  • Relación de la ingestión de vitamina D y la exposición solar con los
           niveles séricos de 25-hidroxivitamina-D en escolares con obesidad

    • Abstract: Abstract: Introduction. The obesity worldwide has produced an increase in obesity-related diseases and can be associated with low concentrations of 25-hydroxyvitamin-D. Also obesity and low physical activity can decrease sun exposure, so the aim was to correlate vitamin D intake with serum 25-hydroxyvitamin-D levels and to assess sun exposure habits in schoolchildren with obesity. Materials and methods. A correlational study was performed from January 2017 to January 2018 on 103 children between 6-12 years of age, with a body mass index ≥+2SD for age and sex, according to the World Health Organization. Blood samples were taken to determine the serum concentrations of 25-hydroxyvitamin-D, a nutritional survey to determine the vitamin D intake and a sun exposure questionnaire were applied. A Spearman correlation coefficient analysis was performed. Results. Forty-seven percent of the children were girls. The median years of age was 10. The median serum 25-hydroxyvitamin-D levels were 35.5 ng/mL, 74.8% had sufficient levels, 25.2% had insufficient levels. The median vitamin D intake was 214.7IU in boys and 231.9IU in girls. Regarding sun exposure, most of the children had excessive levels of sun exposure and inadequate sun protection practices. A positive correlation between vitamin D intake and serum 25-hydroxyvitamin-D was identified only in boys (rho=0.276, p=0.041). Conclusion. A positive correlation between vitamin D intake and serum 25-hydroxyvitamin-D levels was found in obese boys and excessive levels of sun exposure with inadequate sun protection practices in boys and girls.Resumen: Introducción: La obesidad se le ha asociado con distintas comorbilidades, bajas concentraciones séricas de 25-hidroxivitamina-D, sedentarismo que a su vez podría comprometer la exposición solar; por tanto, el objetivo fue relacionar la ingesta de vitamina D con los niveles séricos de 25-hidroxivitamina-D y determinar los hábitos de exposición solar en escolares con obesidad. Materiales y métodos. Estudio correlacional realizado de enero 2017 a enero 2018, en 103 niños entre 6 y 12 años, con un índice de masa corporal ≥+2DE para edad y sexo, según la Organización Mundial de la Salud. Se extrajo muestras sanguíneas para determinar las concentraciones séricas de 25-hidroxivitamina-D, se aplicó una encuesta nutricional para determinar la ingesta de vitamina D y un cuestionario de exposición solar. Se realizó un análisis del coeficiente de correlación de Spearman. Resultados. El 47% de los sujetos eran niñas. La mediana de edad fue de 10. La mediana de los niveles séricos de 25-hidroxivitamina-D fue de 35,5 ng/mL, el 74,8% tenía niveles suficientes, el 25,2% tenía niveles insuficientes. La mediana de la ingesta de vitamina D fue de 214,7UI en niños y de 231,9UI en niñas. Con respecto a la exposición solar, la mayoría de los niños presentaban una exposición excesiva y prácticas inadecuadas de protección solar. Se identificó una correlación positiva entre la ingesta de vitamina D y la 25-hidroxivitamina-D sérica en los niños (rho=0,276, p=0,041). Conclusión. Se identificó una correlación positiva entre la ingesta de vitamina D y los niveles séricos de 25-hidroxivitamina-D en niños obesos y exposición excesiva con prácticas inadecuadas de protección solar en niños y niñas.
       
  • Asociación entre el comportamiento alimentario y la salud auto percibida
           en adultos brasileños

    • Abstract: Abstract: Introduction: Healthy eating habits are related to a lower prevalence of chronic diseases. Objective: Verify the relationship between eating behaviors and self-perceived health (SPH) in young, middle-aged, and older adults. Materials and methods: This is a cross-sectional study with 52166 Brazilian adults aged 18 years and over. The dependent variable was positive SPH, whereas the independent variable was the eating behavior (exchanging lunch for snacks; exchanging dinner for snacks; intake of beans, ultra-processed food the previous day, fruits, vegetables, and processed juices/soft drinks). Poisson regression with robust adjustment for variance was used in the data analysis. Results: The relationship between eating behaviors and SPH was similar between genders. However, a more evident relationship was observed among young adults, regarding the age group. The consumption of fruits and vegetables was most clearly associated with SPH among the investigated behaviors. Furthermore, the higher the number of positive eating behaviors, the higher the prevalence of positive SPH in both genders and all age groups. Conclusion: The findings of this study reinforce the importance of healthy eating behaviors for health and indicate a more direct relationship between these and positive SPH in young adults and as a habit of regularly consuming fruits and vegetables.Resumen: Introducción: Los hábitos alimentarios positivos se relacionan con una menor prevalencia de enfermedades crónicas. Objetivo: Verificar la relación entre las conductas alimentarias y la salud auto percibida (SA) en adultos jóvenes, de mediana edad y ancianos. Materiales y métodos: Se trata de un estudio transversal con 52166 adultos de 18 años o más en Brasil. La variable dependiente fue la SA positiva, mientras que la variable independiente fue la conducta alimentaria (cambio de almuerzo por refrigerio; cambio de cena por un refrigerio, consumo de frijoles, comida ultra procesada, fruta, verdura y zumos/refrescos artificiales el día anterior a la entrevista). En el análisis de datos, se utilizó la regresión de Poisson con ajuste robusto para la varianza. Resultados: La relación entre las conductas alimentarias y la SA fue similar entre los géneros, pero en relación al grupo de edad, se observó una relación más evidente en adultos jóvenes. De los comportamientos investigados, el consumo de frutas y verduras fueron los que se asociaron más claramente con la SA. Cuanto mayor es el número de conductas alimentarias positivas, mayor es la prevalencia de SA positiva, en ambos sexos y en todos los grupos de edad. Conclusión: Los hallazgos de este estudio refuerzan la importancia de un comportamiento alimentario saludable para la mejorar y mantener la salud e indican que, aparentemente, existe una relación más explícita entre éste y la SA positiva en adultos jóvenes, al igual que con el hábito de consumir frutas y verduras con frecuencia.
       
  • Simulación clínica con dramatización, una estrategia de
           enseñanza-aprendizaje para estudiantes de pregrado en nutrición y
           dietética

    • Abstract: Abstract: Introduction: At present, education in Health Sciences requires interaction with real patients, which is made more complex due to the need to ensure their health safety. For this reason, new teaching methodologies are now being implemented, which help to improve and protect safe care. Objective: This study was carried out using a quantitative approach with a non-experimental, descriptive design. Materials and Methods: The sample was non-probabilistic and consisted of 87 undergraduate students. A questionnaire was used, with 18 statements divided into 3 dimensions: dimension 1, simulation structure, with six questions reflecting aspects related to its implementation; dimension 2, learning, with seven questions related to aspects of effective communication, trust, learning and respect for diversity; dimension 3, feedback for learning, with five questions, including feedback by the teacher and the simulated patient, as well as their own views with regard to participating in another similar experience again. A five-point Likert scale was used. Results: The three dimensions studied, simulation structure, learning and feedback for learning, report a high level of positive perceptions. Conclusion: The simulation strategy is an educational tool in health-related careers that enhances the clinical competencies of the students as well as the relevant theoretical and practical skills and abilities in their learning process, promoting integration of the knowledge acquired in previous subjects.Resumen: Introducción: En la actualidad, la formación en Ciencias de la Salud requiere de la interacción con pacientes reales, lo que se hace más complejo por la necesidad de velar por la seguridad de su salud. Por ello, ahora se están implementando nuevas metodologías docentes que ayudan a mejorar y proteger la atención segura. Objetivo: Este estudio se llevó a cabo con un enfoque cuantitativo con un diseño no experimental, descriptivo. Materiales y métodos: La muestra fue no probabilística a conveniencia y estuvo conformada por 87 estudiantes de pregrado. Se utilizó un cuestionario, con 18 enunciados divididos en 3 dimensiones: dimensión 1, estructura de simulación, con seis preguntas que reflejan aspectos relacionados con su implementación; la dimensión 2, aprendizaje, con siete preguntas relacionadas con aspectos de comunicación efectiva, confianza, aprendizaje y respeto a la diversidad; dimensión 3, retroalimentación para el aprendizaje, con cinco preguntas, que incluye la retroalimentación del docente y del paciente simulado, así como sus propias opiniones con respecto a participar nuevamente en otra experiencia similar. Se utilizó una escala tipo Likert de cinco puntos. Resultados: Las tres dimensiones estudiadas, estructura de simulación, aprendizaje y retroalimentación para el aprendizaje, reportan un alto nivel de percepciones positivas. Conclusión: La estrategia de simulación es una herramienta educativa en carreras afines a la salud que potencia las competencias clínicas de los estudiantes, así como las destrezas y habilidades teóricas y prácticas pertinentes en su proceso de aprendizaje, favoreciendo la integración de los conocimientos adquiridos en materias anteriores.
       
  • Microbiota intestinal y modulación del tejido adiposo en la
           patogénesis de la obesidad

    • Abstract: Resumen: Las investigaciones realizadas durante el último siglo relacionadas con la descripción de la Microbiota Intestinal (MI) sugieren una relación concreta entre su composición y la salud del huésped. Su desregulación denominada disbiosis intestinal ha sido asociada a distintos tipos de enfermedades gastrointestinales, metabólicas, oncológicas e incluso psiquiátricas. Destacan numerosos reportes que han informado la condición de disbiosis en la obesidad, tanto en modelos animales como humanos de distintos grupos etarios y regiones del mundo. A su vez, la composición del microbioma también ha logrado asociarse a las diferentes comorbilidades de la obesidad, postulando que la MI posee influencia en la disfunción del tejido adiposo (TA), entendiendo que corresponde al principal modulador de la patogénesis de la obesidad. Sin embargo, aún no es posible establecer una explicación mecanicista plausible. Actualmente, la utilización de tecnologías multiómicas, junto con la evaluación de variables fisiológicas, nos podrían proporcionar una mejor comprensión a la incógnita planteada. Frente a esto, el presente trabajo tiene como objetivo revisar los últimos avances en la comprensión de la influencia de la microbiota intestinal en el TA y su contribución a los mecanismos relacionados con la patogénesis de la obesidad. Entre los principales mecanismos identificados, la evidenciareporta nexos fisiológicos entre la composición de la MI y la modulación de inflamación, permeabilidad intestinal y adipogénesis. Las vías implicadas derivan de la influencia de la disbiosis intestinal en el accionar de ácidos grasos de cadena corta, claudinas, macrófagos, oligosacáridos, entre otros. Los mecanismos implicados, principalmente estudiados en modelos animales, deberían ser considerados para su evaluación en próximos estudios longitudinales y experimentales en humanos con el fin de obtener una mayor comprensión sobre la implicancia de cada mecanismo en la patogenia global de la obesidad.Abstract: The investigations carried out during the last century related to the description of the Gut Microbiota (GM) suggest a concrete relationship between its composition and the health of the host. Its deregulation called intestinal dysbiosis has been associated with different types of gastrointestinal, metabolic, oncological and even psychiatric diseases. Numerous reports that have described the condition of dysbiosis in obesity stand out, both in animal and human models of different age groups and regions of the world. In turn, the composition of the microbiome has also been associated with the different comorbidities of obesity, postulating that MI has an influence on adipose tissue (AT) dysfunction, understanding that it corresponds to the main modulator of the pathogenesis of obesity. However, it is not yet possible to establish a plausible mechanistic explanation. Currently, the use of multi-omics technologies, together with the evaluation of physiological variables, could provide us with a better understanding of the question raised. In view of this, this review aims to review the latest advances in understanding the influence of the intestinal microbiota on AT and its contribution to the mechanisms related to the pathogenesis of obesity. Among the main mechanisms identified, the evidence reports physiological links between the composition of GM and the modulation of inflammation, intestinal permeability and adipogenesis. The pathways involved derive from the influence of intestinal dysbiosis on the action of short-chain fatty acids, claudins, macrophages, oligosaccharides, among others. The mechanisms involved, mainly studied in animal models, should be considered for evaluation in future longitudinal and experimental studies in humans in order to obtain a better understanding of the implication of each mechanism in the global pathogenesis of obesity.
       
  • Incorporation of the NOVA classification into scientific production on
           food and nutrition in Latin America: a scientometric review

    • Abstract: Resumo: Introdução. Em 2009, foi proposta uma classificação de alimentos, denominada classificação NOVA. Países da América Latina tem se destacado em seu uso nas recomendações nutricionais e agenda regulatória. Objetivo. Avaliar como a produção científica em alimentação e nutrição na América Latina tem incorporado a classificação NOVA. Materiais e métodos. A análise da produção científica foi realizada a partir de trabalhos apresentados no Congresso Latinoamericano de Nutrição (SLAN) nos anos de 2012, 2015 e 2018. Os termos utilizados para a busca foram: NOVA, ultraprocessado, processado, processamento e guia alimentar, nos idiomas português, inglês e espanhol. Após a busca, foram aplicados os critérios de exclusão e inclusão e os resumos selecionados foram descritos de acordo com variáveis analíticas previamente definidas. Resultados. Foram analisados 153, sendo 24 publicados em 2012, 20 em 2015 e 109 em 2018. A maioria dos estudos foram desenvolvidos no Brasil (56,2%) e no México (12,4%) e envolvia adolescentes (28,8%), adultos (21,6%) e alimentos (19,6%) como sujeito/unidade de análise. A maioria dos trabalhos foi classificada na área de Nutrição em Saúde Pública (88,9%), era de natureza observacional (82,3%) e empregava método quantitativo (76,5%). A venda e/ou consumo de alimentos (46,4%) e o ambiente alimentar (24,2%) foram os objetos de estudo mais frequentes. Conclusão. A produção científica que considera a classificação NOVA na América Latina aumentou em 2018, com Brasil e México liderando o desenvolvimento dos estudos. Estudos que explorem a relação da classificação NOVA com o preço dos alimentos, habilidades culinárias e políticas públicas são oportunidades de pesquisa.: Introduction. In 2009, a food classification was proposed, called NOVA classification. Latin American countries have stood out in their use in nutritional recommendations and regulatory agenda. Objective. To evaluate how scientific production in food and nutrition in Latin America has incorporated the NOVA classification. Materials and methods. The analysis of scientific production was carried out from annals at the Latin American Congress of Nutrition (SLAN) in 2012, 2015 and 2018. The terms used for the search were: NOVA, ultra-processed, processed, processing and food guide, in Portuguese, English and Spanish. After the search, the exclusion and inclusion criteria were applied and the selected abstracts were described according to previously defined analytical variables. Results. A total of 153 were analyzed, 24 of which were published in 2012, 20 in 2015 and 109 in 2018. Most studies were carried out in Brazil (56,2%), followed by Mexico (12,4%) and involved adolescents (28,8%), adults (21,6%) and food (19,6%) as subject or unit of analysis. Most of the works were classified in the area of Public Health Nutrition (88,9%), were observational (82,3%) and used a quantitative method (76,5%). The sale and/or consumption of food (46,4%) and the food environment (24,2%) were the most common objects of study. Conclusion. The scientific production that considers the NOVA classification in Latin America increased in 2018, with Brazil and Mexico leading the development of studies. Studies that explore the relationship of NOVA classification to food price, culinary skills and public policy analysis are research opportunities.
       
  • "Determinantes Sociales de la Salud", “Habitus” y “Embodiment”
           detrás de un IMC elevado. Un análisis social del actual escenario
           epidemiológico

    • Abstract: Resumen: El índice de masa corporal (IMC) elevado marcaría hoy una nueva etapa dentro de la transición epidemiológica, siendo posible develar el origen social del nuevo perfil de morbimortalidad. Esta revisión busca integrar diferentes conceptos teóricos para explicar los mecanismos a través de los cuales lo social se incorpora en el individuo, delimitando su comportamiento en salud y con ello, su estado nutricional. Las estructuras que representan los determinantes sociales de la salud operarían de manera sistemáticamente distinta sobre las personas, generando posiciones más o menos ventajosas dentro del campo de juego sanitario. Este patrón traduce una realidad que se encarna (embodiment) de manera inconsciente y duradera a través de un habitus, favoreciendo prácticas sociales diferenciadas que terminan por construir clases de cuerpos como expresión biológica de la desigualdad. La toma de decisiones estratégicas en salud dependerá de capacidades y libertades individuales primeramente restringidas por aquellos contextos sociales, entendiéndose estos como las causas estructurales de la salud poblacional.Abstract: A high body mass index (BMI) would characterize a new stage in the epidemiological transition, making it possible to reveal the social origin of the new morbidity and mortality burden. This review seeks to integrate different theoretical concepts to explain the mechanisms through which the social is incorporated into the individual, delimiting their health behavior and with it, their nutritional status. The structures that represent the social determinants of health would operate systematically differently on different social groups, generating positions that are more or less advantageous in the playing field of health. This pattern translates a reality embodied unconsciously and long-lasting through a habitus, favoring differing social practices that result in the construction of bodily classes as a biological expression of social inequality. Strategic decision-making in health will depend on individual capacities and freedoms, which are firstly restricted by those social contexts, being those understood as the structural causes of population health.
       
  • Suplementación prenatal con omega 3 y su efecto en complicaciones
           durante el embarazo

    • Abstract: Resumen: Durante los últimos años se ha incentivado la suplementación con omega 3 durante el embarazo principalmente debido a la evidencia que mostraría beneficios en el desarrollo neuronal y visual del hijo en gestación, y a la prevención de patologías obstétricas asociadas a un aumento de la morbi-mortalidad perinatal. Los ácidos grasos poliinsaturados (PUFAs) omega 3, específicamente el ácido eicosapentaenoico (EPA) y el ácido docosahexaenoico (DHA), poseen propiedades antiinflamatorias, vasodilatadoras, además de propiedades anti-agregantes, las cuales han estimulado el uso de PUFAs en la prevención de enfermedades cardiovasculares. En esta revisión detallamos los efectos de la suplementación con omega 3 en diferentes aspectos del embarazo tales como la prevención del parto prematuro, preeclampsia, depresión post-parto y mejora del metabolismo durante la diabetes gestacional. Si bien existen diversos ensayos clínicos randomizados que estudian la suplementación con omega 3 durante la gestación, la evidencia sigue siendo no concluyente, debido a la variabilidad de las dosis y tiempo de administración. Ciertamente, un mayor número de estudios de calidad son necesarios para determinar el real impacto de la suplementación con omega 3 durante la gestación en la prevención de patologías obstétricas.Abstract: During pregnancy, omega 3 supplementation has raised its popularity due to evidence that it would show not only benefits in the neural and visual development of the unborn child, but also in the prevention of obstetrical pathologies associated with of perinatal morbidity and mortality. Omega 3 polyunsaturated fatty acids (PUFAs), specifically, docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), possess anti-inflammatory, vasodilatory and anti-aggregating properties, which have led to the use of PUFAs in the prevention of cardiovascular diseases. In this review, we detail the effects of omega 3 supplementation on different aspects of pregnancy such as prevention of preterm birth, pre-eclampsia, postpartum depression, and improved metabolism during gestational diabetes. Although there are several randomized clinical trials using omega-3 supplementation during pregnancy, the evidence remains inconclusive, due to variability in dosage and administration time. Certainly, a greater number of high-quality studies including randomized clinical trials are necessary to determine the impact of omega 3 supplementation during pregnancy in the prevention of obstetric pathologies.
