  • Asian American and Native Hawaiian/Pacific Islander Substance Use and

      Abstract: Purpose of Review This study aimed to describe the current state of the science on Asian American (AA) and Native Hawaiian/Pacific Islander (NH/PI) substance use, covering extant research and treatment knowledge and gaps. Recent Findings Despite literature gaps, epidemiological studies revealed sizable population-level differences between AAs, who have among the lowest rates of US substance use, and NH/PIs, who have among the highest. Targeted studies identified key challenges affecting NH/PI communities and some AA subgroups, particularly for alcohol, cigarette, and e-cigarette use. Of major concern, both AAs and NH/PIs demonstrate significant treatment disparities. Summary There is a scarcity of substance use research and interventions specifically targeted for AAs and NH/PIs. Promisingly, a growing body of research is focused on developing/testing culturally targeted interventions for these populations, but more is needed. Recommendations for improving substance use data and intervention research with AAs and NH/PIs—including disaggregating both racial groups and their subgroups—are discussed.

  • Negative Consequences Experienced by Individuals with Gaming Disorder
           Symptoms: A Systematic Review of Available Longitudinal Studies

      Abstract: Purpose of Review Gaming disorder (GD) is a growing concern for health policies worldwide. The aim of the research presented here was to review systematically negative consequences of symptoms of GD by focusing on longitudinal studies that considered the temporal order of the effects. Recent Findings A total of 48 longitudinal studies met the inclusion criteria. The analysis indicated that so far negative consequences of symptoms of GD pertaining to mental health, behavioural problems, social and interpersonal factors, psychological factors, and physical health have been investigated. Summary The most consistent finding is an association between symptoms of GD and depression. In addition, symptoms of GD are an important predictor for symptoms of GD in the future. For other domains, there are few studies and findings are mixed with several studies reporting no association with negative consequences. Future studies should further investigate negative consequences of gaming as well as moderating factors. Regarding prevention and intervention strategies, negative consequences can be addressed in motivational interventions to drive behaviour change.

  • A Narrative Review of Emotion Regulation Abilities, Strategy Use, and
           Their Relationship to Alcohol

      Abstract: Purpose of Review Poor emotion regulation (ER) abilities are associated with coping motives, alcohol use problems, and difficulties recovering from an alcohol use disorder (AUD). This paper reviews the past 5 years of alcohol use and ER research, aiming to advance our understanding of this complex relationship. Recent Findings The indirect effect of self-reported ER abilities on alcohol problems via coping motives has been established. ER strategies are less often investigated and center on the association between reappraisal or suppression and alcohol use problems. Accessing optimal ER strategies and increasing ER abilities, specifically emotional awareness and clarity, appear critical to AUD recovery and need continued research. Summary Studying individual differences in ER abilities while considering ER strategies will provide a clearer understanding of drinking-to-cope and the relationship between ER abilities and alcohol use. Continued investigation of these relationships will help to reduce the public health burden of alcohol-related negative outcomes and AUD.

  • Compulsive Sexual Behavior Disorder and Problematic Pornography Use in the
           Context of Social Ties

      Abstract: Purpose of Review The objective of this review was to (1) outline the current empirical evidence on the impact of social factors on the development and maintenance of CSBs (compulsive sexual behaviors), (2) identify research gaps in this field, and (3) suggest potential future avenues for studying CSBs within a social framework. Recent Findings The evidence highlights insecure attachment as a risk factor for the development of CSBs, indicating a potentially more significant role of attachment anxiety. Higher perceived social support was associated with lower CSB severity; however, the small effect size suggests a supportive role. Loneliness may serve as a risk factor for CSBs across populations. Minority stressors were positively related to CSB severity, while therapy addressing those stressors effectively diminished CSB symptoms. Empirical findings imply complex bidirectional influences between the social bonds’ quality and CSBD/PPU (compulsive sexual behavior disorder/problematic pornography use). Moreover, interpersonal aspects and mutual support are crucial elements in individual, couple, and group treatment for CSBs. Summary Empirical evidence highlights the significance of social factors like loneliness, perceived social support, attachment anxiety, avoidance, and romantic bond quality in CSBD/PPU. Social factors should be considered in clinical practice as they may be involved in the development and maintenance of CSBs. Addressing them in therapy may facilitate the treatment process. However, methodological differences across studies make drawing definitive conclusions impossible. More longitudinal data from multicentered projects using standardized methodologies to clarify causality of the relationships are needed.

  • Compulsive Sexual Behavior in LGBTQ Individuals and Minority Stress
           Theory: A Literature Review and Theoretical Integration

      Abstract: Purpose of Review This review summarizes research on compulsive sexual behavior (CSB) among lesbian, gay, bisexual, transgender, and queer (LGBTQ) individuals and integrates this literature with minority stress theory. Recent Findings Research examining CSB in LGBTQ populations spans assessment, prevalence, etiology, comorbidity, and treatment domains. Assessment research presents concerns that CSB symptoms may be confused with aspects of an LGBTQ client’s sociocultural context, particularly minority stress processes (e.g., LGBTQ-related discrimination, internalized stigma). Due to assessment difficulties, it is unknown whether LGBTQ individuals exhibit a higher CSB prevalence than heterosexual and cisgender populations. Etiologically, minority stress may contribute to the development and maintenance of CSB and its comorbidities, including depression and anxiety. Lastly, LGBTQ-affirmative cognitive behavior therapy, an empirically supported treatment based on minority stress theory, is a promising intervention for CSB. Summary This review suggests that minority stress theory may enhance understanding, assessment, and treatment of CSB in LGBTQ populations.

  • Substance Use and Anxiety Disorders: A Scoping Review

      Abstract: Purpose of Review We aimed to evaluate the significance and directionality of the relationship between substance use disorders (SUDs) and anxiety disorders (ADs), as this remains unclear. Recent Findings Following PRISMA guidelines and a scoping review methodology, 25 articles met inclusion criteria and were searched using PubMed, Ovid, MEDLINE, and PsychInfo through June 2023. Our results reveal strong associations between substance use and ADs, with alcohol and cannabis exhibiting the most robust relationships. Preliminary evidence points to a potential bidirectional relationship between substance use and ADs, however, more longitudinal and experimental studies are needed to demonstrate causal results. Importantly, individuals with comorbid ADs and SUDs were shown to experience greater symptom severity, have poorer treatment response, and increased rates of relapse compared to those with either disorder alone. Summary Our scoping review thus highlights the need for integrated treatment approaches to address comorbid substance use and ADs and improve treatment outcomes for those experiencing dual diagnosis.

  • Digital Interventions for Improving Pain Among Individuals With and
           Without Opioid Use Disorder and Reducing Medical and Non-medical Opioid
           Use: a Scoping Review of the Current Science

      Abstract: Purpose of Review Both chronic pain and opioid use disorder (OUD) are highly prevalent. Emerging technologies may improve pain- and opioid-related outcomes. This scoping review synthesized current evidence on digital interventions for acute and chronic pain in patients with and without OUD and the impact on pain- and opioid-related outcomes. Recent Findings Screening of 403 abstracts yielded 18 studies that were evaluated for eligibility. Of the 18 studies that met review criteria, 14 were randomized-controlled trials (RCTs). Sixteen studies showed positive intervention effects in improving pain and/or medical opioid use outcomes. Internet/app-based interventions yielded slightly more positive results than VR interventions for reducing both pain and medical opioid use and misuse behaviors; engagement in some internet/app-based interventions was limited. Few studies examined pain among OUD populations; no studies evaluated the effects on non-medical opioid use. Summary Digital interventions, particularly internet/app-based strategies, may be feasible and beneficial approaches for improving pain in those with and without OUD and reducing medical opioid use and/or misuse behaviors. Factors influencing intervention engagement require further study. Larger scale RCTs with heterogenous populations may improve the evaluation of intervention efficacy and generalizability.

  • Conceptualizations of Craving and the Pornography Craving Questionnaire: A
           Scoping Review

      Abstract: Purpose of Review Research has demonstrated that craving is a salient aspect of behavioral and substance use addictions. In 2014, the Pornography Craving Questionnaire (PCQ) was published as the first comprehensive scale to assess craving for pornography. This review was undertaken to understand conceptualizations of craving in behavioral addictions and identify all research studies assessing the subjective experience of craving for pornography using the PCQ. Recent Findings Craving has been conceptualized as a passionate attachment, ambivalence, and desire thinking. The PCQ has been administered in multiple studies in various countries. These investigations have indicated that the PCQ is positively related to desire thinking, passionate attachment (i.e., obsessive and harmonious passion), future pornography use (e.g., duration and frequency), mental health concerns (e.g., anger, anxiety, depression, behavioral and substance use addictions), dysfunctional coping (i.e., the tendency to use sex in response to negative mood states and stressors), and problematic pornography use. Summary Craving is a salient dimension of pornography use. Additionally, the PCQ appears of value to measure craving for diagnostic and outcome evaluation purposes. We recommend further evaluation of the PCQ in clinical settings to predict the course and prognosis among help-seeking patients for problematic pornography use.

      Abstract: Purpose of Review In this article, we dig more deeply into the assumptions underpinning common-sense understandings about youth “exposure” to pornography and the ostensible “effects” of this exposure, specifically “pornography addiction”. We trace the emergence of the notion of “pornography addiction”, highlighting how cultural conditions have allowed for its realisation as a socially recognisable and intelligible narrative. Recent Findings Media effects research on the issue of youth “exposure” to pornography is not conclusive, nor is pornography addiction officially recognised as a diagnosable disorder. Moreover, an emerging body of multidisciplinary qualitative research, which, importantly, includes the perspectives and experiences of young people themselves, raises questions about some of the assumptions and conclusions of effects-focused research. Summary Despite inconclusive and contrasting findings, the social narrative of pornography addiction persists both within and beyond academe. We make sense of this persistence in relation to the broader problematisation of youth sexuality, which includes unease and moral objection to young people viewing pornography. It is important to recognise the broader socio-cultural systems supporting the cultural narrative of pornography addiction, and the social functions that this narrative serves, that is, the need for adult intervention into the sexual lives of youth.

  • rTMS for Co-occurring Psychiatric and Substance Use Disorders: Narrative
           Review and Future Directions

      Abstract: Purpose of Review Co-occurring substance use disorders (SUDs) are highly common in individuals with psychiatric illnesses. Individuals with comorbid psychiatric illness and SUDs may experience poorer mental health and decreased treatment efficacy. However, there are no FDA-approved treatments for co-occurring substance use and psychiatric disorders. Repetitive transcranial magnetic stimulation (rTMS), a form of non-invasive brain stimulation that uses an electromagnetic field to change brain activity and behavior, may be a promising treatment for co-occurring disorders. A comprehensive literature search of PubMed, MEDLINE, and Google Scholar databases was conducted, and records were manually reviewed to include all studies testing the effects of rTMS for co-occurring SUDs. Recent Findings Eleven studies met our inclusion criteria. The majority (7/11) assessed rTMS for the treatment of schizophrenia and co-occurring substance use, and the remaining three studies assessed rTMS for the treatment of SUDs in mood and anxiety disorders. Potential neural circuitry targets for the treatment of co-occurring substance use and post-traumatic stress disorder, anxiety disorder, and bipolar disorder are discussed. We identify future directions and considerations for rTMS treatment and research. Namely, we recommend identification of novel treatment targets, the use of pragmatic treatment approaches, the evaluation of rTMS for substance withdrawal, the evaluation of state dependence as a predictor of treatment efficacy, the use of neurobiological measurements to identify underlying neural circuitry, and the assessment of individual predictors of rTMS treatment response. Summary There is preliminary evidence suggesting rTMS may be effective to treat co-occurring disorders, but additional research is needed.

  • Neural Responses in Addictive Eating: a Systematic Review

      Abstract: Purpose of Review It has been suggested that similar brain regions are activated to ultra-processed food as in substance addiction, giving rise to the concept of “food addiction”. However, few neuroimaging studies have used specific assessment tools for food addiction, with obesity often used as a proxy, leading to considerable variability in neural activation patterns associated with food addiction. This systematic review aimed to synthesise published studies using task-related and resting-state fMRI to assess neural responses associated with food addiction, using a validated assessment tool, the Yale Food Addiction Scale (YFAS). A systematic review was conducted from 2008 to July 2022 according to the PRISMA guidelines. Studies were included if they reported YFAS food addiction outcomes in combination with task-related or resting-state fMRI outcomes. Two reviewers independently appraised studies and data were synthesised descriptively. Recent Findings Seven task-related and four resting-state fMRI studies were included, involving 675 predominantly female participants. Brain areas involved in reward, craving, and appetitive responses were associated with YFAS-assessed food addiction in response to high-calorie processed foods in task-related fMRI studies, as well as impaired inhibition in non-food-related tasks. In the resting-state studies, altered connectivity in reward-related regions and cognitive control were commonly reported. However, activation across included studies was not consistent, with some studies reporting no relationship between food addiction scores and neural activation. Summary There was a lack of consistency in activation across studies which may be due to the heterogeneity of study designs. It is difficult to ascertain if there are indeed neural activation patterns that are unique to food addiction and what components of food may have addictive potential. Future studies are required with replicable study designs.

  • Will Internet Availability Fuel Problematic Internet Use' A Cross-Temporal
           Meta-analysis of Patterns in China from 2003 to 2020

      Abstract: Purpose of Review Problematic internet use (PIU) has varied across countries and eras, but how it may relate to internet availability has not been well examined. This cross-temporal meta-analysis aimed to explore the association between PIU severity changes and internet availability development in China over the past few decades and how it may relate to gender and age. Recent Findings The results showed that China’s overall PIU level has increased over time, and these changes were associated with the increase in factors related to internet availability. Gender-related differences in PIU decreased over time, and it appeared related to more gender equality in internet penetration and use. In addition, college students with relatively higher internet availability were found to have higher overall PIU scores than secondary-school students, and that their PIU scores increased over time, while secondary-school students’ scores remained largely consistent. Summary PIU severity increased in China during 2003–2020, and it appears to be associated with the increase in internet availability, which supports the internet availability hypothesis on PIU.

  • Pharmacological Treatments for Alcohol Use Disorder: Considering the Role
           of Sex and Gender

      Abstract: Purpose of Review This qualitative literature review examined sex and gender differences in alcohol use disorder (AUD) pharmacotherapy treatment response. Specifically, we reviewed the clinical and preclinical literature evaluating sex and gender differences in responses to US Food and Drug Administration (FDA) and European Medicine Agency (EMA) approved medications, and repurposed, or off-label, medications for the treatment of AUD. Recent Findings Despite important sex and gender differences in AUD, the effect of sex and gender in the pharmacological treatment of AUD has been understudied. Relatively low representation of women in clinical studies has limited well powered investigations of gender effects on treatment response. Additionally, most preclinical support for AUD medications comes from studies in males. More recent AUD pharmacotherapy research has identified potential sex and gender differences in the efficacy of several pharmacological treatments for AUD. Although findings are mixed, clinical data suggest naltrexone may be more effective at reducing alcohol use in men, while baclofen may be more effective at reducing alcohol use in women. Preclinical studies have indicated sex-differences in the efficacy of topiramate and mifepristone, with putative higher efficacy of topiramate in males and mifepristone in females. Summary Historically, sex and gender differences have not been adequately considered in AUD medication development research. In order to improve translation of preclinical findings and clinical utility, sex and gender must be considered at each stage of drug development research.

  • Cigarette Smoking and Psychiatric Illness Among Individuals with COPD: a
           Systematic Review

      Abstract: Purpose of Review Both smoking and psychiatric illness are independently associated with poorer COPD-related health outcomes, but little is known about the course of COPD among people who smoke and have co-occurring psychiatric illness. This systematic review assessed the links between tobacco smoking and psychiatric illness among individuals with COPD. Recent Findings Depression and anxiety disorders are positively related with current smoking, nicotine dependence, cigarette use for at least 10 pack-years, and mortality risk among patients with COPD. Several studies also identified schizophrenia as an independent risk factor for incident COPD, after adjusting for smoking history. Summary There was an association between depression, anxiety, and schizophrenia with tobacco smoking in COPD. Additional research should examine relationships between other psychiatric illnesses (e.g., personality disorders, PTSD) and tobacco smoking and track COPD-related symptom change over time as a function of smoking cigarettes or newer nicotine products (e.g., e-cigarettes) and with smoking cessation.

  • Substance-Induced Mood Disorders: A Scoping Review

      Abstract: Purpose of the Review Substance-induced mood disorders (SIMDs) are an understudied area with a limited published literature. This scoping review evaluated published evidence to determine differences between SIMDs and independent mood disorders with comorbid SUD. Recent Findings Following Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines, English language articles were searched through October, 2023. Thirty-one studies were identified which explored the relationship between alcohol, cocaine, opioids, methamphetamine, cannabis, and multiple substance use and the induction of mood disorders. Varying symptoms and risk factors were identified for SIMD, specifically for alcohol and opioids. However, confounding factors and heterogeneity in the majority of studies make it difficult to establish clear differences between these groups. Summary Although our findings suggest risk factors and symptoms that may be implicated in SIMD, results are inconsistent. Further research using well-controlled, experimental, and longitudinal designs are needed to parse differences between SIMDs and comorbid mood disorders with SUDs.

  • Genetic Influences on Alcohol Sensitivity: a Critical Review

      Abstract: Purpose of Review The present review provides a summary of the progress that genome-wide association studies (GWAS) have made in identifying the genetic variants that underlie the heritability of alcohol use disorder (AUD) and then provides a similar summary of alcohol sensitivity studies, an important endophenotype that can help to expand our understanding of the genetic architecture of AUD. Recent Findings GWAS of AUD have recently achieved the necessary sample sizes required to identify genome-wide significant risk loci and demonstrated that the disorder is genetically complex with many loci of small effect contributing to its etiology. Consequently, we describe how these studies have begun to explore AUD’s genetic correlations with other alcohol-related traits and with other psychiatric disorders to better understand the genetic etiology of AUD and its underlying mechanisms. GWAS investigating alcohol sensitivity using the Self-Rating of the Effects of Alcohol (SRE) as an AUD endophenotype have struggled to make similar progress. We describe the advances that these studies have made in terms of further demonstrating the heritability of alcohol sensitivity along with limitations in terms of identifying genome-wide significant loci and what we see as the primary challenges facing such studies. We conclude with suggestions for future research and what could be achieved with larger samples. Summary Progress in the molecular genetic study of AUD and related phenotypes is being made. Nonetheless, significant efforts are needed to conduct GWAS of more specific AUD risk factors, such as alcohol sensitivity, to more fully understand how individual genetic loci contribute to AUD risk. In doing so, we will be better positioned to understand the etiologic relations between these AUD risk factors and how their confluence ultimately increases risk for AUD.

  • Use of Digital Health and Digital Therapeutics to Treat SUD in Criminal
           Justice Settings: a Review

      Abstract: Purpose of Review The purpose of this review is to investigate the use of digital health technologies and/or digital therapeutics (DTx) products in the treatment of substance use disorders (SUDs) in the general population and among criminal justice–involved individuals. Recent Findings Despite an expanding evidence base, only three SUD DTxs have received federal regulatory approval. Across studies, DTx products have proven successful in engaging patients in SUD treatment and reducing healthcare costs and resource utilizations. Findings for emerging SUD DTx products show similar results. Still, there is a paucity of evidence regarding the use of digital health technologies and/or DTx among criminal justice populations. Summary DTxs have proven effective for treating multiple SUD types (e.g., nicotine and opioids) among the general population. DTx shows similar promise among justice-involved populations, but additional efficacy and implementation research is needed to address barriers such as cost, cultural resistance, and infrastructure.

  • Within and Beyond the Binary: Sex and Gender Differences in Pain and
           Alcohol Use Disorder

      Abstract: Purpose of Review Alcohol use disorder (AUD) and chronic pain are frequently comorbid conditions. This review aims to communicate recent research describing the bidirectional effects of pain experience and alcohol use, while focusing on sex and gender differences at this intersection with an emphasis on translational research. Recent Findings Most pain syndromes affect people assigned female at birth (AFAB). Although people assigned male at birth (AMAB) typically drink more than AFABs, AFABs progress to AUD more quickly and experience more alcohol-related comorbidities. In the preclinical setting, different alcohol models highlight sex differences in the escalation of alcohol drinking, while a state of alcohol dependence appears to produce sex-specific alterations in withdrawal-induced pain- and anxiety-like behaviors. Summary Preclinical and clinical studies are accelerating in order to elucidate important sex differences in both chronic pain and AUD. A better understanding of the physiological mechanisms underlying pain and AUD will support the discovery of more effective, potentially sex-specific, therapeutic options for these devastating co-morbid conditions.

  • Substance-Induced Psychosis: a Narrative Review

      Abstract: Purpose of Review Novel integrative reviews that give detail information regarding various psychoactive substances (PS) with ability of inducing psychosis in adults are lacking. This review aims to scope the latest knowledge about substance-induced psychosis (SIP) through the comprehensive review of the most common misused PS inducing psychosis. Recent Findings Epidemiological data on the prevalence of SIP are insufficiently harmonized, so clear conclusions for most PS can not be drawn. Etiological mechanisms include three pathophysiological mechanisms: the modulation of GABA transmission, NMDA antagonism, and effects on the monoaminergic system. Beside positive symptoms, PS may induce negative and cognitive symptoms. There are no uniform therapeutic guidelines for specific SIP, but some antipsychotics appear to be more effective than others. It is likely that the conversion rate of SIP to schizophrenia may be higher in the future because novel PS with a higher psychotic potential are increasingly being consumed. Summary This review presents epidemiological data, etiological mechanisms, details of clinical pictures, and treatment options for psychoses induced by specific PS.

  • Alcohol Use Disorder and the Gut Microbiome

      Abstract: Purpose of Review Alcohol use Disorder (AUD) is a widely prevalent health concern, and it profoundly impacts an individual’s mental, physical, and psychosocial well-being. AUD and alcohol-associated liver disease (ALD) therapies are limited and rapidly evolving. Over the past few years, there has been a growing interest in the disruption of the gut microbiome caused by chronic alcohol use and how new therapies targeting the microbiome could be potentially used to alter the course of this disease. Recent Findings There are significant changes in the gut microbiome caused by alcohol consumption. These changes lead to the breakdown of the intestinal mucosal barrier, allowing the translocation of gut-derived inflammation agents, which subsequently cause inflammation in the brain and liver. Therapies focused on restoring a normal gut microbiome in patients with AUD are being explored. The alterations in microbiome constitution and gut dysfunction could be relatively easy to reverse by following abstinence within a short period of time. Preliminary data strongly show that supplementing the microbiome with probiotics and fecal microbiota transplantation may be a useful intervention strategy. Elimination of the state of abundant pathological organisms with antibiotic therapy has also been investigated, with potential implications in microbiome modulation to obtain beneficial effects. Summary There is a huge potential for the use of microbiome-directed therapies for the management of AUD and ALD. Further long-term and controlled studies in humans are highly needed.
