  • Assistance of a military health unit deployed in «Institucion Ferial de
           Madrid» field hospital during SARS-CoV-2 crisis

    • Abstract: RESUMEN Introducción: Durante la pandemia por coronavirus SARS-CoV-2 que afectó a España durante la primavera del año 2020, muchos de los hospitales se colapsaron por la afluencia de estos pacientes. Por este motivo se desplegó en la Institución Ferial de Madrid (IFEMA) un hospital de campaña. La única unidad de críticos de este hospital fue militar. La unidad responsable de su montaje, mantenimiento y repliegue fue la Unidad Médica Aérea de Apoyo al Despliegue (UMAAD) de Madrid perteneciente al Ejército del Aire. Objetivo: Describir la asistencia médica prestada en la unidad de críticos de IFEMA. Material y métodos: Estudio descriptivo prospectivo realizado del 19 de marzo al 30 de abril de 2020. Se consiguió la autorización militar de la Dirección de Sanidad del Ejército del Aire para la realización del estudio. Resultados: Durante el periodo de estudio 25 pacientes ingresaron en la unidad durante la Operación «Balmis». La mayoría fueron varones y de raza hispana, el motivo de ingreso fue por neumonía por SARS-CoV-2. Se aisló la vía aérea en el 88% de los pacientes. Las principales complicaciones fueron trombosis, hemorragia e hipertensión, falleciendo tres pacientes durante la realización del estudio. Conclusión: La experiencia adquirida en este despliegue, demuestra, una vez más, la importancia de los apoyos que la Sanidad Militar puede prestar a la Sanidad Civil en el que el componente sanitario de la gestión de crisis en territorio nacional.SUMMARY Introduction: During the coronavirus SARS-CoV-2 pandemic that affected Spain in spring of 2020, many of hospitals were collapsed by these patients. For this reason, a field hospital was deployed at the «Institución Ferial de Madrid» (IFEMA). The only critical unit of this field hospital was military. The unit responsible for its deployment, maintenance and withdrawal was UMAAD (Madrid Deployment Support Air Medical Unit belonging to the Air Force). Objective: To describe medical assistance provided in the IFEMA critical unit. Material and methods: Prospective descriptive study carried out from March 19 to April 30, 2020. The pertinent military authorization (Air Force Health Service Director) was obtained to carry out the study. Results: During the study time, 25 critically ill patients were admitted. Most of them were male and Hispanic race. SARS-CoV-2 pneumonia was the reason for hospital admission. Airway was performed in 88% of patients. Principal complications were thrombosis, hemorrhage and hypertension. Three patients died during this research paper. Conclusion: The experience acquired in this deployment demonstrates, once again, the importance of the support that Military Health can provide to Civil Health in the component of crisis management in national territory.
       
  • Analysis of clinical cases in the Oftalmology area attended by the
           Telemedicine Service of the Hospital Central de la Defensa «Gómez Ulla»
           during the years 2017, 2018 and 2019

    • Abstract: RESUMEN Antecedentes y Objetivos: Evaluación de casos clínicos de Oftalmología atendidos mediante el Servicio de Telemedicina del Hospital Central de la Defensa Gómez Ulla en 2017, 2018 y 2019 estudiando edad, sexo, empleo, antecedentes oftalmológicos, tiempo de respuesta, pruebas, diagnóstico, tratamientos, evacuaciones, medio de comunicación y lugar. Material y Métodos: Estudio observacional retrospectivo de los 37 casos clínicos de la base de datos del Servicio de Telemedicina. Resultados: La especialidad de oftalmología está entre las 10 más demandadas y las solicitudes aumentan cada año. La patología con mayor prevalencia es traumatismo en la conjuntiva o excoriación corneal por accidentes. En un 65% fue posible el tratamiento en la misión gracias a las indicaciones del oftalmólogo. El personal desplegado carecía de los fármacos necesarios en al menos 6 casos. En algunos pacientes se encontraron antecedentes oftalmológicos que podían ser causa de la enfermedad que aconteció. Conclusiones: Sería aconsejable una mayor protección ocular dado que la mayoría de los casos consisten en accidentes traumáticos y salpicaduras. Convendría llevar a cabo exámenes exhaustivos pre-misión revisando antecedentes que puedan desencadenar problemas en misiones. Sería recomendable proveer a cualquier sanitario desplegado los medicamentos recogidos en este trabajo que han sido recomendados por los oftalmólogos. El Servicio de Telemedicina ha hecho posible que los especialistas del área de Oftalmología hayan brindado un valioso apoyo al personal sanitario desplegado en un amplio abanico de ubicaciones remotas. Además de evitar evacuaciones médicas innecesarias, dicho servicio facilita evacuaciones apropiadas que de otro modo podrían haberse retrasado.SUMMARY Background and Objectives: Evaluation of Ophthalmology clinical cases attended to by the Telemedicine Service of the Central Defense Hospital Gómez Ulla in 2017, 2018 and 2019, studying age, sex, employment, ophthalmological history, response time, tests, diagnosis, treatments, evacuations, media and place. Material and Methods: Retrospective observational study of the 37 clinical cases from the database of the Telemedicine Service. Results: Ophthalmology is among the 10 most highly demanded specialities and requests are increasing every year. The pathology with the highest prevalence is conjunctival injury or corneal abrasion due to accidents. In 65% of the cases, thanks to the ophthalmologist’s instructions, treatment within the mission was possible. The deployed health staff lacked the necessary medication in at least 6 cases. A history of ophthalmological diseases was found in some patients, which could be the cause of the disease that was observed. Conclusions: Greater eye protection would be advisable, since the majority of cases consist of traumatic accidents and splashes. Extensive pre-mission reviews should be conducted by reviewing possible issues that may trigger mission problems. It would be advisable to equip all deployed healthcare staff with the necessary medications that were identified in this work, which have been recommended by ophthalmologists. The Telemedicine Service has made it possible for specialists in the area of Ophthalmology to provide valuable support to deployed healthcare staff in a wide range of remote locations. In addition to avoiding unnecessary medical evacuations, this service facilitates appropriate evacuations that might otherwise may have been delayed.
       
  • Review of effectiveness criteria in the application of Telemedicine

    • Abstract: SUMMARY Telemedicine is a very useful tool and resource for medical diagnosis and treatment. Since its inception in the 70s it has developed progressively, demonstrating the clinical benefit it brings to patients and being increasingly present in the daily activities of clinicians. Different studies have tried to define and limit its effectiveness at the time of application versus face-to-face assistance, this being an important variable to clarify. This article reviews the different variables that have been coined to define the effectiveness when trying to demonstrate the cost-effectiveness of including telemedicine in daily healthcare activity.RESUMEN La telemedicina es una herramienta y recurso de gran utilidad para el diagnóstico y tratamiento médico. Desde su inicio en la década de los 70 se ha desarrollado de forma progresiva, demostrando el beneficio clínico que aporta a los pacientes y estando cada vez más presente en la actividad diaria de los clínicos. Diferentes estudios han tratado de definir y acotar su efectividad a la hora de aplicarse versus la asistencia presencial, siendo esta una variable importante a despejar. Este artículo realiza una revisión de las diferentes variables que se han acuñado para definir efectividad a la hora de tratar de demostrar la rentabilidad de incluir la telemedicina en la actividad asistencial diaria.
       
  • Aortoiliac thrombosis related to COVID-19: a case report

    • Abstract: RESUMEN Se presenta un caso de isquemia arterial aguda en extremidad inferior izquierda por trombosis aortoiliaca, en paciente diagnosticada de neumonía bilateral con compromiso respiratorio severo durante la infección por SARS-CoV-2. Se realiza angio TC abdominopélvico ante sospecha de afectación iliofemoral izquierda y femoropoplitea derecha sin factores de riesgo cardioembólicos ni clínica previa de claudicación. A pesar del mal estado general de la paciente con necesidad de aporte de oxígeno a alto flujo se realiza cirugía para salvamento de extremidad. La intervención se realizó bajo anestesia local y en posición anti-Trendelemburg para asegurar la saturación de oxígeno. Se realiza tromboembolectomía iliofemoral de la extremidad sintomática, consiguiendo la recuperación funcional de la extremidad.SUMMARY We present a case of acute arterial ischemia in left lower limb due to aortoiliac thrombosis in a patient with SARS-CoV-2 pneumonia with severe respiratory distress. Abdominal CT angiography was performed with high suspect of left iliofemoral and right femoropopliteal compromise without cardioembolic risk factors nor previous intermittent claudication. Despite patient’s poor condition, who needed supplementation of high flow oxygen, a salvage surgery was performed in order to preserve the extremity. The intervention was performed under local anesthesia and reverse Trendelenburg position in order to keep appropriate oxygen saturation. The procedure consisted of mechanical iliofemoral thromboembolectomy of the symptomatic limb achieving functional recovery.

Se trata de opiniones técnicas positivas que son previas a la autorización y puesta en el mercado del medicamento.SUMMARY The drugs assessed by the Spanish Agency for Medicines and Health Products or European Medicines Agency made public in April and June of 2020, and considered of interest to the healthcare professional, are reviewed. These are positive technical reports prior to the authorization and placing on the market of the product.

  • Performance of the General Defense Hospital in Zaragoza during the
           pandemic caused by the new SARS-CoV-19 coronavirus

    • Abstract: RESUMEN La pandemia provocada por el virus SARS-CoV-2 ha supuesto una verdadera tragedia sanitaria a nivel mundial, generando graves consecuencias de morbimortalidad, especialmente en personas mayores. El Hospital General de la Defensa (HGDZ) ha participado en la Operación Balmis de las Fuerzas Armadas (FAS) para combatir esta pandemia. Esto ha exigido al HGDZ una gran flexibilidad y capacidad de adaptación para afrontar la gran demanda de ingreso de pacientes infectados por este virus. Para ello, ha Implantado un conjunto creciente de medidas centradas en reforzar la respuesta en el ámbito de la salud y reducir las tasas de contagio.SUMMARY The pandemic caused by the SARS-CoV-2 virus has been a true health tragedy worldwide, generating serious consequences of morbidity and mortality, especially in older people. The General Defense Hospital (HGDZ) has participated in Operation Balmis of the Armed Forces (FAS) to fight against this pandemic. It has required HGDZ great flexibility and adaptability to face the high demand for admission of patients infected with this virus. To this aim, it has implemented a growing set of measures focused on strengthening the response in the health field and reducing contagion rates.

           performance during Balmis Operation

    • Abstract: RESUMEN El Instituto de Medicina Preventiva de la Defensa (IMPDEF) ha jugado un papel importante durante la Operación Balmis en materia de medicina preventiva y salud pública en el ámbito de las Fuerzas Armadas. La finalidad ha sido proporcionar una guía para la vigilancia de casos COVID-19 y adopción de medidas de protección sanitaria de la fuerza, mantener su operatividad y proporcionar información como parte de promoción de la salud. El presente artículo pretende dar a conocer la gestión del COVID-19 realizada desde el IMPDEF en el entorno extrahospitalario de territorio nacional y de las operaciones militares en el exterior, y su evolución durante la Operación Balmis.SUMMARY The Military Institute of Preventive Medicine for the Defence (IMPDEF) has played an important role during Balmis Operation in preventive medicine and public health in the field of the Armed Forces. The aim has been to provide guidance for case surveillance of COVID-19 and the adoption of measures related with force health protection, maintenance of force operativity and provide information as part of medical education. This article aims to present the role of the IMPDEF during the Balmis Operation providing recommendations for public health policy and interventions in the units in Spain and for the deployed forces.

    • Abstract: RESUMEN Desde hace tiempo se utilizan tratamientos para algunas enfermedades infecciosas con plasma hiperinmune de donantes convalecientes de esa enfermedad con resultados prometedores. Ante la pandemia de COVID-19 se plantea su eficacia dado que no existen tratamientos óptimos para abordar la enfermedad. El Comité Científico de Seguridad Transfusional del Ministerio de Sanidad permite la realización de ensayos clínicos y estudios observacionales para avanzar en este terreno mientras se obtiene un tratamiento eficaz.SUMMARY Treatments for some infectious diseases with hyperimmune plasma from convalescent donors of this disease have been used for some time with promising results.   • Presente y futuro de la Telemedicina Militar Española. 25 años de la
           Unidad de Telemedicina en el Hospital Central de la Defensa «Gómez
           Ulla»

    • Abstract: SUMMARY Telemedicine consists of the exchange of health information between healthcare professionals, and between healthcare professionals and patients, through the use of information and communications technologies (ICT). It is already considered an emerging technology in what is commonly called the «Productivity Plateau». It is also probably an exponential growth technology, in which the «trigger» for such growth will be a mixture of new technologies such as portable sensors/wearables that detect multiple patient data (Blood Pressure, Heart Rate, Respiratory Rate, Glycaemia, Temperature, etc.), better communications (5G), augmented and mixed reality (augmented and virtual), artificial intelligence systems to improve diagnosis, etc. In Spain, Military Telemedicine is a pioneer in the field. The main mission is to provide remote health support to health professionals or military personnel deployed in Operations and remote or difficult-to-access locations. In 2021 the Spanish Telemedicine Unit at Central Defense Hospital «Gómez Ulla» will celebrate its 25Th anniversary. This article discusses the aforementioned aspects of telemedicine as an emerging technology and describes the current mission, organization and capabilities of Spanish military telemedicine, as well as its future.RESUMEN La Telemedicina consiste en el intercambio de información sanitaria entre profesionales sanitarios, o entre profesional sanitario y paciente, mediante el uso de las tecnologías de la información y comunicaciones (TIC). Se considera ya una tecnología emergente en el denominado «Plateau de productividad». Probablemente se trate de una tecnología de crecimiento exponencial, en la cual el «gatillo» para dicho crecimiento será una mezcla de diferentes tecnologías, como nuevos sensores portátiles que detecten múltiples datos de los pacientes (Tensión Arterial, Frecuencia cardiaca, Frecuencia respiratoria, Glucemia, Temperatura, etc), mejores comunicaciones (5G), realidad aumentada y mixta (aumentada y virtual), sistemas de inteligencia artificial para ayuda al diagnóstico, etc. En España, la Telemedicina Militar es pionera en este campo. Su misión fundamental es el apoyo y asesoramiento sanitario a distancia a personal sanitario y también no sanitario, tanto en Zona de Operaciones (ZO) como en situación de aislamiento y/o localización remota. En 2021 el Servicio de Telemedicina del Hospital Central de la Defensa «Gomez Ulla» celebrará su 25 aniversario. En el artículo se tratan los mencionados aspectos acerca de la Telemedicina como tecnología emergente y se describe la misión, organización y capacidades actuales de la Telemedicina Militar Española, así como sus perspectivas de futuro.
