  • Clinical and criminal profile of internet offenders with ASD

      Abstract: Autism-specific characteristics have been associated with internet criminal activities. Internet and non-internet offenders differ on a series of demographic, psychological and offending variables. However, the clinical and criminal presentation of individuals with autism spectrum disorder (ASD) in forensic secure care settings has been underexplored. This paper aims to explore the profiles of internet offenders with ASD admitted to a secure psychiatric unit. This study provides the results of a service evaluation of individuals with ASD. The demographic, clinical and criminal characteristics of a small sample of internet offenders with ASD admitted to secure care are described and discussed. Internet offenders present in secure care with high rates of comorbid disorders, histories of violence and traumatic experiences, mood disorders and difficulties with relationships. Of the 24 internet offenders discussed, 18 of them committed an offence of a sexual nature involving children. This paper highlights the potential risks for individuals with ASD in using the internet and the possible difficulties associated with detecting this because of rapid advancements in technology.

  • Causes and consequences of delayed diagnosis of autism spectrum disorder
           in forensic practice: a case series

      Abstract: The purpose of this paper is to highlight the difficulties in identifying adults with autism spectrum disorder (ASD) and to illustrate the consequences of late diagnoses in forensic settings. This paper presents an overview of the literature relevant to the topic and presents the details of three clinical cases. Delayed diagnosis is not uncommon in forensic services. Cases may remain unidentified for considerable periods despite being under evaluation from clinical teams. Diagnosis in forensic settings is challenged by case complexity. Barriers to prompt diagnosis include time, resources and clinical competency. Timely diagnosis may lessen the risk of future offences, improves clinical risk management and can bring direct benefits to those with the condition. This is the first case series which explores the issue of late diagnosis in forensic settings. The illustration of the extent to which a diagnosis can be missed may prompt clinicians to review and identify missed cases. The discussion of the issue may also support services to review their approach to screening and diagnosis of ASD.

  • “Back off means stay with me”. Perceptions of individuals with mild
           intellectual disability or borderline intellectual functioning about the
           group climate in a secure forensic setting

      Abstract: The purpose of this study is to provide an in-depth account of how individuelas with a mild intellectual disabilitiy or borderline intellectual functioning (MID-BIF; IQ 50–85) perceive their group climate in a secure forensic setting. Giving voice to these service users may provide relevant insights for secure forensic settings. The interpretative phenomenological analysis was used to explore what individuals with MID-BIF experience with regard to their group climate. In the interviews about the four domains of group climate (i.e. repression, support, growth and atmosphere), five overarching dimensions appeared, namely, autonomy, uniformity, recognition, competence and dignity. Depending on the person and the (treatment) context in which he/she resides, these five dimensions relate to all four factors of the group climate instrument. From the perspective of individuals with MID-BIF, this study contributes by providing a framework to “fine-tune” group climate on five dimensions. Training socio-therapists to be sensitive to interpret ambiguous signals on these dimensions can contribute to optimizing group climate in secure forensic settings.

  • Comparing co-production approaches to dynamic risk assessments in a

      Abstract: A co-produced clinical practice that aims to improve outcomes through a partnership with service users is becoming increasingly important in intellectual disability (ID) services, yet these approaches are under-evaluated in forensic settings. This study aims to explore and compare the feasibility of two approaches to co-production in the completion of dynamic risk assessments and management plans in a secure setting. A convenience sample of adults admitted to a secure specialist forensic ID service (N = 54) completed the short dynamic risk scale (SDRS) and drafted risk management plans under one of two conditions. In the first condition, participants rated the SDRS and risk management plan first, separately from the multidisciplinary team (MDT). In the second condition, participants and MDTs rated the SDRS and risk management plan together. In total, 35 (65%) participants rated their risk assessments and 25 (47%) completed their risk management plans. Participants who rated their risk assessments separately from the MDT were significantly more likely to complete the SDRS (p = 0.025) and draft their risk management plans (p = 0.003). When rated separately, MDT scorers recorded significantly higher total SDRS scores compared to participants (p = 0.009). A series of Mann-Whitney U tests revealed significant differences between MDT and participant ratings on questions that required greater skills in abstraction and social reasoning, as well as sexual behaviour and self-harm. Detained participants with an ID will engage in their dynamic risk assessment and management plan processes. The study demonstrates the impact of different co-production methodologies on engagement and highlights areas for future research pertaining to co-production.

  • Feasibility and reliability of screening on mild to borderline
           intellectual disabilities using SCIL through video-administration

      Abstract: In the Netherlands, the Screener for Intelligence and Learning Disabilities (SCIL) was developed to aid recognition of mild to borderline intellectual disability (MBID) early in the criminal justice system or health-care trajectory. In situations where physically meeting the suspect or client is not feasible, administration of the SCIL using a video-link might be a solution. This paper aims to examine whether the SCIL is still reliable when administered remotely instead of face-to-face. The SCIL was administered twice to a total of 89 respondents: once face-to-face, once using a video link, in varying order and with an interval of at least six weeks. A laptop with a Skype connection was used for the remote administration, while an assistant was present to make sure the respondents did not have to perform technical actions. After the second SCIL administration, respondents were asked to answer a series of evaluation questions. Respondents were generally satisfied with both methods of administration of the SCIL. However, they were in general more positive about face-to-face administration. Nevertheless, most respondents would be willing to undergo administration through video-link in future. On average, respondents scored slightly lower on the SCIL when administered remotely (µ = 16.31, SE = 0 0.77) than with face-to-face administration (µ = 16.94, SE = 0.78), t(88) = 2.47, p = 0 0.015. Calculation of the reliability of the assessment “suspected MBID” showed a (linear weighted) Kappa of 0.77, p = 0.000, 95% RI: 0.64-0.90. The results of this study show that with some caution, the SCIL can be administered remotely. When doing this, the SCIL will only lead to a small number of respondents being wrongly labelled as “suspected MBID”.

  • Study of the psychometric properties and population norms of the C-SHARP
           in a representative French population

      Abstract: To fill this gap, this paper aims to translate the children’s scale of hostility and aggression: reactive/proactive (C-SHARP) into French and then tested its psychometric properties and established the first French standards. This paper assess the aggressive behavior of 305 children and adolescents with intellectual disability (ID). This paper tested the scale’s factor structure (confirmatory factor analysis), internal consistency and inter-rater reliability. The results support a factor structure similar to the US version. Factor analysis yielded a five-factor solution, namely, verbal aggression (12 items), bullying (12 items), covert aggression (11 items), hostility (9 items) and physical aggression (8 items). The French-speaking version of the C-SHARP shows good reliability (internal consistency) and good inter-rater reliability. The French version demonstrated encouraging evidence of validity. This paper also established French norms that were similar to US norms. This paper concludes that the French-speaking version of the C-SHARP must benefit from additional psychometric analyzes to confirm its adaptation for research and the clinic. There is no tool currently available in France for the evaluation of aggressive behavior in children and adolescents with ID. The C-SHARP (Farmer and Aman, 2009) is the only questionnaire that specifically assesses aggression in children and adolescents with ID, its form, frequency and its proactive or reactive qualities. The French version of the C-SHARP could be a useful diagnostic tool to assess aggressive behavior in children and adolescents with ID and to identify and set up appropriate support strategies.
