      Psychotherapy training is intrinsically shaped by the way we think about the human being. What, in human existence, exposes us to (psycho)pathology' What can be considered beneficial and transformative in the therapeutic exchange' And similarly, what approach in training, as well as in therapy, could be more humane' Referring to phenomenological thought, the authors examine the singular statute around which psychotherapy and therapy revolve: the pathic phenomenon. "Pathic" refers to the multiform vulnerability that characterizes our human condition and that also marks our openness, our fundamental sensitivity to what happens in the encounter. The pathic dimension of the illness - more than the symptoms - constitutes the immediate and meaningful reality for the patient. Therapy can be defined as an intersubjective process of encounter, of common path and exchange, where the therapist places his or her theoretical knowledge, know-how, and presence at the service of the person under treatment in order to strive for a better way of being with self, with others and in the world. In the same way, training is called upon to achieve a transformative encounter between trainers and students. Training in psychotherapy should be based on this living and thinking initiation to the human condition, to its fundamental "pathic" nature, marked by the possibility of perdition and annihilation as well as trust and fulfilment. In their journey, the students must be able to rely not only on experience but also on the relationship with committed instructors who support and guide them, and on the active solidarity and cooperative spirit of the group of companions to face both the ‘wonderful’ and the ‘disconcerting’ in the encounter with pathos.

  • The art of training in psychiatry

      To speak of training in the field of psychiatry is to address a set of questions concerning the identity of psychiatry as an expression of knowledge and practice that relate to humankind, especially the suffering human. In other words, a reflection on the contents and methodological principles that are considered suitable for training professionals raises the question of what the primary vocation of psychiatry is, what its assumptions and aims are, or should be. First of all, we need to ask ourselves whether the object of study and intervention of this discipline is such as to justify its place in the field of medicine; whether the task of psychiatry is limited to the exercise of clinical competence or whether it should include responsibility of an ethical and political nature, in the sense of a tendency to comprise in its field of action and research the relationship between insanity and normality, between the forms of treatment of mental suffering and the languages of a culture of mental health. Without any intention to set limits to a discourse that needs to remain open and flowing, the aim is to reflect on the potential of training as an experience of transformation that involves the individual practitioner and the entire care team. Starting from the question of psychiatry’s fate in the face of the challenges inherent in the relationship with insanity, the reflection examines the value of a dialogue with philosophy in order to leave the narrow confines of the clinic and restore to psychiatry the original sense of a privileged point of view on the human being. Lastly, there is the testimony of an experience of research and training intrinsic to therapeutic practice, that which took shape in the 1990s from the encounter between the Basagliainspired psychiatry of the Orzinuovi Mental Health Centre and the original dialectical philosophy of Italo Valent.

  • Ludwig Binswanger and Bleuler’s "spiritual figure"

      By a brief and focused analysis of published and unpublished documents, this paper investigates the complex of personal, intellectual and scientific relationship between Ludwig Binswanger and his "revered teacher and friend" Eugen Bleuler from 1907 to 1939. As known, Binswanger’s career in psychiatry began at the "Burghölzli" Clinic in Zurich. Although Binswanger showed particular admiration for Bleuler’s "spiritual figure" and for some of his specific theoretical innovations in the clinical-structural sense, he did not fail to criticize some features of his conception of human nature, like the determinist monism conveyed in his 1921 Naturgeschichte der Seele.

  • Learning Psychotherapy: The art workshop

      The term bottega d’arte (art workshop) is borrowed from the artistic tradition of the Renaissance. In this place, the transmission of knowledge takes place from the master to the apprentices, in a way that respects individual creativity. This is the functional system that Giovanni Carlo Zapparoli and his collaborators, starting from the experience of the Psychology Centre of the Province of Milan, have developed to facilitate the transmission of knowledge in the field of psychotherapy. The author highlights its strengths in clinical wisdom and in the ability to differentiate essential and superfluous aspects within the therapeutic process.

  • Philosophical-phenomenological Anthropology as Ludwig Binswanger’s
           Metodological "Presupposition"

      The aim of the essay is to show how some aspects of the philosophicalphenomenological anthropology are a guide regarding Ludwig Binswanger’s diagnosis and therapy. And that is a valid "presupposition" of his clinical praxis. In my paper I follow the phenomenological method pinpointing how the analysis of clinical cases examined by Binswanger confirms the human tripartition according to Husserl’s position. The validity of phenomenological approach is confirmed by the comparison between Husserl and Freud. In the conclusion I observe that knowledge of the structure of the human being is useful to the individuation of the clinical cases in a fruitful circularity, which underscore the complexity of the human being, his/her strength or his/her frailty, not forgetting his/her dignity.

  • The psychiatrist: Who was he' Reflections on the crack of the mirror

      The forms of psychiatric knowledge and practice are far from being established once and for all. The current academic instruments of education have long flattened on a simplistic and reductionist line. In the second half of the Twentieth Century, a series of conditions had given the figure of the psychiatrist a centrality never had before in history: the closure/reorganization of asylums, the introduction and spread of psychopharmaceuticals, the advent of the concept of "community mental health". The aforementioned reductionism, however, has recklessly decomplexised the figure of the psychiatrist, whose identity problem has, in this way, become even more pressing, to the point of imploding, like the image at the breaking of a mirror. Psychiatry, in fact, unlike other medical branches, puts into play an anthropological factor, that of "madness" and its modalities of constitution and manifestation in the world. If this factor, historically, was considered decisive in the structuring and revealing of the so-called "mental illness", in more recent times the drainage of psychiatry from the swamps of madness has produced, in our opinion, a crack in the psychiatrist’s identity. Who is the psychiatrist if he is no longer able to keep the dialogue between society and madness alive' The question of madness sets in motion, like wood that feeds the fire of the human, the search for meaning and care. The eradication of madness from the territory of the human has produced a class of psychiatrists "in search of author" [1], participants of a daily clinical staging, without any claim to understanding, care and freedom. The anthropological factor, indeed, is the only one that can allow a not deformed mirror - through the human prism of the psychiatrist - of "madness" and "norm", as both expressions of life.

  • What ‘evidence’ do people really want and need for their
           recovery'

      Having access to lived-experience wisdom and knowledge is no longer optional but essential for help seekers to live well, and in turn for help providers to deliver more relevant and meaningful services. To date, mental health research agendas have primarily been concerned with producing clinical evidence that guides help providers as to the interventions that best reduce or ameliorate mental illness symptoms. This paper flips the focus to the nature of the type of ‘evidence’ people, who experience mental illness want and need in order to guide, activate and lead their own recovery. The authors’ draw both upon their shared anecdotal experiences of recovery, to explore the relevance and use of ‘clinical’ and ‘personal’ recovery evidence in people’s individual recovery journeys. People’s needs for evidence stretch beyond the ceiling of what ‘clinical’ recovery evidence currently offers. To thrive beyond the impact of mental ill health, people want to know more than how to manage symptoms. They want to know and experience: (i) recovery is real and possible, (ii) the notions underpinning personal recovery, not just clinical recovery, (iii) the lived experience collective wisdom and, (iv) most of all, how to protect themselves from any iatrogenic harm arising out of seeking help, such as institutionalisation, discrimination, stigma and oppression. Depending on their core beliefs and practice, mental health providers will either hinder or facilitate access to and utilisation of this knowledge. Decades of first-hand accounts provide testimony to the personal effort required to overcome the impacts of mental illness and its associated treatments. Lived experienced produced research provides rigour and strength to the ‘personal’ recovery evidence base and can stand side by side with its ‘clinical’ evidence counterpart. Both knowledge bases, whilst appearing tangential, are useful for people in recovery. Maintaining their separateness is unhelpful and limits access to necessary recovery knowledge for all. Only when research agendas synthesise these two wisdoms into a single evidence base will a new and more effective way of delivering services evolve.

  • The educational role of publishing

      During the twentieth century, several publishing companies acted as major cultural players. Publishing companies were often guided by prominent thinkers: writers, journalists, poets. They played also a political role through professional commitment. The educational function of publishing got into crisis in the last decades of the twentieth century probably due to the reduction of social conflict and political engagement and, at the same time, due to new rising management models that put economic results above reputation. At the beginning of the new millennium, Italian and International publishing is dominated by corporations owning historical publishing brands. Scientific publishing adapted itself to a managerial approach and, not surprisingly, has been able to take advantage by change: one of the most discussed issues concerns the quality of content and the tools for prepublication evaluation (peer review) and after publication (bibliometric indexes are a determinant of the careers of health professionals). Access models to journals have also changed, with open access in great rise despite it may discriminate low-income nations’ researchers. In a changed cultural, social and political context, even the choices and decisions of the less politically engaged publishers may contribute to building points of view on the world. Who knows that - thanks to the work of the publishers - these perspectives on the future cannot inspire the return to the guiding values of the 20th century.
