  • Asociación entre la exposición al riesgo biológico y signos y síntomas
           clínicos en asistentes de laboratorio

    • Abstract: La presencia de signos y síntomas en el personal que se encuentra expuesto a riesgo biológico puede pasar desapercibida, siendo éstos, el preámbulo de aparición de una infección laboral de tipo superficial y/o sistémica. El artículo determina la asociación entre signos y síntomas clínicos con la exposición al riesgo biológico entre asistentes de laboratorios. El diseño del estudio es de corte transversal. Variables independientes: características sociodemográficas, infraestructura de los laboratorios, prácticas y procedimientos en bioseguridad, documentación y salud e higiene en el trabajo. Variables dependientes: síntomas de cefalea, faringitis a repetición, hipersensibilidad respiratoria, infecciones respiratorias y rash. Participaron 19 asistentes de 24 laboratorios de la Facultad de Ciencias Agrarias y del Medio Ambiente de una institución educativa universitaria, la mediana (Me) de edad se encuentra en 36 años con rango intercuartil 30-39, 68.4% mujeres, 73.7% de los asistentes tienen exposición directa al riesgo biológico. Infraestructura, almacenamiento de residuos, uso de elementos de protección personal obtuvo más del 80% de cumplimiento. No se presentó asociación estadística entre las variables cefalea (p=0.305), faringitis (p=0.345) infecciones respiratorias (p=0.529), infección de oído (p=0.372) rash (p=0,259) con la exposición al riesgo biológico; pero, se determinó que los asistentes expuestos al riesgo biológico tienen un OR= 3.75 en cefalea y OR=2.7 en faringitis. Se concluye que se hace necesario reforzar en medidas preventivas en relación con las prácticas y procedimientos en los laboratorios así como iniciar un Sistema de Vigilancia Epidemiológica con miras a la prevención de enfermedades relacionadas con este tipo de riesgo.Introduction. Mild clinical signs and symptoms in personnel exposed to biological risk may be undetected and would be the beginning of superficial and / or systemic occupational infection. The aim of this study was to determine the association between clinical signs and symptoms with biological risk exposure among laboratory assistants. Material and methods. This cross-sectional trial studied explanatory variables like sociodemographic characteristics, laboratory infrastructure, practices and procedures in biosafety, documentation and health and hygiene at work, and outcome variables like: headache, pharynx, low and high respiratory tract infections and rash. Results. Nineteen participants from 24 laboratories from Agricultural Sciences and the Environment Faculty of Francisco de Paula Santander University, were selected and underwent to clinical record analysis. The median (Me) of age were found in 36 years with interquartile range 30-39, 68.4% women, 73.7% of the assistants have direct exposure to biological risk. Infrastructure, waste storage, use of personal protection elements obtained more than 80% compliance. No statistical association was found between the variables headache (p = 0.305), pharyngitis (p = 0.345) respiratory infections (p = 0.529), ear infection (p = 0.372) rash (p = 0.259) with exposure to biological risk; but, it was determined that the attendants exposed to biological risk have an OR = 3.75 in headache and OR = 2.7 in pharyngitis. Conclusions. It is necessary to reinforce preventive measures in relation to practices and procedures in laboratories as well as to initiate an Epidemiological Surveillance System with a view to the prevention of diseases related to this type of risk.
  • Componentes Bioactivos del Asai (Euterpe oleracea Mart. y Euterpe
           precatoria Mart.) y su efecto sobre la salud

    • Abstract: La calidad de vida depende de múltiples factores siendo la salud, un elemento determinante. Así, la dieta y la nutrición son pilares fundamentales en la prevención y tratamiento de enfermedades crónicas. La ingesta suficiente de frutas y verduras disminuye el riesgo de aparición de estas patologías, debido a un alto aporte de sustancias biológicamente activas (vitaminas, minerales y antioxidantes). El Asai (Euterpe oleracea Mart. y Euterpe precatoria Mart.) es el fruto de una palma que se encuentra en las llanuras de marea de los ríos de la región amazónica. La literatura informa que por su alto contenido de antocianinas, este fruto disminuye los niveles de radicales libres, la peroxidación lipídica y el estado proinflamatorio. El objetivo de esta revisión es explorar la fitoquímica, potencial terapéutico y acción farmacológica del Asai. La evidencia reporta una posible acción anticancerígena y cardioprotectora in vivo e in vitro.The quality of life depends on multiple factors health being a decisive factor. Thus, diet and nutrition are cornerstones in the prevention and treatment of chronic diseases. Sufficient intake of fruits and vegetables reduces the risk of developing these conditions due to a high intake of biologically active substances (vitamins, minerals and antioxidants). The Asai (Euterpe oleracea Mart and precatoria Mart) is the fruit of a palm found in the tidal flats of the rivers of the Amazon region. The literature reports that for its high content of anthocyanins, this fruit reduces the levels of free radicals, lipid peroxidation and proinflammatory state. The aim of this review is to explore the phytochemical, pharmacological and therapeutic potential of Asai. Evidence reported a possible cardioprotective and anticancer action in vivo and in vitro.
  • Componentes Bioactivos del Shiitake (Lentinula edodes Berk. Pegler) y su
           impacto en la salud

    • Abstract: El Shiitake (Lentinula edodes), conocido por sus atributos sensoriales, nutricionales y funcionales, es el segundo hongo comestible más cultivado del planeta, antecedido por el champiñón blanco (Agaricus bisporus). El píleo (sombrero) del hongo es muy apetecido mientras que su estípite (tallo) generalmente es separado y no es comercializado debido a que dentro de sus componentes hay alto contenido de fibra. El objetivo de esta revisión es profundizar sobre el estudio del contenido nutricional del Shiitake (Lentinula edodes) y reconocer la presencia de sus componentes bioactivos de mayor relevancia en el píleo y estípite; así como, sus posibles efectos beneficiosos sobre la salud. La literatura científica reporta un alto contenido de proteína y fibra en el hongo, además que resalta el papel de sus metabolitos secundarios como el Lentinan y la Eritadenina como agentes biológicamente activos en la prevención de determinados tipos de cáncer y por tener un efecto antioxidante y en bajar el colesterol con posible efecto en la prevención de enfermedades cardiovasculares.The Shiitake (Lentinula edodes), known by its sensorial, nutritionals and functional attributes, is the second most cultivated fungus eatable of the planet, preceded by mushroom white (Agaricus bisporus). The pileus (hat) of the fungus is quite desired; while his stem or stipe is usually separated from the fungus, and this part is not commercialized due its high fiber content. The objective of this review is to deepen on the study of the nutritional content of the Shiitake (Lentinula edodes), recognizing its major bioactive components in the pileus and stipe; as well as to elucidate its beneficial effects on the health. Indeed, the scientific literature is reporting a high content of protein and fiber, additionally emphasizing the paper of secondary metabolites present in the fungus such as, the Lentinan and the Eritadenina. Both of them are biologically active agents useful in the prevention of certain types of cancer, like antioxidant; as well as, in lowering the cholesterol with possible effect in the prevention of cardiovascular diseases.
  • Evaluación pronóstica de los parámetros ecocardiográficos de
           disfunción diastólica en pacientes con infarto agudo de miocardio con
           elevación del segmento ST

    • Abstract: Introducción: Las propiedades diastólicas del ventrículo izquierdo están alteradas en el miocardio infartado. Se han empleado varios métodos para su evaluación. La ecocardiografía doppler tiene la ventaja de ser incruenta, de menor costo y de eficacia comparable al resto de los procedimientos. Objetivo: Evaluar los parámetros ecocardiográficos de disfunción diastólica como indicador pronóstico en pacientes con infarto agudo de miocardio con elevación del segmento ST (IMCEST). Metodología: Se realizó una investigación analítica y prospectiva en 30 pacientes ingresados en el Servicio de Cardiología del Hospital General del Sur de Maracaibo con diagnóstico de IMCEST. Se les realizó ecocardiograma transtorácico en las primeras 72 horas y posterior seguimiento a 6 meses. Según la aparición de eventos (angina/reinfarto, insuficiencia cardíaca, arritmia maligna o muerte) se separaron en dos grupos: grupo 1, pacientes sin eventos y grupo 2, con eventos. Resultados: Los pacientes con eventos cardiovasculares tuvieron una relación E/e’ de 11,69±5,71, en los pacientes sin eventos fue 7,39±2,33 (p=0,010). Así mismo se observaron diferencias significativas en la relación S/D del flujo de las venas pulmonares y la relación E/Vp. Los pacientes con relación E/e’ ≥ 8 presentaron 25,91 veces más riesgo de presentar eventos con respecto a aquellos con relación E/e’ < 8 (OR: 25,91; IC95%: 1,24-59,28; p=0,03) Conclusiones: En la evaluación de la función diastólica en pacientes con IMCEST mediante ecocardiograma transtorácico, los cocientes E/e’, E/Vp y S/D mostraron diferencias significativas en el seguimiento a 6 meses. La relación E/e’ constituye un importante predictor de riesgo.Introduction: Left ventricular diastolic properties are altered in infarcted myocardium. Various methods have been employed for its evaluation. Doppler echocardiography has the advantage of being noninvasive, lower cost and with certain efficacy compared to other procedures. Objective: To evaluate echocardiographic parameters of diastolic dysfunction as a prognostic indicator in patients with acute myocardial infarction with ST-segment elevation (STEMI). Methods: An Analytical and prospective study was conducted. 30 patients admitted to the cardiology department of the Southern General Hospital of Maracaibo diagnosed with STEMI were studied. Transthoracic echocardiography was performed the first 72 hours and repeated after 6 months. According to the occurrence of events (angina / reinfarction, heart failure, malignant arrhythmia or death) patients were separated into two groups: group 1, patients without events and group 2 with events. Results: Patients with cardiovascular events had an E/e’ ratio of 11.69 ± 5.7, in patients without events it was 7.39 ± 2.33 (p = 0.010). Furthermore significant differences in the S/D ratio of the flow of the pulmonary veins and E/Vp ratio were observed. Patients with E/e’ ≥ 8 had 25.91 times more risk of events with respect to those with E/e’ <8 (OR 25.91, 95% CI 1.24 to 59.28; p = 0.03). Conclusions: Evaluation of diastolic dysfunction in patients with STEMI by transthoracic echocardiography, the E/e’ ratios, E/Vp and S/D all showed significant differences with control after 6 months. The relationship E/e’ is an important predictor of risk.
  • Manifestaciones cutáneas como diagnóstico precoz de enfermedades
           endocrinas en pacientes pediátricos

    • Abstract: Objetivos: El objetivo del presente estudio fue determinar la frecuencia de endocrinopatías en pacientes con enfermedades dermatológicas y su asociación con el tipo de endocrinopatía. Materiales y Métodos: Se realizó un estudio retrospectivo en 107 pacientes pediátricos de la consulta de dermatología del Hospital Roberto Gilbert Elizalde que cumplieron con la elaboración completa de pruebas endocrinas para la estimación de alteraciones endocrinológicas. Se realizó un modelo de regresión logística múltiple para la determinación de endocrinopatías. Resultados: La prevalencia de endrocrinopatías correspondió al 28,0%. Se encontró una asociación significativa entre la presencia de endocrinopatía con la edad y los diagnósticos dermatológicos. El grupo etario de escolares/ adolescentes tuvo un riesgo significativo para endocrinopatía (OR= 5,64; IC95%: 1,15-27,58; p=0,033), asimismo la presencia de acantosis nigricans presentó 4,56 veces mayor riesgo de ocurrencia de endocrinopatía comparados los pacientes que no presentaron acantosis nigricans (OR= 4,56; IC95%: 1,62-12,77; p=0,004). Conclusiones: Se presenta la estrecha relación entre las dermopatías y endocrinopatías, en particular con respecto a la acantosis nigricans, recomendándose la evaluación endocrina protocolar en estos pacientes para la detección precoz de alteraciones endocrinas. Esto reafirma la importancia de la evaluación integral y holística de los individuos con alteraciones cutáneas.Objectives: The aim of this study was to determine the frequency of endocrine disorders in patients with skin diseases and their association with different types of endocrine disease. Materials and Methods: A retrospective study was conducted in 107 pediatric patients from the dermatology service of the “Roberto Gilbert Elizalde” Hospital who had a full workup of endocrine tests to estimate endocrine disorders. A model of multiple logistic regression was performed to determine the odds ratios for endocrinopathies. Results: The prevalence of endocrinopathy was 28,0%. A significant association was found between the presence of endocrine disease with age and dermatologic diagnoses. The age group of school children/adolescents had a significant risk for endocrine disease (OR= 5.64, 95% CI: 1.15-27.58; p= 0.033), also the presence of acanthosis nigricans conferred 4.56 times greater risk for endocrine disease (OR= 4.56, 95% CI: 1.62-12.77; p= 0.004). Conclusions: The close relationship between skin diseases and endocrine disorders is presented, in particular with regard to acanthosis nigricans. Endocrine evaluation is recommended in these patients for early detection of endocrine disruptions. This reaffirms the importance of comprehensive and holistic assessment of individuals with skin disorders.
  • Comportamiento epidemiológico de la obesidad y factores de riesgo
           asociados en la población rural de Cumbe, Ecuador

    • Abstract: Introducción: La obesidad constituye actualmente una de las más importantes pandemias a nivel mundial con importantes consecuencias personales, familiares y gubernamentales. Los datos epidemiológicos de la obesidad en poblaciones rurales de Ecuador son limitados, por lo que el objetivo de esta investigación fue evaluar la prevalencia y los factores asociados a la obesidad en individuos adultos de la parroquia rural de Cumbe, Ecuador. Materiales y métodos: Se realizó un estudio de campo, analítico, transversal con muestreo aleatorio multietápico, que incluyó a 374 individuos de ambos sexos mayores a 18 años residenciados en la parroquia rural de Cumbe, a quienes se les aplicó historia clínica completa. La obesidad se definió según los criterios de la Organización Mundial de la Salud (OMS) en normopeso, sobrepeso y obesidad. Se construyó un modelo de regresión logística multivariante para la estimación de Odds Ratios (IC95%) para factores asociados a la obesidad. Resultados: La prevalencia de sobrepeso y obesidad fue del 34,7% (n=140) y 19% (n=71). Las mujeres presentaron una mayor prevalencia de obesidad (21,8%), que los hombres (13,5%). Los factores asociados a obesidad fueron: el sexo femenino (OR=3,61; IC95%:1,38-9,47; p=0,009), el antecedente personal de diabetes mellitus tipo 2 (OR=3,54; IC95%:1,10-11,39; p=0,034), ser exfumador (OR=3,48; IC95%:1,02-11,83; p=0,045), realizar actividad física baja (OR=2,17; IC95%:1,02-4,63; p=0,043), la circunferencia abdominal elevada (OR=3,23; IC95%:1,37-7,62, p=0,007) y el antecedente de hipertensión (OR=5,44, IC95%:2,04- 14,45; p=0,001). Conclusiones: Se evidenció una baja prevalencia de obesidad similar a las reportadas en el país, pero bajas en comparación de otras latitudes. Ser mujer, exfumador, realizar actividad física baja, hipertenso, diabético o con obesidad abdominal fueron los factores asociados a obesidad, por lo que deben tomarse en cuenta al momento de realizar la prevención y promoción de la salud.Introduction: Obesity is one of the most important pandemics worldwide with important personal, family and governmental consequences. The epidemiological data of obesity in rural populations of Ecuador are limited, so the objective of this research was to estimate the prevalence and associated factors of obesity in individuals of the rural parish of Cumbe, Ecuador. Materials and methods: This cross-sectional, analytical field study included 371 individuals of both sexes over 18 years of age residing in the rural parish of Cumbe, who were applied a complete clinical history. Obesity was defined according to the criteria of the World Health Organization (WHO) in normopeso, overweight and obesity. A multivariate logistic regression model was constructed for the estimation of Odds Ratios (95% CI) to determine associated factors with obesity. Results: The prevalence of overweight and obesity was 34.7% (n = 140) and 19% (n = 71). Women had a higher prevalence of obesity (21.8%) than men (13.5%). Associated factors with obesity were: be a woman (OR=3.61; 95%CI:1.38-9.47, p=0.009), the personal history of type 2 diabetes mellitus (OR=3.54; 95%CI:1.10-11.39; p=0,034), be a former smoker (OR=3.48, 95%CI:1.02-11.83, p=0.045), had low physical activity (OR=2.17; 95%CI:; p=0.043), increased waist circumference (OR=3,23; IC95%:1,37-7,62; p=0.007) and personal history of hypertension (OR=5.44; IC95%:2.04-14.45; p=0.001). Conclusions: It was evidenced a low prevalence of obesity, similar to those reported in the country, but low in comparison with other latitudes. Being a female, former smoker, low physical activity, hypertensive, diabetic or with increased waist circumference were the associated factors with obesity, so they should be taken into account at the time of prevention and health promotion.
