  • Development of an Effective Client-Practitioner Therapeutic Alliance in

      Abstract: imageLow back pain (LBP) is a significant problem in industrialized society with a considerable economic impact. There are many treatment approaches for LBP; however, many are expensive and possess questionable efficacy. An often-overlooked aspect of care for individuals with LBP is establishment of a positive therapeutic alliance. The therapeutic alliance essentially describes the relationship between the patient or client and practitioner that embodies trust, empathy, and mutual respect. Evidence suggests that when a positive therapeutic alliance exists, outcomes are improved. Unfortunately, practitioners are rarely instructed in how to specifically develop a positive therapeutic alliance. This article intends to highlight the overarching elements of establishing a therapeutic alliance for LBP management.

  • The Reliability of Tensiomyography for Assessment of Muscle Function in
           the Healthy Population: A Systematic Review

      Abstract: imageTensiomyography (TMG) is a noninvasive tool used to assess contractile tissue properties during an isometric muscle contraction. Owing to portability and versatility for assessing muscle parameters, TMG may be of value to the strength and conditioning (S&C) specialist. The purpose of this systematic review was to investigate the reliability of TMG measurements. PubMed, PEDro, MEDLINE, and Cochrane databases were searched up to September 2021 by 2 authors, who independently examined all titles and abstracts to determine initial eligibility. Inclusion criteria included any study assessing the reliability of TMG parameters, published in English, published in a peer-reviewed scientific journal, and included participants with no significant musculoskeletal conditions. Exclusion criteria included the following elements: TMG assessment process not clearly delineated, reliability of specific TMG parameters not clearly defined, and statistical methods for determining reliability not clearly defined. All studies underwent a quality assessment using the Modified Downs and Black checklist for assessing quality studies, and results were extracted from qualified articles. In total, 635 studies were identified, with 16 studies retained after full-text reviews. Twelve studies had poor quality, whereas 4 had fair quality. Noninvasive TMG has good to excellent absolute and interrater reliability for measuring the properties of skeletal muscles.

  • Resistance Training in Individuals With Hip and Knee Osteoarthritis: A
           Clinical Commentary With Practical Applications

      Abstract: imageOsteoarthritis (OA) has been identified as 1 of the 4 most prevalent chronic musculoskeletal conditions, with both prevalence and health care spending rising as the population continues to age. The current US health care system incorporates a model that is driven by individualized one-on-one treatment, which has been successful in the management of certain medical diseases; however, chronic musculoskeletal diseases, such as OA, may be better managed from a population health perspective for more cost-efficient and effective utilization of resources. Although the benefits of resistance training are indisputable, a wide variety of factors (social determinants of health, access to quality care, contextual factors, and comorbidities) may hinder the successful implementation of resistance training programs in older individuals with OA. Considering the complexity and flaws of the health care system, health care professionals involved in movement and exercise should consider innovative models of care that incorporate population health, psychosocial factors, and social determinants of health, in conjunction with individualized treatment of disease, injuries, and pathologies to enhance independence and quality of life in individuals with OA. Therefore, this commentary will discuss innovative strategies to overcome the inherent challenges associated with the implementation of resistance training programs for individuals with hip and knee OA.

  • Anemia: Considerations for the Athletic Population

      Abstract: imageAnemia is a condition characterized by too few red blood cells, leading to diminished oxygen delivery, which can significantly impact exercise participation and athletic performance. The etiology of anemia is multifactorial. Some forms of anemia develop as a result of deficiencies in specific vitamins and minerals and may be modifiable, whereas other types are inherited disorders. Although nonmodifiable, the latter may be managed with the assistance of trained medical professionals. This article will provide an overview of the pathophysiology associated with common forms of anemia and provide an introduction to diagnostic testing procedures and treatment options. Additionally, the signs and symptoms will be discussed to help strength and conditioning professionals recognize this potentially impactful condition. This article concludes with a discussion of the influence iron deficiency has on sports participation.

  • Blood Flow Restriction Training for Individuals With Osteoarthritis

      Abstract: imageResearch suggests that healthy eating and exercise decrease the likelihood of developing osteoarthritis (OA) with age. Despite this, OA is a prevalent chronic condition that typically causes joint pain at rest and during exercise, making it difficult to develop effective training programs. Recently, blood flow restriction (BFR) training has shown to be a beneficial alternative to traditional resistance training to improve muscle function. In this article, we provide a rationale as to how BFR may be a beneficial resistance training alternative that would allow individuals with osteoarthritis to experience similar improvements in muscle function compared with traditional resistance training using lower relative intensities.

      Abstract: imageExercise is an effective tool for managing and treating type II diabetes (T2DM) and may be a useful component of a diabetes management program. Both short- or long-term exercise interventions can improve physical fitness, metabolic health, and cardiovascular function in individuals with poor glycemic control. Incorporating aerobic and resistance exercise as part of a comprehensive diabetes management plan can play a vital role in the fight against this chronic metabolic disease. Progression and individualized exercise programming are critical for an effective and enjoyable exercise experience in a gym setting. With respect to safety, exercise professionals should be aware of special considerations and potential complications persons with T2DM can experience when exercising.

      Abstract: imageFemale participation in sport as a masters athlete has grown considerably in recent years. Functional losses and physiological changes in the cardiovascular, musculoskeletal, and endocrine systems that occur with female aging require recognition and thoughtful exercise prescription. In working with this population, the strength and conditioning professional should be able to recognize the biology of aging, factors affecting recovery and performance, and application of appropriate training parameters to keep the female athlete in their sport while mitigating the effects of aging and maximizing performance. This special populations article outlines key age-related changes in the female athlete and presents comprehensive, practical training guidelines for this growing population.

  • Trunk Neuromuscular Function and Anterior Cruciate Ligament Injuries: A
           Narrative Review of Trunk Strength, Endurance, and Dynamic Control

      Abstract: imageTrunk strength, endurance, and dynamic control may have an effect on anterior cruciate ligament (ACL) injury rates and biomechanical ACL loading variables during athletic tasks. Individuals responsible for training athletes at risk of ACL injuries should implement training programs that address these components of athletic performance. In ski racers, deficits in trunk flexion/extension strength and decreased trunk flexion/extension strength ratios have been identified as ACL injury risk factors. Trunk strength training alone is not sufficient to decrease biomechanical ACL loading, and there is no clear association between trunk endurance and ACL injury risks. Trunk dynamic control training may improve trunk and knee movements associated with decreased ACL loading during athletic tasks. Dynamic, unanticipated, and perturbed trunk functional assessments and training are recommended to challenge the trunk more during athletic tasks. Injury prevention programs should involve exercises using unstable surfaces, sports-related dual tasks, and perturbations to address trunk dynamic control. More investigation is still needed to further understand the associations between trunk neuromuscular functions and ACL injury risks during athletic tasks.

  • A Collective Review of the Research on Training the Oldest-Old

      Abstract: imageTo synthesize the scientific evidence on physical exercise protocols for the oldest-old we searched PubMed, Scopus, Web of Science, CINAHL, Medline, and gray literature up to January 2021. Eligibility criteria included investigation of the oldest-old population (≥80 years); use of intervention protocols with appropriate and effective variables for a physical exercise program (type, intensity, frequency, and duration); reporting positive effects of the interventions on physical fitness, execution of activities of daily living, and quality of life of the oldest-old; an experimental and/or quasiexperimental (randomized or nonrandomized) study design. Twenty studies were included. Physical exercise protocols for the oldest-old should include strength, power, aerobic endurance, flexibility, balance, and joint mobility exercises. These exercises are effective in attenuating or minimizing the declines that occur during aging, improving factors such as gait, fall rate, and functional capacity.
