  • Volume 18 Issue 1 - An intervention to improve confidence in nursing
           students from culturally and linguistically diverse backgrounds: A pilot
           study

      Background Communication skills are essential for healthcare professionals. Nursing students from culturally and linguistically diverse backgrounds are more likely to face challenges when communicating with patients in a healthcare setting.

      Method This pilot study was conducted at the University of South Australia in 2018 and involved seven undergraduate nursing students. The participants' perceived self-confidence in their communication skills was assessed using a self-administered questionnaire before and after the intervention of immediate, personalised feedback following a scenario. The aim was to determine the effectiveness of the developed questionnaire for potential future use in larger studies. Additionally, the collected data was entered into Graph Pad software and analysed using a Wilcoxon test.

      Results All participants involved in this study showed a statistically significant improvement in their perceived self-confidence after the provision of immediate, personalised feedback about their communication skills.

      Conclusions The developed tool was effective in obtaining the required data from participants and could be used in studies with a larger population.

      Colin Street Day Surgery (CSDS), operational since 1987, is a freestanding, private day hospital located in Perth. The facility has two GA theatres, five recovery beds and five recovery chairs, and conducts surgeries for both adult and paediatric patients. Up to last year, after 31 years of operation, CSDS was fortunate to have not had one notifiable event. This was to change in August 2018.

  • Volume 18 Issue 2 - Eight techniques to support high-quality perioperative
           communication: Reflections on the NASSF WA survey

      It is widely accepted that communication is critical in the perioperative context as it has important implications for both patients and clinical staff. In this article, we report the findings of a survey of Nurses Association of Short Stay Facilities Western Australia (NASSF WA) members to identify common barriers to effective communication as well as to identify techniques that may contribute to raising the bar on communication. Results of the survey point to perceived power differences, hierarchical work structures, multiple steps in communication chains, and a general reluctance to speak up as primary barriers to effective communication. We also identify eight potentially under-utilised techniques designed to bolster communication. First we identify three techniques designed to enhance communication through structure - ISOBAR, huddles or briefings, and stand back. We then highlight two techniques that ensure the message is understood - closed loop communication and readback. Finally, we identify three techniques to support communication transmission - using graded assertiveness techniques, for example the PACE/CUSS models, having a designated conduit, and using an individual's name. Each technique is discussed in turn and we conclude with some practical advice on ways in which these techniques can be introduced and practised, with the view of making high-quality communication a norm in daily clinical practice.

      This article explores issues surrounding accessing Aboriginal and Torres Strait Islander health in the rural Queensland setting. As a diabetes educator, I will address some barriers that are faced on a daily basis, and consider whether some of these are perceived or real. In addition, do these beliefs come from my clients' cultural beliefs and customs or are they encountered due to the current health system' These will be some of the issues I hope to address in this article.

      Many cancers follow a somewhat predictable pathway, beginning as a small lesion that develops through a series of well-defined stages. Ovarian cancer, on the other hand, does not have a clearly defined origin nor stages of development and there are multiple subtypes. There is no clear lesion and low prevalence. This makes ovarian cancer difficult to diagnose and treat. This paper will explore the primary, secondary and tertiary approaches to the prevention and management of ovarian cancer in different populations.

      In a 2016 survey of workforce trends, Deloitte found the single greatest change in the way organisations work is an increasing reliance on teamwork. While an interesting trend overall, teamwork is nothing new to healthcare, let alone the perioperative context that relies heavily on effective coordination both within and between often dynamic teams. When teams work effectively, patients benefit from higher quality of care, the medical system runs more efficiently, and medical staff experience lower levels of stress5,6. While many team-enhancing practices exist3, few have been empirically demonstrated to yield increases in individual and team performance to the extent of debriefing. The secret' Debriefing enhances individual and team learning which in turn fosters the systematic translation of those learnings into actions that ultimately results in positive outcomes such as enhanced patient care. When teams debrief they are investing in their future, not just as a team but also as individuals working in a highly complex and unpredictable context where the stakes often could not be higher. However, despite their potency and widespread use in other high-risk industries, debriefings are largely underutilised in the medical context.

      Since the 1990s until now, we have seen a dramatic increase in opioid use, misuse and overdose in Western countries, and deaths due to opioid overdose has exploded. At the same time, the administration of opioids during general anaesthesia has become almost systematic, using increasing doses over the years. The extensive indications of surgical interventions in the ambulatory setting expose patients to the same risk as the in-patient setting when opioids are used, certainly when used above a reasonable dose.

      Is there a relation between opioid exposure during anaesthesia, postoperative hyperalgesia, hospital readmission, opioid addiction, cancer recurrence, immune system impairment, higher risk of postoperative delirium and cognitive dysfunction' The recent literature is starting to confirm some of these hypotheses. Due to all these questions and the growing evidence of an opioid crisis, the concept of opioid free anaesthesia (OFA) was born. Is this the solution' Is it a real paradigm shift'

      However, it is important to keep in mind that stress-induced postoperative pain has a negative influence on outcome. The use of opioids in the postoperative phase could be necessary but must be titrated and reduced to the minimal required dose, certainly in ambulatory surgery. The possibility of postoperative OFA is an emerging debate in the literature.

Colonoscopies comprise a significant proportion of such endoscopic procedures, and a large majority of colonoscopies involve the removal of colonic polyps. Colonoscopies may be performed for the indication of gastrointestinal symptoms, but a significant proportion of these procedures are performed for screening measures.

      The anaesthetic department at St Vincent's Private Hospital Melbourne works cohesively to pre-empt and manage emergencies in the perioperative environment. Inadequate access to appropriate equipment at the time of an emergency was recognised as a challenge to anaesthetists in emergency management. With the implementation of an emergency resource area in each theatre, the anaesthetic department endeavours to ensure best practice in emergency management.
