  • Inhibitory Activity of Parsea americana Mill. Peels Extract and Fraction
           Containing Phenolic Compound Against Staphylococcus aureus ATCC 25923

      Abstract: Parsea americana Mill. is a natural resource that has been studied for its antibacterial properties. The pulp, peel, and seed of Parsea americana Mill. have potential as an antibacterial agent. This study aimed to determine the inhibitory activity and phenolic content of Parsea americana Mill. peels extract and fraction against Staphylococcus aureus ATCC 25923. Parsea americana Mill. was macerated with 96% ethanol and then fractionated with n-hexane, ethyl acetate, and methanol solvent. Determination of the fraction that has the greatest antibacterial activity against S. aureus was carried out using the Kirby Bauer method.  The total phenolic content in the extract and fraction was calculated as gallic acid equivalent (GAE) using the Folin-Ciocalteu method spectrometrically. Antibacterial activity test of the 96% ethanol extract, ethyl acetate fractions, and methanol fractions at a concentration of 10% w/v showed activity with a measurable inhibition zone. On the other hand, the n-hexane fraction showed no inhibition zone. The highest inhibition zone was the ethyl acetate fraction with approximately 8.33 ± 0.58 mm. The ethyl acetate fraction of Parsea americana Mill. resulted in 536.26 ± 14.29 mg GAE/g fraction. The conclusion was that the ethyl acetate fraction had the highest total phenolic content and was the most active fraction in inhibiting the growth of Staphylococcus aureus.

  • Drug Utilization 90% Profile of Antibiotics Use during the Period of 2013
           – 2017 at a Private Teaching Hospital in Yogyakarta

      Abstract: Evaluation of antibiotics use plays an important role in antimicrobial stewardship programs to support the rational use of antibiotics at all healthcare facilities. This study aimed to describe the profile of drug utilization 90% (DU90%) of antibiotics use for hospitalized patients at a private teaching hospital in Yogyakarta over period of 2013 – 2017. Aggregate data of all systemic antibiotics having an anatomic therapeutic chemical (ATC) code and used for hospitalized patients during 2013 – 2017 were included in this study. Quantity of antibiotics use was expressed in defined daily dose (DDD) and final quantity was expressed in DDD/100 bed days. DU90% profile was obtained by calculating the cumulative use of antibiotics from the highest to the lowest percentage. There were 44 antibiotic agents used each year over the period of 2013 – 2017. However, there were only 10 – 12 agents within the DU90% segment. Cephalosporins group was identified as the most used antibiotics consistently over the period and the top three antibiotics that were consistently prescribed were: ceftriaxone, levofloxacin, and cefixime. It can be concluded that only about a quarter of the total antibiotic agents used in hospitals are within the DU90% segment and those antibiotic agents are relatively consistent, especially for the top three of the highest used antibiotics.

  • Effect of Antihypertensive Therapy Compliance Using Medication Possession
           Ratio Method on Blood Pressure in Patients with Hypertension

      Abstract: Hypertension is a circulatory system disorder that causes an increase in blood pressure, specifically systolic blood pressure ≥140 mmHg or diastolic blood pressure ≥90 mmHg. Hypertension can cause serious medical conditions such as damage to various organs. The complications caused by hypertension can be prevented with treatment regularly to achieve controlled blood pressure. This study determines the relationship between antihypertensive therapy compliance to blood pressure in hypertensive patients. This type of research is analytical observational with retrospective cohort design and purposive sampling techniques. The data were obtained from patients’ medical records, including demographic data, blood pressure, and prescribed drugs. The subject's compliance was measured using the Medication Possession Ratio (MPR). A total of 124 subjects met the inclusion criteria. The data were analyzed using a chi-square test with Yates correction. Most patients had uncontrolled blood pressure as much as 101 patients. The statistical analyses show that antihypertensive therapy compliance affects controlled blood pressure in patients with hypertension (p= 0.00). More efforts to support adherence to hypertensive treatment should be initiated to have greater effect on therapy compliance.

  • Users’ Views Regarding Electronic Prescribing Implementation: A
           Qualitative Case Study in A Private Hospital of Magelang City, Indonesia

      Abstract: The use of the Internet and digital system in a hospital named the Hospital Information System (HIS) has grown rapidly and remarkably, including in Indonesia. One of the HIS components is electronic prescribing (e-prescribing). Although the Indonesian government has mandated HIS in hospitals since 2011, successful implementation is not without challenges. Therefore, this study aimed to explore users' views regarding e-prescribing implementation through a case study in a private hospital in Magelang City, Indonesia. This study applied a qualitative approach. Participants were e-prescribing users who were selected purposively. Data were gathered using interviews. The Capability, Opportunity, Motivation, and Behavior (COM-B) model was used to structure the interview guideline. Data were transcribed verbatim and analyzed using a thematic approach. Twenty participants were interviewed. Five themes emerged through the interviews: self-reflection, perceived advantages, policy, capacity building, and quality assurance. The themes express optimism in implementing e-prescribing. Users are central to e-prescribing system acceptance, as is explained in the self-reflection and perceived advantages themes. Furthermore, top management plays a pivotal role in improving the capacity and ensuring system implementation quality. This study’s findings imply that the user, system, and organization play crucial roles in enhancing health care service quality through e-prescribing.

  • Characterization of 1H NMR and 13C NMR Spectrometry and Antibacterial
           Activities of 4-tert-Butylbenzoyl Amoxicillin

      Abstract: The synthesis of 4-tert-butylbenzoyl amoxicillin has been done by reacting amoxicillin with 4-tert-butylbenzoyl chloride. The product was characterized using proton and carbon-13 spectrometry. Antibacterial activity of 4-tert-butylbenzoyl amoxicillin against Escherichia coli, Bacillus subtilis, and Staphylococcus aureus was tested using paper disk diffusion method. Results showed that 4-tert-butylbenzoyl amoxicillin had a specific spectrum activity. Antibacterial test of 4-tert-butylbenzoyl amoxicillin showed clear zones around the disc paper for Escherichia coli, Bacillus subtilis, and Staphylococcus aureus. The study results indicated 4-tert-butylbenzoyl amoxicillin has antimicrobial properties but is categorized as a less-effective antibiotic agent based on the response to bacterial growth inhibition.

  • Synthesis of Silver Nanoparticles Using Premna serratifolia Linn. Leaf
           Extract as Reducing Agent and Their Antibacterial Activity

      Abstract: Premna serratifolia Linn. leaf extract has been used as a reducing agent in the synthesis of silver nanoparticles (AgNPs). Various synthesis parameters such as reaction time, concentration and the pH of the Premna serratifolia Linn. leaf extract, and silver nitrate concentration were investigated. In addition, the stability of synthesized AgNPs and their activity against Staphylococcus aureus and Escherichia coli have also been investigated. The results of the experiment showed that Premna serratifolia Linn. leaf extract reduced silver ions resulting in AgNPs. In addition, the AgNPs colloid showed a gradual change in color from transparent green to yellow. At the same time, its ultraviolet (UV)-Visible spectra exhibited the typical surface plasmon resonance peak at around 400-415 nm. The optimum reaction conditions in the formation of AgNPs are 40 minutes of reaction time using silver nitrate 1.5x10-4 M and Premna serratifolia Linn leaf extract 80 ppm at pH 10. The particle size of synthesized AgNPs distributes from 48.3-157 nm with an average size of 58.7±14.4 nm and is stable at least for 1-month storage under ambient conditions. The antibacterial test shows that synthesized AgNPs are effective against both Escherichia coli and Staphylococcus aureus.

  • Systematic Review: Anti-Osteoporosis Potential Activities Of Phytoestrogen
           Compounds In Chrysophyllum cainito L., Elaeis guineensis Jacq., Lannea
           acida Rich., Marsilea crenata Presl., and Medicago sativa L.

      Abstract: Phytoestrogens are compounds from plants that have a structure and function similar to estrogen (17β-estradiol). Phytoestrogens can be found in Chrysophyllum cainito L., Elaeis guineensis Jacq., Lannea acida Rich., Marsilea crenata Presl., and Medicago sativa L. This systematic review aimed to prove that all of these five plants have phytoestrogens which were observed with several instruments, and to assess activities and bone forming mechanisms from these five plants in the female mice (Mus musculus) and female rats (Rattus norvegicus). This systematic review was done by identifying articles in several databases (Google Scholar, PubMed, and Science Direct). The process of selecting the articles used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines to create a flowchart with inclusion and exclusion study criteria. Meta-synthesis was done to analyze, identify, and interpret all of the data in the articles systematically. 31 articles in total were obtained from the selection process, with 27 articles containing a discussion about chemical compound content and 4 articles describing research results for in vivo testing of the plants that were reviewed. The results showed that these five plants have phytoestrogens and by in vivo testing have the activity in increasing trabecular bone density in the experimental animals.
