  • Hygienic-sanitary conditions of lupine ceviche street food stalls in
           Riobamba-Ecuador and bacteriological evaluation of the product and its
           ingredients

    • Abstract: RESUMEN Los alimentos expendidos en la vía pública constituyen una fuente de ingreso para familias en países en vías de desarrollo, sin embargo, pueden ser fuentes potenciales de enfermedades transmitidas por alimentos. En Riobamba-Ecuador entre los alimentos de mayor consumo en la vía pública está el plato típico “ceviche de chochos (Lupinus mutabilis)”, nutritivo, sensorialmente agradable y con precio accesible. Este estudio evaluó la calidad microbiológica y condiciones sanitarias de 10 puestos de venta. Se realizó el análisis microbiológico a 100 muestras correspondientes a: ceviche e ingredientes por separado (jugo de tomate, chochos desamargados, piel cocida de cerdo y cebollas). Estas muestras se tomaron en dos días diferentes y se cuantificó aerobios mesófilos, enterobacterias y Staphyloccocus aureus. Las condiciones sanitarias de los puestos de venta fueron evaluadas mediante una lista de verificación que incluyó los siguientes parámetros: diseño, ubicación, manipulador, preparación, transporte, comercialización y saneamiento. Los resultados revelan que todas las muestras estaban contaminadas con enterobacterias y S. aureus, 80% presentaban aerobios mesófilos > 6 log10 UFC/g, siendo el ingrediente chocho desamargado, el que aportó mayor carga microbiana en 6 de los 10 puestos de venta. Los resultados de la lista de verificación muestran que las principales deficiencias son: la no utilización de agua potable circulante y la manipulación de manera conjunta de los alimentos y el dinero. En conclusión, existe gran fluctuación del cumplimiento de cada parámetro dentro de las Prácticas Correctas de Higiene por puesto de venta, creando focos o entradas de contaminantes, que se evidencia en los altos recuentos de microorganismos indicadores de calidad sanitaria en el producto final.ABSTRACT Street food constitutes a source of income for several families in the world, mainly in developing countries and emerging economies; however, it can be a potential source of foodborne diseases. In Riobamba-Ecuador, among the most consumed food streets is the typical dish “ceviche de chochos (Lupinus mutabilis) “, a nutritious and pleasant sensory dish at an affordable price. In this study, the microbiological load and sanitary conditions of 10 lupine ceviche stalls were evaluated. Samples of ceviche and its ingredients (tomato juice, debited lupins, pork skin, and onions). These samples (n= 100) were taken on two different days and analysed to quantify mesophilic aerobes, enterobacteria, and S. aureus. The sanitary conditions of the stalls were analysed using a check list consisting of sections focused on design, location, handler, preparation, transportation, marketing and sanitation. The results show that 100% of the samples were contaminated with enterobacteria and S. aureus, 80% of these had mesophilic aerobics > 6 log CFU / g, and the debittered lupin ingredient is the one that provides the highest microbial load in 6 of the 10 stalls evaluated. The results of the check list showed that the main deficiencies are the non-use of circulating drinking water and the joint manipulation of food and money. In conclusion, there is a great fluctuation in the compliance of the Good Hygiene Practises parameters in each street food stall. It causes multiple foci or entrances of contaminants, which is evidenced in the high counts of microorganisms that are indicators of sanitary quality in the final product.
       
  • New formulations of fermented milk drinks with fruit pulp added:
           Physicochemical characteristics during storage and nutritional profile

    • Abstract: ABSTRACT The aim of this study was to evaluate the nutritional profile and the physicochemical characteristics during storage of newly developed formulations of fermented milk drinks with added pineapple, mango and passion fruit pulp. The fermented drinks showed a high content of protein, iron, and calcium. The passion fruit milk drink had the lowest pH (4.13) and highest acidity (0.95%, expressed in % of lactic acid), which was significantly different (p<0.05) from the pineapple and mango drinks. As for syneresis and sedimentation, the pineapple milk drink had the highest rates at 14 days storage, with 34.33% and 6.50%, respectively and was significantly different (p≤0.05) when compared to the mango and passion fruit milk drinks. In conclusion, newly developed fermented milk drinks with added fruit pulp were a source of several nutrients. We observed physical-chemical characteristics suitable for a fermented milk product during storage.RESUMEN El objetivo de este estudio fue el desarrollo de nuevas formulaciones de bebidas lácteas fermentadas adicionadas de piña, mango y maracuyá, para evaluar el perfil nutricional y las características fisicoquímicas durante el almacenamiento. Las bebidas fermentadas mostraron un alto contenido en proteínas, hierro y calcio. En cuanto a las características fisicoquímicas durante el almacenamiento, la bebida láctea de maracuyá presentó el pH más bajo (4,13) y la acidez más alta (0,95%, expresada en % de ácido láctico), con una diferencia significativa (p < 0,05), en comparación con las bebidas de piña y mango. En cuanto a sinéresis y sedimentación, la bebida láctea de piña presentó los mayores índices a los 14 días de almacenamiento, con 34,33% y 6,50%, respectivamente, y con diferencia significativa (p ≤ 0,05) al compararla con las bebidas lácteas de mango y maracuyá. En conclusión, las bebidas lácteas fermentadas con adición de pulpa de fruta son una fuente de varios nutrientes, y de características físico-químicas adecuadas para un producto lácteo fermentado durante el almacenamiento.
       
  • Knowledge of physiological aspects, clinical management, and nutrition
           among patients with type 2 diabetes mellitus

    • Abstract: ABSTRACT Objective: To evaluate the knowledge about physiological aspects, clinical management, and nutrition of primary care patients with type 2 diabetes mellitus by sociodemographic, economic, and clinical variables. Material and methods: We conducted an observational cross-sectional study in 22 Family Health Strategy units in the city of Montes Claros, Minas Gerais, in 2015 among 353 patients registered with type 2 diabetes mellitus. A structured questionnaire was used to assess sociodemographic and clinical characteristics. Knowledge was assessed using the diabetes knowledge scale questionnaire. Data were presented with absolute frequencies and percentages and analyzed using Poisson regression. Results: Most respondents demonstrated satisfactory knowledge in the field of physiology (60.30%) and unsatisfactory knowledge in clinical management (67.7%) and nutrition (61.5%). Schooling was associated with knowledge in the areas of physiology, clinical management and nutrition, and treatment associated with clinical management. Conclusion: Knowledge about type 2 diabetes mellitus was related to sociodemographic characteristics and treatment. The importance of health education for coping with this chronic condition is evident.RESUMEN Objetivo: Evaluar los conocimientos sobre aspectos fisiológicos, manejo clínico y nutrición de usuarios con diabetes mellitus tipo 2 registrados en unidades de Atención Primaria y su análisis según variables sociodemográficas, económicas y clínicas. Métodos: Se trata de un estudio observacional y transversal realizado en 22 unidades de la Estrategia Salud de la Familia de la ciudad de Montes Claros, Minas Gerais, en 2015 con 353 pacientes registrados con diabetes mellitus tipo 2. Se utilizó un cuestionario estructurado para evaluar las características sociodemográficas y clínicas. El conocimiento se evaluó mediante el cuestionario de la escala de conocimiento de la diabetes. Los datos se presentaron en frecuencias absolutas y porcentajes. El análisis de datos utilizó la regresión de Poisson. Resultados: La mayoría de los encuestados demostró conocimientos satisfactorios en el campo de fisiología (60,30%) y conocimientos insatisfactorios en manejo clínico (67,7%) y nutrición (61,5%). La escolaridad se asoció con conocimientos en las áreas de Fisiología, Manejo Clínico y Nutrición e treatment con Manejo Clínico. Conclusión: El conocimiento sobre la diabetes mellitus tipo 2 está relacionado con las características sociodemográficas y el. tratamento. La importancia de la educación en salud para el enfrentamiento de esta condición crónica es evidente.
       
  • Plasma levels of Vitamin D in older adults in Metropolitan Lima, Peru

    • Abstract: RESUMEN Objetivo. Determinar los niveles plasmáticos de vitamina D y su relación con la ocupación, procedencia y los valores de calcio, leucocitos, hemoglobina y plaquetas en adultos mayores de Lima Metropolitana. Método. Estudio observacional descriptivo de corte transversal, participaron 100 adultos mayores de uno u otro sexo de Lima Metropolitana, ellos fueron reclutados durante las estaciones de invierno-primavera del 2022, los niveles de vitamina D se categorizó como suficiente, insuficiente y deficiente, las concentraciones séricas de vitamina D se midieron por radioinmunoensayo. La relación de variables se realizó con el coeficiente de correlación de Spearman y regresión logística. Resultados. La edad media fue 69.6 años, el 71%, fueron del sexo femenino, la concentración media de vitamina D fue 36.56 ng/ml, el 13% tuvieron niveles de deficiente, y el 32% de insuficiente y el 53% suficientes. Según sexo, el 77,7% de los que tuvieron niveles de deficiente/insuficiente fueron mujeres. En el análisis bivariado no hubo correlación entre los valores de vitamina D con calcio, hemoglobina leucocitos y plaquetas, los que procedieron de distritos de menor temperatura tuvieron 2,25 veces más riesgo de tener niveles insuficientes/deficiente de vitamina D. Conclusiones. El 45% de los adultos mayores tuvieron niveles deficientes/insuficiente de vitamina D, siendo más frecuente en las mujeres y de los procedentes de distritos de menor promedio de temperatura.ABSTRACT Objective. To determine the plasmatic levels of vitamin D and its relationship with the occupation, origin and the values of calcium, leukocytes, hemoglobin and platelets in older adults of Metropolitan Lima. Methods Observational descriptive cross-sectional study, 100 older adults of either sex from Metropolitan Lima participated, they were recruited during the winter-spring seasons of 2022, vitamin D levels were categorised as sufficient, insufficient, and deficient, serum vitamin D concentrations were measured by radioimmunoassay. The relationship of variables was established with the Spearman correlation coefficient and logistic regression. Results. The mean age was 69.6 years, 71% were women, the mean vitamin D concentration was 36.56 ng/ml, 13% had deficient levels, 32% were insufficient and 53% sufficient. By sex, 77.7% of those with deficient/insufficient levels were women. In the bivariate analysis, there was no correlation between vitamin D values with calcium, hemoglobin, leukocytes and platelets; those who came from districts with lower temperatures had a 2.25 times greater risk of having insufficient/deficient levels of vitamin D. Conclusions. 45% of older adults had deficient/insufficient levels of vitamin D, more frequent in women and those coming from districts with lower average temperatures.
       
  • Does the Brazilian cash transfer program (bolsa familia): Protect
           nutritional deviations among infants and breastfeeding practices in a
           capital of the Central Region of Brazil'

    • Abstract: ABSTRACT Cash transfer programs are strategies used by countries, intended for impoverished families, which play an essential role in promoting access to public services such as health, education, and social protection. Programs may also promote food and nutrition security. The Brazilian Cash Transfer Program (“Bolsa Familia”) (BFP) aims to alleviate immediate poverty and combat hunger. The aim of this study is to characterize the nutritional and breastfeeding status of children under two years old among both beneficiaries and non-beneficiaries of BFP. Data from the Brazilian Food and Nutritional Surveillance System, available in the primary healthcare service system of Goiania, Brazil, in 2013 were collected. The following variables were evaluated: sex, weight, height/length, age, and breastfeeding status. Data from 4,567 children under 24 months old were assessed, of which 2.72% (n= 124) were BFP beneficiaries. Beneficiaries had a lower odd of receiving breast milk compared to non-beneficiaries (OR= 0.46, 95% CI:0.31; 0.66, p= 0.0001). Regarding nutritional status, 18.14% (n= 790) of children were diagnosed with nutritional deviation, and overweight was the most prevalent (n=352, 8.04%). Beneficiaries presented a lower odd of developing stunting when compared to non-beneficiaries of BFP (OR= 0.44, 95% CI:0.25; 0.77, p= 0.006). Being a BFP beneficiary was a protective factor for the stunting in children under 24 months old in Goiania, Brazil. However, measures to promote and support breastfeeding should be intensified in primary healthcare service, aimed primarily at children in social vulnerability.RESUMEN Los programas de transferencias en efectivo son estrategias utilizadas por los países, destinadas a familias en situación de pobreza y pobreza extrema, que juegan un papel fundamental en la promoción del acceso a los servicios públicos como salud, educación y protección social, además de promover la seguridad alimentaria y nutricional. El Programa Brasileño de Transferencias en Efectivo (“Bolsa Familia”) (BFP) tiene como objetivo aliviar la pobreza inmediata y combatir el hambre. El objetivo de este estudio es caracterizar el estado nutricional y de lactancia de los niños menores de dos años, tanto beneficiarios como no beneficiarios del BFP. Se recogieron datos del Sistema de Vigilancia Alimentaria y Nutricional Brasileña, disponibles en el sistema de atención primaria de salud de Goiânia, Brasil, en 2013. Se evaluaron las siguientes variables, sexo, peso, talla, edad y estado de lactancia. Se evaluaron datos de 4.567 menores de 24 meses, de los cuales el 2.72% (n=124) eran beneficiarios del BFP. Las beneficiarias presentaron menos posibilidades de recibir leche materna en comparación con las no beneficiarias (OR= 0.46, IC95%:0,31; 0,66, p= 0,0001). En cuanto al estado nutricional, el 18,14% (n= 790) de los niños fueron diagnosticado con desviación nutricional, siendo el sobrepeso el más prevalente (n= 352, 8, 04%). Los beneficiarios presentaron menos probabilidad de desarrollar talla baja en comparación con los no beneficiarios del BFP (OR= 0.44, IC95%: 0.25; 0.77, p= 0.006). Ser beneficiario del BFP fue un factor de protección para la baja talla en menores de 24 meses en Goiania, Brasil. Sin embargo, se deben intensificar las medidas de promoción y apoyo a la lactancia materna en los servicios de atención primaria de salud, dirigidas principalmente a los niños en situación de vulnerabilidad social.
       
  • Effect of the Etonogestrel contraceptive sub-dermal implant on body weight
           in reproductive age’s women

    • Abstract: RESUMEN Objetivo: Analizar el efecto sobre el peso corporal de la anticoncepción hormonal continua mediante implante sub-dérmico liberador de etonogestrel (ENG) en mujeres en edad reproductiva atendidas en la consulta de planificación familiar de un hospital venezolano. Métodos: Investigación comparativa, con diseño cuasi experimental, de casos y controles, a etiqueta abierta, y prospectivo; con una muestra intencionada de 60 mujeres separadas para recibir bien sea un implante subdérmico (Implanon NXT®; casos) o un dispositivo intrauterino (DIU) de cobre (T de cobre; controles). Se evaluaron el peso corporal e índice de masa corporal antes y posterior de doce meses del uso del contraceptivo; así como las características demográficas de las usuarias, efectos adversos y efectividad anticonceptiva de cada método. Resultados: Luego de un año con el implante de ENG no se encontraron variaciones significativas respecto a las mediciones iniciales del peso corporal (61,21±8,30 vs. 61,23±9,50, p>0,5) e IMC (25,23±3.89 vs. 25,26 ±4,30; p>0,05); contrariamente, a lo observado entre las usuarias del DIU donde tanto el peso corporal como el IMC tuvieron un aumento significativo (P<0,05). Asimismo, la mayoría de las usuarias se mantuvieron en el mismo rango de peso donde se encontraban al iniciar el método (p<0,001); mientras que la ganancia ponderal fue mayor entre las usuarias del DIU (1,530±2,04 vs. 3,700±3,02; p<0,05). Conclusiones: El implante de ENG no produce aumento del peso corporal luego de 12 meses de uso, con mínimos efectos adversos y alta efectividad contraceptiva.ABSTRACT Aim: To analyse the effect on body weight of continuous hormonal contraception by releasing subdermal implant etonogestrel (ENG) in women of reproductive age treated in the family planning consultation of a Venezuelan hospital. Methods: Comparative and applied research, with quasiexperimental, case-control, open label and prospective design, with an intentional sample of 60 women separated to receive the contraceptive implant (Implanon NXT®; cases) or a cooper intrauterine device (IUD) (Cooper T; controls) was carried out. Body weight and body mass index were evaluated before and after 12 months of contraceptive use; as well as demographic characteristics of users, side effects, and contraceptive effectiveness of each method. Results: After one year with the ENG implant, no significant variations were found with respect to initial measurements of body weight (61.21±8.30 vs. 61.23±9.50, p>0.5) and BMI (25.23±3.89 vs 25.26±4.30; p>0.05); on the contrary, to what was observed among IUD users where both body weight and BMI had a significant increase (P<0.05). Likewise, most users remained in the same weight range as when starting the method (p<0.001); while weight gain was greater among IUD users (1,530±2.04 vs. 3,700±3.02; p<0.05). Conclusions: The ENG implant does not produce an increase in body weight after 12 months of use, with minimal adverse effects and high contraceptive efficacy.
       
  • Nutritional knowledge questionnaire applied in sport: Initial proposal for
           the instrument

    • Abstract: RESUMEN El conocimiento nutricional es clave para la formación de hábitos alimentarios y en la modulación del comportamiento alimentario. La alimentación y nutrición son cruciales durante la adolescencia sobre todo en jóvenes deportistas. El objetivo de esta investigación fue analizar la validez teórica y los índices de discriminación y dificultad del cuestionario de conocimientos nutricionales aplicados en el deporte utilizando la teoría de respuesta al ítem en deportistas juveniles de 14 a 19 años. Este estudio instrumental se desarrolló en cinco fases: generación de ítems, revisión por panel de expertos, pilotaje, aplicación y depuración final. Participaron 261 adolescentes de 14 a 19 años e integrantes de disciplinas colectivas en selecciones Nacional y clubes profesionales. Se determinaron los índices de dificultad y de discriminación para cada uno de los ítems. Los ítems 1 (0,354) y 2 (0,285) del factor antes, 3 (0,315) y 6 (0,408) del factor durante, 7 (0,354) y 10 (0,408) del factor después, 3 (0,254) y 9 (0,231) del factor hidratación y, 2 (0,362) y 5 (0,323) del factor suplementación, fueron los que presentaron mayor grado de discriminación. El cuestionario de conocimientos nutricionales aplicados en el deporte está compuesto por 30 ítems divididos en cinco factores cada uno de ellos con seis ítems. La propuesta inicial del cuestionario es una herramienta valida teóricamente y con índices de dificultad y discriminación adecuados para evaluar el conocimiento nutricional en jóvenes deportistas. Su aplicación permite conocer potenciales brechas en conocimientos claves y diseñar estrategias educativas específicas para subsanarlas.ABSTRACT Nutritional knowledge plays a fundamental role in the formation of dietary habits and the modulation of eating behaviour. Nutrition and diet are crucial during adolescence, especially for young athletes. The purpose of this research was to analyse the theoretical validity and indices of discrimination and difficulty of the questionnaire on nutritional knowledge applied in sports using item response theory in youth athletes aged 14 to 19. This instrumental study was conducted in five phases: item generation, expert panel review, pilot testing, application, and final refinement. A total of 261 adolescents aged 14 to 19, who were members of national team and collective disciplines of professional clubs, participated. Difficulty and discrimination indices were determined for each item. Items 1 (0.354) and 2 (0.285) of the before factor, 3 (0.315) and 6 (0.408) of the during factor, 7 (0.354) and 10 (0.408) of the post factor, 3 (0.254) and 9 (0.231) of the hydration factor, and 2 (0.362) and 5 (0.323) of the supplementation factor showed the highest degree of discrimination. The questionnaire on nutritional knowledge applied in sports consists of 30 items divided into five factors, each with six items. The initial questionnaire proposal is a theoretically valid tool and with adequate difficulty and discrimination indices to evaluate nutritional knowledge in young athletes. Its application allows for identifying potential gaps in key knowledge and designing specific educational strategies to address them.
       
  • Nutritional, techno-functional properties, and food application of mung
           bean (Vigna radiata)

    • Abstract: RESUMEN El frijol mungo (Vigna radiata) es una leguminosa ampliamente producida y consumida en países asiáticos. Esta leguminosa gradualmente ha ido tomando importancia dentro de la gastronomía de occidente, tanto por su valor nutricional como por sus propiedades biológicas y tecnológicas. Dentro de sus propiedades nutricionales se destaca el contenido de proteínas, carbohidratos, fibra y compuestos fenólicos. Las semillas de frijol mungo con un adecuado tratamiento, ya sea de germinación, fermentación o aislamiento, ha demostrado tener propiedades biológicas como la antioxidante, antidiabética, antihipertensiva, antiinflamatoria y anticancerígena. Por otro lado, dentro de las propiedades tecnológicas podemos destacar las propiedades emulsificante, espumante, gelificante, absorción de aceite y de agua. Todas estas propiedades mencionadas hacen que el frijol mungo sea un ingrediente de interés para la industria de alimentos, por lo cual, se hace necesario realizar una revisión de los estudios recientes acerca de los atributos nutricionales, tecno-funcionales y aplicaciones en el área de alimentos.ABSTRACT The mung bean (Vigna radiata) is a legume widely produced and consumed in Asian countries. This legume has gradually gained importance in western gastronomy for its nutritional value and biological and technological properties. Among its nutritional properties, the content of protein, carbohydrates, fibre, and phenolic compounds stands out. With proper treatment, whether it is germination, fermentation or isolation, mung beans have been shown to have biological properties such as antioxidant, antidiabetic, antihypertensive, anti-inflammatory, and anticancer. However, we can highlight the properties of emulsifying, foaming, geling, oil, and water absorption within the technological properties. All these properties make the mung bean an ingredient of interest for the food industry, for which it is necessary to review recent studies on the nutritional, techno-functional attributes and applications in the food area.
       
  • Diet therapy in rheumatoid arthritis: A review of the evidence

    • Abstract: RESUMEN La artritis reumatoide (RA), es una enfermedad crónica reumática, caracterizada por un daño articular progresivo y manifestaciones extraarticulares, que pueden desencadenar una discapacidad. La etiología de las patologías autoinmunes, incluyendo la RA, es compleja, abarcando factores de tipo genético, hormonal, dietarios y ambientales. Sin embargo, los mecanismos fisiológicos por los que estos agentes contribuyen a la patogenia aún no están definidos completamente. Estudios sugieren un impacto de la disbiosis del microbioma intestinal en la patogénesis de la artritis reumatoide sugiriendo también la alteración de la permeabilidad intestinal como una posible causa. Esta se puede relacionar con patrones dietarios influyentes en la prevención de esta patología así como una propuesta de tratamiento complementario al manejo farmacológico tradicional, considerando el riesgo aumentado de estos pacientes de desarrollar síndrome metabólico y “caquexia reumatoide”, una acumulación de tejido adiposo acompañado de una disminución de tejido muscular debido a los cambios catabólicos debido a la inflamación crónica e inactividad física a consecuencia de las dificultades motoras. Ante evidencia reciente que propone patrones de alimentación e intervenciones dietarias como tratamiento complementario a la terapia farmacológica en el manejo de la RA, la siguiente revisión narrativa cualitativa se propone revisar la efectividad de intervenciones propuestas en términos de dolor y actividad de enfermedad, encontrando dentro de la amplia heterogeneidad de intervenciones propuestas, aquellos patrones dietéticos con efecto modulador de la microbiota intestinal como la dieta mediterránea y sus variaciones en contexto de la promoción de un estilo de vida serían positivas como terapia complementaria a esta patología.ABSTRACT Rheumatoid arthritis (RA) is a chronic rheumatic disease characterised by progressive joint damage and extra-articular manifestations that can lead to disability. The aetiology of autoimmune diseases, including RA, is complex, involving genetic, hormonal, dietary, and environmental factors. However, the physiological mechanisms by which these agents contribute to pathogenesis have not yet been fully defined. More recent studies suggest an impact of gut microbiome dysbiosis on the pathogenesis of autoimmune diseases in both animal and human models, including rheumatoid arthritis, also suggesting altered intestinal permeability as a possible cause. This modification of the gut microbiome may be related to influential diet patterns in the prevention of this pathology, as well as a proposed complementary treatment to traditional pharmacological treatment, also considering the increased risk for these patients of developing metabolic syndrome and “rheumatoid cachexia”, an accumulation of adipose tissue accompanied by a decrease in muscle tissue due to catabolic changes due to chronic inflammation and physical inactivity as a consequence of motor difficulties in RA. With the emergence of recent evidence proposing dietary interventions and eating patterns as adjunctive treatment to drug therapy in the management of RA, the following narrative qualitative review proposes reviewing recent evidence on the effectiveness of various proposed dietary interventions in the treatment of rheumatoid arthritis in terms of pain and disease activity, finding between a high heterogeneity of proposed interventions, those dietary patterns with modulating effect on the gut microbiome as Mediterranean diet and its variations in the context of promoting a healthy lifestyle would be positive as a complementary therapy for this disease.
