      Abstract: imageThe number of potentially curative treatments for advanced, metastatic lung cancer has increased dramatically following the Cancer Moonshot investments that started in 2016. Many of these treatments target the effects of cancer-causing genetic variants, known as driver mutations. Immunotherapy treatments that release the full power of the patient's own immune system produce amazing results. Advances in radiation therapy yield markedly better results with fewer treatments, reducing the need for surgical intervention. Emphasis on cancer prevention, earlier identification through cancer screenings, and highly effective treatments that can precisely target the characteristics of a patient's specific tumor have had a significant impact on outcomes. The field of oncology is changing dramatically, and agencies that recognize this phenomenon and respond will have the competitive edge. We are no longer just getting patients ready to accept hospice; we need to also focus on getting patients ready for long-term survival with the highest possible quality of life. Understanding the basics about cancer treatment options today is a core competency for home health clinicians and managers. There is an emerging need for home health nurses, therapists, and social workers with advanced education and certification in oncology who can anticipate, identify, and clearly communicate potential disease and treatment-related concerns to other members of the healthcare team. Teaching patients how to maintain functional status, manage symptoms, and to prevent avoidable complications promotes a high quality of life as well as long-term survival.

Engaging the health care team, including older adults and their family caregivers, with the 4Ms framework can help to ensure that every older adult gets the best care possible, is not harmed by health care, and is satisfied with the care they receive.

  • What Constitutes Patient-Centered Care in Home Care' A Descriptive
           Study of Home Health Nurses' Attitudes, Knowledge, and Skills

      Abstract: imageIn their seminal paper outlining the proposed Future of Home Care, Landers et al. (2016) stated that “patient-centered care” (PCC) is one of the “pillars” of home care. They then asked the question, what is PCC in home care and how is it measured? A qualitative descriptive study explored the answer to this question. In-depth interviews were conducted with 20 home health nurses to identify how they incorporated patient-centered and culture-sensitive care (CSC) into their assessment and care planning practices. The data were categorized into attitudes, knowledge, and skills (including relationship-building, assessment, and care planning skills) associated with patient-centered/culture-sensitive care. The home health nurses had developed multiple strategies for delivering PCC, despite a lack of education in how to provide this care. They primarily learned their techniques through their caring for patients as unique, highly valued persons and their ability to form warm caring relationships with their patients. Together they painted a portrait of the attitudes, knowledge, and skills needed for PCC and CSC. PCC and CSC are mutually reinforcing concepts essential to the high-quality, equitable care needed to mitigate healthcare disparities prevalent in home healthcare. A teaching resource for incorporating PCC/CSC into home health clinician practice was derived from the data.

  • Addressing Compassion Fatigue in Family Caregivers using a
           Mindfulness-based Smartphone Application

      Abstract: imageThe number of Americans receiving some level of healthcare at home is rapidly increasing, often leaving family members responsible for providing care for their loved ones. The impact on these unpaid caregivers, who must also balance the stressors of work and other daily responsibilities, can lead to compassion fatigue (CF). A growing body of evidence suggests that the practice of mindfulness meditation can reduce the symptoms of CF, such as stress, anxiety, and depression. This quality improvement project sought to determine if using a mindfulness-based smartphone app daily for 6 weeks would have any impact on CF in family caregivers. The project sought to determine whether it was feasible for caregivers to use the app and examined the impact on stress, depression, and anxiety. Participants completed two validated instruments before and after the 6-week intervention and results were compared using Wilcoxon signed rank tests. Results indicated a significant reduction in stress (p = .043) and a marginal reduction, though not significant, in depression (p = .075) and anxiety (p = .149). The caregivers found the app to be useful. Future projects should investigate the patterns of use by family members and use of the app as an adjunct to other interventions.

  • Review of National Council on Aging Fall Prevention Programs for the Home
           Health Setting

      Abstract: imageFalls are a major concern for community-dwelling older adults and receive substantial attention from community agencies, clinicians, and researchers. A review process completed by the Administration for Community Living has identified 16 evidence-based fall prevention programs listed by the National Council on Aging. Although most of the programs are designed for group settings in the community, four have evidence supporting their effectiveness in the home setting. These are CAPABLE, FallScape, FallsTalk, and the Otago Exercise Program. We reviewed the literature about these four programs and identified positive outcomes in addition to reduction in fall risk. Although the results varied among the programs, potential benefits include improvement in activities of daily living, decreased Medicare expenditure, increased fall awareness and implementation of behaviors to reduce fall risk, decreased pain, and improved balance. These factors should be considered by community agencies looking to select programs to address falls for older adults.
