  • Impact of the COVID-19 pandemic on the success of bariatric surgeries in
           patients with severe obesity

    • Abstract: Introduction: SARS-CoV-2 infection (COVID-19) pandemic may influence the weight outcomes of bariatric surgeries (BS). Here, we intended to compare the weight outcome of patients who underwent bariatric surgery before and during the pandemic time. Methods: In a retrospective, single-center study, the information of two groups of patients; first COVID-19_Group (n=51) consisted of those that underwent BS during the pandemic and completed a year of follow-up, second NonCOVID-19_Group included 50 patients who underwent BS and were followed up before the pandemic. All the patients’ anthropometric and obesity-related disease data were compared between groups. Results: Weight loss and the decrease of body mass index (BMI) one year after the surgery, as well as excess weight loss and total weight loss, were significantly higher in the NonCOVID-19_Group compared to the COVID-19_Group (p < 0.05). Although the rate of remission for diabetes mellitus, hypertension, and dyslipidemia was higher in the NonCOVID-19_ Group, the differences were not statistically significant (p> 0.05).Conclusion: We showed a significantly poorer weight outcome at the one-year follow-up of the BS during the pandemic compared to the pre-pandemic. These results need further investigations to determine the preventive measures and management by evaluating the associated factors.

  • The EASO new investigator award in clinical research 2021. Role of
           chronotype in obesity.

    • Abstract: Background: Chronotype is the expression of the timing of circadian rhythmicity of a subject, and three categories of chronotype have been identified: morning, evening and intermediate chronotype. Subjects with morning chronotype prefer to carry out most of their daily activities in the morning while subjects with evening chronotype perform most of their daily activities in the second half of the day. Intermediate chronotype is in an intermediate position between the above reported categories. Recently evening chronotype has been associated with an increased risk of developing chronic diseases. Thus, the aim of this manuscript is to review the current evidence on the role of chronotype categories on the risk of developing obesity and the most common obesity-related comorbidities (cardiometabolic and neoplastic complications). Summary: Subjects with evening chronotype have been reported to be at high risk of developing obesity and this was mostly due to the tendency of these subjects to follow unhealthy lifestyle mostly characterized by sedentary behavior and high intake of unhealthy food. In addition, sleep disturbances are a common finding in subjects with evening chronotype that in turn could further contribute to the risk of obesity. The impairment of insulin sensitivity, melatonin, adiponectin and clock genes function along with increase of leptin secretion detected in subjects with evening chronotype could also represent a favorable milieu for the onset of obesity-related cancer. The current evidence are limited to breast, prostate and endometrial cancer. Key Messages: The chronotype categories could be easy assessed in subjects with obesity and at the same time provide an important information on an additional risk factor predisposing to the onset of obesity-related comorbidities. Since chronotype could be modified through a behavioral-driven approach thus potentiating the efficacy of anti-obesity treatment, the assessment of chronotype categories should be included in the management of obesity.

  • Rapid emergence of appetite and hunger resulting in weight gain and
           improvement of eating disorder symptomatology during and after short-term
           off-label metreleptin treatment of a patient with anorexia nervosa

    • Abstract: Off-label treatment of a 15-year-old female patient with anorexia nervosa with human recombinant leptin (metreleptin) for nine days was associated with self-reported increments of appetite and hunger resulting in rapid weight gain, substantial improvement of eating disorder cognitions and of depression. The results further substantiate the effects of metreleptin on both AN and depression. We contrast these results with the widespread view that leptin is an anorexigenic hormone. Randomized controlled trials are warranted to confirm the described effects.

  • European Association for the Study of Obesity (EASO) position statement on
           medical nutrition therapy for the management of overweight and obesity in
           children and adolescents developed in collaboration with the European
           Federation of the Associations of Dietitians (EFAD)

    • Abstract: Introduction: This position statement on medical nutrition therapy (MNT) in the management of overweight or obesity in children and adolescents was prepared by an expert committee convened by the European Association for the Study of Obesity (EASO) and developed in collaboration with the European Federation of the Associations of Dietitians (EFAD).Methods: It is based on the best evidence available from systematic reviews of randomised controlled trials on child and adolescent overweight and obesity treatment and other relevant peer-reviewed literature. Results: Multi-component behavioural interventions are generally considered to be the gold standard treatment for children and adolescents living with obesity. The evidence presented in this position statement confirms that dietary interventions can effectively improve adiposity-related outcomes. Dietary strategies should focus on the reduction of total energy intake through promotion of food-based guidelines that target modification of usual eating patterns and behaviours. These should target increasing intakes of nutrient-rich foods with a lower energy density, specifically vegetables and fruits, and a reduction in intakes of energy dense nutrient-poor foods and beverages. In addition, higher intensity, longer duration treatments, delivered by interventionists with specialised dietetic-related skills and co- designed with families are associated with greater treatment effects.Discussion: Such interventions should be resourced adequately so that they can be implemented in a range of settings and in different formats, including digital or online delivery, to enhance accessibility.

  • Clinical impact of Glucagon like peptide-1 receptor analogs on the
           complications of obesity

    • Abstract: Background: Obesity is a chronic disease associated with increased morbidity and mortality due to its complications. The aims of obesity treatment are primarily to accomplish weight loss, and prevention or treatment of its complications. Lifestyle changes along with behavioral therapy constitute the first line treatment of obesity followed by pharmacotherapy. Glucagon-like peptide receptor analogs (GLP-1 RA’s) are among the approved pharmacotherapy options. Their central effect on suppressing appetite results in considerable weight loss. However their effect on the complications of obesity have not been very well recognized. This review aims to analyze the effects of GLP-RA’s on the complications of obesity, as diabetes mellitus, hypertension, nonalcoholic steatohepatitis (NASH), cardiovascular diseases, polycystic ovary syndrome, infertility, obstructive sleep apnea (OSA) and central nervous system problems. Summary: Data from preclinical studies and clinical trials have been thoroughly evaluated. Effects regarding the complications as far as the scope of this review has covered can be summarized as blood glucose lowering, blood pressure lowering, resolution of NASH, improving major cardiovascular events, improving fertility and sex hormone levels, improvement in OSA symptoms and in cognitive scores. Although the mechanisms are not fully elucidated, it is clear that the effects are not solely due to weight loss, but some pleiotropic effects like decreased inflammation, oxidative stress and fibrosis also play role in some of the complications. Key messages: Treating obesity is not only enabling weight loss but ameliorating complications related with obesity. Thus any anti-obesity medication has to have some favorable effects on the complications. As far as the GLP-RA’s analogs are concerned, there seem to be improvement in many of the complications regardless of the weight loss effect of these medications.

  • Early time-restricted eating reduces weight and improve glycemic response
           in young adults: a pre-post single-arm intervention study

    • Abstract: Introduction: Time-restricted eating (TRE) has cardiometabolic health benefits by optimizing circadian rhythms. However, limited data are available on the effect of early TRE in young adults. The objective of this pre-post single-arm trial was to test the effect of TRE on body composition and cardiometabolic risk factors and to evaluate changes in meal and sleep timing by TRE among young adults with typically late bedtime. Methods: This 4-week intervention was conducted in healthy young adults aged 18-30 years. Dietary records with time logs were collected before and during the intervention and nutrient intake and meal timing were evaluated. Snack packages containing 20 g/day of protein were provided weekly. Body composition was measured weekly using bioelectrical impedance analysis. Blood samples were collected before and after the intervention, and cardiometabolic parameters were evaluated. Results: Of the 36 screened participants, 34 completed the study (completion rate 94.4%). The average age was 23.4 ± 2.9 years with 64.7% female. The mean wake-up time and bedtime were 09:16 ± 01:26 and 01:51 ± 01:39 with average sleep duration of 7.4 ± 1.4 hour. Body weight and fat mass, excluding muscle mass, were significantly reduced over 4 weeks compared to baseline only in the early TRE group starting before noon. The early TRE group also showed significantly reduced fasting glucose, fasting insulin, and serum triglyceride (TG) levels after 4 weeks. However, the late TRE group starting after noon showed no significant changes except a reduced TG level. The meal timing was changed by TRE, where the first meal was delayed and the last meal was shifted. Neither sleep duration nor timing was significantly changed by TRE. Energy intakes was not different, but protein intake increased from 19.2% to 22.6% due to snack packages during intervention. However, no significant correlation between nutrient intakes and body composition changes was found. There were no adverse events related to study participation. Conclusions: An early TRE regimen may be a feasible and effective strategy to manage body composition and cardiometabolic risk factors in young adults, without altering the sleep–wake cycle.   • Body Composition Assessment by Dual-Energy X-Ray Absorptiometry: A Useful
           Tool for the Diagnosis of Lipedema

    • Abstract: Introduction: Lipedema is a poorly known condition. Diagnosis is based almost exclusively on clinical criteria, which may be subjective and not always reliable. This study aimed to investigate regional body composition (BC) by dual-energy X-ray absorptiometry (DXA) in patients with lipedema and healthy controls and to determine cut-off values of fat mass (FM) indices to provide an additional tool for the diagnosis and staging of this condition. Methods: This study is a single-center case-control study performed at Lausanne University Hospital, Switzerland. Women with clinically diagnosed lipedema underwent regional BC assessment by DXA. The control group without clinical lipedema was matched for age and body mass index (BMI) at a ratio of 1:2 and underwent similar examination. Regional FM (legs, arms, legs and arms, trunk, android and gynoid FM) was measured in (kg) and divided by FM index (FMI) (kg/m2) and total FM (kg). The trunk/legs and android/gynoid ratios were calculated. For all indices of FM distribution showing a significant difference between cases and controls, we defined the receiver operating characteristic (ROC) curves, calculating the area under the curve (AUC), sensitivity, specificity, and Youden’s index. Types and stages of lipedema were compared in terms of FM indices. Correlation analyses between all FM distribution indices and lipedema stages were performed. Results: We included 222 women (74 with lipedema and 148 controls). Overall, the mean age was 41 years (standard deviation [SD] 11), and mean BMI was 30.9 kg/m2 (SD 7.6). A statistically significant difference was observed for all DXA-derived indices of FM distribution between groups, except for arm FM indices. The ROC curve analysis of leg FM/total FM, as a potential indicator of lipedema, resulted in an AUC of 0.90 (95% confidence interval 0.86–0.94). According to Youden’s index, optimal cut-off value identifying lipedema was 0.384. Sensitivity and specificity were 0.95 and 0.73, respectively. We found no significant differences between lipedema types and stages in terms of FM indices, nor significant correlations between the latter and lipedema stages. Discussion/Conclusion: BC assessment by DXA, and particularly calculation of the leg FM/total FM index, is a simple tool that may help clinicians rule out lipedema in doubtful cases.

  • A representative percentile chart for prediction of weight loss trend
           after sleeve gastrectomy

    • Abstract: Introduction: It could be valuable for surgeons and patients to use one chart in different groups and evaluate weight loss during the post-surgery period. Methods: This retrospective study used the Iran National Obesity Surgery Database. patients with clinically severe obesity aged 18-70 undergoing sleeve gastrectomy participated in this study. Body mass Index (BMI) reduction and 5 other metrics measured over the study period were modeled using LMS (Lambda-mu-sigma) method. Our data were split into the train (70%) and test (30%) sets. Results: In this study, 1,258 patients (75% female) met the eligibility criteria to participate. Mean age and initial BMI were 36.87 ± 10.51 and 42.74 (40.37-46.36), respectively. Percentile charts for various metrics have been presented for the first 2 years after surgery. Conclusions: For sleeve surgery, all metrics are acceptable for clinical applications. Using the statistical view, BMI reduction is the most acceptable metric according to the lowest bias values and its variation between all the metrics.
