Abstract: RESUMEN Introducción: Las infecciones por micobacterias pueden causar lesiones cutáneas y pulmonares; las micobacterias pueden hallarse en fuentes de agua intrahospitalarias, pudiendo llegar a los pacientes inmunosuprimidos y provocar enfermedades severas. Objetivo: Aislar y caracterizar micobacterias no tuberculosas en el sistema de distribución de agua en un hospital de Lima. Materiales y métodos: Se aislaron las micobacterias en medios convencionales a base de huevo; y se emplearon pruebas bioquímicas y herramientas moleculares como la reacción en cadena de la polimerasa ―PCR― y secuenciación para su identificación. Para el análisis de secuenciación se empleó el GenBank y para el árbol filogenético se utilizó MEGA 6. Resultados: Fue posible el aislamiento de bacilos ácido alcohol resistentes en muestras de agua potable fría, aunque no en agua caliente del hospital. Se aislaron e identificaron varias especies de micobacterias no tuberculosas en las muestras de agua, siendo el 100 % de crecimiento rápido, estas fueron: Mycobacterium canariasense; M. frederiksbergense; M. mucogenicum; Mycobacterium fortuitum y Mycobacterium chelonae. Conclusiones: Esta es la primera vez que se aíslan micobacterias en agua potable de un hospital en Perú. Se concluye que la existencia de micobacterias en el agua del hospital podría ser fuente potencial de brotes nosocomiales, por lo que debería tenerse en cuenta su estudio periódicamente.ABSTRACT Introduction: Mycobacterial infections can cause skin and lung lesions. Mycobacteria can be found in intra-hospital water sources which can reach immuno-suppressed patients and cause severe disease. Objective: To isolate and characterize non-tuberculous mycobacteria in the water distribution system in a hospital in Lima. Materials and Methods: Mycobacteria were isolated in conventional culture egg-based media and biochemical tests and molecular tools such as polymerase chain reaction (PCR) and sequencing were used for their identification. The GenBank was used for sequencing analysis and MEGA 6 was used for the phylogenetic tree. Results: Isolation of acid-fast bacilli was possible in samples of cold drinking water, although not in the hospital hot water. Several species of non-tuberculous mycobacteria were isolated and identified in the water samples being a 100% of rapid growth such as Mycobacterium canariasense, M. frederiksbergense, Mycobacterium fortuitum, Mycobacterium chelonae, and M. mucogenicum. Conclusions: This is the first time that mycobacteria are isolated in drinking water at a hospital in Perú. It is concluded that the existence of mycobacteria in hospital water could be a potential source of nosocomial outbreaks and should be analyzed periodically.
Abstract: RESUMEN Introducción: Las especies de Staphylococcus spp. son patógenos oportunistas que causan enfermedades como osteomielitis y bacteriemias. Estas bacterias pueden adquirir resistencia a antibióticos, lo que hace que se conviertan en un problema de salud pública debido a la restricción de opciones terapéuticas. Objetivo: Evaluar la tendencia de los perfiles de resistencia antimicrobiana de S. aureus y S. epidermidis aislados en un hospital de Cartagena entre 2010 y 2016. Materiales y métodos: Estudio de corte transversal. El método de microdilución en caldo fue usado para la determinación de la concentración mínima inhibitoria de 12 agentes antimicrobianos. Se estudió el comportamiento de la resistencia antimicrobiana de acuerdo a la especie Staphylococcus, el sitio de infección y el período de tiempo. Resultados: Se aislaron 1218 cocos grampositivos, de los cuales 42,7 % fueron S. aureus y 18,9 % S. epidermidis. El 47,5 % de S. aureus fueron resistentes a meticilina y se encontraron principalmente en secreciones (43,3 %); mientras que el 68,7 % de S. epidermidis fueron meticilino resistentes y aislados principalmente en sangre (76,9 %). Conclusión: Se identificaron aislamientos clínicos de S. aureus y S. epidermidis con perfil de multirresistencia. Se observo un comportamiento constante en sus perfiles de resistencia durante el período de estudio, excepto en los dos últimos años en los que se identificó una reducción significativa de la meticilino resistencia en S. epidermidis.ABSTRACT Introduction: Staphylococcus spp. are opportunistic pathogens that cause diseases like osteomyelitis and bacteremia. These bacteria can acquire resistance to antibiotics being a public health problem due to the restriction of therapeutic options. Objective: To evaluate the tendency of antimicrobial resistance profiles of Staphylococcus aureus and Staphylococcus epidermidis isolated in a hospital in CartagenaColombia between 2010 and 2016. Materials and methods: Cross-sectional study. The broth microdilution method was used to determine the minimum inhibitory concentration of 12 antimicrobial agents. The behavior of antimicrobial resistance was studied according to the Staphylococcus species, the site of infection and the time period. Results: A total of 1218 Gram-positive cocci were isolated of which 42.7% were S. aureus, and 18.9% belonged to S. epidermidis. Of the isolates of S. aureus, 47.5% were resistant to methicillin and were found mainly in secretions (43.3%). In the case of S. epidermidis, 68.7% were resistant to methicillin and were found mainly in the blood (76.9%). Conclusion: Clinical isolates of S. aureus and S. epidermidis with a multi-resistance profile was identified. A constant behavior in their resistance profiles was detected during the study period except in the last two years in which a significant reduction of methicillin resistance in S. epidermidis was recognized.
Abstract: RESUMEN Introducción: La cateterización percutánea por punción de una vena de gran calibre y flujo sanguíneo conocido como catéter venoso central ―CVC― es un procedimiento que no deja de ser invasivo, por lo tanto no está exento de potenciales complicaciones que pueden generar un riesgo para la vida de los pacientes como lo es el neumotórax iatrogénico secundario. Objetivo: Establecer la incidencia de neumotórax secundario iatrogénico como complicación inmediata post inserción de catéter venoso central. Materiales y métodos: Se trata de un estudio prospectivo-descriptivo y observacional de las historias clínicas de los pacientes entre 15 y 60 años de edad, de ambos sexos, que ingresaron a los diferentes servicios de hospitalización del hospital Dr. Adolfo Pons del Instituto Venezolano de los Seguros Sociales ―IVSS― (Maracaibo, Venezuela), durante el período comprendido entre enero de 2016 y junio de 2017; los cuales ameritaron inserción de un catéter percutáneo para el abordaje venoso central como indicación para manejo terapéutico o de constantes fisiológicas. Resultados: Se incluyeron 140 pacientes, en el 15,6 % (22) se identificó la presencia de neumotórax secundario en los controles radiológicos post procedimiento cuando el abordaje era la vena subclavia. Conclusiones: Se debe tener en cuenta que el neumotórax secundario representa la complicación mecánica más común en la cateterización venosa central, la cual puede poner en riesgo la integridad del paciente. Este conocimiento es fundamental para detectar esa complicación por el médico operador y poder darle la solución más acorde y adecuada para evitar las complicaciones y lograr así una medicina de mejor calidad.ABSTRACT Introduction: Percutaneous catheterization by puncture of a large caliber vein and blood flow known as the central venous catheter (CVC), is a non-invasive procedure and, therefore, it is not free of potential complications that may generate a risk for the life of patients, as it is the secondary iatrogenic pneumothorax. Objective: To establish the incidence of secondary iatrogenic pneumothorax as an immediate complication secondary to the insertion of a CVC. Materials and methods: This is a prospective, descriptive, observational study of the medical records of patients between 15-60 years of age of both sexes, who were admitted in the different hospitalization services at Dr. Adolfo Pons Hospital from the Social Insurance Institute of Venezuela ―IVSS for its acronym in Spanish― (Maracaibo, Venezuela) during the period between January 2016 and June 2017, patients who required the insertion of a percutaneous catheter for the central venous approach, as an indication for therapeutic management or physiological constants. Results: A total of 140 patients were included; in 15.6% (22) the presence of secondary pneumothorax was identified in radiological controls when the subclavian vein was approached. Conclusions: It should be considered that secondary pneumothorax represents the most common mechanical complication in central venous catheterization that can put the integrity of the patient at risk. This knowledge is essential to detect this complication by the attending physician and can give the most adequate and appropriate solution to avoid complications and thus achieve better quality medicine.
Abstract: RESUMEN El Centro de Estudios Epidemiológicos (CES) de los Estados Unidos introdujo con la escala para depresión (CES-D) y otra escala para cuantificar en forma rápida ideación suicida (CES-IS) durante las dos semanas más recientes; sin embargo, se conoce poco del rendimiento psicométrico de este instrumento. El objetivo fue verificar el desempeño psicométrico de la CES-IS en adolescentes escolarizados de un municipio de Boyacá, Colombia. Se realizó un estudio de validación con la participación de 363 estudiantes de un municipio de características rurales en edades entre 10 y 17 años (media=13; DE=1,8); 55,4% de los estudiantes de sexo femenino y 44,5% de sexo masculino. Los adolescentes diligenciaron los cuatro ítems de la CES-IS, una versión de diez ítems de la (CES-D-10) y la escala de Rosenberg para autoestima (ERA). Se estimó la consistencia interna (alfa de Cronbach y omega de McDonald), la estructura dimensional (análisis factorial confirmatorio, AFC) y la convergencia con la CES-D-10 (correlación de Spearman, rs). Las puntuaciones en la CES-IS se observaron entre 4 y 20 (M=6,1; DE=3,5; Me=4, RIC=4-7) y en la CES-D entre 11 y 44 (M=23,3; DE=6,8; Me=22; RIC=18-27). La CESIS mostró alfa de Cronbach de 0,86 y omega de McDonald de 0,87, una dimensión que explicó el 71,2% de la varianza, los indicadores de ajuste RMSEA=0,00; IC90% 0,00-0,09; CFI=1,00; TLI=1,00; y SRMR=0,01, correlación convergente de rs=0,59 (p=0,001) con la CES-D-10 y correlación divergente de rs=-0,38(p=0,001) con la ERA. Se concluye que la CES-IS en adolescentes de un municipio de Boyacá presenta alta consistencia interna, una estructura unidimensional y alta convergencia con la CES-D-10 y divergencia con la ERA. Se necesita investigar el rendimiento de este instrumento en adolescentes colombianos con otras características distintivas.ABSTRACT The Center for Epidemiological Studies (CES) of the United States introduced, along with the Depression Scale (CES-D) and another scale to rapidly quantify suicidal ideation (CED-IS) during the two most recent weeks. However, little is known about the psychometric performance of this instrument. The objective of this study was to verify the psychometric performance of the CES-IS in school enrolled adolescents from a small town in Boyacá, Colombia. A validation study was carried out with the participation of 363 students between 10 and 17 years of age (mean=13, SD=1.8); 55.4% female students and 44.5% male students. The adolescents completed the four items of the CES-IS, a ten-item version of the CES-D-10 and the Rosenberg Self-esteem Scale (RSS). Internal consistency (Cronbach alpha and McDonald omega), dimensional structure (confirmatory factorial analysis, AFC) and convergence with CES-D-10 (Spearman correlation, rs) were estimated. The CES-IS scores were observed between 4 and 20 (M=6.1, SD=3.5, Me=4, IQR=4-7) and the CES-D between 11 and 44 (M=23.3, SD=6.8, Me=22, IQR=18-27). The CES-IS showed Cronbach alpha of 0.86 and McDonald’s omega of 0.87, a dimension that explained 71.2% of the variance, the adjustment indicators RMSEA=0.00, 90%CI 0.00-0.09; CFI=1.00, TLI=1.00 and SRMR = 0.01, convergent correlation of rs=0.59 (p=0.001) with the CES-D-10 and divergent correlation of rs=-0.38 (p=0.001) with the RSS. It is concluded that the CES-IS among adolescents of a small town in Boyacá, Colombia, has high internal consistency, a onedimensional structure and high convergence with CES-D-10 and divergence with the RSS. It is necessary to investigate the performance of this instrument in Colombian adolescents with other distinctive characteristics.
Abstract: RESUMEN Actualmente los trastornos neurodegenerativos representan un grave problema de salud pública con una prevalencia en ascenso mundial, si bien se ha tratado de armonizar criterios de diagnóstico para estas enfermedades, aún existen obstáculos que dificultan su correcta diferenciación, dando lugar a errores posteriores en etapas terapéuticas. El objeto de esta revisión pretende mostrar el potencial de tres técnicas de neuroimagen (tomografía por emisión de positrones, resonancia magnética de difusión, resonancia magnética estructural), en la identificación de biomarcadores que apoyen el proceso diagnóstico, en tres de las afecciones neurodegenerativas más comunes (enfermedad de Alzheimer, deterioro cognitivo leve, demencia frontotemporal). Se realizó una revisión mediante búsqueda electrónica de literatura. Se destaca el uso de bases de datos como ScienceDirect, PubMed, SciELO, IEEE, para localizar información sobre hallazgos estructurales y funcionales representativos y el poder diagnóstico de estas técnicas. Como lo confirman los estudios, las neuroimágenes ponen en evidencia su potencial para el establecimiento de patrones en la diferenciación de trastornos neurodegenerativos. La resonancia magnética estructural permanece como herramienta central de diagnóstico, en la identificación de patrones de atrofia corticales y subcorticales. Por otro lado, los avances en la tomografía por emisión de positrones han abierto la puerta a un diagnóstico ante-mortem, y una identificación preclínica temprana. Asimismo, el enfoque reciente de la resonancia magnética de difusión posibilita la caracterización de la integridad microestructural de la materia blanca cerebral y su relación con el deterioro cognitivo en el contexto de la enfermedad neurodegenerativa. Al integrar información de diferentes dominios se apoya las herramientas clínicamente aceptadas, garantizando mejor precisión del diagnóstico y la predicción de la aparición de la enfermedad. Los resultados demuestran que, a través de enfoques multimodales, colaboraciones multicentro, armonización de las metodologías y parámetros de adquisición, es posible incluir estas herramientas en el arsenal clínico para la identificación de estas enfermedades.ABSTRACT Currently, neurodegenerative disorders represent a serious public health problem, with an increasing prevalence worldwide. Even though there has been an attempt to harmonize the diagnostic criteria for these disorders, there are still obstacles that hinder their correct differentiation, leading to subsequent errors in therapeutic stages. This review aims to demonstrate the potential of three neuroimaging techniques (positron emission tomography, diffusion-weighted magnetic resonance, and structural magnetic resonance) in the identification of discriminating biomarkers that support the diagnostic process in three of the most common neurodegenerative disorders (Alzheimer’s disease, Mild Cognitive Impairment, frontotemporal dementia). A review was done via an electronic literature search. The use of ScienceDirect, PubMed, SciELO, and IEEE databases to find information on representative structural and functional findings, as well as the diagnostic power of these techniques, is highlighted. As the studies confirm, neuroimages show their potential to establish patterns in the differentiation of neurodegenerative disorders. The structural magnetic resonance remains as a central tool in the identification of cortical and subcortical atrophy patterns. On the other hand, advances in positron emission tomography have enabled not only antemortem diagnosis but also early preclinical identification. Likewise, the recent approach of diffusion magnetic resonance allows to characterizing the microstructural integrity of the cerebral white matter and its relationship with cognitive deterioration in the context of the neurodegenerative disorder. By integrating information from different domains, the clinically accepted tools are supported, guaranteeing better diagnostic accuracy and the prediction of the onset of the disorder. The results show that through multimodal approaches, multicenter collaborations, harmonization of methodologies and acquisition parameters it is possible to include these tools in the clinical repertoire for the identification of these disorders.
Abstract: RESUMEN Introducción: La ciclosporosis es una enfermedad emergente caracterizada por producir cuadros clínicos de gastroenteritis en humanos luego de la ingesta de agua o de alimentos contaminados, siendo más severa en niños e inmunosuprimidos; es ocasionada por el coccidio Cyclospora cayetanensis cuyo cuerpo de conocimiento está en pleno proceso de construcción. Objetivo: Recopilar y presentar en forma comprimida lo hallado hasta ahora sobre la biología, manifestaciones clínicas, diagnóstico, tratamiento y comportamiento epidemiológico de C. cayetanensis. Metodología: Se trató de un estudio documental con base en la revisión minuciosa de la literatura científica mundial. Resultado y conclusión: Se ofrece a los profesionales de la salud e instituciones sanitarias oficiales, herramientas teóricas necesarias para el control de la parasitosis.ABSTRACT Introduction: Cyclosporiasis is an emergent disease characterized by producing clinical profiles of gastroenteritis in humans after the intake of water or contaminated food, and it is more severe in children and immunosuppressed patients. It is caused by the coccidia Cyclospora cayetanensis, whose body of knowledge is in the process of construction. Objective: To compile and show in compressed form what has been found so far on the biology, clinical manifestations, diagnosis, treatment and epidemiological behavior of C. cayetanensis. Methodology: a documentary study based on a thorough review of the world scientific literature. Results and conclusion: the theoretical tools necessary for the control of parasitosis are offered to health professionals and official health institutions.
Abstract: RESUMEN El síndrome de Leriche es una condición causada por la obstrucción de las ramas principales de la aorta abdominal por debajo de la salida de las arterias renales con compromiso de todo el árbol arterial, incluyendo las iliacas y femorales. Se presenta un paciente masculino de 62 años de edad con antecedentes de tabaquismo y que consulta al servicio de urgencias por un cuadro clínico de 3 meses de evolución consistente en dolor de miembros inferior, linfedema que predomina en el miembro inferior izquierdo, además de ausencia de pulsos en miembros inferiores. Al aplicar el test IIEF-5 se obtiene una disfunción eréctil moderada. Es importante resaltar este síndrome y la necesidad de diagnosticarlo tempranamente debido a sus consecuencias catastróficas, haciendo énfasis en identificar e intervenir en los factores de riesgo que conllevan a esta presentación inusual de la enfermedad arterial periférica.ABSTRACT Leriche syndrome is a condition caused by obstruction of the main branches of the abdominal aorta below the outlet of the renal arteries, with involvement of the entire arterial tree, including the iliac and femoral arteries. . We present a 62-year-old male patient with history of smoking who consults the emergency department with clinical features of a 3-month course consisting in lower limb pain, lymphedema more predominant in the left lower limb, and the absence of pulses in the lower limbs. When applying the IIEF-5 test, moderate erectile dysfunction is found. It`s important to highlight this syndrome and the necessity of an early diagnosis for its consequences can be devastating, also to identify and intervene in risk factors that lead to this unusual presentation of peripheral artery disease.
