  • The Timelessness of the Scar in the Borderline Personality Disorder BPD: A
           Systemic Intervention in a Case of Child Sexual Abuse

      Abstract: Abstract Chronic sexual abuse in childhood can be directly associated with the formation of personality disorders such as BPD, with the majority of this abuse occurring in the family context. The objective of this article is to reference the importance of clinical and psychotherapeutic intervention from a systemic approach, prioritizing the analysis of interpersonal relationships, behavioral patterns repeated during the patient’s history and the adaptation of family roles learned during childhood to adulthood, obtaining extremely positive results in terms of remission of symptoms and general recovery of the patient.

  • The 7th Annual Psyche Awards: Honoring the Most Valuable Papers in Mental

      Abstract: Abstract Scholarly publications play a vital role in the advancement of psychotherapy, as a creative treatment strategy and as an empirically supported approach. The best scholarly articles integrate scientific methodology and clinical practice. Unfortunately, many mental health professionals devote their work responsibilities to one domain or the other, choosing to become academic researchers or full-time clinicians. The Psyche Awards were developed to highlight the most valuable journal articles that were published during the previous year. A total of 168 journals were screened to identify 128 interesting and useful articles, which were then sorted into 13 award categories. The 53 nominated papers were reviewed by a panel of four judges to identify the most valuable article in each topical area.

      Abstract: Abstract This manuscript challenges the notion of “therapist drift”—the deviation from evidence-based practices due to the partial application or non-adherence to treatment protocols—proposing that such deviations often reflect good clinical judgment and a commitment to personalized patient care. Drawing on recent research, we argue against the conventional wisdom that adherence to empirically supported treatments based on narrow diagnostic criteria guarantees superior therapeutic outcomes. We highlight the “dodo bird verdict,” which suggests the equivalence of different psychotherapy approaches in effectiveness, and scrutinize the American Psychological Association’s endorsements of empirically supported treatment relationships, emphasizing the move towards personalized psychotherapy. We argue that due to validity concerns of prevalent diagnostic taxonomies and the heterogeneity of desired therapy outcomes across diverse methods and patient needs, randomized controlled trials comparing treatments for fixed diagnoses are inadequate for guiding clinical decisions. We propose adjusting therapy to the patient’s unique characteristics and desired outcomes—not strict protocol adherence—indicates responsiveness and clinical acumen, necessitating a shift toward more nuanced, patient-centered therapeutic models.

  • Cognitive Behavioral Therapy for Suicidal Ideation: Extending the Stepped
           Care Model

      Abstract: Abstract Cognitive behavioral therapy (CBT) demonstrates clear efficacy in addressing suicidal thoughts and behaviors. However, considerable barriers limit the accessibility of CBT interventions for those experiencing suicidal crises. As a solution to these limitations, a stepped care model is adapted which presents a stratified framework attuned to suicidal ideation severity that may expand access and optimize outcomes. Through four levels - basic, intermediate, advanced, critical - calibrated to match intensity of therapeutic support to patient risk profiles and clinical needs, the Stepped Care Model promotes efficient, responsive care. Basic levels focus on community education, crisis lines, screening to enable early intervention for mild cases; while critical levels feature emergency hospitalization and stabilization care plans for individuals facing severe, acute suicidality. Challenges include gaps in assessing suicide risk complicating level assignments, ethical complexities in involuntary interventions, workforce preparedness in terms of CBT and suicidology competencies, and promoting interdisciplinary coordination. Recommendations include specialized clinical training, localized adaptation within health systems, technological integration to bolster care continuity, and ongoing collection of patient outcomes data. Refining implementation guidance will support sites in transforming service delivery models to be more preemptive, patient-centered and evidence-based - contributing significantly to suicide prevention while maximizing scarce resources. Success requires balancing clinical responsiveness, practical feasibility and scientific rigor in this Stepped Care Model approach for one of healthcare’s most vulnerable populations. Therefore, the aim of this review article was to explore and review the considerations for successfully implementing a Stepped Care Model approach that is optimized for treating suicidal ideation and behaviors.

  • An Integrative Model of Psychotherapeutic Interventions Based on a
           Predictive Processing Framework

      Abstract: Abstract There are numerous approaches to psychotherapy with distinct underlying theories. Yet, it is unlikely that any of these is conclusive because they broadly yield similar therapeutic effects. A different approach attempts to derive the common factors from the specific treatments and proposes that these common factors primarily promote therapeutic effects. But although generally promising, these common factors have not been integrated into a more fundamental explanatory framework that also considers the effectiveness of specific factors. Therefore, an integrative model of psychotherapeutic interventions is still missing. The aim of this paper is to help fill that gap by applying the theory of predictive processing to interventions from various psychotherapeutic approaches, including cognitive behavioural therapy, client-centred therapy, and psychoanalysis. The analysis results in the predictive processing model of psychotherapeutic interventions (3PI model for short). The new model reveals that psychotherapy comprises two main functions: to provide a safe and positive environment to which the patient can adapt, and to facilitate the process of updating a dysfunctional generative model. Further, it shows that common factors primarily promote the first function, while specific factors mainly support the second function, offering a new perspective to the debate on common and the specific factors.

  • “Close to the Problem” or “Close to Your Problem”' The
           Contribution of Bond to the Structure of Early and Advanced Therapeutic
           Alliance

      Abstract: Abstract The study analyzed the contribution of bond in the structure of the therapeutic alliance. Results were reported for two stages of the psychotherapeutic process: early and advanced. The study included 415 psychotherapist–patient dyads in the process of individual psychotherapy. Alliance estimation was based on Bordin’s model of alliance. The hypothesis was tested through structural equation modeling analysis. In both the early and advanced phases of psychotherapy, patient–therapist bond made a large contribution to the structure of the alliance, similar to that of the remaining two components: goals and tasks. The results indicate that bond is a crucial and permanent element in the structure of the relationship in the psychotherapeutic dyad, regardless of the stage of the process. In the advanced phase, the distance in weight between bond and the remaining two dimensions of alliance is minimal. The contribution of bond to the alliance structure as high as the contributions of goals and tasks signals not only the importance of understanding the patient’s problem but also the importance of mutual trust and respect.

  • Living the Language of Psychotherapy: How Therapists Use Their Experiences
           from Being Patients

      Abstract: Abstract Personal therapy is a requirement in several psychotherapy-training programs, and more than 80% of therapists have been psychotherapy patients. No quantitative evidence of the effects on their patients’ outcomes has been presented. However, qualitative studies and expert experiences highlight the value of therapists having been patients themselves. To investigate this further, we used in-depth qualitative methods to explore the experiences of eight early career psychologists. For our data collection, the participants wrote reflexive notes on two occasions, and met for a full day of focus groups. The notes and transcriptions of the focus groups were analyzed from the perspective of reflexive hermeneutic phenomenology. In all, the therapists had been patients in 19 courses of various psychotherapies. The core experiential themes were: (1) their own suffering had been for real; (2) ambivalence of sharing vulnerability; (3) the significance of experiencing theoretical concepts – the language of psychotherapy – in real life; (4) dealing with shame; (5) the need to know that therapists may become so important that the patient can easily be hurt; and (6) the therapist became an inner model that the patient could use. The therapists seemed to use their psychotherapy experiences in their personal development and professional lives. Therapists may have a unique opportunity to benefit from psychotherapy due to the language they have acquired for sharing experiences with their therapists.

  • “Let Us Begin Well Together”: A Preparation-Positivity-Purpose
           Checklist for Helping Beginning Supervisors Optimize the Start of
           Supervision

      Abstract: Abstract Becoming a psychotherapy supervisor is no easy feat, and conceptual/practical material and empirical data suggest that beginning (compared to more advanced) supervisors are at their most vulnerable and most apt to struggle with issues of identity, confidence, self-efficacy, and competency development. Beginning supervisors can benefit from being helped to navigate through these normative developmental struggles and being transitionally eased into the supervisory role. We consider one way to make that transition and navigation process easier, presenting a supervisor preparation-positivity-purpose action checklist that is organized around three key areas: educating supervisees about the supervision experience; fostering positive supervisee expectations; and collaboratively formulating reasonable supervision goals. Distributed across those three areas, we identify and elaborate upon 20 specific supervisor actions that provide direction about optimizing supervision’s start.       Abstract: Abstract In ths paper I outline my development over fifty years as a therapist, researcher and theorist. I started with empathy as a core helping process seeing it as a moment-bymoment information processing aid as well as a provision of the curative effects of human understanding. Over the years I added a focus on emotion as central to change, and proposed a number of necessary distinctions that needed to be made when working with emotion and concluded that one of the best ways to change emotion was with another emotion. In addition to my focus on moment by moment process I saw that identification of in-session experiential states and case formulation went beyond just being in the moment and helped provide more focus to an essentially phenomenologically based experiential approach.

  • My Evolution as a Psychotherapist

      Abstract: Abstract Psychotherapy has evolved over the past 50 years and many seasoned psychotherapists have evolved as well. In the current paper, Dr Paul Wachtel summarizes his evolution as a psychotherapist. Dr. Wachtel is a Distinguished Professor in the Doctoral Training Program in clinical psychology at the City University of New York. For the past 50 years, Dr. Wachtel has been a prolific writer, a skilled clinician, and the chief advocate for integrating different theoretical orientations to enhance the field of psychotherapy. His extensive training in psychoanalysis provided a strong foundation on which to build a comprehensive and integrative view of psychotherapy.   • Working Alliance Instability in the Inpatient Treatment of Borderline
           Personality Disorder

      Abstract: Abstract Borderline personality disorder (BPD) is characterized by a pathognomonic instability of interpersonal relationships. Therefore, a good working alliance in the treatment of BPD may go beyond qualitative aspects like agreement on goals and mutual trust, and in particular depend on a balancing of instability. Yet, little is known about working alliance dynamics in the treatment of BPD. Using daily ratings of the Therapy Process Questionnaire, we retrospectively investigated assessments of the stability of the therapeutic alliance during inpatient treatment in 51 patients with BPD and 66 patients with major depressive disorder. Regression analyses were used to investigate whether working alliance was more unstable in patients with BPD and how it was associated to outcome. The working alliance was found to be significantly more unstable in patients with BPD. Yet, a high dynamic complexity of the working alliance was found to be linked to better outcomes in BPD, possibly hinting at the importance of rupture and repair.

      Abstract: Abstract Eye Movement Desensitization and Reprocessing (EMDR), while recognized as evidence-based, continues to be viewed as a novel and controversial treatment. At the same time, numerous alternative eye movement therapies have been introduced, each of which requires its own set of randomized controlled trials (RCTs) to assess remarkable claims of cure. The present situation is untenable in our opinion because any clever entrepreneur can claim a new method and trademark a new acronym. Recommendations are made for more stringent criteria to establish science-based methods that guide clinical practice.

  • ‘Free from BFRB’: Efficacy of Self-Help Interventions for Body-Focused
           Repetitive Behaviors Conveyed via Manual or Video

      Abstract: Abstract Self-help may help fill the large treatment gap in individuals with body-focused repetitive behaviors (BFRBs). We examined the efficacy of three self-help techniques for BFRBs: habit reversal training (HRT), decoupling (DC), and decoupling in sensu (DC-is) and also tested whether a video demonstration is more efficacious to written instructions. A total of 224 participants with at least one BFRB were randomly assigned to two intervention groups (video, manual) receiving access to all techniques or a wait-list control group (1:1:1). A 6-week follow-up assessment was conducted. The Generic Body-Focused Repetitive Behavior Scale (GBS-9) served as primary outcome. The intention-to-treat analysis showed significant improvements for the manual group compared to the wait-list control group on GBS total score and severity and impairment scores. For the per-protocol analyses (i.e., participants had at least read/watched the manual/video), 26.9% of completers in the manual group improved at least 35% in GBS compared to 23.3% (video group) and 15.9% (control). A dose–response relationship emerged for the video group. Subjective ratings by the video and manual groups did not differ; treatment satisfaction was greater for HRT and DC than for DC-is. Results suggest that the manual group showed somewhat stronger effects than the video group when usage frequency was low. With more frequent usage, improvements in the video condition increased. Future studies should investigate potential order effects of the techniques as well as add-on effects when techniques are used along with other treatment approaches.

  • The Efficacy of Using Psychotherapy Interventions to Minimize Symptoms of
           Trichotillomania and Trichophagia: A Scoping Review

      Abstract: Abstract Trichophagia, or eating one’s own hair, commonly occurs in individuals with trichotillomania. Minimizing the symptoms of trichotillomania and trichophagia and maintaining remission can encourage them to have healthy lives. Conduct a scoping review to investigate the efficacy of using psychotherapy interventions to minimize symptoms of trichotillomania and trichophagia among individuals with trichotillomania. Using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews guidelines to screen 19,552 studies published between 2010 and 2022. A total of 19 studies that fulfill the eligibility criteria reported that psychotherapy interventions had been effective and applicable approaches to minimizing the symptoms of trichotillomania and trichophagia. Psychotherapy interventions are a promising treatment that is regarded as a credible treatment approach to ensure long-term positive outcomes among individuals with trichotillomania. This scoping review verifies the formalization and incorporation of psychotherapy interventions into standard practice and highlights the importance of mental health practitioners using them to ensure long-term positive outcomes for individuals with trichotillomania. Moreover, practitioners need further specialized training on a variety of psychotherapy interventions because not all of them are familiar with these interventions.

  • A Phenomenological Analysis of Mental Health Providers’ Experience
           of Client Suicide

      Abstract: Abstract Suicide is the 11th leading cause of death in the U.S. It is estimated that one third of people who died by suicide received mental health services in the year preceding their death and one fifth had received services in the previous month. Given these statistics, many mental health professionals are very likely to work with a client who dies by suicide at some point in their career. This qualitative study aimed to investigate the experiences of therapists after a client suicide and to identify those supports and responses the therapist found helpful in the aftermath. The nine participants were licensed mental health providers. Each had a client die by suicide at least 2 years prior and was actively providing treatment or had been providing treatment in the 6 months prior to the client’s death. Findings revealed that a client’s death by suicide can have a profound impact on the provider, including sadness, anger and anxiety about litigation. Such reactions speak to the importance of intentional self care for individuals who work with high risk populations. Additionally, results indicated that discussing client suicide in advance helps prepare clinicians to be better able to cope and heal following the actual event. This study enhances our understanding of this experience and fosters the development of actionable postvention steps clinicians, supervisors, and mental health agencies can take to support providers after a client suicide.

  • Vicious Relational Circles and Chronic Couple Conflictuality: An Empirical
           Study

      Abstract: Abstract The study aims to empirically assess the control-mastery theory hypothesis that considers chronic couple conflictuality as the repetition of relational vicious circles—that is, interactions where both partners test their pathogenic beliefs and fail their reciprocal tests, confirming their reciprocal pathogenic beliefs. In addition, the study aims to verify if interpersonal guilt is more activated during couple conflicts. Our study involved 11 couples treated by four experienced therapists and nine trained, independent judges who, after reading verbatim transcripts of the couples’ psychotherapy sessions, used the Patient Scale of Couple Testing (PSCT), and the Patient Interpersonal Guilt Rating Scale (PIGRS) to rate segments of couple sessions. The results were obtained by applying generalized estimating equations and confirm our hypotheses: we could observe a greater presence of testing activity and confirmation of pathogenic beliefs in segments classified as conflictual for both partners and a stronger presence of interpersonal guilt in conflictual versus nonconflictual interactions. These findings support the idea that conflict interactions can be seen as failed attempts by both partners to disconfirm their pathogenic beliefs.

  • Targeting Beliefs About Emotions via Meta-Emotion Therapy for Adolescents
           with Anxiety: A Case Series Study

      Abstract: Abstract Research suggests that best-practice psychotherapies only have moderate effects on reducing anxiety in youths. Such limitations in outcomes prompt consideration of alternative conceptualisations of anxiety and its treatment. Based upon the metacognitive model, we propose a novel therapy targeting metacognitive knowledge (beliefs about emotions and emotion regulation) for the treatment of anxiety in adolescents. The current case series study aimed to provide a proof of concept for the effectiveness of Meta-Emotion Therapy for adolescents (MET-A) for the treatment of adolescent anxiety. Four adolescents with elevated anxiety participated in this case series study. All participants received 10 individual sessions of MET-A. Participants and a parent completed the Revised Children’s Anxiety and Depression Scale at baseline, weekly and at one-month follow-up. The participants also completed the Beliefs About Emotions Questionnaire and the Difficulties with Emotion Regulation Scale at baseline, post-therapy and one-month follow-up. The results showed that all participants improved or recovered from symptoms of anxiety and depression from pre- to post-treatment, when taking into consideration both self- and parent-reported ratings (however, these ratings were often non-concordant). Gains were maintained at one-month follow-up. Further, participants who self-reported reductions in anxiety and depression also cited reductions in several maladaptive beliefs about emotions and improvements in aspects of emotion regulation. The participants endorsed the therapy as highly acceptable, appropriate, and feasible. Overall, the results of this study provide preliminary proof of concept for MET-A as a promising intervention for the treatment of adolescent anxiety Further investigation of MET-A is warranted to optimise treatment outcomes and measurement of change.   • Broken Narratives by Stressful Life Events: An Intervention for Narrative
           Identity Reconstruction

      Abstract: Abstract The present paper aims to introduce a constructivist narrative intervention for Narrative Identity Reconstruction (NIR) in working with clients with a history of stressful life events (SLE). This intervention is based on a proposed conceptual framework explaining how SLE affects narrative identity. First, we provide a conceptual model to show the theoretical links between SLE and narrative identity through the three functions of making meaning, unity, and purpose. The present conceptualization proposes that concepts of meaning-making, self-concept clarity, and agency should improve for narrative identity reconstruction. For designing therapy steps, we were inspired by the procedure of the Life Design process for reconstructing narrative identity. We defined the following steps: (a) construct self-story through small stories, (b) deconstruct these stories, (c) reconstruct them into an identity narrative or life portrait, and (d) co-construct intentions that lead to the next action episode in the real world. We hope NIR can help clients construct a robust self-narrative in which the SLE is not a confusing and distressing experience leading to a broken narrative; instead, it can make sense and integrate into the bigger picture of self-narrative. It is suggested that future research empirically examine the NIR to explore the feasibility and effectiveness of the intervention in mental health settings.
