  • The child as "therapy" of the post-traumatic manic defence

      With this contribution of mine I wish to bring an articulated criticism to the scant attention paid to the several processes of the so called function of parentality and, at the same time, to consider these processes as unconscious collusive aspects that are often fundamental not only for the phenomenology of the relationship between partners, but also for a precise relation between the symptomatology of the children and the aspects of collusive pathology organized by the couple. This statement implies the whole complex field or the choice of the partner.This collusive mutuality emerges in a variety of forms. For example, mutual motivation of deprived, traumatized, shocked or deeply depressed individuals, to choose similar partners in order to perpetuate their mood without investing or being invested affectively by the other: "How can I speak of my own grief knowing the immensity of yours!". In this connection, Winnicott pointed out the "False Restitution" of the children towards the parents. This false restitution and reparation espressi t self through the identification of the child with the part of the parents that did not went through the dissociation of manic defence of theirs fails, the painful core, never worked through, the traumatic interruption of both their selves, that senses like a non turning point. There is in the child a sort of omnipotent attempt to reestablish continuity.

  • Transgenerational transmission of pain: an expression of inhabiting the
           other

      The authors highlight how, in the clinical practice of the psychoanalytic multifamily group, the emergence of traumatic memories and related emotional experiences allows us to perceive the existence of pathogenic "presences" in the internal world of suffering people. Such "presences" possess a permanent, secret and occult power of conditioning, very hard to grasp in other contexts. Pathogenic identifications that block the development of their own egoic re-sources are structured around these "presences" and the subject will live inhabited and pos-sessed by others who prevent him from being himself. This paper describes how the specific functioning of the group makes it possible to start processes for the construction of transform-ative identifications which are gradually able to reduce the pathogenic power of such presences. Mutual mirroring, the functioning of the group as an "enlarged mind" and the emergence of healthy personal virtuality are the elements that allow the person to start think-ing, through the other, what was impossible to think on his or her own. In particular with re-spect to his or her own pathogenic interdependencies. The presence of the other becomes of support in the disclosure of an authentic Self and in the construction of an integrous subjective identity.

      According to Jorge Garcia Badaracco, the gaze with which a patient is observed greatly in-fluences the outcome of the treatment. To help the people to discover their "healthy virtuality", it is necessary that first the person in charge of the treatment observes it. Starting from this assumption, the authors reflect on what are the factors (intrinsic and extrinsic) that influence the gaze, and how important it is to modify first the way in which the caregivers observe pa-tients and their families. Fear is a core element in contributing to the raise of defenses. It is the normality that can frighten a person suffering from psychosis, as well as his family and his caregivers, preventing him/her from seeing his/her "healthy virtuality". Have access to it implies tolerating deep pain; doing it within a multi-family psychoanalysis group help family members and practitioners to feel less frightened and less suffering.

      Reflections on the psychotherapeutic pathway in dissociative disorders, between individual dimension and multifamily psychoanalysis group.In the course of individual psychotherapy, it often becomes clear to the therapist that important dissociated parts are present, as well as the perception of how much these dissociative defences were necessary, the only ones available to protect the subject from the fear of the flood of painful and intolerable, unthinkable feelings; from the fear of experienced traumas and their repetition. As Bromberg states, these are necessary defenses to maintain the necessary continuity and integrity of the Self. The simultaneous participation in the multifamily psychoanalysis group can be very important for the subject to have the possibility to get in touch with dissociated parts of himself, otherwise kept at a distance, even if in the meantime, one engages in individual psychotherapeutic work. Listening to the patient during the session can show us, even with great clarity, how much the participation in the group allows him to approach his suffering, supported also by a network of relations and shared meanings. The contribution of the two therapeutic dimensions can allow, by enlarging and alternating the two fields of work, to open spaces of elaboration and new awareness, usable to favour a tolerable integration for the subject and for the therapeutic couple itself.

  • Multifamily groups in a therapeutic community: the exercise of the
           extended mind

      Abstract:
      The authors deal with an experience carried out in therapeutic communities and try to un-derstand the functioning of the expanded mind within the multifamily psychoanalytic group (GMF). They propose the reasons for choosing this methodological model, referring to anti-institutional experiences and the specific characteristics of group therapy. They then highlight the classical and particular therapeutic factors of GMF, as well as those inherent in community treatment. They then propose some clinical vignettes from which they deduce a theoretical hy-pothesis concerning the development of a previously absent fantasy of care that involves all the members of the group through the development of an enlarged mind. The hypothetical mechanism of change of the bonds between the members of the group and in the internal groupality of the individuals is described.They concludes by describing the significant characteristics of the use of the multifamily group within a therapeutic community.

      Within a long-lasting analysis of a patient, who suffered from two past psychotic epi-sodes, the author examines a peculiar phenomenon: the patient sets up some lively interactions with the animal world, very different from the usual relationships with a pet. It regards quite a lot of contacts with animals (mainly with birds), that stay in their environment and with that the patient meets up on the border between their world and hers; the contact is carried out by means of pre-verbal and body language; the experience has got a special intensity as an important and mysterious moment for both. According to the author, in addition to the rich transitional area development, for the patient the motivation to know, the epistemophilic drive, be-fore involved in the patient’s dynamic psychopathology, is paramount in these contacts in a new libidinal way.

  • Considerations about the difficult experience of giving psychological
           support to migrant asylums seekers

      The authors reflect on the difficulty in the understanding - resulting inattempts at homolo-gation and misinterpretations - and on the complex psychic mechanisms that occur among the staff of the receptioncentres for migrants and even among the psychotherapists appointed to providethe migrants with psychological support.They consider the manifestations of the post-traumatic suffering of those who endure the dramatic experiences of uprooting, life-threatening events, different forms of slavery, impossibility or incapacity to plan for the future, and of an abrupt, harshdescent into a world with rules migrants don’t understand. A world that, despite its pretence of welcoming them, controls them, segregates them, and then suddenly releases them from the reception centresin the assumption that they have the skills and resilience to adapt and integrate. Often, and over a lengthy period of time, traumatic experiences don’t find words nor adequate symbolic representations, and henceforth they "speak" through somatiza-tions, physical complaints, references to examples and models belonging to the culture of origin, all of which results in bewilderment and helplessness among those who are supposed to listen and comprehend. According to the authors, cultural differencespose not only the problem of the correct in-terpretation of the uneasiness and symptoms mentioned, but also questions on the adequacy of the setting and the interpretative keys utilised in our culture. The article mentions some trans-cultural approaches and the psychotherapists’ need for a shared space for comparison on counter-transferal life experiences, often overwhelming, which, if not understood and processed, may lead to attitudes of renunciation and avoidance.

      In a prison that is closing for good, an unexpected administrative obstacle prevents the transfer of twelve inmates, forcing five officers to remain on duty. A film shows us the inter-actions in that prison of two cohabiting microcosms and we associate images and emotions experienced in the face of the reactions between operators, patients and family members who find themselves working together.
