  • Faktor Risiko Masalah Mental Emosional Pada Anak Prasekolah Di Kota

      Abstract: Impairment of mental and emotional development in children can be an early sign of crime at a young age. Risk factors for mental and emotional disorders in children are influenced by external and internal factors. This study aims to determine the prevalence and factors influencing mental-emotional problems in pre-school children in Sukabumi City in 2020. The study design was cross-sectional. The number of samples of pre-school children is 385 children. The data was collected with the help of a checklist and a questionnaire on psychological emotional problems (KMME). Logistic regression was used in the data analysis. The result of the prevalence of mental emotional disorders is 25.7% (99 children). The results of the multivariate analysis showed that emotional psychological problems in children caused by authoritarian upbringing [OR = 5.88 (95% CI; 3.45-10.02)], birth complications [OR = 3.36 (95% CI); 1.95-5.81)] is affected. , divorced parents [OR = 3.00 (95% CI; 1.09–8.23)], working mother [OR = 1.77 (95% CI; 1.04–3.00)] and mother with a low level of education [OR = 1.74 (95% CI; 1.02–2.95.))], authoritarian upbringing is the dominant factor in emotional psychological problems in children. Routine detection by health workers is required so that mental emotional problems in children can be detected as early as possible. Abstrak Perkembangan mental emosional anak yang terganggu dapat menjadi tanda awal kejahatan pada usia remaja. Faktor risiko masalah mental dan emosional pada anak dipengaruhi faktor eksternal dan internal. Studi ini bertujuan untuk mengetahui prevalensi dan faktor-faktor yang mempengaruhi masalah mental emosional pada anak prasekolah di Kota Sukabumi pada tahun 2020. Disain studi potong lintang. Jumlah sampel anak prasekolah adalah 385 anak. Pengambilan data dilakukan dengan menggunakan daftar ceklist dan juga kuesienor masalah mental emosional (KMME). Analisis data menggunakan regresi logistik. Hasil prevalensi masalah mental emosional sebesar 25,7% (99 anak). Hasil analisis multivariat menunjukkan bahwa masalah mental emosional terdapat pada anak dipengaruhi oleh pola asuh otoriter [OR =5,88 (CI 95%;3,45-10,02)], komplikasi saat lahir [OR=3,36 (CI 95%;1,95-5,81)], orangtua bercerai [OR=3,00 (CI 95%; 1,09-8,23)], ibu bekerja [OR= 1,77 (CI 95%;1,04-3,00)], dan pendidikan ibu rendah [OR=1,74 (CI 95%; 1,02-2,95)], Faktor yang paling dominan terhadap masalah mental emosional anak adalah pola asuh otoriter. Perlu dilakukan deteksi secara rutin oleh tenaga kesehatan agar masalah mental emosional pada anak dapat terdeteksi sedini mungkin.

  • Orang dengan Disabilitas: Situasi Tantangan dan Layanan di Indonesia

      Abstract: People with disabilities experience discrimination for their rights. This discrimination makes them experiencing difficulties in their lives. This article explores the challenges facing people with disabilities and what services are available to the disability population in Indonesia. A literature study used to capture the phenomenon of challenges and services for the disability population in Indonesia. This study indicated that people with disabilities still experience social injustice and discrimination. These challenges demonstrate the public perception to the disability population that those were not independent or depends on the other compassion. The services are centralized in some cities so that the challenge appeared for those living in the countryside. Social workers may actively provide excellent services by considering local wisdom and commitment to social justice by fulfilling the rights of people with disabilities. In addition, they also promote the participation of the beneficiaries at all levels to fulfilment of needs. Abstrak Orang dengan disabilitas mengalami diskriminasi dalam keterpenuhan haknya. Diskriminasi dalam pemenuhan hak tersebut membuat orang dengan disabilitas kesulitan dalam kehidupannya. Artikel ini bertujuan untuk mengeksplorasi tantangan yang dihadapi orang dengan disabilitas dan layanan apa yang tersedia bagi populasi disabilitas di Indonesia. Metode yang digunakan adalah studi kepustakaan tentang fenomena tantangan dan layanan bagi populasi disabilitas di Indonesia. Hasil penelitian menemukan bahwa di Indonesia, populasi disabilitas masih mengalami ketidakadilan sosial dan diskriminasi. Tantangan tersebut menunjukkan cara pandang orang umum terhadap populasi disabilitas sebagai populasi yang tidak mandiri dan tergantung pada bantuan orang lain yang berdasar pada belas kasihan. Layanan yang tersedia bagi populasi disabilitas di Indonesia terpusat di kota-kota besar Indonesia, sehingga menjadi tantangan tambahan bagi populasi disabilitas terutama yang tinggal di pedesaan. Profesi pekerjaan sosial memberikan layanan kompeten dengan mempertimbangkan budaya yang berkembang di masyarakat, kemudian berkomitmen untuk mengusung keadilan sosial dengan mengupayakan pemenuhan hak orang dengan disabilitas, serta mempromosikan partisipasi penerima layanan di semua level usaha pemenuhan kebutuhan. .

  • Gambaran Pelaksanaan Program Vaksinasi Human Papilloma Virus (HPV) Di Dua
           Puskesmas Di Kota Jakarta Pusat Tahun 2020

      Abstract: Human Papillomavirus (HPV) is the main risk factor for  cervical cancer. HPV vaccination among adolescents may prevent the disease. Jakarta Province has been implementing an HPV vaccination program since 2016. This research aimed to know the overview of the HPV vaccination program in Central Jakarta in 2020. This research was a qualitative study conducted in the City of Central Jakarta from March till June 2020. Informants of the research were 12 persons from Central Jakarta Health Office, PHCs, Central Jakarta Education Office, and teachers from elementary schools selected purposively. Primary data was collected through interview using a structured questionnaire. Secondary data was from the report of the Jakarta Provincial Health Office. Data were analyzed using content analysis for planning, organizing, implementing, monitoring, evaluation, intersectoral collaboration and coverage. The study results  showed that the HPV vaccination program was conducted through planning, organizing, implementing, monitoring and evaluation, with intersectoral collaboration. The problem was vaccine availability, invalid target data, lack of teachers involved, rejection from parents and lack of PHC providers. Monitoring and evaluation were not conducted adequately in the schools, and there was an obstacle in private schools for vaccination. The vaccinaton coverage was 90.98% in 2016, 95.40% in 2017, and 94.55% in2018.  In conclusion, the HPV vaccination program in Central Jakarta was running but needed strengthening. It is recommended to strengthen the coordination in vaccines supply, update target data, increase education, involve the Education office and schools in monitoring and evaluation, and collaborate with community and religious leaders. Abstrak Human PapillomaVirus (HPV) merupakan faktor risiko utama terjadinya kanker serviks.  Vaksinasi HPV pada remaja dapat mencegah kanker serviks. Provinsi DKI Jakarta telah mengembangkan program vaksinasi HPV sejak tahun 2016. Penelitian ini bertujuan untuk mengetahui gambaran program vaksinasi HPV di Jakarta Pusat tahun 2020. Penelitian ini merupakan studi kualitatif yang dilaksanakan di Kota Jakarta Pusat pada Maret - Juni 2020. Informan penelitian adalah 12 orang dari Sudin Kesehatan dan Puskesmas, Sudin Pendidikan, dan guru Sekolah Dasar yang dipilih secara purposif. Data primer diperoleh melalui wawancara mendalam menggunakan kuesioner terstruktur. Data sekunder dikumpulkan dari Dinkes Provinsi DKI Jakarta. Data dianalisis dengan analisis konten untuk mengetahui perencanaan, pengorganisasian, pelaksanaan, monitoring dan evaluasi, kerja sama lintas sektor, serta cakupan program vaksinasi HPV. Hasil penelitian menunjukkan program vaksinasi HPV dilaksanakan melalui perencanaan, pengorganisasian, pelaksanaan, monitoring dan evaluasi, dengan kerja sama lintas sektor. Kendala dan hambatan yang terjadi adalah ketersediaan vaksin, ketidaksesuaian data sasaran, kurangnya guru yang terlibat, penolakan orang tua dan kurangnya petugas kesehatan. Monitoring dan evaluasi belum dilaksanakan sepenuhnya di sekolah dan masih ada hambatan vaksinasi pada sekolah swasta. Cakupan program vaksinasi HPV sebesar 90,98% pada 2016, 95,40% pada 2017 dan 94,55% pada 2018.  Kesimpulan penelitian adalah program vaksinasi HPV di Jakarta Pusat dilaksanakan tetapi perlu penguatan. Saran yang diberikan adalah penguatan koordinasi dalam penyediaan vaksin HPV, pemutakhiran data sasaran, peningkatan edukasi, pelibatan Sudin Pendidikan dan sekolah dalam monitoring dan evaluasi, dan kerja sama dengan tokoh masyarakat dantokoh agama.

  • Faktor Pendukung Perilaku Seksual Remaja Di Provinsi Maluku

      Abstract: Premarital sexual behavior among adolescents of Maluku Province is four percent, which was higher than the national figure, making it the fifth-highest in adolescents’ premarital sexual behavior, after West Papua (10%), Papua (5%), North Sulawesi (5%) and North Maluku (4%). This study is based on the 2019 Adolescents’ Performance and Accountability Survey of Population, Family Planning, and Family Development to analyze the relationship between enabling factors and adolescents’ sexual behavior. This research is a quantitative study, which is correlative analytic with a cross-sectional study approach. The total sample was 241 adolescents age 10-24 years and not married. The analysis found significant factors related to adolescents’ sexual behavior were exposure to family planning information (p = 0.011), exposure to“genre” information (p = 0.015), exposure to STIs information (p <0.001), exposure to HIV / AIDS information (p = 0.001), exposure to drugs information (p = 0.026) and discussion of menstruation / wet dreams (p <0.001). This study suggests a breakthrough innovation in providing information which is not only through mass media but also social media such as Instagram, Facebook, Twitter, YouTube, or reproductive health applications. Abstrak Maluku merupakan provinsi yang berada pada urutan kelima dengan persentase hubungan seksual pranikah di kalangan remaja tertinggi, setelah Papua Barat (10%), Papua (5%), Sulawesi Utara (5%) dan Maluku Utara (4%). Angka hubungan seksual pranikah di Provinsi Maluku pada tahun 2019 tiga persen lebih tinggi dari angka nasional, yaitu empat persen. Penelitian ini bertujuan untuk menganalisis faktor pendukung perilaku seksual remaja di Provinsi Maluku berdasarkan data Survei Kinerja dan Akuntabilitas Program Kependudukan, Keluarga Berencana dan Pembangunan Keluarga (SKAP) Remaja Tahun 2019. Penelitian ini merupakan penelitian kuantitatif yang bersifat analitik korelatif dengan pendekatan cross sectional study. Besar sampel dalam analisis 241 remaja yang berusia 10-24 tahun dan belum menikah. Hasil regresi logistik menunjukkan lima variabel keterpaparan informasi yang signifikan berhubungan dengan perilaku seksual remaja, antara lain keterpaparan informasi tentang KB (p=0,011), keterpaparan informasi tentang generasi berencana (p=0,015), keterpaparan informasi tentang IMS (p<0,001), keterpaparan informasi tentang HIV/AIDS (p=0,001), keterpaparan informasi tentang NAPZA (p=0,026) dan diskusi haid/mimpi basah (p<0,001). Disarankan perlunya inovasi dalam saluran pemberian informasi, tidak hanya media massa tetapi juga melalui media sosial seperti Instagram, facebook, twitter, melalui youtube ataupun juga melalui aplikasi khusus mengenai kesehatan reproduksi.

  • Religiositas Dan Kelengkapan Imunisasi Dasar Pada Anak

      Abstract: Information on the absence of basic immunization due to religious reasons and the general overview of immunization coverage among religious believers in Indonesia is still limited. This study aims to examine the relationship between parental religiosity and basic immunization status on children because there are still many children who are not fully immunized in Indonesia. This study used cross-sectional data of the Indonesia Family Life Survey (IFLS) 2007 and 2014 with the unit of analysis for families having children aged 1-5 years. Our estimations using ordered logistic regression confirm that there is a relationship between religiosity on the parents' decision to perform basic immunization on their children. The differences in religiosity patterns in 2007 and 2014 illustrate changes in individual religious behavior towards the basic immunization program. These changes occur through religious social support. Abstrak Informasi tidak dilakukannya imunisasi dasar karena alasan agama masih terbatas dan gambaran cakupan imunisasi di antara pemeluk agama di Indonesia secara aktual belum dapat diketahui. Penelitian ini bertujuan untuk menjelaskan hubungan antara religiositas orang tua dengan status imunisasi dasar pada anak dikarenakan masih banyaknya anak yang tidak diberikan imunisasi secara lengkap di Indonesia. Penelitian ini menggunakan desain cross sectional dengan sumber data Indonesia Family Life Survey (IFLS) Tahun 2007 dan 2014 dengan unit analisis keluarga yang memiliki anak usia 1-5 tahun dan dianalisis menggunakan regresi ordered logistic. Hasil analisis menunjukkan adanya hubungan antara religiositas terhadap keputusan orang tua untuk melakukan imunisasi dasar pada anaknya. Perbedaan pola religiositas tahun 2007 dan 2014 menggambarkan adanya perubahan perilaku beragama individu terhadap program imunisasi dasar. Perubahan tersebut dapat terjadi melalui dukungan sosial keagamaan.

  • Rokok Dan Hipertensi

      Abstract: Hypertension was one of the non-communicable diseases that may cause death in Indonesia. Through Healthy Indonesia Program with Family Approach (PIS-PK), the public health centers conduct home visits to collect family’s health status, such as risk factors and health outcomes involving smoking and hypertension. This study investigates the correlation of smoking and hypertension in two heath centers to the lowest and highest healthy family index (IKS) in Metro City, Lampung Province. The data analysis used multiple logistic regression. The results indicated Puskemas Mulyojati had 11.18% of people diagnosed with hypertension, while Puskesmas Iringmulyo had 5.14%. More smokers were higher in Mulyojati (27.30%) than those in Iringmulyo (23.38%). The proportion of smokers with hypertension in Mulyojati was 2.40% (OR 0.70; 95%CI 0.59-0.84; p 0.006) and in Iringmulyo was 1.09% (OR 0.87 95%CI 0.66-1.14; p 0.115). In Mulyojati, the odds of smokers contracting hypertension was 0.56 times lower than the non-smokers (95%CI 0.44-0.71; p <0,000); meanwhile, the smoking-hypertension relationship in Iringmulyo was not significant despite a bigger odds ratio (OR 0.83, 95%CI 0.59-1.17; p 0.293.) As a recommendation, both Puskesmas promote campaigns for a smoke-free movement, smoke-free areas and smoking cessation counseling to reduce the prevalence of hypertension. Abstrak Hipertensi merupakan salah satu penyakit tidak menular penyebab kematian di Indonesia. Melalui Program Indonesia Sehat dengan Pendekatan Keluarga (PIS-PK), puskesmas melakukan kunjungan rumah dan mengumpulkan data profil kesehatan keluarga, meliputi faktor risiko dan kejadian penyakit termasuk kebiasaan merokok dan hipertensi. Penelitian ini bertujuan mengetahui hubungan antara kebiasaan merokok dengan hipertensi di dua puskesmas yang memiliki Indeks Keluarga Sehat (IKS) terendah dan tertinggi di Kota Metro, Provinsi Lampung. Data dianalisis dengan regresi logistik berganda. Sebanyak 11,18% orang di Puskesmas Mulyojati didiagnosis hipertensi, sedangkan Puskesmas Iringmulyo sebesar 5,14%. Perokok lebih banyak ditemukan di Puskemas Mulyojati (27,30%) daripada di Puskesmas Iringmulyo (23,38%). Proporsi orang yang merokok dan didiagnosis hipertensi di Puskesmas Mulyojati sebanyak 2,40% (OR 0,70; IK 95% 0,59-0,84; p 0,000) sedangkan di Puskesmas Iringmulyo 1,09% (OR 0,87; IK 95% 0,66-1,14; p 0,325). Odd rasio orang merokok untuk mengidap hipertensi 0,56 kali dibandingkan dengan orang yang tidak merokok dengan hasil signifikan (IK 95% 0,44-0,71; p <0,000) di Puskesmas Mulyojati. Di Puskesmas Iringmulyo, meskipun odd rasionya sebesar 0,83, namun hasilnya tidak signifikan (IK 95% 0,59-1,17; p 0,293). Kedua Puskesmas disarankan untuk melakukan kampanye gerakan tanpa asap rokok, area bebas rokok dan konseling berhenti merokok untuk menurunkan prevalensi hipertensi.

  • Kepemilikan Asuransi Kesehatan Dan Hubungannya Dengan Persalinan Pada
           Dukun Bayi

      Abstract: Delivery by traditional birth attendants (TBAs) has become a culture in several regions in Indonesia. The absence of health insurance ownership and lack of knowledge have supported the decision to choose TBA by the expectant mothers. This study aimed to determine the relationship between health insurance ownership with delivery by TBA. This research was a cross-sectional study with 120 samples of women aged 15-49 years who had ever given birth in the last five years and lived in Sangiangtanjung Village, Lebak, Banten. Multivariate logistic regression analysis was used in this study consist of health insurance ownership with delivery by TBAs controlled by age, education, socioeconomic, knowledge, antenatal care provider, frequency of antenatal care, and danger of pregnancy. The results showed that women who did not have health insurance were 3.20 times higher (95%CI= 1.10-9.30) for choosing delivery by TBAs compared to women who had health insurance after being controlled by knowledge. This study concluded that there was a significant relationship between health insurance ownership with delivery by TBAs. This study suggests that it is needed to conduct maternity care about health insurance ownership and positive health-seeking behavior, and the system of integrating TBAs with skilled birth attendants especially midwives in which TBAs as birth companions. Abstrak Persalinan oleh dukun bayi sudah menjadi budaya di beberapa daerah di Indonesia. Tidak adanya jaminan kesehatan dan pengetahuan menjadi pendorong dalam pemilihan dukun sebagai penolong persalinan. Penelitian bertujuan mengetahui hubungan antara kepemilikan asuransi kesehatan dengan persalinan oleh dukun bayi. Penelitian kuantitatif ini menggunakan desain potong lintang dengan 120 sampel. Sampel merupakan wanita berusia 15-49 tahun yang melahirkan dalam lima tahun terakhir dan tinggal di Desa Sangiangtanjung, Lebak, Banten. Analisis multivariat regresi logistik digunakan dalam penelitian ini terdiri dari kepemilikan asuransi kesehatan dengan persalinan oleh dukun bayi dan dikontrol oleh usia ibu, pendidikan ibu, sosial ekonomi, pengetahuan, tenaga pemeriksa kehamilan, frekuensi pemeriksaan kehamilan, dan bahaya kehamilan. Hasil penelitian menunjukkan bahwa ibu yang tidak memiliki asuransi kesehatan 3,20 kali lebih tinggi (95% CI = 1,10-9,30) untuk melakukan persalinan oleh dukun bayi dibandingkan dengan ibu yang memiliki asuransi kesehatan setelah dikontrol oleh pengetahuan. Penelitian ini menyimpulkan bahwa terdappat hubungan antara kepemilikan asuransi kesehatan dengan persalinan oleh dukun bayi. Diperlukan asuhan maternitas tentang kepemilikan asuransi dan perilaku pencarian kesehatan yang positif, serta pengintegrasian dukun bayi dengan tenaga kesehatan terutama bidan di mana dukun bayi perlu ditetapkan kembali perannya sebagai pendamping kelahiran.

  • Pengelompokan Provinsi berdasarkan Penyakit Menular dan Penyakit Tidak
           Menular untuk Upaya Pengendalian Penyakit dengan Pendekatan
           Multidimensional Scaling (MDS)

      Abstract: This study aims to group provinces in Indonesia based on the prevalence of communicable and non-communicable diseases (CDs and NCDs) for disease control efforts. The results of grouping can find out the priority of communicable and non-communicable disease control areas based on seven variables related to infectious diseases and ten variables related to NCDs based on Basic Health Research 2018. A Multidimensional Scaling (MDS) technique was used as the analytical strategy. The MDS analysis resulted in four groups of provinces based on the prevalence of CDs and NCDs. Provincial groups with the highest prevalence of infectious diseases (group 2) were NTT, Central Kalimantan, Maluku, West Papua, and Papua. Provincial groups with the highest NCDs prevalence (group 3) were Bangka Belitung, DKI Jakarta, DI Yogyakarta, East Kalimantan, North Kalimantan, and North Sulawesi. The two groups of provinces were the priority groups in controlling CDs and NCDs. The focus of communicable disease control is URI, hepatitis, malaria, and filariasis in the highest priority groups of provinces with the highest prevalence of infectious diseases. In groups of provinces with the highest NCDs prevalence, the NCD control should focus on asthma, cancer, diabetes, heart disease, hypertension, stroke, chronic renal failure, and joint disease. Further research is suggested adding risk factor analysis variables for CDs and NCDs using the MDS method to help provides a more comprehensive picture of regional groupings. Coordination between central and local governments is needed to accelerate efforts to control CDs and NCDs in priority area groups. Abstrak Tujuan penelitian ini adalah melakukan pengelompokan provinsi di Indonesia berdasarkan prevalensi penyakit menular (PM) dan penyakit tidak menular (PTM) dalam upaya pengendalian penyakit. Hasil pengelompokan dapat diketahui prioritas wilayah pengendalian PM dan PTM berdasarkan tujuh variabel terkait PM dan 10 variabel terkait PTM dari Riset Kesehatan Dasar (Riskesdas) 2018. Jenis penelitian ini adalah cross-sectional menggunakan data sekunder Riskesdas 2018. Analisis pengelompokan provinsi menggunakan  Multidimensional Scaling (MDS). Analisis MDS menghasilkan empat kelompok provinsi berdasarkan prevalensi PM dan PTM. Kelompok provinsi dengan prevalensi PM tertinggi (kelompok 2) adalah NTT, Kalimantan Tengah, Maluku, Papua Barat dan Papua. Kelompok propinsi dengan prevalensi PTM tertinggi (kelompok 3) adalah Bangka Belitung, DKI Jakarta, DI Yogyakarta, Kalimantan Timur, Kalimantan Utara, dan Sulawesi Utara. Kedua kelompok provinsi ini merupakan kelompok provinsi prioritas dalam pengendalian PM dan PTM. Pada kelompok provinsi dengan PM tertinggi, fokus pengendalian PM adalah ISPA, hepatitis, malaria dan filariasis. Fokus pengendalian untuk PTM adalah asma, kanker, diabetes, penyakit jantung, hipertensi, stroke, gagal ginjal kronis, dan penyakit sendi. Penelitian selanjutnya disarankan menambahkan variabel analisis faktor risiko PM dan PTM dengan menggunakan metode MDS untuk membantu memberi gambaran yang lebih lengkap pada pengelompokan wilayah. Diperlukan koordinasi sinergi antara pemerintah pusat dan daerah untuk percepatan upaya pengendalian PM dan PTM di kelompok wilayah prioritas.
