    • Abstract: A Campanha "Não à Medicalização da Vida", organizada pelo Conselho Federal de Psicologia, critica contundentemente a identificação e tratamento de crianças com transtornos de aprendizagem e com o Transtorno do Déficit de Atenção e Hiperatividade. Isso gerou uma resposta imediata de entidades ligadas à psiquiatria, neurociências e neuropsicologia. O propósito deste artigo foi explicar o que a Campanha entende por medicalização e apresentar seus argumentos contra essa prática (o argumento da falta de objetividade científica, da pressão da indústria farmacêutica, da estigmatização e dos efeitos indesejados). A análise desses argumentos conclui que todos eles são falhos e que a campanha mistura uma preocupação terapêutica legítima em relação à medicação excessiva e mal-informada (uso não-racional) com suposições incompatíveis com a ciência.The "No to the Medicalization of Life" campaign, organized by the Federal Council of Psychology, criticizes forcefully the identification and treatment of children with learning disabilities and Attention Deficit Hyperactivity Disorder. It begot an immediate response from entities related to psychiatry, neurosciences and neuropsychology. The purpose of this article was to explain what the campaign means by medicalization and to present its arguments against this practice (especially the argument of lack of scientific objectivity, the argument of the pressure from the pharmaceutical industry, the argument of stigmatization and the argument of adverse effects). The analysis of those arguments concludes that they are all flawed and that the campaign mixes a legitimate concern with the excessive and ill-informed medication (non-rational) with assumptions incompatible with science.

    • Abstract: As funções executivas (FE) são funções mentais complexas, responsáveis pela autoregulação. Apesar do crescente interesse científico no estudo dessas funções, ainda existem indefinições acerca do construto. Este artigo objetiva agregar evidências de pesquisas no campo da avaliação neuropsicológica infantil. Trata-se de revisão sistemática de publicações indexadas nas bases de dados PsycINFO e PubMed, nos últimos cinco anos (2008-2013). Foram identificados 25 artigos. Os resultados indicam aumento da produção científica sobre o tema nos três últimos anos. Os principais instrumentos de avaliação das FE identificados foram o BRIEF, as escalas Wechsler e tarefas baseadas nos paradigmas Go/No-Go e Stroop. Observou-se relativo consenso acerca da natureza multidimensional das FE. Contudo, permanecem divergências teóricas e metodológicas quanto à natureza e à quantidade desses componentes.The executive functions (EF) are complex mental functions, responsible for autoregulation. Despite growing scientific interest in the study of these functions, there are still uncertainties about the construct. This article aims to add evidence of research in the field of neuropsychological assessment of children. This is a systematic review of publications indexed in the databases PsycINFO and PubMed in the last five years (2008-2013). Twenty-five articles were identified. The results indicate an increase of scientific literature on the topic in the last three years. The main identified tools for evaluating EF were BRIEF, Wechsler scales and tasks based on Go/No-Go and Stroop paradigms. A relative consensus about the multidimensional nature of the EF was observed. However, theoretical and methodological differences remain as to the nature and amount of these components.

    • Abstract: O objetivo foi identificar estudos que investigaram o processamento auditivo (PA) e linguístico na avaliação clínica e/ou neuropsicológica de crianças/adolescentes com TDAH. Buscamos em base de dados PubMed de 2000 a 2012 pela sintaxe [language or linguistic] AND [auditory processing or auditory perception] AND [attention deficit or hyperactivity]. Os abstracts foram analisados por double blind review com terceiro juiz para consenso. De 202 resumos, foram analisados 17 artigos completos, com 15 incluídos; 46,67% dos estudos avaliaram a linguagem (palavra e sentença), 33,33%, PA (integração binaural, processamento temporal e figura-fundo auditiva) e 13,33%, ambos. O transtorno de leitura foi a comorbidade mais prevalente (46,67%). Em geral, o desempenho linguístico e de PA não foram analisados para fins neuropsicológicos específicos, mas apenas para identificar comorbidades.The aim of this paper was to identify studies that investigated auditory processing (AP) and language along clinical and/or neuropsychological assessment of children/teenagers with ADHD. We searched on PubMed database from 2000 to 2012 by means of the syntax [language or linguistic] AND [auditory processing or auditory perception] AND [attention deficit or hyperactivity]. s were analyzed by a double blind review with a third expert for a consensus. From 202 abstracts, 17 full texts were analyzed and 15 ones were included; 46.67% of the studies assessed language (word and sentence levels), 33.33% evaluated AP (binaural integration, temporal processing and auditory figure-ground) and 13.33% examined both of them. Reading disorder was the most prevalent comorbidity (46.67%). In general, AP and linguistic performance were not analyzed for specific neuropsychological purposes, but only for comorbidities identification.

    • Abstract: Visando delimitar as funções neuropsicológicas comprometidas nos quadros de dificuldade específica em compreensão leitora, o desempenho de maus compreendedores (leitura de palavra preservada e compreensão leitora deficitária; n=19) e bons leitores (leitura de palavras e compreensão leitora adequadas; n=58) foi comparado em uma bateria de avaliação neuropsicolinguística, objetivando verificar as funções neuropsicológicas relacionadas com o desempenho em compreensão. Os valores foram ajustados para série e tipo de escola (a amostra de alunos de escolas públicas e privadas se distribuía entre a quarta e a sexta série). A variável grupo - que expressa a diferença entre maus compreendedores e bons leitores - foi capaz de explicar significativamente a variabilidade no desempenho de quatro tarefas: memória de trabalho visuoespacial, fluência verbal ortográfica e semântica, e escrita de palavras e pseudopalavras. Memória de trabalho e funções executivas devem integrar avaliação e intervenção nas dificuldades específicas em compreensão leitora.Aiming to define impaired neuropsychological functions in cases of specific reading comprehension difficulties, poor comprehenders (high performance in word reading and low performance in reading comprehension; n=19) and good readers (high performance in word reading and reading comprehension; n=58) underwent a battery of neuropsycholinguistic assessments in order to verify which neuropsychological functions were related to comprehension performance. The values were adjusted for grade and type of school (the sample ranged from fourth to sixth grade, and the students attended public and private schools). The variable group - which expresses the difference between good readers and poor comprehenders - was able to significantly explain the variability in the performance of four tasks: visuospatial working memory, orthographic and semantic verbal fluency and writing words and pseudowords. Working memory and executive functions should integrate assessment and intervention in specific reading comprehension difficulties.

    • Abstract: This study aimed to present the neuropsychological assessment of MC, a seven-year-old girl, who was diagnosed with acute viral cerebellitis. The protocol included cognitive tests, stress and mood scales, behavioral observation, and interviews with family and school. In the neuropsychological assessment, difficulties in crystallized skills, especially in language and numerical cognition domains, were observed. The results of the neuropsychological assessment were discussed to detail her neurocognitive profile and verify to what extent her impairment could be attributed to the acute viral cerebellitis.O estudo teve como objetivo apresentar a avaliação neuropsicológica de MC, menina de sete anos, diagnosticada com cerebelite viral aguda. Foram utilizados testes cognitivos, escalas de humor e estresse, observação comportamental, além de entrevista familiar e escolar. Na avaliação neuropsicológica, foram observadas dificuldades nas habilidades cristalizadas, em especial nos domínios da linguagem e cognição numérica. Os resultados da avaliação neuropsicológica foram discutidos visando detalhar o seu perfil neurocognitivo e verificar em que medida seus prejuízos poderiam ser atribuídos à cerebelite viral aguda.

    • Abstract: Recentes achados no campo da neuropsicologia apontam para possíveis prejuízos neurocognitivos associados ao Transtorno Bipolar (TB) em crianças e adolescentes. O presente estudo buscou avaliar o funcionamento cognitivo de adolescentes com TB. Participaram dele 23 adolescentes diagnosticados com TB e 20 adolescentes controles. Utilizou-se uma bateria neuropsicológica e clínica para avaliar as múltiplas funções. Os grupos apresentaram desempenho similar nas tarefas neurocognitivas, entretanto, evidenciou-se prejuízos relacionados à memória de trabalho visuo-espacial, planejamento e tomada de decisão. A identificação precoce de prejuízos neurocognitivos pode ser um fator determinante para minimizar o impacto de tais prejuízos no funcionamento global de crianças e adolescentes. Além disso, possibilita maior eficácia das estratégias de intervenção e prevenção de recaídas nestes indivíduos.Recent researches in neuropsychology indicate neurocognitive impairments in children and adolescent with Bipolar Disorder (BD). This study aims to assess the cognitive functioning of adolescents diagnosed with BD. In this, 23 adolescents with BD and 20 healthy controls were enrolled. A clinical and neuropsychological battery was used to assess multiple functions and conditions of the participants. Both groups showed similar performance in the neurocognitive tasks, however, there were evidences of impairments related to visual working memory, planning and decision making. The early identification of neurocognitive impairments could be the key for minimize the impact of these deficits in the global functioning of children and adolescents. Besides, early identification increases efficacy of strategies of prevention of relapse in those individuals.

    • Abstract: Objetivo: Este estudo visou ao desenvolvimento e avaliação de consistência interna de tarefas para avaliação de distúrbios da linguagem na abordagem psicolinguística seguindo os critérios da bateria Psycholinguistc Assessment of Language. Métodos: Cinco tarefas de compreensão e quatro de produção de linguagem foram desenvolvidas em português do Brasil e administradas a um grupo clínico com diagnóstico de afasia (n=26) e um grupo controle (n=55). As amostras não foram pareadas, pois o foco recaiu sobre as tarefas. Resultados: Como esperado, o grupo clínico obteve escores significativamente mais baixos, porém consistentes. Conclusões: Ao permitirem discriminar entre os grupos, as tarefas foram adequadas à aplicação em populações com distúrbio de linguagem adquirido. Permitiram ainda, conhecer as habilidades preservadas no grupo clínico, já que os subsistemas linguísticos avaliados por cada tarefa revelaram-se diferentemente comprometidos.Objective: This study aimed at developing and testing for internal consistency of tasks for the assessment of language disorders based on the psycholinguistic approach following the criteria of the Psycholinguistc Assessment of Language. Methods: Five language comprehension and four language production tasks in Brazilian Portuguese were designed and tested in a clinical group diagnosed as aphasics (n=26) and a control group (n=55). Groups were not paired since the tasks were at stake. Results: As expected, the clinical group scored significantly lower, but with consistent results. Conclusions: Since it discriminated between groups, the tasks were appropriate for use among the population with acquired language impairment. The psycholinguistic subsystems were distinctively impaired within the clinical group, by means of which it is possible to identify the spared language abilities.

    • Abstract: Estudos demonstram a importância dos núcleos da base e do tálamo em memória, atenção, seleção e monitoramento de informações. Nesta pesquisa analisamos quatro casos com lesão subcortical unilateral de hemisfério esquerdo pós-AVC. As participantes foram avaliadas com o Questionário de Aspectos de Saúde e Socioculturais e o Instrumento de Avaliação Neuropsicológica Breve NEUPSILIN. Objetivou-se verificar e discutir discrepâncias no desempenho em diferentes tarefas de memória de pacientes com lesões subcorticais. Todas as pacientes apresentaram prejuízo na tarefa de evocação imediata e nenhuma na de evocação tardia. Os resultados parecem estar relacionados à menor capacidade para criar estratégias, organizar as informações e reter novos estímulos. Ressalta-se o envolvimento dessas estruturas em circuitos que, em conjunto com regiões corticais, fazem parte do processamento da memória operacional e das funções executivas. Estudos futuros devem incluir amostras mais amplas e controlar a influência das variáveis sociodemográficas.Studies show the importance of the basal ganglia and thalamus in memory, attention, selection and monitoring of information. In this research we analyze four cases with unilateral subcortical lesions after stroke. The participants were assessed with the “Questionnaire of sociocultural and health aspects” and the “NEUPSILIN Brief Neuropsychological Assessment Instrument”. The objective was to analyze and discuss discrepancies in performance on different memory tasks in patients with subcortical lesions. All patients showed prejudice in the immediate recall task and none in the delayed recall task. The results appear to be related to a lower capacity to create strategies, organize information and retain new stimuli. We emphasize the involvement of these structures in circuits that are part of, together with cortical regions, the processing of working memory and executive functions. Future studies should include larger samples and control the influence of sociodemographic variables.

    • Abstract: Introduction: The Frontal Assessment Battery (FAB) is a for the assessment of executive functions. We investigated its validity and reliability and proposed normative data in a representative sample of older adults. Method: 391 healthy elderly subjects and 93 Alzheimer's disease (AD) patients performed the FAB. Internal consistency, convergent correlations, ROC curve and logistic regression were used to assess its psychometric properties. Results: We found good internal consistency, significant correlations with other tests and moderate accuracy (64%) for AD diagnosis. The use of specific subtests increased diagnostic potential (75%). Normative data stratified by age and education were proposed. Conclusion: The FAB showed adequate psychometric properties. The normative values stratified by age and education can improve its clinical use for neuropsychological assessment.Introdução: A Bateria de Avaliação Frontal (BAF) é um teste de rastreio para o exame das funções executivas. Investigou-se sua validade e fidedignidade e propuseram-se dados normativos em uma amostra representativa de idosos. Método: 391 idosos saudáveis e 93 pacientes diagnosticados com doença de Alzheimer (DA) realizaram a FAB. Consistência interna, correlações convergentes, curva ROC e regressão logística compuseram a avaliação das propriedades psicométricas do teste. Resultados: Foi encontrada boa consistência interna, correlações significativas com outros testes e acurácia moderada (64%) para o diagnóstico de DA. O uso de subtestes específicos aumentou o potencial diagnóstico (75%). Normatização estratificada por idade e escolaridade foi proposta. Conclusão: A BAF apresentou propriedades psicométricas adequadas. Os valores normativos estratificados por idade e escolaridade podem melhorar o uso desse instrumento no exame neuropsicológico.

    • Abstract: This study investigates discourse coherence and its relation with cognitive deficits in Alzheimer's disease (AD). Participants consisted, in two groups of individuals, 18 with AD in the moderate and moderate severe stages of cognitive decline, and 16 older adults without dementia matched by age, sex and education. Discourse tasks differed according to the presence of non-informative and informative prompts. Verbal comprehension, semantic memory, episodic memory and working memory were tested. Findings showed that global coherence was affected in AD participants. Correlations between discourse and cognitive variables were observed. The strongest correlations found related global coherence to episodic and semantic memory in the informative prompt task. Results are discussed according to clinical and theoretical implications for the understanding of discourse production in AD.Este estudio investiga la coherencia del discurso de las personas con enfermedad de Alzheimer (EA) y las posibles relaciones con déficits cognitivos. Los participantes fueron 18 personas con EA en las fases moderada y severa-moderada y 16 idosos sanos pareados por edad, educación y género. Dos tareas discursivas fueron usadas, una sin ayudas informativas y otra con ayudas informativas. La comprensión verbal, la memoria semántica, la memoria episódica y la memoria de trabajo fueron evaluadas. Los resultados mostraron coherencia global afectada en participantes con EA. Se observaron correlaciones entre el discurso y las variables cognitivas, especialmente entre coherencia y memoria episódica y semántica. La discusión debate la evaluación clínica discursiva y sus implicaciones para entender la comunicacion de personas con Alzheimer.

    • Abstract: Este trabalho investigou a existência de dissociação entre seres vivos e artefatos em dois estudos: um com adultos típicos e outro com a descrição de dois pacientes que receberam diagnósticos de Demência Semântica (DS) ou de Doença de Alzheimer (DA). Para tanto, foram utilizadas as provas de fluência verbal, nomeação, compreensão e definição da Bateria de Memória Semântica de Cambridge. Entre os adultos típicos, o número de acertos em seres vivos foi maior na nomeação, definição e fluência verbal. Diante do controle das covariáveis sociodemográficas correlacionadas com as tarefas, não se encontraram diferenças significativas. Já a investigação dos dois casos identificou diferentes perfis de distúrbio nas provas. Sugere-se que o paciente com DS teve dupla-dissociação em relação aos controles e à paciente com DA. Os resultados foram discutidos com base em modelos de organização do conhecimento semântico e correlatos neuroanatômicos.This study investigated dissociations between nouns representing living and nonliving entities in cognitively unimpaired adults and in two patients, one with Semantic Dementia (SD) and another with Alzheimer's disease (AD), using tests from the Cambridge Battery of Semantic Memory Tests. Among controls a higher number of correct responses in naming, definition and verbal fluency was observed in nouns representing living concepts but differences did not reach statistical significance when we controlled for sociodemographic variables correlated to the tasks. The description of the performance of two patients allowed for recognition of different profiles in the tasks. It is suggested that the patient with SD presented a double-dissociation compared to controls and to the patient with AD. Results are discussed in terms of models of semantic memory organization and neuroanatomical correlates.

A partir desta revisão, recomendamos o uso de medidas de magnitude de efeito na mensuração da eficácia de intervenções em reabilitação neuropsicológica, com especial atenção para o uso do d de Cohen e delta de Cliff, bem como a descrição do poder estatístico dos testes utilizados e seus respectivos intervalos de confiança.The aim of this study was to offer guidelines on the use of quantitative measures to verify the neuropsychological rehabilitation effectiveness, presenting the assumptions for the use of effect size measures. The method was literature review, based on the arbitrary choice of articles in scientific journals and books that addressed the topic. From this review, we recommend the use of effect size measures in the mensuration of the treatment effectiveness in neuropsychological rehabilitation, with special attention to Cohen's d and Cliff's delta usage, as well as the description of the statistical power of the applied tests and their respective confidence intervals.
