  • Death in a bathtub of an adolescent with neurofibromatosis type 2
           exhibiting meningioangiomatosis with white matter involvement

      Abstract: Abstract Neurofibromatosis type 2 (NF2) is a neurocutaneous syndrome characterized by the development of multiple benign tumors, including vestibular schwannomas and meningiomas, in the nervous system. Seizures are rarely associated with NF2, and the lethality of this condition typically stems from tumor growth and related complications, leaving the incidence of sudden death largely unreported. This report discribes a 16-year-old girl with a history of NF2 and occasional seizures who died unexpectedly in a bathtub. Postmortem examination revealed multiple tumors in the cranial nerves (schwannoma), under the dura mater (meningioma), and in the upper cervical cord (neurofibroma). Typical signs of drowning, such as foam in the airways, were not present. Upon histological examination, meningioangiomatosis (MA) was observed in the cerebellum and the cerebral cortex, specifically in the frontal lobe, temporal lobe, and insula. The MA extended into the white matter, exhibiting severe perivascular fibrosis and cystic dilatation of perivascular spaces in the frontal lobe and cerebellum. Additionally, glial microhamartomas were detected both around and separate from the MA. These autopsy findings suggest that sudden unexpected death in epilepsy (SUDEP) was the cause of death rather than drowning. Moreover, while NF2-associated MA is typically asymptomatic, unlike sporadic MA, which commonly presents with seizures, the spread of MA into the white matter is unusual in an NF2 patient. Therefore, MA with the white matter involvement could have been a factor causing the seizures and the occurrence of SUDEP in this NF2 patient.

  • Police arrest-related death: about four cases and review of the literature

      Abstract: Abstract Unexpected deaths may occur in individuals during a police arrest. These individuals usually have had both physical and emotional exertion, associated or not with agitation and restraint by police. Further investigations in these cases often show an impregnation with drugs. Autopsy findings may raise medico-legal problems in these cases. We report four cases of deaths during police arrest in which multiple factors contributed to the death. In the first two cases, there was a combination of emotional and physical stress due to police pursuit and, the presence of mechanical asphyxia. We noticed the presence of cannabis and amphetamine; however, we couldn’t relate the death to a drug intoxication. In the third case, the deceased lost consciousness when he was arrested by the police with no link to the intervention of law enforcement authorities. Death in this case was due to sepsis complicating endocarditis. The fourth case reports a rare cause of asphyxia that can occur in a police arrest related to a drug package. Death was due to upper airway obstruction in this case. The forensic pathologist should take into consideration every factor when explaining these deaths. Thorough autopsy and investigations are critical to understand the cause and circumstances of death.

  • Terminal ballistics performance of 9 × 19 mm cartridges in 10%
           ballistic gelatin: FBI Protocol based analysis of Brazilian-made law
           enforcement ammunition

      Abstract: Purpose Understanding that a projectile entering the human body can cause damage or destruction to live tissues through a variety of wounding mechanisms – permanent cavity, temporary cavity, and fragmentation – is crucial for researching terminal ballistics and understand the patterns of gunshot wound configuration. Methods The present work tested four different types of ammunition in caliber 9 × 19 mm (Full Metal Jacketed, Gold Hex, Copper Bullet Tactical and Bonded), using ballistic gelatin at 10% as soft tissue surrogate. The tests were based on the Federal Bureau of Investigation Protocol and included shots through bare gelatin, heavy clothing, plywood, steel sheets and auto glass. As a comparison parameter, the American-made Federal™ HST, used by several law enforcement agencies in the USA, was also tested in the same conditions. Results The Full Metal Jacketed cartridge had a uniform performance throughout the experiment, showing high penetration levels and no expansion, as expected. Gold Hex demonstrated a strong tendency to fragment with low levels of penetration and weight retention. Copper Bullet Tactical did not achieve the 12” minimum penetration in the soft barrier phases but expanded aggressively. Finally, Bonded only failed to achieve the 12” mark of penetration in phase 5 (auto glass), the hardest barrier in the whole Protocol. Tested for comparison purposes, Federal HST showed aggressive expansion in the initial phases (over 100%), after surpassing the 12” threshold. Conclusion The study concluded that heavier projectiles (CBC Bonded and Federal HST) performed better than lighter and faster bullets in terms of terminal ballistics.

  • The application of vibrational spectroscopy in forensic analysis of
           biological evidence

      Abstract: Abstract Vibrational spectroscopy is a powerful analytical domain, within which Fourier transform infrared spectroscopy (FTIR) and Raman spectroscopy stand as exemplars, offering high chemical specificity and sensitivity. These methodologies have been instrumental in the characterization of chemical compounds for an extensive period. They are particularly adept at the identification and analysis of minute sample quantities. Both FTIR and Raman spectroscopy are proficient in elucidating small liquid samples and detecting nuanced molecular alterations. The application of chemometrics further augments their analytical prowess. Currently, these techniques are in the research phase within forensic medicine and have yet to be broadly implemented in examination and identification processes. Nonetheless, studies have indicated that a combined classification model utilizing FTIR and Raman spectroscopy yields exceptional results for the identification of biological fluid-related information and the determination of causes of death. The objective of this review is to delineate the current research trajectory and potential applications of these two vibrational spectroscopic techniques in the detection of body fluids and the ascertainment of causes of death within the context of forensic medicine.

  • Assessing the impact of corrosive acids on human bone integrity in
           forensic context

      Abstract: Abstract Dissolving bodies is a contemporary method of disposing of human remains and has been practiced throughout the years. This research article discussed the impact of readily available corrosive chemicals, such as Hydrochloric acid (HCl), Sulfuric acid (H2SO4), and Expelz™ toilet cleaner, on human bone. Unfortunately, the existing literature on this subject is scarce, with only a few studies conducted on human bones. To address this gap, we conducted an experimental analysis using human sternum bone fragments. These samples were subjected to immersion in different acids, namely HCl and H2SO4, at two concentrations (10% and 37%), and Expelz™ toilet cleaner. This research aims to describe the impact of toilet cleaner, sulphuric acid, and hydrochloric acid on bone tissue deterioration at various time intervals. Further, the morphological alterations weight loss, and cytological analysis of bone tissue residue with various acid concentrations and immersion times (3.5, 9, 24, 36, 48, 60, and 72 h) were analyzed. Among the chemicals examined, it is evident that 37% hydrochloric acid (HCl) and sulfuric acid (H2SO4), regardless of refreshment, exhibit the highest reactivity, resulting in the complete decomposition of hard tissue within a few hours. In contrast, the behaviour of 10% H2SO4 reveals a more intricate degradation process characterized by fluctuations in weight loss. These distinct reactivity profiles under different conditions are crucial in understanding the potential for rapid weight loss, signifying high reactivity and more gradual weight loss, indicating a faster decomposition rate. Notably, refreshment of the solution enhanced the reactivity of both HCl and H2SO4.

      Abstract: Abstract A deceased man in his 50 s was found with his neck over a broken glass door frame, with blood around the body. A non-contrast postmortem computed tomography (PMCT) scan revealed subcutaneous hemorrhage, temporal bone fracture, and cerebral contusion. Also, wounds extending from the anterior to posterior neck and the presence of air in the cervical vessels suggested cervical vascular injury. A virtual angioscopy image reconstructed from PMCT angiography data revealed a ruptured left common carotid artery and allowed accurate measurement of the injury. This case demonstrates the effectiveness of postmortem virtual angioscopy for visualization and evaluation of vascular injuries, providing valuable insights for forensic investigation.

  • Positive identification through comparative dental analysis in mass
           disaster: a systematic review and meta-analysis

      Abstract: Abstract Purpose: The study aimed to assess the probability of achieving positive identification through comparative dental analysis (CDA) and to determine the factors that influence its success rate in mass disaster scenarios. Methods: An electronic literature search was conducted across six databases for observational studies that reported both the total number of mass disaster victims and the count of victims identified through CDA alone. A random-effect meta-analysis, using the proportion of victims identified with CDA as the effect size, was conducted alongside subgroup analyses based on the type of disaster (natural or non-natural), the disaster classification (open or closed), and the geographical region (i.e., Europe, Asia). Results: The search yielded 3133 entries, out of which 32 studies were deemed eligible. Most of the studies (96.8%) presented a low risk of bias. The meta-analysis revealed a mean weighted-proportion probability of 0.32, indicating that forensic odontology could identify about one-third of the victims in a mass disaster. The probability of comparative dental identification was three times higher in closed mass disasters compared to open disasters (p < 0.05) and was higher in mass disasters occurring in North America and Europe compared to other regions (p < 0.05). Conclusion: The current result suggested that CDA can identify approximately 32% of a victim in a hypothetical scenario, emphasizing the integral role of teeth and forensic odontology in victim identification framework.

  • Postmortem biochemistry of GFAP, NSE and S100B in cerebrospinal fluid and
           in vitreous humor for estimation of postmortem interval: a pilot study

      Abstract: Abstract Postmortem interval (PMI) is a challenging issue in forensic practice. Although postmortem biomarkers of traumatic brain injury (TBI) are recognised as an emerging resource for PMI estimation, their role remains controversial. This study aims to evaluate postmortem concentrations of three TBI biomarkers (GFAP, NSE and S100B) in two matrices (cerebrospinal fluid and vitreous humor), in order to find out if these markers could be adopted in PMI estimation. Thirty-five deceased individuals with known PMI who underwent forensic autopsy at the University of Parma were examined. Matrices were collected during autopsy, then biomarker concentrations were determined through the enzyme-linked immunosorbent assay. Statistical significance of the data in relation to PMI was studied. The correlation of biomarkers with PMI, examined with samples divided into six groups according to the number of days elapsed since death, was not statistically significant, although S100B in cerebrospinal fluid showed an increasing trend in cases from 1 to 5 days of PMI. Comparison between cases with 1 day of PMI and those with 2 or more days of PMI showed a statistically significant correlation for GFAP and NSE in cerebrospinal fluid. GFAP and NSE in cerebrospinal fluid represent appropriate biomarkers in PMI estimation to distinguish cases with one day of PMI from those with two or more days of PMI. The current study was limited by the scarcity of the cohort and the narrow spectrum of cases. Further research is needed to confirm these observations.

  • Decomposition of partially submerged remains: a study on the reliability
           of insect colonisation for PMI/PMSI estimation

      Abstract: Abstract The terrestrial decomposition of remains and associated insect colonisation have been highly researched, and recently studies have expanded to investigate the aquatic decomposition of remains. However, there are instances where remains may experience both terrestrial and aquatic conditions simultaneously due to partial submersion in tidal areas, or influx or efflux of water caused by flood or drought. Decomposition and post-mortem interval (PMI) research to date has focused on remains wholly exposed to either terrestrial or aquatic environments, with limited consideration of dual simultaneous exposure. This study was conducted in artificial lentic environments to ascertain how simultaneous zones of terrestrial and aquatic environments on a single body may impact decomposition. Three trials were completed over a period of 12 months, with each trial consisting of 12 stillborn piglets; three partially submerged head exposed, three partially submerged abdomen exposed, three fully submerged aquatic controls and three terrestrial controls. Decomposition stage and rate were inferred from physical characteristics and insect activity. The decomposition rate of the exposed region of each piglet was significantly faster than the submerged region. The exposed zone of each was colonised by insects and reached skeletonization, whereas the submerged zone without orifice exposure had no insect activity and had a significantly slower decomposition rate. This indicated the ability to utilise terrestrial entomological approaches to estimate a minimum PMI for the exposed portion of the remains. However, without the ability to determine the amount of time the remains may have been submerged for, this estimation represents only a minimum PMSI, with the possibility the remains were submerged for a period of time without insect access and colonisation.

           radiators/heaters in children

      Abstract: Abstract Contact burns in children are not uncommon and are often due to accidental contact. Medico-legal assessment is of paramount importance in these contexts to identify cases of abuse. In three cases of burns caused by contact with radiators or a portable heater -two accidental and one deliberate- thorough medico-legal assessment, combined with on-site event reconstruction, enabled accurate diagnoses. Accidental burns displayed a ‘pattern’ compatible with the incandescent instrument but were more irregular, with different depths and in different parts of the body. In contrast, intentional burns were uniform in depth, distribution and localisation, inconsistent with accidental events. In this context, the on-site inspection and direct evaluation of the objects involved were crucial in the medico-legal assessment. These are indispensable elements for a thorough analysis and abuse recognition.

  • Characterization of a novel SCN5A mutation associated with long QT
           syndrome and arrhythmogenic right ventricular cardiomyopathy in a family

      Abstract: Abstract Sudden cardiac death represents a significant diagnostic challenge for forensic pathologists, particularly in inherited arrhythmia syndromes or cardiomyopathies resulting from genetic defects. Molecular autopsies can reveal the underlying molecular etiology in such cases. In this study, we investigated a family with a history of sudden cardiac death to elucidate the molecular basis responsible for sudden cardiac death. The proband underwent a comprehensive forensic examination. Family members received thorough clinical evaluations, including electrocardiogram, Holter monitoring, echocardiography, and cardiac magnetic imaging. Whole exome sequencing and genetic analysis were performed on the deceased and her parents. In addition, Western blotting and patch-clamp recordings were employed to evaluate the expression and function of the mutant protein in vitro. Forensic examination diagnosed arrhythmogenic right ventricular cardiomyopathy (ARVC) as the cause of sudden death. Genetic analysis identified a novel missense mutation in SCN5A (p.V1323L), which was assessed as likely pathogenic by the ACMG guideline. Another family member carrying the mutation manifested long QT syndrome and mild cardiac fibrosis. The cellular electrophysiological study demonstrated that the mutation resulted in an enhanced late sodium current, suggesting it was a gain-of-function mutation. This study characterizes a novel SCN5A mutation that putatively causes long QT syndrome and may contribute to the development of ARVC. Our work expands the pathogenic spectrum of SCN5A variants and underscores the importance of molecular autopsy in sudden death cases, especially in those with suspected genetic disorders.

  • Case report: Electrocardiographic (ECG) recording during the hanging
           process

      Abstract: Abstract A 48-year-old woman was found hanged in the bathroom. She was wearing a Holter monitor, which was later analysed by a cardiologist. During autopsy, findings congruent with atypical hanging were collected. The ECG showed a 20 s asystole and four minutes later bradycardia, which progressed to a second-degree AV-block Mobitz I, then Mobitz II, then to a third-degree AV-block. Finally, only P waves could be observed, before heart action ceased. This is one of few cases reporting ECG-changes during hanging and might give further insight into the complex pathophysiology of this type of death.

  • Trabecular bone microstructure parameters as predictors for chronological
           age: a systematic review

      Abstract: Abstract Estimating chronological age is crucial in forensic identification. The increased application of medical imaging in age analysis has facilitated the development of new quantitative methods for the macroscopic evaluation of bones. This study aimed to determine the association of age-related changes in the trabecular microstructure with chronological age for age estimation in forensic science through different non-invasive imaging techniques. This systematic review was reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. An electronic search was performed with PubMed/MEDLINE, Scopus, and Cochrane databases as well as with a Google Scholar search. Qualitative synthesis was performed using the Anatomical Quality Assessment tool. A detailed literature search yielded 3467 articles. A total of 14 articles were ultimately included in the study. A narrative approach was employed to synthesize the data. Microcomputed tomography, high-resolution peripheral quantitative computed tomography, and cone beam computed tomography have been used for the quantitative estimation of age. These imaging techniques aid in identifying the trabecular bone microarchitectural parameters for chronological age estimation. Age-related changes in trabecular bone included a decrease in the bone volume fraction, trabecular number, and connectivity density and an increase in trabecular separation. This study also revealed that morphometric indices vary with age and anatomical site. This study is registered with the International Prospective Register of Systematic Reviews (PROSPERO) with the registration number CDRD42023391873.

  • Prevalence of Frank’s sign in healthy young individuals, morphological
           characteristics, and its association with family history of chronic
           disease

      Abstract: Abstract Frank’s sign (FS, earlobe crease) is known to be associated with many chronic diseases i.e. coronary, cerebrovascular, and peripheral vascular diseases. The objective of this study is to investigate the presence and grade of FS in healthy, young-adult individuals aging between 18 and 25. Furthermore, to evaluate the relationship between FS and the chronic disease history of first-degree family members. 853 healthy people with a mean age of 20.09 years were imvolved in the research on a voluntary basis. Of 853 individuals, 85.3% (n:728) had no FS, whereas 14.7% (n:125) had. Of the 125 people observed FS, 41.6% (n:52) were female and 58.4% (n:73) were male. Of the 125 people diagnosed with FS, 32.8% (n:41) had no family history of any chronic disease, and 67.2% (n:84) had at least 1 family history of chronic disease as well. It was determined that there was a statistically significant (p < 0.05) relationship between FS identified in healthy individuals and familial chronic disease history. In this study, while no FS was observed in 85.3% of the participants, grade 1 was detected in 12.4% (n:105), grade 2a in 1.9% (n:16), grade 2b in 0.2% (n:2), and grade 3 in 0.2% (n:2). To the best of our knowledge, this study investigated the relationship between the presence and grade of FS in the healthy, young, Turkish population and the presence/absence of chronic disease in the first-degree relatives of individuals for the very first time. As a result of larger studies, being aware of the presence of FS, especially in young healthy individuals, may help predict some chronic diseases, and caution may be taken to avoid these diseases at an early age in at-risk individuals.

  • A case of a lidocaine intoxication of a 12 week old boy due to a
           circumcision in a domestic setting – clinical, toxicological and
           forensic aspects

      Abstract: Abstract Lidocaine is a local anaesthetic commonly used during circumcision for dorsal penile nerve block (DPNB). We describe a case of a 12-week-old infant who presented generalized seizures due to local anesthetic systemic toxicity after Lidocaine administration for circumcision in a non-hospital setting. Serum concentrations of Lidocaine (16.4 mg/L) and its main active metabolite monoethylglycinexylidide (MEGX, 1.36 mg/L) were determined by HPLC-DAD, in a sample collected shortly after administration, which were higher than in comparable cases reported in literature. The reason for the overdose was assumed to be accidental systemic application. Due to suspicion of an improperly performed circumcision and bodily harm, police was involved and a clinical forensic examination was carried out. Here, we present analytical, clinical and forensic aspects of this case.

  • Tension hydrothorax complicating syringopleural shunt for post-traumatic
           syringomyelia

      Abstract: Abstract Syringomyelia is a rare phenomenon that is typically associated with Chiari malformations. However, they can occur in the setting of post-traumatic spinal injury. Potential diversion treatments include syringopleural (SPS), syringoperitoneal and syringosubarachnoid shunts. Short-term complications have been reported in the literature, however, long term complications are not well documented. This case report is of a 43-year-old woman found deceased in bed. She had a history of a traumatic spinal injury following a motor vehicle accident 15 years prior to death. This was complicated by a syringomyelia/syrinx requiring a SPS insertion. Post-mortem imaging and autopsy findings demonstrated a large right tension hydrothorax with dense fibrosis of the pleural surfaces, contralateral diffuse alveolar damage lung changes histologically and a patent SPS. Her cause of death was registered as “Right tension hydrothorax and diffuse alveolar damage complicating right syringopleural shunt and thoracic syrinx”.

  • Sudden cardiac death due to an obstructive large right atrial myxoma

      Abstract: Abstract Sudden cardiac death is a phenomenon that affects more than 200,000 individuals annually in the United States. Among sudden cardiac deaths, only a fractionally small portion is thought to be attributable to intracardiac neoplasms. Though rarely implicated as the primary cause of death, cardiac myxomas are the most common benign neoplasms of the heart and have been known to cause death due to multifactorial embolic events, fatal arrhythmias, and frank obstructive events. In this case, a healthy, asymptomatic young male was found unresponsive in his residence. Examination of the heart at autopsy revealed dilation of the right atrium and a large, hemorrhagic, irregular, and obstructing mass arising from the right atrium, consistent with cardiac myxoma. When unusual cardiac findings are discovered at autopsy, a systematic investigation of the heart, including retaining the organ for cardiac pathology consultation, is mandatory to correctly identify the cause of death.
