      Abstract: Key summary points Aim To enquire the perceived quality of care delivered through VC at a geriatric outpatient clinic from a healthcare professional’s perspective triangulated with the views of others. Findings The implementation of video consulting at the geriatric outpatient clinic was slow due to the absence of many facilitating factors, but participants believe video consulting can be of future use for particular geriatric patients. Both efficiency and comfort gains and losses were mentioned for and by healthcare professionals and patients. Message In the geriatric population, consideration should be given to the cognitive functioning of the patient and the presence of a digitally literate person when adopting video consultations.

Findings Available studies are observational and limited by incomplete statistical adjustment required for robust causal analysis. There may be a signal for improved outcomes in acute coronary syndrome patients treated invasively vs conservatively. Message Robust studies are needed to inform the optimal selection of coronary interventions in frail older patients with acute coronary syndrome.

      Abstract: Key summary points Aim To describe the diagnoses at index and readmission, and the health characteristics of a geriatric population acutely admitted and readmitted to the same geriatric department. Findings Geriatric patients are mainly readmitted because of a new acute disease, and they suffer from polypharmacy, multimorbidity, disability, and longer index length-of-stay. Message To prevent acute readmission, health professionals need to be particularly vigilant towards detecting new signs of health deterioration following discharge of vulnerable geriatric patients.

      Abstract: Key summary points Aim This study aims to investigate the effects of a synchronous tele-exercise program on physical fitness, quality of life, loneliness, and mood change when applied to older people under social isolation during the coronavirus pandemic. Findings Synchronous tele-exercise seems to be an effective method to provide improvements in the physical fitness level of community-dwelling older people and maintain their quality of life. In addition, it can make positive contributions to coping with loneliness in the older people and improve their mood. Message To maintain the physical fitness level and quality of life of community-dwelling older people, the exercises can be applied effectively via the synchronous tele-exercise method.

  • Cognitive frailty and 5-year adverse health-related outcomes for the
           Malaysian elders longitudinal research (MELoR) study

      Abstract: Key summary points Aim To determine the risk of adverse outcomes between prefrail and frail individuals with and without cognitive impairment as well as those with cognitive impairment in isolation compared to robust individuals. Findings Individuals with cognitive frailty were at higher risk of falls and hospitalisation compared robust individuals. Being prefrail was associated with memory worsening while frailty in isolation [7.70 (1.55–38.20)], being prefrail and cognitively impaired [3.35 (1.76–6.39)] and cognitive frailty [6.15 (2.35–16.11)] were associated with sarcopenia at 5-year follow-up. Message The increased risk of falls and hospitalization among those with cognitive frailty appeared to be accounted for by ethnic disparities. In addition, the presence of sarcopenia at 5-year follow-up was associated with frailty at baseline regardless of cognitive status as well as the presence of mild cognitive impairment in those who were prefrail.

      Abstract: Key Summary Points Aim To examine the factors associated with community ambulation in community dwelling older adults attending a day hospital. Findings Self-efficacy and gait speed were found to be independently associated with community ambulation in community dwelling older adults attending a day hospital. The associations between an individual’s physical ability, psychological well-being and their participation in community ambulation, highlight the importance of completing a holistic assessment of this population. Message The findings demonstrate the complexity and multifactorial nature of community ambulation suggesting the need to adopt a broader approach to the management of older adults, to promote the achievement of independent community ambulation.

      Abstract: Key summary points Aim The aim was to identify tools for classifying the risk of unplanned hospitalization among home care clients utilizing the Resident Assessment Instrument-Home Care (RAI-HC). Findings The Detection of Indicators and Vulnerabilities for Emergency Room Trips (DIVERT) Scale predicts unplanned hospitalizations in home care clients. In the oldest age groups, however, it works poorly. Message DIVERT Scale can be used for identifying high-risk home care clients needing urgent care planning to prevent unplanned hospital admissions and their potential adverse consequences. Clients scoring high in the scale and experiencing the outcome earlier than others, should be the primary group for more detailed assessment.

      Abstract: Key summary points Aim Frailty appears to be an important risk factor for COVID-19-related deaths, but studies to data have important limitations. The aim of this study was to describe the relationship between COVID-19, frailty risk and mortality in older people. Findings This study examined outcomes for all older people hospitalised with COVID-19 between March 2020 and July 2021 and showed that mortality risk was increased with higher Hospital Frailty Risk Scores. Message Any level of elevated frailty risk should be considered an important prognostic marker for older people hospitalised with COVID-19.

      Abstract: Key summary points Aim To investigate the association of polypharmacy with adverse health outcomes, in relation to comorbidity and frailty. Findings Excessive polypharmacy (≥ 10 medications) is highly prevalent in older adults at the emergency department and associated with falls, mortality and readmission. Frailty and comorbidity partly drive the association of polypharmacy with adverse health outcomes. Message Trials that target polypharmacy and inappropriate prescribing are needed to answer the lingering question of causality in the observed polypharmacy–mortality association and to evaluate whether medication review improves health outcomes in older patients at the ED.

      Abstract: Key summary points Aim To examine the effect of cumulative smoking exposure on the association between peak expiratory flow rate (PEFR) and skeletal muscle mass in middle-aged and older adults. Findings The skeletal muscle mass progressively reduced with decreasing PEFR levels in individuals with non-smoking and light-to-moderate smoking exposure. However, the association between low PEFR level and reduced skeletal muscle mass was not clearly observed in individuals with heavy smoking exposure. Message Although decreased PEFR in the older population is associated with the age-related loss of systemic skeletal muscle mass, smoking exposure levels of individuals should be taken into consideration when using PEFR as an indicator of sarcopenia.

      Abstract: Key summary points Aim The aim was to evaluate the relationship between ADL improvement and skeletal muscle mass changes among older patients during VCF rehabilitation. Findings Skeletal muscle mass increase was one of the independent factors associated with ADL improvement in those patients. The rehabilitation to gain skeletal muscle was effective in improving ADLs in elderly patients with VCF. Message We might encourage those patients to undertake rehabilitation for building skeletal muscle and improving their ADLs.

      Abstract: Key summary points Aim To carry out a meta-analysis to reveal and describe the association between sarcopenia and cognitive impairment in the older people. Finding Patients with sarcopenia were at higher risk of cognitive impairment and there was statistically significant difference, confirming their correlation. Message Comprehensive screening and active prevention of sarcopenia are in need for those older people with cognitive decline, which may help enhance the quality of everyday life.
