  • Faktor-faktor Determinan Perilaku Mencuci Tangan Setelah Masa
           ‘Adaptasi Kebiasaan Baru’

      Abstract: Changes in policies implemented in Indonesia before and after the New Normal policy affect the prevention habits that have been implemented by the community. Therefore, a study to determine factors affecting washing hands behavior after the implementation of the new normal policy during the COVID-19 period has been conducted in order to address this issue. This study uses two analyzes, the paired T-test which aims to determine whether there are differences in handwashing behavior among respondents before and after the new normal period, and a multivariate test using logistic regression analysis which aims to determine the factors that influence the lack of hand washing behavior. The results of this study's data analysis showed that there was a decrease in the number of respondents who had washed their hands well before and after new normal policy (73.75% to 67.75%). The results of the analysis also show that the lack of hand washing habits is related to factors such as education, intensity of seeking information related to COVID-19, have had positive for COVID-19, and anxiety due to information received. Meanwhile, age, gender, and income have no effect on this behavior. Hopefully, these results can be used as a foundation for the making of policies to improve health promotion and education on proper handwashing to targeted communities, such as people with low education level, rarely seek information, or are not anxious about COVID-19 disease. Image media can also be used to increase the interest of these targeted people towards information related to COVID-19. Abstrak Perubahan kebijakan yang diterapkan di Indonesia sebelum dan setelah kebijakan “New normal”/Adaptasi kebiasaan baru (AKB) dapat mempengaruhi upaya pencegahan penyakit di masyarakat. Informasi terkait perubahan perilaku masyarakat setelah diberlakukannya suatu kebijakan pemerintah sangat penting diketahui. Oleh karena itu, studi tentang faktor-faktor penentu kebiasaan mencuci tangan setelah diterapkannya kebijakan Adaptasi Kebiasaan Baru (“New normal”) AKB pada masa COVID-19 telah dilakukan untuk menjawab persoalan tersebut. Studi ini melakukan dua analisis yaitu uji beda dengan menggunakan uji T berpasangan yang bertujuan untuk menentukan apakah terdapat perbedaan perilaku mencuci tangan pada responden sebelum dan setelah masa adaptasi kebiasaan baru dan uji multivariat menggunakan analisis regresi logistik yang bertujuan untuk menentukan faktor yang berpengaruh pada kurangnya perilaku mencuci tangan pada responden. Hasil analisis data studi ini menunjukkan bahwa terjadi penurunan jumlah responden yang tadinya mencuci tangan dengan baik pada masa sebelum adaptasi kebiasaan baru (73,75%) menjadi kurang baik di masa setelah adaptasi kebiasaan baru (67,75%). Hasil analisis juga menunjukkan bahwa kurangnya kebiasaan mencuci tangan berhubungan dengan faktor-faktor seperti pendidikan, intensitas pencarian informasi terkait COVID-19, pernah positif COVID-19, dan kecemasan akibat informasi yang diterima. Sedangkan umur, gender, dan pendapatan tidak berpengaruh terhadap perilaku ini. Diharapkan hasil studi ini dapat digunakan sebagai dasar untuk pengembangan kebijakan promosi dan edukasi kesehatan terkait kebiasaan cuci tangan yang benar terhadap orang-orang yang ditargetkan, seperti orang-orang dengan tingkat pendidikan rendah, jarang mencari informasi, atau orang-orang yang tidak khawatir terhadap penyakit COVID-19. Media gambar juga dapat digunakkan untuk meningkatkan ketertarikan masyarakat yang ditargetkan ini terhadap informasi terkait COVID-19.

  • Fungsi Kognitif Anak Dengan Epilepsi Umum

      Abstract: Children with epilepsy may experience cognitive impairment due to the effects of structural lesions or seizure activity. This study aims to determine the cognitive function of children with general epilepsy. This study was a cross-sectional design that included children aged 6 to 68 months with general epilepsy in the polyclinic and pediatric ward of Sanglah Hospital Denpasar from September 2013 to January 2015. All children who participated in this study were subjected to a Mullen scale examination. Statistical analysis was performed using Chi-square test and multivariate analysis with logistic regression. In 93 children with general epilepsy, there was a significant correlation between the level of cognitive function and seizure frequency >10 times with an odds ratio 6.067 (95% CI 2.002-18.388, p value 0.001) and seizure duration ≥15 minutes with an odds ratio 6.006 (95% CI 1,817-20,246, p value 0.003). In this study, it can be concluded that seizure frequency >10 times and seizure duration ≥15 minutes in children with general epilepsy have a positive relationship of 6 times with the abnormal cognitive function. Further studies are needed to determine cognitive development of children with general epilepsy. Abstrak Anak dengan epilepsi dapat mengalami gangguan kognitif berkaitan dengan efek dari lesi struktural maupun aktivitas kejang. Penelitian ini ditujukan untuk mengetahui fungsi kognitif anak dengan epilepsi umum. Penelitian ini merupakan penelitian dengan desain potong lintang yang mengikutsertakan anak usia 6 sampai 68 bulan dengan epilepsi umum di poliklinik dan ruang perawatan anak RSUP Sanglah Denpasar pada bulan September 2013 hingga Januari 2015. Semua anak yang ikut serta dalam penelitian dilakukan pemeriksaan skala Mullen. Analisis statistik dilakukan dengan uji kai kuadrat dan analisis multivariat dengan regresi logistik. Pada 93 anak dengan epilepsi umum, didapatkan hubungan yang signifikan antara fungsi kognitif dengan frekuensi kejang >10 kali dengan rasio odds 6,067 (nilai p=0,001, 95% CI 2,002-18,388) dan lama kejang ≥15 menit dengan rasio odds 6,006 (nilai p=0,003, 95% CI 1,817-20,246). Pada penelitian ini dapat disimpulkan bahwa frekuensi kejang >10 kali dan lama kejang ≥15 menit pada anak dengan epilepsi umum memiliki hubungan yang positif sebesar 6 kali terhadap fungsi kognitif abnormal. Penelitian lanjutan perlu dilakukan untuk mengetahui perkembangan fungsi kognitif anak dengan epilepsi umum.

  • Implementasi Program Pelayanan Kesehatan Lanjut Usia Di Dinas Kesehatan
           Provinsi Jawa Timur

      Abstract: The elderly is vulnerable group because they suffer from many diseases (multimorbidity), functional disability and dependence on others. East Java Province has an old population structure so that it needs good handling, especially when related to health. This study aimed to analyze the implementation of the elderly health service program carried out by the East Java Provincial Health Office. This research was an observational study with a cross sectional design. Data collection employed in-depth interviews, focus group discussions, review of coverage data and related regulatory documents. Data were analized descriptively. The results showed that the East Java Provincial Health Office had a policy baseline for the Elderly health program, where the program included activities inside and outside the building. Program barriers include infrastructure, budget, human resources, and policies. Efforts to improve the elderly program are carried out with long-term care, advocacy, training and mentoring at the Puskesmas. The coverage of elderly health services in East Java Province are: 1) hospitals with Integrated Geriatric Team services (3.13%), 2) Elderly Posyandu integrated with Posbindu (7.3%) and 3) Community Health Centers for the Elderly (42.9%). As recommendation, it is necessary to increase the capacity building of human resources through training as well as to ensure the availability of facilities and infrastructure for elderly health services at Health Center. Abstrak Lansia merupakan kelompok rentan karena mengidap banyak penyakit (multimorbiditas), ketidakmampuan secara fungsional dan ketergantungan pada orang lain. Provinsi Jawa Timur memiliki struktur penduduk tua, sehingga perlu penanganan yang baik terutama terkait kesehatan. Penelitian ini bertujuan menganalisis implementasi program pelayanan kesehatan Lansia yang dilaksanakan oleh Dinas Kesehatan Provinsi Jawa Timur. Penelitian ini merupakan penelitian observasional dengan desain cross-sectional. Pengumpulan data dilakukan dengan wawancara mendalam, diskusi kelompok terarah, telaah data cakupan dan dokumen regulasi terkait. Analisis data secara deskriptif. Hasil penelitian menunjukkan Dinas Kesehatan Provinsi Jawa Timur telah memiliki landasan kebijakan program kesehatan Lansia, di mana kegiatan program meliputi kegiatan dalam gedung Puskesmas Santun Lansia dan kegiatan luar gedung. Hambatan program meliputi, sarana prasarana, anggaran, SDM dan kebijakan. Upaya perbaikan program Lansia dilakukan dengan perawatan jangka panjang, advokasi, pelatihan dan pendampingan Puskesmas. Cakupan pelayanan kesehatan Lansia di Provinsi Jawa Timur yaitu: 1) RS dengan pelayanan Tim Geriatri Terpadu (3,13%), 2) Posyandu Lansia yang terintegrasi dengan Posbindu (7,3%) dan 3) Puskesmas santun Lansia (42,9%). Direkomendasikan, perlunya peningkatan kapasitas SDM melalui pelatihan serta menjamin ketersediaan sarana dan prasarana untuk Yankes Lansia di Puskesmas.

  • Faktor Risiko Hipertensi Pada Usia Produktif Di Indonesia Dan Upaya
           Penanggulangannya

      Abstract: The prevalence of hypertension in the world is about 1.13 billion. Most people with hypertension are undiagnosed and unaware that they have it. The low level of medication in patients with hypertension affects uncontrolled blood pressure. The long-term effect may risk cardiovascular disease, kidney failure, stroke and even death. The study aims to determine the prevalence, awareness, treatment, control, and risk factors of hypertension toward productive age in Indonesia. The study design was cross-sectional with secondary data analysis from the Indonesian Family Life Survey (IFLS) 5 with the sample aged 15-64 years. The Chi-square test is applied to see the correlation between variables with p-values of 0.05 and 95%CI. The findings indicated hypertension prevalence (22.49%), awareness (10.60%), treatment (23.73%) and control (27.97%). Significant factors risk hypertension were obesity (PR 2.75; 95% CI 2.59-2.93), aged groups; 25-34 years (PR 1.51; 95% CI 1.35-1.68), 35-44 years (PR 3.52; 95%CI 3.17-3.90), 45-54 years (PR 7.89; 95% CI 7.11-8.77), 55-64 years (PR 13.36; 95% CI 11.92-14.97). Other variables showed no significant relationships. The variables such as low awareness, treatment, and control of hypertension for productive age increase the morbidity and mortality of non-communicable diseases in the future. Health screening efforts must be carried out by a routine blood pressure check for productive ages and optimizing the role of Integrated Non-Communicable Diseases Post (Posbindu PTM).   Abstrak Prevalensi hipertensi di dunia sekitar 1,13 miliar. Kebanyakan penderita hipertensi tidak terdiagnosis dan tidak menyadari bahwa mereka menderita hipertensi. Rendahnya konsumsi obat pada penderita hipertensi membuat tekanan darah tidak terkontrol. Efek jangka panjangnya bisa berisiko penyakit kardiovaskular, gagal ginjal, stroke, bahkan kematian. Penelitian ini bertujuan untuk mengetahui prevalensi hipertensi yang diaganosis, pengobatan, terkontrol dan faktor risiko hipertensi pada usia produktif di Indonesia. Desain penelitian adalah cross-sectional dengan analisis lebih lanjut dari data Indonesian Family Life Survey (IFLS) 5 dengan sampel 15-64 tahun. Data dianalisis dengan uji chi-square untuk melihat hubungan variabel dengan p-value 0,05 dan 95% CI. Hasil studi menunjukkan prevalensi hipertensi (22,49%), awareness (10,60%), treatment (23,73%) dan control (27,97%). Faktor signifikan risiko hipertensi adalah obesitas (PR 2,75; IK 95%  2,59-2,93), kelompok umur; 25-34 tahun (PR 1,51; IK 95% 1,35-1,68), 35-44 tahun (PR 3,52; IK 95% 3,17-3,90), 45-54 tahun (PR 7,89; IK 95% 7,11-8,77) , 55-64 tahun (PR 13.36; IK 95%11.92-14.97). Variabel lain tidak menunjukkan hubungan yang signifikan. Rendahnya awareness, treatment, dan control hipertensi pada usia produktif akan meningkatkan morbiditas dan mortalitas penyakit tidak menular di masa yang akan datang. Upaya skrining kesehatan harus dilakukan dengan rutin memeriksakan tekanan darah bagi usia produktif dengan mengoptimalkan peran Pos Binaan Terpadu Penyakit Tidak Menular (Posbindu PTM).

  • Peran Rumah Tangga Dan Lingkungan Tempat Tinggal Terhadap Kondisi Air
           Minum Sumur Gali Di Indonesia

      Abstract: Water is essential for human life. The availability of decent dan safe water for drinking, food processing, and day to day necessity is required to prevent water-borne diseases. This study analyzes the role of household factors (compositional) and rural/urban village factors (contextual) on the condition of dug-wells water in Indonesia. The analysis used data integration of the 2018 Basic Health Research (Riskesdas) – the National Socioeconomic Survey (Susenas)in March 2018 and the 2018 Village Potential (Podes) data. These three surveys employed a cross-sectional design. We applied multilevel logistic regression modeling using STATA in the analysis. Samples were households that used dug wells in 12,259 rural/urban villages in Indonesia. The result found that 76,5% of households use basic drinking water facilities, in which 54% were in poor condition. The role of household-level (level 1) and rural/urban village-level (level 2) were almost the same, which were 50.13% and 49.87% respectively. In conclusion, there were no differences between household factors and rural/urban village factors in explaining the condition of drinking water from dug wells in Indonesia. To maintain proper and safe dug-wells-drinking-water facilities for all family members, it is necessary to provide environmental education to the community, to encourage community service and to reforest the living environment. Abstrak Air sangat dibutuhkan dalam kehidupan manusia. Ketersediaan air yang layak dan aman sangat diperlukan oleh masyarakat untuk keperluan minum, pengolahan makanan dan kebutuhan sehari-hari agar dapat terhindar dari penyakit yang ditularkan melalui air. Penelitian ini menganalisis peranan faktor rumah tangga (komposisional) dan faktor desa/kelurahan (kontekstual) terhadap kondisi air minum sumur gali di Indonesia. Analisis menggunakan data integrasi Riset Kesehatan Dasar (Riskesdas) 2018 - Survei Sosial Ekonomi Nasional (Susenas) Maret 2018 dan Potensi Desa (Podes) 2018. Ketiga survei tersebut menggunakan rancangan potong lintang. Analisis menggunakan pemodelan multilevel logistic regression dengan program STATA. Sampel diambil dari rumah tangga yang menggunakan sumur gali pada 12.259 desa/kelurahan di Indonesia. Dari hasil diketahui bahwa 76,5% rumah tangga menggunakan sarana air minum dasar, dengan 54% kondisinya tidak baik. Besar peran tingkat rumah tangga (level 1) dan desa/kelurahan (level 2) hampir sama besar yaitu 50,13% dan 49,87%. Dapat disimpulkan bahwa tidak ada perbedaan faktor rumah tangga dan desa dalam menjelaskan kondisi air minum dari sumur gali di Indonesia. Untuk menjaga keberlangsungan sarana air minum sumur gali yang layak dan aman bagi seluruh anggota keluarga, diperlukan penyuluhan lingkungan bagi masyarakat, menggiatkan kerja bakti dan melakukan penghijauan lingkungan tempat tinggal.

  • Tantangan Kebijakan Satu Data Indonesia

      Abstract: Accurate vital statistics can be used to formulate health programs and policies. The problem is the overlapping and fragmented data sources in several agencies that record data on deaths and causes of death. The Indonesia’s One Data policy was created to organize the data generated by each agency that performs public data collection, recording and reporting. The purpose of this study is to examine the challenges of implementing the Indonesia’s One Data policy in the registration system of deaths and causes of death in DKI Jakarta Province. Data analysis uses the World Wide Web Foundation's framework of data openness, including legal frameworks, political barriers, coordination, and data quality. This study found that there are many regulations regarding data integration, but there are still overlaps in the recording and reporting systems. Coordination between agencies and political egos of each agency affect the quality of the data produced. In conclusion, weak coordination and sectoral political egos are still obstacles to implementing the One Data Indonesia policy. This study suggests eliminating data fragmentation and conducting data interoperability so that it is more accurate to use as a basis for policymaking. Abstrak Statistik hayati kematian dan penyebab kematian yang akurat dapat dimanfaatkan sebagai dasar penyusunan program dan kebijakan kesehatan berbasis bukti. Permasalahan yang ada adalah sumber data yang tumpang tindih dan terfragmentasi di beberapa instansi yang melakukan fungsi pencatatan data kematian dan penyebab kematian. Kebijakan Satu Data Indonesia dibuat untuk mengorganisir data yang dihasilkan oleh setiap instansi yang melakukan pengumpulan, pencatatan dan pelaporan data publik. Tujuan penelitian ini mengkaji gambaran tantangan implementasi kebijakan Satu Data Indonesia pada sistem pencatatan kematian dan penyebab kematian di Provinsi DKI Jakarta. Analisis data menggunakan teori menurut World Wide Web Foundation tentang keterbukaan data yang terdiri dari variabel legal framework, political barriers, koordinasi, serta kualitas data. Hasil penelitian ini menemukan dari sisi regulasi telah banyak diatur tentang integrasi data, tetapi masih ada tumpang tindih sistem pencatatan dan pelaporan. Koordinasi antar instansi dan ego politik masing-masing instansi berpengaruh pada kualitas data yang dihasilkan. Kesimpulannya, hambatan koordinasi dan ego politik sektoral masih menjadi kendala dalam pelaksanaan kebijakan Satu Data Indonesia. Penelitian ini menyarankan untuk membuka fragmentasi data dan melakukan koordinasi interoperabilitas data sehingga data yang dihasilkan lebih akurat untuk digunakan sebagai dasar pembuatan kebijakan.

  • Faktor Yang Berhubungan Dengan Kejadian Cedera pada Pengendara Sepeda
           Motor Siswa SMK Negeri 1 Puring Kabupaten Kebumen

      Abstract: Traffic accidents are the biggest cause of injury. Two of the three main causes of death are injuries from traffic accidents, which are expected to increase further than other causes of death. The number of injuries caused by riding in Kebumen District from 2014 to 2016 involves mainly motorcycles, occurring in the 15-30 year age group, with private and student status that did not possess any driving license. This study determines factors related to the incidence of injury on motorcyclist students of 1 State Vocational High School Puring, Kebumen District academic year 2016/2017 based on host factors, agent factors, and environmental factors. This study was a cross-sectional design with a random sampling technique on 313 motorcyclists. Questionnaires disseminated as the data collection. The analysis was either univariate or bivariate with a significance level p-value<0.05. Drivers were dominantly male. Most occupation of the parents was farmers. The majority of motorists never participate in any safe driving socialization, and the average age was 11 years old when they started to ride a motorcycle. The average mileage per day was 11 km. Bivariate analysis results no correlation among sex, parental occupation, age of first started to ride a motorcycle, use of personal protective equipment, the safe driving socialization, the vehicle feasibility, the driving license ownership, and parental support for safe driving with the incidence of injury. There was a relationship between distance to school, knowledge of safe driving, and friend's motivation to safe driving with injuries. Abstrak Kecelakaan lalu lintas merupakan penyebab cedera terbesar. Salah satu penyebab utama kematian adalah cedera akibat kecelakaan lalu lintas, yang diperkirakan terus meningkat dibandingkan dengan penyebab kematian lain. Jumlah kejadian cedera akibat berkendara di Kabupaten Kebumen tahun 2014−2016, banyak melibatkan kendaraan sepeda motor dengan pengendara usia 15−30 tahun, berstatus swasta, pelajar serta tidak memiliki SIM. Studi ini mengidentifikasi faktor yang berhubungan dengan kejadian cedera pada pengendara sepeda motor siswa SMK Negeri 1 Puring Kabupaten Kebumen tahun ajaran 2016/2017 berdasarkan host, agent dan environment factors. Penelitian menggunakan desain potong lintang. Teknik pengambilan sampel menggunakan random sample pada 313 pengendara sepeda motor. Pengumpulan data menggunakan kuesioner. Analisis data dilakukan dengan univariat dan bivariatengan tingkat signifikansi p-value < 0.05. Terdapat pengendara terbanyak usia 16 tahun sejumlah 145 pengendara, pengendara yang berusia 17˗21 tahun sejumlah 129 pengendara, didominasi laki-laki dan pekerjaan orangtua petani. Mayoritas pengendara tidak mengikuti sosialisasi berkendara yang aman. Sejak usia 11 tahun mereka mulai aktif mengemudikan sepeda motor dengan Jarak tempuh 11 km perhari. Hasil analisis bivariabel menunjukkan tidak ada keterkaitan antara usia, jenis kelamin, pekerjaan orangtua, usia awal pengendara sepeda motor mulai aktif mengemudikan sepeda motor, penggunaan alat pelindung diri, sosialisasi berkendara yang aman, kepemilikan SIM C, faktor kelayakan kendaraan dan dukungan orangtua dalam berkendara yang aman dengan kejadian cedera. Terdapat hubungan antara jarak tempat tinggal dengan sekolah, pengetahuan tentang berkendara yang aman dan dukungan teman dalam berkendara yang aman dengan kejadian cedera. Dukungan teman sebaya menjadi faktor paling dominan menentukan terjadinya cedera. Siswa yang tanpa dukungan berkendara aman memiliki risiko cedera. Maka, sosialisasi berkendara oleh pihak kepolisian dan kolaborasi dengan sekolah menjadi hal yang wajib untuk siswa berkendara aman dan selamat di jalan.

  • Proyeksi Admisi Rawat Inap Dan Kebutuhan Tempat Tidur di Rumah Sakit an
           Puskesmas dalam Universal Health Coverage Di Indonesia

      Abstract: Strengthening service facilities is an important aspect of universal health complaints and this is consistent with the achievement of sustainable development goals (SDGs). The bed occupancy rate in the inpatient facility a measure of the ability of inpatient facilities to function safely and effectively. This study aims to find out the projection of inpatient admissions and beds needs in hospitals and Public Health Center in Indonesia. Analysis to projection was conducted used the Need based model method with 2015 population reference from SUPAS 2015. The results showed that the yearly inpatient admission rate in 2023 increased by around 71% (115,554 TT) compared to 2019. Conclusion The projection of inpatient admissions for PHC in 2023 increases by 20%, the average number of days of hospitalization increases by 18.38%, and the number of Beds increases by 29% compared to 2018. The projection of hospital admissions in 2023 increases by 60%, the average day of hospitalization increases by 81.62%, and the number of inpatient beds increased by 71% compared to 2018. The suggestions are needed to build inpatient facilities, strengthen the FKTP, IT development to with applications or software model need based. Abstrak Penguatan fasilitas layanan merupakan aspek penting dalam pencapaian cakupan kesehatan universal dan ini sejalan dengan pencapaian sustainable development goals (SDGs). Akses dan ketersediaan rawat inap menjadi perhatian utama untuk memenuhi kebutuhan peserta JKN. Tingkat hunian di fasilitas rawat inap telah diakui sebagai ukuran kemampuan fasilitas rawat inap agar berfungsi secara aman dan efektif. Untuk memenuhinya diperlukan estimasi admisi dan kebutuhan tempat tidur. Tujuan penelitian ini untuk memproyeksikan kebutuhan tempat tidur rawat inap di Indonesia tahun 2023. Analisis proyeksi menggunakan metode Need based model dengan acuan penduduk Indonesia SUPAS tahun 2015. Hasil penelitian menunjukkan tingkat admisi rawat inap tahun di tahun 2023 meningkat sekitar 71% (115.554 TT) dibanding dengan tahun 2019 dikarenakan proyeksi pertambahan jumlah penduduk Indonesia dan kebutuhan menuju Universal Health Coverage. Kenaikan ini menyebabkan kebutuhan tempat tidur rawat inap meningkat. Kesimpulan Proyeksi admisi rawat inap Puskesmas tahun 2023 naik 20%, rerata hari rawat inap naik 18,38%, dan jumlah TT naik 29% dibanding tahun 2018. Proyeksi admisi rawat inap RS tahun 2023 naik 60%, rata hari rawat inap naik 81,62%,dan jumlah tempat tidur rawat inap naik 71% dibanding 2018. Saran: Pemenuhan FKTP selain PKM yaitu klinik dan praktek dokter dan FKTL dengan peningkatan kelas RS. Penguatan regulasi pola rujukan berjenjang yang komprehensif melibatkan swasta. Penguatan SDM serta pembenahan sarana prasarana FKTP dan FKTL yang memadai sesuai kebutuhan. Optimalisasi peran pemerintah dalam penetapan kebutuhan tempat tidur, membuka seluas-luasnya kesempatan kepada klinik dan RS swasta untuk bekerjasama dengan BPJS.
