  • Benefits and challenges of conversational agents in older adults

      Abstract: Background Commercial conversational agents (CAs) bear the promise of low threshold accessibility for individuals with limited digital competencies. This applies not only for healthy aging older adults but also for specific subgroups such as those with life-long intellectual disabilities (ID). Objective This scoping review aims to synthesize the current evidence on benefits and challenges of CAs for older adults with and without ID. In doing so, we hope to inform future research as well as practical decision-making in the context of CAs as potential quality of life enhancers for older adults with various competence levels. Material and methods A literature search was conducted in form of a scoping review. A total of 841 publications were screened for benefits and challenges of CAs, resulting in an extraction of 18 articles targeting healthy aging older adults (60 years+) and 5 articles targeting older adults with ID (50 years+) for synthesis. Results The existing evidence suggests that CAs come with more benefits than challenges, e.g., general ease of use, easier information access, and feelings of companionship. Higher perceived agency due to using a CA seems to be a specific issue for older adults with ID. Challenges concern mostly learning how to use a CA and privacy concerns. Conclusion The results indicate that CAs can serve as quality of life enhancers both in healthy aging adults and in older adults with ID; nevertheless, thoughtful preparation is necessary, especially in relation to learning needs, capabilities present and privacy concerns.

  • Mehr Freiraum für mich' Effekte einer Gruppenintervention für
           pflegende Angehörige anhand von Routinedaten der SVLFG-Pflegekasse

      Abstract: Zusammenfassung Hintergrund Die Intervention ist ein mehrtägiges und bundesweit angebotenes Gesundheitsprogramm der Sozialversicherung für Landwirtschaft, Forsten und Gartenbau (SVLFG) für pflegende Angehörige. Ziel der Arbeit Erhöht die Interventionsteilnahme die Inanspruchnahme von Leistungen zur Fremdhilfe, sodass Freiräume für Selbstfürsorge entstehen' Material und Methoden Im Rahmen eines quasiexperimentellen „Within“-Designs werden jährliche Leistungsdaten der SVLFG-Pflegekasse für die Cluster-Stichprobe Bayern von 2017 bis 2020 und mit Intervention in 2018/2019 analysiert. Mittels „Fixed-effects“-Panelregressionen wird die Effektheterogenität für Pflegebeziehung, -dauer und -grad unter Adjustierung von Periodeneffekten ermittelt (Interventions- bzw. Vergleichsgruppe: 88 bzw. 6045 Personen mit 207 bzw. 16.091 Beobachtungen). Ergebnis Nach der Intervention ist die Inanspruchnahme von Leistungen zur Fremdhilfe tendenziell höher als vorher. Mit zunehmendem Pflegegrad, bei Pflege innerhalb der Partnerschaft und bei kürzerer Pflegedauer ist der Effekt signifikant deutlicher. Bei Pflege innerhalb der Partnerschaft, Pflegebeginn vor einem Jahr und Pflegegrad 4/5 wird nachher für 1160 € (p < 0,001) mehr Fremdhilfe genutzt als vorher. Diskussion Design und statistische Methode würden eine kausale Schlussfolgerung erlauben. Angesichts der Limitationen insbesondere zur Fallzahl in der Interventionsgruppe und der nur begrenzt verfügbaren Merkmale zur Kontrolle zeitdynamischer Heterogenität ist hier jedoch Zurückhaltung geboten.

  • Using wearables to promote physical activity in old age

      Abstract: Background Wearables provide new opportunities to promote physical activity also among older adults but data on effectiveness and user friendliness are rare. Objective The effects of a comprehensive self-regulative intervention on moderate to vigorous physical activity (MVPA) and number of steps were examined using commercially available activity trackers. Acceptance regarding the devices was analysed in various domains. Methods In this study 80 older adults (mean = 67.03 years, standard deviation = 3.97 years; 59% women) wore a Fitbit Charge HR for 21 days including a baseline, a postintervention and a follow-up week. The intervention comprised feedback, goal setting and planning and 50% of the participants were additionally randomized to a role model component. Social cognitive predictors based on the health action process approach (HAPA) and user experience were assessed via questionnaires. Results The MVPA increased by an average of 19 min per week and steps by 1317 per day. An additional benefit of the role model component could be observed for MVPA. In the follow-up, the intervention effect was still significant for the number of steps, while MVPA dropped back to baseline. Multilevel models including HAPA variables explained small but significant amounts of variance in MVPA (8% within-person, 26% between-person) and steps (11% within-person, 12% between-person). User experience was rated as very high. Conclusion Providing an intervention based on established behavior change techniques and self-monitoring via wearables seems to be effective for increasing physical activity among older adults. The HAPA variables seem to play a limited role to explain activity levels. Acceptance of wearables can be expected to be high.

  • Digitales geriatrisches Self-Assessment – ein narratives Review

      Abstract: Zusammenfassung Hintergrund Digitale Gesundheits-Apps haben ein großes Potenzial für das selbstständige Screening sowie das Monitoring älterer Menschen hinsichtlich der Aufrechterhaltung ihrer Selbstständigkeit. Aufgrund des demografischen Wandels und des Fachkräftemangels in der Medizin könnten diese prämedizinischen Self-Assessment-Apps in Zukunft einen großen Stellenwert einnehmen. Ziel der Arbeit Dieses narrative Review soll die Einschätzung ermöglichen, ob ein digitales geriatrisches Self-Assessment für ältere Menschen ≥ 70 Jahren mithilfe aktuell verfügbarer Apps durchführbar ist. Material und Methode Es erfolgte die Recherche nach Apps, die ein Self-Assessment in den folgenden Domänen ermöglichen: physische Kapazität, Kognition, Emotion, Ernährung, Sensorik und Kontextfaktoren. Anhand definierter Kriterien werden Apps ausgewählt und vorgestellt. Ergebnisse In 4 der 6 Domänen konnten Self-Assessment-Apps identifiziert werden: physische Kapazität, Kognition, Emotion, Sensorik. Insgesamt 5 Apps werden exemplarisch vorgestellt. Für Ernährung und Kontextfaktoren wurden keine Apps identifiziert. Zahlreiche Self-Assessment-Apps wurden für den Bereich physische Aktivität identifiziert. Schlussfolgerung Zum aktuellen Zeitpunkt ist ein Self-Assessment für einige Domänen des umfassenden geriatrischen Assessments möglich. Weitere vielversprechende Apps befinden sich in der Entwicklungsphase. Insgesamt bedarf es der wissenschaftlichen Überprüfungen der App-Testgütekriterien und App Usability. Darüber hinaus wird eine Plattform benötigt, die die einzelnen Assessment-Apps integriert, um Nutzer*innen einen Überblick über die Ergebnisse zu geben und Empfehlungen abzuleiten.

  • Pharmakologische Therapie kardiologischer Erkrankungen im Alter

      Abstract: Zusammenfassung Kardiovaskuläre Erkrankungen haben die höchste Prävalenz im fortgeschrittenen Lebensalter. Trotzdem sind höhere Altersgruppen in Randomized Controlled Trials (RCT) häufig unterrepräsentiert. Die Folge ist eine in vielen Fällen ungenügende Evidenz. Empfehlungen aus Leitlinien können daher nur bedingt auf diese Altersgruppe übertragen werden. Aufgrund der Komplexität und Vulnerabilität geriatrischer Patienten bedarf es häufig individueller Überlegungen in der pharmakologischen Therapie. Im vorliegenden Beitrag wird die Pharmakotherapie einiger relevanter kardiovaskulärer Erkrankungen aus der Perspektive eines geriatrischen Behandlungsansatzes diskutiert.

      Abstract: Background The article addresses commercial socially assistive robots (SAR). There is evidence of the impact of SAR on older persons in institutional settings. Family care at home has not yet been the subject of systematic research; however, especially at home SAR could help to prevent loneliness, provide emotional support, and foster communication. Objective Inspired by evidence on the potential impact of SAR, the article analyzes their potential for supporting family care at home. A participatory research and development (R&D) project stands as an example of how SAR can improve communication and relations at home. Material and methods Apart from the results of selected randomized controlled trials (RCT) in institutional settings, the article provides results from tests that were run in the R&D project. Results SAR can increase the quality of life of users at home. They are capable of improving relations within family care. They inspire communication, change routines, and provide relief to family caregivers; however, it takes trained human companions to support the use of SAR at home and address ethical aspects. Conclusion If SAR are supposed to deliver their potential for care at home, older persons and their caregivers must be actively involved in research and development. These processes must be systematically evaluated.

  • Telemedizin in der mobilen geriatrischen Rehabilitation

      Abstract: Zusammenfassung Hintergrund Um das Prinzip „Reha vor Pflege“ umzusetzen, bedarf es adaptiver Konzepte für geriatrische Patienten. Patienten mit Sehbehinderungen, eingeschränkter Kommunikationsfähigkeit, psychischen Erkrankungen oder kognitiven Defiziten sind in einer Rehabilitationsklinik oft nicht oder nur unzureichend behandelbar. Die mobile geriatrische Rehabilitation (MoGeRe) schließt diese Lücke im Versorgungssystem, ist aber in ihrer Reichweite begrenzt. Über die 22 Standorte in Deutschland ist es bislang nicht ansatzweise möglich, eine flächendeckende MoGeRe zu ermöglichen. Hier bietet die Telemedizin Lösungsmöglichkeiten. Fragestellung Telemedizinische Ergänzungen der MoGeRe in Form von Videovisite und Videoaufnahme wurden hinsichtlich ihrer Machbarkeit und Akzeptanz in einer hochaltrigen Zielgruppe untersucht. Methode Mit 25 Patienten wurden 101 Videovisiten und 26 diagnostische Videoaufnahmen durchgeführt. Interviews von Patienten und Teammitgliedern wurden mithilfe einer qualitativen Inhaltsanalyse ausgewertet. Ergebnisse und Diskussion Insbesondere die Akzeptanz der Videovisite war bei allen Beteiligten hoch. Ihr Potenzial liegt in der Anpassung der individuellen Behandlung, Motivation, ärztlichen Lenkung sowie Supervision des Teams. Die Videoaufnahme kann die Chance bieten, den interdisziplinären Austausch zu bereichern und das therapeutische Prozedere zu evaluieren und anzupassen. Spezifische Strategien wie Begleitung von Angehörigen, Erklären des Prozedere und günstiges Timing sind bei kognitiv beeinträchtigten Patienten notwendig. Unsere Ergebnisse sind ein weiterer Beleg dafür, dass auch ältere Menschen als Nutzer digitaler Medien zu berücksichtigen sind.

  • Geriatric assessment in the inpatient sector

      Abstract: Background Geriatric assessment is an integral part of geriatrics and leads to various improvements in the treatment of geriatric patients. Material and methods Analysis of assessment data and patient characteristics in a large inpatient geriatric patient population. Evaluation of data from the Geriatrics in Bavaria Database (GiB-DAT) over 20 years as well as a cross-sectional study from 2020. Presentation of data for patients in inpatient acute geriatrics as well as continuing geriatric rehabilitation. Results The number of patient records and participating hospitals has steadily increased for both inpatient care types to 821,913 (status 31 March 2021). The Barthel index and other assessment results show differentiated values between acute geriatrics and continuing geriatric rehabilitation. Conclusion The results demonstrate the differences in patient outcomes between the two types of care as well as changes in the setting over time. The constancy of the applied assessment instruments contributes to the comparability of the different hospitals; however, it is necessary to introduce new and alternative assessment instruments and to further develop inpatient geriatrics.

      Abstract: Background Professional caregivers take care of an increasing number of migrants with dementia who may show behavioral changes. Objective Insight into the experiences of professional caregivers concerning the care for people with a migration background who suffer from dementia and behavioral changes. Material and methods Semi-structured interviews with 20 professional caregivers providing community and residential care. Results The caregivers’ experiences focus on behavioral changes itself and on their efforts to offer person-centered care. Caregivers are aware of the mismatch between the cultural background of the client and their relatives and the professional care context. The different care perspectives of relatives and professionals sometimes result in conflict and poor collaboration. Over time, the reflections on experiences result in an awareness that more expertise and support is needed. Conclusion Professional caregivers deserve support in their aim to provide high quality care for migrants with dementia and behavioral changes.

  • Dementia and migration: culturally sensitive healthcare services and
           projects in Germany

      Abstract: Background There are approximately 96,500 people with a migration background (PwM) with dementia in Germany. They and their families face not only dementia-related challenges but also the challenge of having little knowledge about the healthcare system and its services and thus more difficulty in accessing support. Germany’s national dementia strategy recognises these individuals as a risk group and thus aims to expand the provision of culturally sensitive information and healthcare services. Objective To determine the amount of culturally sensitive information and healthcare services as well as projects on dementia and migration. Method With a scoping review the PsycInfo, PsycArticles and Psychology & Behavioral Sciences Collection databases, Google Search, the network map (Netzwerkkarte on the website www.demenz-und-migration.de) and the websites of various research funding bodies were used to find culturally sensitive information and healthcare services as well as current projects on dementia and migration. Results Listed are 45 care services as well as 3 additional projects that deal with dementia and migration at the local level. The geographical distribution of the offers shows that most of the services can be found in federal states where most PwM with dementia live. Discussion It is necessary to provide information and healthcare services in all regions and to adapt them to PwM. Different aspects and culturally sensitive measures are important when informing PwM with dementia, as such information can enable these individuals to access the healthcare system and help to provide them with care. It is important to bring together relevant stakeholders to provide access and services that improve the situation of PwM with dementia and their families.

  • Sakrumfrakturen beim geriatrischen Patienten

      Abstract: Zusammenfassung Sakrumfrakturen beim geriatrischen Patienten nehmen zu und bilden eine eigene Entität. Sie können auch mit unspezifischer Symptomatik imponieren und werden daher leicht übersehen. Stabilitätsbeurteilung und Therapieentscheidung erfordern eine Betrachtung des gesamten Beckenrings. Die konventionelle Röntgenuntersuchung ist die Primärdiagnostik, jedoch ist zur Beurteilung des hinteren Beckenrings regelmäßig eine schnittbildgebende Untersuchung erforderlich. Die FFP-Klassifikation (FFP: „fragility fractures of the pelvis“) ist für die anzustrebende chirurgische Therapie richtungweisend. Bei stabilen und auch im Verlauf nicht wesentlich dislozierenden Frakturen ist diese konservativ. Bei Instabilität, fehlender Mobilisierbarkeit und anhaltenden Schmerzen kommen je nach Frakturform minimal-invasive Verfahren zur Stabilisation des hinteren und vorderen Beckenrings bis hin zu aufwendigen Verfahren wie der spinopelvinen Fixation in Betracht.
