- Post-COVID eosinophilia – A report on its Ayurvedic
management Authors: Manjiri Jayprakash Walinjkar, Mandip Goyal Pages: 53 - 57 Abstract: Manjiri Jayprakash Walinjkar, Mandip Goyal Journal of Ayurveda Case Reports 2022 5(2):53-57 Eosinophilia is one of the common laboratory findings of Post-COVID syndrome with lingering symptoms such as cough, dyspnea, chest discomfort, and wheezing. This can be correlated with Vataja kasa by Ayurvedic point of view. Conventional management can control raised eosinophilia and provide symptomatic relief, but relapses are often observed. A 40-year-old male patient has been diagnosed with Post-COVID eosinophilia based on his presenting complaints. He was given Chatushashti prahari pippali as an intervention. After completion of eight weeks of the treatment, the patient got complete symptomatic relief and no symptoms relapsed during the follow-up period of eight weeks and patient's increased eosinophilic count turned down to its normal limits. The observations infer that the Chatushashti prahari pippali has significant benefits in Post-COVID eosinophilia. This illustrates that there is a wide scope to explore the variety of imperative medicines present in Ayurvedic pharmacopoeia, which can be used more rationally to treat various Post-COVID complications. - Lichenoid dermatitis treated with Ayurveda therapies: A case report

Eczema can be correlated with Vicharchika (~wet eczema) described in Ayurveda. In acute conditions, this disease responds well to treatment, but in chronic presentations, it is difficult to manage due to its recurrent nature. Panchakarma (~five biopurification therapies) is well-known measure to improve the quality of life and prevention of recurrence in chronic diseases. In this case report, a 27-year-old male patient presented with chronic lichenoid dermatitis having symptoms of Kandu (~itching), Daha (~burning sensation), Ruja (~pain), Pidaka (~papules/eruptions), Srava (~discharge), Vaivarnya (~discoloration), and Rukshata (~dryness) treated with Ayurvedic interventions. Virechana (~therapeutic purgation) along with Ayurvedic oral medicines and wholesome dietary regimen were given for 12 weeks. Encouraging relief in various symptoms was observed after treatment. Rasayana (~rejuvenation) was also given to patient for prevention of recurrence. The patient has shown well tolerance and satisfactory adherence to treatment. No adverse event was reported during the course of therapy. After three months of treatment, disease progression was fully stopped and lesions resolved completely. - Ayurvedic management of Dadru (dermatophytosis): A case report

Dadru is a common clinical condition, which is considered under the broad heading of Kustha (~skin disorders). Dadru is a Pitta-Kapha Dosha predominant disease which is characterized by Raga (~reddish discoloration), Pidaka (~eruptions), Kandu (~itching at the site of the lesion), and Mandala (~elevated circular patches). On the basis of clinical features, it has the closest resemblance with dermatophytosis which is a superficial infection caused by a group of fungi. It is a major health problem having faster spreading in nature. We present here a case of 24-year-old male having lesions on the medial side of the lower left leg and left knee, thus diagnosed with Dadru with chronicity of four years. He was treated with the internal administration of Siddharthaka yoga kwatha and external application of Siddharthaka yoga ointment. The total treatment duration was 28 days. Significant remission in clinical features of Kandu, Raga, Pidika, and Mandala was observed within 28 days of treatment without any complication, which becomes an evidence and base for further research works with a larger sample size. - Customized Ayurveda approaches in modulating brain injury: A case report

Its manifestation, symptoms, and pathogenesis can be understood with Pakshaghata (~cerebrovascular stroke) in Ayurvedic parlance. This can be managed through Ayurveda principles with appropriate Panchakarma procedures and oral medications. A case of 23-year-old male who was diagnosed with sudden severe stroke of totally dependent and severe disability with intracranial acute hematoma and hemorrhagic abnormalities was successfully treated with planned Ayurveda interventions. Significant improvement in the degree of disability was found and the interventions modulated him into partially dependent and moderate disability. The minimal strategic intervention used resulted in the significant recovery in overall quality of life. Consequently, the study validates that the treatment planned with Ayurveda principles along with suitable Panchakarma therapies and oral medications has a potential role in the management of Pakshaghata. - A report on the management of Galaganda (colloid multinodular goiter)
through Ayurveda approaches Authors: Anshu Sharma, Shalini Rai, Anand More Pages: 74 - 79 Abstract: Anshu Sharma, Shalini Rai, Anand More Journal of Ayurveda Case Reports 2022 5(2):74-79 Multinodular goiter (MNG) is one of the most common endocrine disorders affecting the thyroid gland with a higher prevalence in the female population. It may be due to abnormalities of iodine supplies or metabolism or dyshormonogenesis, resulting in nodules of different sizes filled with colloid-rich material. This case report documents the successful management of Galaganda (~colloid MNG [Bethesda Grade 2]) with Ayurvedic oral medicines. A 60-year-old female, with chief complaints of Galashotha (~swelling in the throat), Sankocha (~tightness and tense feeling in the neck), Amlapitta (~hyperacidity), Vibaddhata (~constipation), and Aadhmana (~gas formation) has visited for the Ayurvedic treatment for her condition in September 2018. She was administered Ayurveda treatment based on the principles of Deepana–pachana (~improving the digestion and metabolism), Sroto shodhana (~cleansing of the bowel and circulatory channels), Sanshamana (~pacifying treatment), and Satwavajaya chikitsa (~patient counseling). Based on the condition, Amarasundari vati, Vridhivadhika vati, Kanchanara kashayam and a herbo-mineral compound in capsule form along with a regulated diet and lifestyle were prescribed for the initial three months. The patient showed improvement in subjective and objective parameters with improvement in quality of life. However, since recurrences are commonly reported in MNG, the medicines were continued for a period of 12 months. No side effects have been observed during the therapeutic period. The patient was followed up for another 12 months and no recurrences have been noted even after stopping the treatment. Agadayogas: A case report Authors: Anjali Nandakumar Pages: 80 - 83 Abstract: Anjali Nandakumar Journal of Ayurveda Case Reports 2022 5(2):80-83 Bhallataka (Semecarpus anacardium Linn.) is a drug mentioned under Upavisha varga belonging to Anacardiaceae family. It is known to produce potential and sometimes life-threatening anaphylaxis. The utility of Agada yogas in cases of hypersensitive allergic responses due to contact, particularly urushiol-induced contact dermatitis, is magnified in this case report. A 23-year-old male patient with clinical presentation of blisters associated with burning sensation and itching over both the hands with a history of contact with marking nut tree is presented here. The case was treated with Ayurvedic approaches and formulations such as Vibhitaki twak (bark of Terminalia bellirica Linn.), Kottamthagaradi agada, Bilwadi agada, Punarnavasava, and Avipattikara churna, which gave a successful and satisfactory result within a short span of time. - A report of a non-healing burn wound managed through Ayurveda

Burn wounds are common during household chores. Normally, the partial-thickness burn does not take more than two to three weeks to re-epithelialize. If it does not, then the cause may be that the wounds have become deeper over time. A 62-year-old female, having Kapha vata prakriti, with complaints of non-healing burn wound with severe pain and visible purulent discharge visited to Ayurvedic hospital. On local examination, an unhealed wound was noticed on the posterolateral side of the right leg above the ankle joint. The wound was covered with debris associated with discharge, foul smell, and raised local temperature, which confirmed the presence of secondary infection. Before visiting the Ayurvedic hospital, the patient had twice completed the course of antibiotics and had been taking analgesics regularly. Although some relief was found, some part of the wound was not healed completely and was associated with severe pain. The patient's caregiver brought her to the outpatient department of Ayurvedic medicine for further management in hope. The patient was treated with internal and external medicines where significant wound healing was achieved in a span of 28 days. This case report sets an example of the benefits of Ayurvedic medication in chronic case of wounds when there are no other options available or fail to respond. - Effect of Nitya virechana before intrauterine insemination: A case report

When infertility evaluation failed to detect any cause of infertility, it is called “Unexplained Infertility.” Intra-uterine insemination (IUI) is a basic treatment option in such unexplained cases. In Ayurvedic classics, Ritu (~menstruation), Kshetra (~uterus), Ambu (~nutrition), and Beeja (~ovum and/or sperm) are described as essential factors for conception. Adequate physiological functioning of these factors is also necessary with the absence of causative factors to achieve conception. Along with that normal psychology is also described as an important aspect for conception. A 35-year-old female presented to Stree Roga Evum Prasuti Tantra OPD, on December 2019 with chief complaints of long-standing unexplained infertility. The patient was advised IUI along with Ayurvedic management. Vata is a principle Dosha in Vandhyatva (~infertility) and Mridu sanshodhana (~mild purification) described as a line of treatment to treat vitiated Vata dosha. Virechana is also maintained for “Agarbha” Stree (~infertile woman). Hence, the patient was treated with Nitya virechana (~daily therapeutic purgation) by Eranda sneha (~castor oil). Eranda sneha maintains Agni (~digestive power) and normalizes Vata dosha by Vata anulomana karma (~carminatives). It also affects the psychological status of the patient by Medhya (~intellect power) and Smritikara (~improving memory) properties. This concept was followed in this case to manage infertility. After the second cycle of IUI, urine pregnancy was achieved in April 2020, and delivered a healthy male baby in November 2020.
