      Introduction: Ayurveda is one of the most renowned Indian traditional health-care systems and has been adopted globally in recent decades. In Ayurveda Kanchanara guggulu (KG) tablet is one of the Indian traditional medicines used in the treatment of various indications such as tonsilitis, cyst, abdominal lump, and fistula-in-ano. Methods: In the present study, KG tablet was prepared and standardized on basis of physical, physico-chemical properties, high-performance thin-layer chromatography (HPTLC) fingerprint, and phytochemical analysis. Heavy metal test of lead, arsenic, mercury, and cadmium was carried out to confirm the permissible limit as per the AYUSH and WHO standards. The antibacterial activity of the KG tablet was evaluated on two human pathogenic bacteria Escherichia coli and Staphylococcus aureus. Results: The heavy metal limit was found below the limit of quantification and within the allowable limit as per the AYUSH and WHO standards. The tablet showed comparable antimicrobial activity against both strains. The tablets were found to be having satisfactory quality parameters such as hardness, disintegration test, and friability values as per the pharmacopeia standards. Conclusion: The tested parameters based on the present study can be used as a reference approach for quality control of KG formulation. The HPTLC study was found to be an effective tool for quality control of herbal formulation to check the inter-batch batch consistency of the formulation. The antimicrobial activity against human pathogens substantiates its traditional uses.

      Introduction: According to the WHO, alcoholism is the third leading cause of disease burden. Harmful use of alcohol results in 2.5 million deaths per year. Years of alcohol abuse harm the liver, causing alcohol liver disease. Patoladi Gana Kashaya, a polyherbal Ayurveda formulation, alleviates the adverse effects of alcohol as well as liver disorders caused by it. Aims and Objectives: To study the effectiveness of Patoladi gana kashaya on Madya janya yakrit vikara (alcohol liver disease). Methods: The present study implemented an open-label, single-arm before and after a study conducted at SDM College of Ayurveda Hospital, Hassan with 30 individuals (who fulfilled the inclusion and diagnostic criteria). A convenience sampling method was used for the selection. The trial drug Patoladi Gana Kashaya was given in the dose of 30 ml thrice daily after food with lukewarm water for 30 days. A predefined symptom scale and laboratory investigation were used to measure the efficacy before and after the treatment. Statistical analysis of subjective parameters was assessed using Cochran's Q and McNemar test and objective parameters by t-test. Results: The trial drug showed statistical significance in liver function test and lipid profile. In subjective parameters, statistical significance was obtained in Mandagni, Jwara, Chardi, Udarashoola, Baddhashakrith, and Peetanetrashakruthmootrata (P < 0.05). Conclusion: The polyherbal formulation (Patoladi Gana Kashaya-30 ml t.i.d. after food with 30 ml of warm water) is effective in the management of Madya Janya Yakrit Vikara (alcohol liver disease) and requires further investigations using a more vigorous method.

      Introduction: Obesity is a complex disease with serious social and psychological dimensions, affecting almost all ages and socioeconomic groups and overwhelmingly developed and developing countries. The objective of this study is to compare the outcome of specific diet, yogic practices, and Ruksha Udvartana in the management of obesity (Sthaulya). Methods: A total of 75 obesity patients who met the proposed selection criteria were enrolled and randomly divided into three groups. After the prescribed interventions for total 3 months, changes in different subjective and objective parameters were assessed at three different follow-ups. Results: Results of the study showed that Udarparsvavrddhi (increased abdominal girth) was the most common symptom, 100% (n = 75) seen in patients, followed by Svedadhikya (Excessive sweating) 90.66% (n = 68) and Utsahahani (Lack of enthusiasm) 85.33% (n = 64). At 3rd follow-up, statistically significant results were seen in most of the subjective parameters. On intragroup comparison of objective parameters, statistically highly significant results were found in all three groups. Conclusion: This study reveals the beneficial effects of specific diet, yoga practices, and Ruksha Udvartana in controlling obesity (Sthaulya). These scientific evidence regarding indulgent mode of action of diet, yoga, and Ruksha Udvartana would help in worldwide acceptance of these cost-effective interventions in the management of obesity.

      Introduction: Amavatari Rasa (AR) (mercury-containing herbo-mineral formulation), widely used in the management of Amavata (rheumatoid arthritis) has been being manufactured by many pharmaceutical companies in India for long. However, till today, the standard manufacturing process (SMP) for this formulation is not available, and hence, the present study was carried out to set the SMP of AR along with its quality control parameters. Methods: Pharmaceutical procedures such as Shodhana (pre-processing) of Parada (mercury), Gandhaka (sulfur), Chitraka (Plambago zeylanica Linn.), and Guggulu (Commiphora wightii Arn.); preparation of lime water, Triphla Kwatha (decoction) and AR were carried out at Rasa shastra and Bhaishajya Kalpana Department of ITRA, Jamnagar. Physicochemical parameters, high-performance thin-layer chromatography (HPTLC), and inductively coupled plasma atomic emission spectroscopy (ICP-AES) were carried out at the pharmaceutical chemistry Lab, ITRA, Jamnagar and Sophisticated Analytical Instrument Facility, IIT Powai, Mumbai respectively. Results: Average of 54.33 g of castor oil was required for 354.33g of AR. Mean ± standard deviation (SD) of pH, loss on drying, ash value, acid insoluble ash, water soluble extractive, and alcohol soluble extractive of AR was 3.77 ± 0.20, 6.29 ± 0.41%w/w, 5.46 ± 0.72%w/w, 0.40 ± 0.15%w/w, 23.2 ± 5.15%w/w and 30.13 ± 7.19%w/w, respectively. HPTLC study showed 4 peaks in 254 nm and 5 peaks in 366 nm ultraviolet lights. AR contains 7.38% mercury, 0.0031% arsenic, and 0.0032% lead. Conclusion: The adopted method for preparation of AR was convenient to replicate and hence can be considered SMP. Results of physicochemical parameters, HPTLC, and ICP-AES of this study could be used as quality parameters for Amavatari Rasa prepared by this method.

      Introduction: Wheat, a nutrient-dense cereal grass, has been eaten since the dawn of humanity. Various Ayurvedic literatures described its properties. They prescribed it for diabetes (Madhumeha), obesity (Sthaulya), hyperlipidemia (Medhaj and Kaphaj Rogas), and other diseases. Food preparation (Samskara) is highly valued in the Ancient System of Indian Medicine. It involves feeding ruminants (cows, horses, etc.) wheat and collecting the grains from their feces. It is believed that when grains pass through these ruminants' guts, they acquire beneficial properties that can be used to treat diseases such as diabetes. The current study involves feeding wheat grains (WGs) to cows and comparing the grains recovered from their excrement to conventional wheat that has not been subjected to this technique. Methods: Cows were fed with WGs. The grains were collected from cow dung. These grains were washed properly and shade dried. These grains were then subsequently tested for phytochemical analysis to compare the properties between unfed normal wheat and cow fed wheat. Results: Sugar levels in the cow fed partially digested wheat are below the detectable levels, and even the carbohydrates level is low as compared to normal unfed wheat. Conclusion: Sugar levels in the cow fed partially digested wheat are below the detectable levels, and even carbohydrates level is low while as compared to normal unfed wheat. Results synchronize to what has been mentioned in the Ancient Indian System of Medicine. This might be why it is advised for the management of Madhumeha diabetes mellitus.

      Introduction: Ayurvedic formulations have a long history of use and have increased amazingly to follow the global interest in natural products. However, for many classical ayurvedic formulations, no quality parameters of standardization are available. Vyoshadi Churna, a classical Ayurvedic polyherbal formulation that cures indigestion and diarrhea, is being used since ancient times. However, there is no standard method available for its formulation and evaluation. Hence, the current study aims to develop the quality standards for Vyoshadi churna. Methods: The standard procedures for preparation and standardization of Churna described in Ayurvedic Pharmacopoeia of India (API) were used in the present study. Standardization includes pharmacognostical, chromatographic, and physico-chemical studies. The other essential studies were also done in accordance to API and World Health Organization guidelines to find the limits of microbial load, aflatoxin, heavy metals, and pesticide residue using modern analytical techniques such as gas chromatography-mass spectroscopy (MS) and inductively coupled plasma-MS/MS. Results: Vyoshadi Churna is of brown color and bitter in taste. Pharmacognostical studies have shown the characteristic features of almost each contributory crude drug. A unique chemical profile was also developed using toluene: ethyl acetate: methanol: formic acid (4:4:1:0.5) as the mobile phase to resolve maximum components. Furthermore, the standardized limits for the physicochemical studies, microbial load, heavy metals, aflatoxins, and pesticide residues were established and found in limits. Conclusion: This is for the first time when the standard operating procedure of Vyoshadi Churna was developed. Hence, the current technical work would be practically helpful for the future preparation and standardization study of Vyoshadi Churna.

      Objective: In the absence of vaccines and effective pharmaceutical interventions, an economic way to deal with such a prolonged epidemic of COVID-19 is the need of the hour. Fever is a primary response of the immune system to infections, and it resembles the disease termed jwara. Ayurveda describes immunity through bala (strength) and its preservation through the nonpharmacological protocol in the first phase of jwara. The present study evaluated the physiological action of each component of the jwara management protocol. It aims to appraise an economic, safe, and efficient way to regulate immunity in fever from the perspective of bala. Review Methods: Classical Ayurvedic texts were searched for the reference of jwara and related aspects. Data Source: Physiology of fever, jwara, and assessment of the impacts and detriments of each component of the treatment protocol was explored via electronic search. Contemporary studies appraising the physiological evidence and the therapeutic and immunological significance of the jwara management protocol are selected. Results: Niruha basti and anjana, the nonconventional routes of medications and the nonpharmacological jwara management approaches such as langhana, bitter taste diet, and dhoopana, directly impact the immune system. Bala or immunity-regulating actions of jwara management can be categorized as those that can decrease the metabolic rate for the proper functioning of the immune system, those that provide nutrition to regulate innate immune responses, and those approaches that alleviate damage due to inflammatory responses. Conclusion: Jwara management protocol may be an economic way to increase the chances of self-recovery by homogenizing the immunity in infections with fever.

      Objective: The primary objective is to elaborate the various concepts concerning genetics and epigenetics in Ayurveda. Data Source: The classical Ayurveda texts, along with the commentaries and peer-reviewed articles, were referred. The classical information was further analyzed and concluded. Review Method: Systematic review after studying changes in phenotypes with a view-through epigenetics will be a scientific contribution to generate interest among medical fraternity for better public health contribution. Results: The four major factors influencing the phenotypes are lifestyle and behavior, diet and digestion, stress and the environment we live in. It is not only genes that are the cause of numerous disorders our children are facing but, also the surrounding (both living and nonliving) that lays an impact, thus changing the “gene expression,” thus changing a whole lot of traits. Conclusions: Genes are affected by several factors and processes including development in utero and childhood, environmental chemicals, drugs, and pharmaceuticals, aging, and diet thus inducing epigenetic changes through developmental plasticity which may land up into diseases with transgenerational inheritance. This comprehension will lead to better integration with the current medical system in managing optimal health of the public sector.

      Objectives: Ajirna manjari is a discrete reference book on indigestion authored by Sri Dattarama on multitudinous facets of indigestion. This Samskrita language based text authored in earlier part of nineteenth century. As it is a short reference book it contains only 105 verses and there are no chapter divisions. To review the important concepts and advancement of Ajirna manjari; To perform literary analysis on Ajirna manjari and Ajirnamrita manjari text books. Data Source: All printed version of Ajirna manjari and Ajirnamrita manjari with Hindi and English translations were procured for this literature study. Review Methods: Initially literary search has been done verse by verse to find out the basic structure, details of author, time of inscription, language usage and important concepts narrated in Ajirna manjari. Further advancement of Ajirna manjari is explored by considering six such similar works. As it has close connection with Ajirnamrita manjari, their relationship is tried to establish by clearing dubieties. Results: Crux of the text is, single drug and varied number of counter food articles and 35 formulations for the treatment of indigestion. Rigorous quest disclosed more than 35 genre of Ajirna manjari and related manuscripts are found across India and overseas. Current text underwent six adaptations in due course of time. Among these, Ajirna manjari is having close connection Ajirnamrita manjari. Conclusion: Ajirna manjari is an exceptional and exclusive reference book on indigestion available in Ayurveda. Six fold adaptations of Ajirna manjari suggests that it is a updated version of Ajirnamrita manjari text.

  • A critical analysis of Madanpal Nighantu &#8211; An ancient lexicon
           of Ayurvedic literature

      Since ancient times, there is a tradition of composition of lexicons in Ayurveda, which are called Nighantu. The Ayurvedic physicians who do not know the names of drugs, they become suspicious and do not recognize the medicine even after seeing. We gain this knowledge from Nighantu, so the study of Nighantu is essential for Ayurvedic physicians. In the tradition of Ayurvedic lexicons, Madanvinod/Madanpal Nighantu is a sophisticated treatise of Dravya-guna. It is composed by Nrip Madanpal in 1374 A. D. This lexicon has thirteen varga (sections) and at the end Granthakartriprashasti, 14th section is mentioned. In this treatise, along with ancient medicinal substances, new substances like Ahifen are also described. Objective: The objective is to study and analyze the importance and utility of Madanpal Nighantu, in the illumination of the knowledge of handwritten manuscripts. Data Source: Preprinted versions and handwritten manuscripts of Madanpal Nighantu, contemporary journals, articles, internet material, and previous research papers related to these subjects have been used. Review Methods: All the published and unpublished literature related to Madanpal Nighantu is reviewed by comparing and analyzing the different meanings and thoughts of classical as well as contemporary authors. A separate analysis of commentaries on classical texts is done for better understanding. Result and Conclusion: This article highlights the utility and importance of handwritten manuscripts for the sophistication of Madanpal Nighantu. By which the identification, properties and actions of drugs and medical treatment can be made more effective and powerful.

  • Management of yovanpidaka by vaman karma

      Introduction: Yovanpidaka, also known as Mukhadushika, is described under Kshudra Rogas in Ayurveda Samhitas. It can be compared with acne vulgaris. It is self-limiting in the majority but severe forms, however, lead to disfiguring scars, which can be prevented. Hence, it is essential to look out for treatment modalities which would help prevent the disfigurement caused by acne. Main Clinical Finding: A 20-year-old female presented with eruptions, pain, closed comedones, whiteheads, itching, and redness over her face for the past 2 years. Diagnosis: Yovanpidaka. Interventions: Administered Vaman with Madanphal Yoga along with milk and Yashtimadhu Phanta after proper internal oleation with Panchatikta Ghruta. Outcome and Conclusions: Marked improvement after Vaman in the signs and symptoms of the patient. Improvement in complexion was also seen.Significant result was seen in Yovanpidaka after Vaman. In cases where local treatments prove to be futile, Shodhan Chikitsa can deliver wonderful results.
