  • Differential Maxillary Expander as an Alternative Device for Early
Treatment of Posterior Crossbite: Case Report

    • Authors: Alisson Gabriel Idelfonso Bistaffa, Luciana Belomo-Yamaguchi, Marcio Rodrigues de Almeida, Ana Claudia de Castro Ferreira Conti, Paula Vanessa Pedron Oltramari, Mariane Casadei Bravo Santana, Thais Maria Freire Fernandes
      Pages: 02 - 05
      Abstract: Abstract
Rapid maxillary expansion (RME) is the protocol of choice for early treatment of transverse malocclusions and it is achieved with fixed expanders that produce heavy forces to achieve midpalatal suture opening. One third of patients with maxillary constriction have a greater transversal deficiency at the intercanine width than at the intermolar. The aim of this article is to present a protocol for early treatment of posterior crossbite with the use of the expander with differential opening (EDO) for RME. It is a new appliance to perform RME and its main advantage would be to allow the expansion individualization. EDO was proposed aiming to promote greater expansion on the anterior rather than on the posterior region. A 9 year-old female sought treatment. The interceptive treatment plan was based on using EDO for RME. The post-expansion orthopedic response showed an opening of 7.5 mm between the maxillary central incisors, at the level of the incisal edge. It was observed an opening of 4.8mm in the midpalatal suture. The opening at a distance of 10 mm and 20 mm from the crest to posterior at the midpalatal suture were 3.9mm and 2.8mm. The upper intercanine distance showed an increase of 9.31 mm. The upper intermolar distance had increased 8.04 mm. The upper arch perimeter showed a difference from 74.02 mm to 80.11 mm . And the upper arch length, from 29.83 mm to 31.56 mm. The posterior crossbite was 2 mm overcorrected. Early diagnosis and treatment of posterior crossbite has a very favorable prognosis. A distância intermolares superiores aumentou 8,04 mm. Abstract: Abstract
The individuals with Hansen’s disease experience situations of prejudice that, together with stigma and discrimination, culminate in social isolation and restrictions in social relationships. The aim of this study was to assess the perception of leprosy patients about their quality of life. This is a cross-sectional, quantitative study, whose convenience sample consisted of 94 leprosy patients, undergoing treatment, who attended health units in the city of Cuiabá, MT, Brazil. A structured questionnaire with sociodemographic data and information about the patient's knowledge about leprosy, prejudice, self-esteem and quality of life, was applied between April and August 2018.The results showed in the investigated population a predominance of males (55.3%), individuals with high school education (28.7%) and with an income ranging from 1 to 3 minimum wages (67%).The interviewees pointed out the side effects (44.7%) and the duration period (28.7%) as the greatest difficulty in the treatment. Most of the investigated (72.3%) had great knowledge about the disease, among which 26.6% had already suffered the leprosy reaction.40.4% of individuals were depressed and sad, and 69.1% had problems in employment after the diagnosis of the disease. When the quality of life was investigated, 45.7% of leprosy patients classified the domains evaluated between bad and very bad. Physical pain was associated with 43.7% of the individuals being unable to perform daily tasks. It can be concluded that leprosy causes suffering beyond pain and discomfort, with great social and psychological impact. Keywords: Leprosy. Quality of Life. Combined Modality Therapy.
• Repair Techniques in CAD/CAM System Ceramics: a Literature Review

    • Authors: Christianny de Souza Carvalho, Fernanda Silva de Assis, Andrei Rosa, Amandah Thalita Cortês Silva, Ricardo Danil Guiraldo, Mateus R Rodrigues Tonetto
      Pages: 12 - 16
      Abstract: Abstract
Currently, CAD / CAM systems have been increasingly used in Dentistry, however due to the characteristic of the technique, since it is milled from a ceramic block, some flaws, such as fractures, become a challenge after the piece is cemented. To avoid the complete replacement of the part, the technique through intraoral repair of the fractured region is ideal for these cases. There are several protocols for this technique, this study aims to review the literature regarding the techniques and protocols for ceramic repair of the CAD / CAM system. A literature review was carried out on the Pubmed database using the terms Ceramic repair, Adhesive and Composite resin searching for studies published in the period from 2000 to 2020. 104 articles were found, after reading 32 articles were selected because they presented a greater correlation with the present study. There was unanimity among the authors about the importance of a surface treatment on the ceramic to be repaired, 70% of them still indicated the roughness with drills and conditioning with hydrofluoric acid due to the ease of the technique and good result. The combination of several surface treatments in the same region to be repaired produces better tensile strength. The use of the silane agent in a separate step presents better bonding strength results when compared to the universal type adhesive system with silane incorporated in the formula. Keywords: Repair. Ceramics. Adhesive. Composite Resins.

• Determining the Presence of 5-Fluorouracil in Hamster Saliva by HPLC

    • Authors: Bernar Benites, Fernando Nogueira, Luana Campos, Raquel Cardoso, Ernani Pinto, Cristhiane Almeida Leite da Silva, Alyne Simões
      Pages: 17 - 22
      Abstract: Abstract
Introduction: Various methods of analysis for the assay of chemotherapeutic agent 5-Fluorouracil (5-FU) in human and animal biological fluids have previously been reported. However, there is no standardization for detecting 5-FU in the hamsters’ saliva that received the chemotherapeutic agent. Objective: Considering that the administration of 5-FU in some way changes the morphology and function of the salivary glands, and that the presence of the chemotherapeutic agents in the oral mucosa may lead to some oral complications, the aim of this study was to determine the presence of 5-FU in the hamsters’ saliva that received the chemotherapeutic agent, by means of the High Performance Liquid Chromatography technique (HPCL) since this animal model is used in studies of 5-FU induced oral mucositis and glandular hypofunction. Methods: Twelve animals were divided into 4 groups: CP and CPI, in which the animals received pilocarpine (CP) or pilocarpine + isoproterenol (CPI) and the chemotherapy vehicle intraperitoneally; and Groups QP and QPI, in which the animals received the same secretagogues listed above, and the chemotherapeutic agent 5-FU, respectively. After the secretagogue administration, saliva was collected from all the animals for a period of 60 mins. Subsequently, the saliva was frozen at -80 ˚C for later determination of the chemotherapeutic agent by HPLC. After the the chromatograms analysis, and based on the results obtained, it was possible to identify the presence of 5-FU in the saliva samples from hamsters that received the chemotherapeutic agent intraperitonally, by the HPLC technique. Keywords: Antimetabolites. Chromatography, High Pressure Liquid. Saliva Resumo
Pages: 23 - 27
      Abstract: Abstract
There is an increasing demand for orthodontic treatment by adult patients in dental offices. Orthodontic treatment in adults has particular characteristics, because, in addition to not showing active growth, there are situations that demand interrelation with other areas of dentistry, such as, Periodontics, Dental Prosthesis, Restorative Dentistry and Implantology. The objective of this work was to describe a clinical case of an adult patient with periodontal involvement, establishing the main considerations to be taken in the orthodontic treatment of adult patients with periodontal involvement. The results obtained in the clinical case showed that it is possible to treat patients with periodontal involvement as long as some precautions are taken, such as strict hygiene control, in association with the specialty of periodontics, a reduced sequence of wires, in order to shorten treatment time, using low intensity forces, to minimize damage to periodontal tissues. The orthodontic treatment performed, gave the patient the desired smile functionality and aesthetics, prioritizing a strict hygiene control, the use of light forces, simplification in orthodontic movement, as well as the use of fixed and permanent restraints in areas of reduced periodontium. Keywords: Orthodontic Treatment. Adult. Periodontics. Alveolar Bone Loss.
Abstract: Abstract
With a high global impact, the main target of recent health-related studies focuses on the current pandemic caused by COVID-19, a zoonotic infection mediated by the SARS-CoV-2 virus, which shows high transmission and mortality capacity. This paper aims to analyze, by means of a careful literature review , the current evidence on the manifestations of COVD-19 in the oral cavity, seeking to understand whether these are primarily from the virus, or secondary factors. This is a narrative literature review based on the research of articles carried out on the platforms: Pubmed, Google scholar, Scielo and Lilacs, in the period from June-August 2020, using the crossing of Decs / Mesh descriptors with the use of the Boolean operator AND. Only articles from 2020 were selected, in English or Portuguese and, after the search was refined, a total of 24 articles were selected. Among the manifestations expressed in the oral cavity, taste changes are present in most cases. In addition, vesiculobullous and ulcerative oral lesions have been observed in some case reports, however, due to the scarcity of information coupled with the recent emergence of this infection, further studies should be carried out in an attempt to prove or discard the association of COVID-19 with these oral manifestations. Abstract: Abstract
The partial vascular occlusion (PVO) associated with exercises, proposed by the KAATSU Training method, has the objective of strengthening and muscle hypertrophy with low joint overload. Currently, PVO has been associated with exercises to strengthen then quadriceps, however it is not known about its influence on postural control or the possibility of imbalances during the performance of exercises. Objectives: To evaluate the effect of quadriceps vascular occlusion on postural control in women with patellofemoral pain syndrome (PFPS). Methods: The sample in this study was composed of four sedentary women, aged between 18 and 40 years, with a clinical diagnosis of patellofemoral dysfunction. Participants responded to the Visual Analog Pain Scale (VAS) and the Anterior Knee Pain Scale (AKPS). They were submitted to an evaluation of postural control over the force platform, by means of dynamic one-legged squat activity, with and without PVO. The variables of postural control analyzed were Area of pressure center (A-COP), Anteroposterior (AP) and medium-lateral (ML), Velocity AP and ML. Results: No significant differences were found for the variables of postural control analyzed when comparing the moments with and without peripheral vascular occlusion during single-legged squat activities. Conclusion: The single leg squat with vascular occlusion does not change the postural control of patients with PFPS with and without PVO, and that the method can be used for training these patients, without impairments related to postural control. Abstract: Abstract
Identified in China in December 2019 as a new class of viral pneumonia of unknown origin, the new Coronavirus has already passed the milestone of 100 million people infected worldwide. The clinical course of this infection is characterized by fever, cough, upper airway congestion and complications related to Acute Respiratory Distress Syndrome. In addition, the virus can have repercussions that go beyond the impairment of the respiratory system, affecting other systems. Therefore, the aim of this study is to describe the correlation between Covid-19 and thromboembolic and cardiovascular events. It is a Systematic Review that was submitted to searches in the PubMed, SciELO, Lilacs and Bireme databases. Six observational studies comprising 1539 patients were included in this review. The quality of the articles was evaluated according to the New Castle-Ottawa scale. After extracting data from the studies, it was observed that hospitalized patients diagnosed with Covid-19 infection are more likely to develop Venous Thromboembolism (VTE) and Pulmonary Embolism (PE), as well as cardiovascular events, although the latter are less frequent. These patients also have alterations in the pulmonary parenchyma, being proportional to the severity of the case, as well as the prevalence of d-dimer, the rate of thrombosis in the pulmonary artery, the need for intensive care and the mortality rate. Thus, the severe form of infection by Covid-19 manifests consequences that do not only involve the respiratory system, compromising the blood clotting of affected patients, leading to a higher incidence of thromboembolic and cardiovascular events. Abstract: Abstract
Tuberculosis is a disease that has affected the population for more than centuries, it is caused by Mycobacterium tuberculosis with high incidence and prevalence in the population. In Brazil, active tuberculosis is the condition with the greatest impact on mortality in people living with the human immunodeficiency virus, since this part of the population is more likely to develop the disease. In this sense, in order to elucidate the epidemiological profile of patients coinfected with Tuberculosis/HIV, a retrospective study was carried out with a quantitative approach in a referral hospital in the care of these patients in the State of Alagoas. 956 cases of tuberculosis and HIV co-infection were reported, with a higher incidence in males (63.49%), in mixed race (82.42%) and in the age group corresponding to the economically active population, between 21 and 50 years (84,.08%). In addition, the pulmonary clinical form (73.95%) is predominant among the cases, with the peripheral lymph node (32.12%) being the most frequent among the extrapulmonary forms (19.87%). Regarding the clinical outcome of these patients, it was observed that there was a higher rate of treatment drop-out in patients reported with alcoholism, consequently reducing the cure rate of these patients. Therefore, it is evident that socioeconomic variables directly influence the incidence and outcome of patients co-infected with Tuberculosis/HIV and that despite the therapeutic advances and the existing support network, it is still a disease that generates negative impacts on the social development of the country. Abstract: Abstract
About 5.5 million people in the world have respiratory limitations, which is the 5th largest cause of hospitalizations in Brazil. Covid-19 has contributed negatively to this scenario. The choice of adequate respiratory support contributes to a better clinical outcome, also supporting an improvement in quality of life. A new way of providing oxygen therapy, with lower cost and capable of managing hypoxia, may be a promising alternative. With the use of common supplies, it is possible to provide a high flow of compressed air and oxygen, heated and humidified, optimizing ventilation-perfusion. With a view to addressing hypercapnia and hypoxemia, this newly designed new form can contribute to clinical practice. The results of this study showed the effectiveness of this strategy when using flows from 30l/min, since, in this way, the loss of gas supply to the patient is insignificant. In addition, the financial viability makes the strategy accessible to less favored institutions. The magnitude of respiratory disorders aroused the need for new approaches to this clinical scenario, as conventional oxygen therapy does not always meet these demands and the high-flow nasal catheter has already shown a promising strategy in cases of respiratory limitations.
