  • Loneliness in the elderly: association with health variables, pain, and
           cognitive performance. A population-based study

    • Abstract: ABSTRACT The aim of this population-based study is to analyze the association of loneliness and physical health, pain, health-related quality of life, mental health, cognitive performance, morbidity, and use of health services. Based on the census, a stratified random sampling was chosen. A telephone interview was conducted with 2,060 people over 65 years old, using the GHQ-12 and the COOP-Wonca. An association of loneliness with General Mental Health (GHQ-12) was found, OR = 1.43 (1.34, 1.52) and depression, OR = 2.34 (1.54, 3.53). In cognitive performance the highest effect variable was “memory problems disturb your daily life”, OR = 3.11 (1.25, 7.72); illnesses-related variables: the highest effect variables were perception of health status, OR = 1.37 (1.13, 1. 67) and quality of life (COOP-Wonca), OR = 3.03 (2.32, 3.94); pain-related variables: the highest effect variables were non-localized pain, OR = 2.67 (1.87, 3.83) and arthritis/arthrosis, OR = 1.94 (1.38, 2.72); impaired vision, OR = 2. 62 (1.84, 3.73) and hearing, OR = 1.81 (1.21, 2.72). It is concluded that loneliness is a complex phenomenon associated with variables of different nature. This fact should be taken into account at the time of planning possible solutions.RESUMEN El objetivo de este estudio poblacional es analizar la asociación de la soledad no deseada (SND) con salud física, dolor, calidad de vida relacionada con la salud, salud mental, rendimiento cognitivo, morbilidad y uso de servicios de salud. Recurriendo al censo, se realizó un muestreo aleatorio estratificado en Madrid. Se llevó a cabo una entrevista telefónica a 2,060 mayores de 65 años utilizando el GHQ-12 y el COOP-Wonca. Encontramos asociación de la SND y la salud mental general (GHQ-12), OR = 1.43 (1.34, 1.52) y depresión, OR = 2.34 (1.54, 3.53). En rendimiento cognitivo, la variable de mayor efecto fue “los problemas de memoria alteran su vida cotidiana”, OR = 3.11 (1.25, 7.72); variables relacionadas con enfermedad: las de mayor efecto fueron “percepción del estado de salud”, OR = 1.37 (1.13, 1.67) y calidad de vida (COOP-Wonca), OR = 3.03 (2.32, 3.94); variables de dolor: dolor general, OR = 2.67 (1.87, 3.83) y artritis/artrosis, OR = 1.94 (1.38, 2.72); dificultades para ver, OR = 2.62 (1.84, 3.73) y oír, OR = 1.81 (1.21, 2.72). Se concluye que la SND es una situación compleja, lo que debe considerarse al programar soluciones que sean efectivas.
       
  • Mental illness stigma. A comparative cross-sectional study of social
           stigma, internalized stigma and self-esteem

    • Abstract: ABSTRACT The aim of this study was to explore the role of stigma in different diagnoses of mental illness. A cross-sectional study (N = 255) was developed in two groups: users of a rehabilitation network for people with severe mental illness (Group-I) and people with common diagnoses in an ambulatory psychiatric service (Group-II). Internalized stigma, social stigma, self-esteem, and sociodemographic variables were measured. Mean comparisons, ANOVAs, and independent linear regression models were carried out. Similar overall scores were obtained for the internalized stigma, but Group-I reported more discrimination and resistance to stigma and also had less social stigma. The regression model for Group-I revealed social stigma and self-esteem as predictors, while in Group-II only self-esteem was significant. The study reveals differences in internalized stigma according to the care resource and diagnoses, suggesting different intervention lines and underlining the importance of further research on this topic.RESUMEN El objetivo de este estudio ha sido explorar el papel del estigma en los diferentes diagnósticos de la enfermedad mental. Se llevó a cabo un estudio transversal (N = 255) en dos grupos: usuarios de una red de rehabilitación para personas con enfermedad mental grave (grupo I) y personas con diagnósticos comunes en un servicio psiquiátrico ambulatorio (grupo II). Se midió el estigma internalizado, el social, la autoestima y las variables sociodemográficas. Se llevaron a cabo comparaciones medias, ANOVA y modelos de regresión lineal independientes. Se obtuvieron puntuaciones generales similares para el estigma internalizado, pero el grupo I manisfestó más discriminación y resistencia al estigma y también menor estigma social. El modelo de regresión para el grupo I mostró como predictores el estigma social y la autoestima, mientras que en el grupo II sólo la autoestima era significativa. El estudio muestra diferencias en el estigma internalizado según el recurso de atención y los diagnósticos, lo que sugiere diferentes líneas de intervención y subraya la importancia de seguir investigando en este tema.
       
  • Quality of life of informal caregivers of cervical cancer patients:
           analysis of anxiety and spirituality

    • Abstract: ABSTRACT This study analyzed the association and contribution of anxiety, spirituality, coping and burden to quality of life (QOL) in informal caregivers of patients with cervical cancer. The sample included 119 informal caregivers of patients undergoing treatment for cervical cancer at a reference hospital in the state of Amazonas, Brazil. The caregivers answered instruments that assessed sociodemographic variables, burden, coping, anxiety, spirituality, and QOL. Female caregivers used more spirituality (reflection and positive interpretation) than male caregivers. The number of hours of care dedicated by caregivers was positively correlated with their burden. Burden had a positive association with anxiety and was negatively associated with the caregivers’ mental QOL. Spirituality (reflection and positive interpretation) moderated the relationship between anxiety and caregiver’s mental QOL. The results showed the importance of interprofessional teams in oncology to intervene with caregivers, focusing on psychological morbidity to prevent burden and the spirituality as a coping strategy.RESUMEN Este estudio analiza la asociación y contribución de la ansiedad, espiritualidad, afrontamiento y sobrecarga con la calidad de vida (CV) en cuidadores informales de pacientes con cáncer de cuello uterino. La muestra fue de 119 cuidadores informales de pacientes en tratamiento de cáncer de cuello uterino en un hospital de referencia en el estado de Amazonas, Brasil. Los cuidadores cumplimentaron cuestionarios que evaluaban variables sociodemográficas, sobrecarga, afrontamiento, ansiedad, espiritualidad y calidad de vida. Las mujeres cuidadoras utilizaron más espiritualidad (reflexión e interpretación positiva) que los varones cuidadores. El número de horas de atención dedicadas por los cuidadores se correlacionaba positivamente con su sobrecarga. Esta tuvo una asociación positiva con la ansiedad, asociándose negativamente con la calidad de vida mental de los cuidadores. La espiritualidad (reflexión e interpretación positiva) moderaba la relación entre la ansiedad y la calidad de vida mental del cuidador. Los resultados mostraron la importancia de que los equipos interprofesionales de oncología intervengan con los cuidadores, centrándose en la morbilidad psicológica para prevenir la sobrecarga, así como en la espiritualidad como estrategia de afrontamiento.
       
  • Specific phobia disorder symptoms in people over 65 in Madrid: a network
           analysis

    • Abstract: ABSTRACT A symptom network analysis of specific phobia disorder (SPD) was conducted according to gender and age in 555 people over 65 in the Region of Madrid (Spain). The network was estimated using the InsingFit package that implements a procedure called eLasso. These results reveal differences in the strength, closeness, and betweenness of the different networks according to gender but not for the 65-74 and 75-84 age groups. The gender variable presents a greater presence of symptoms in women than in men. For all types of specific phobia, the symptom of greater occurrence for both groups was fear. The main difference between men and women over 65 with SPD lies in the situational type, where centrality is more distributed and where the most central symptoms are distress for women and fear for men. These differences in the networks seem to support the conceptualization of SPD from a dimensional point of view.RESUMEN Se realizó un análisis de redes de síntomas del trastorno de fobia específica (TFE) según el género y la edad en 555 personas mayores de 65 años de la Comunidad de Madrid (España). La red se estimó utilizando el paquete InsingFit que implementa un procedimiento llamado eLasso. Los resultados presentan diferencias en la fuerza, cercanía e intermediación de las distintas redes según el género, pero no para los grupos de edad de 65-74 y 75-84 años. El análisis de la variable género muestra una mayor presencia de síntomas en las mujeres que en los hombres. Para todos los tipos de fobia específica, el síntoma de mayor presencia para ambos grupos fue el miedo. La principal diferencia entre hombres y mujeres mayores de 65 años con TFE radica en el tipo situacional, donde la centralidad está más distribuida y donde los síntomas más centrales son la angustia para las mujeres y el miedo para los hombres. Estas diferencias en las redes parecen reforzar la conceptualización del TFE desde un punto de vista dimensional.
       
  • Resilience moderates the associations between bipolar disorder mood
           episodes and mental health

    • Abstract: ABSTRACT Resilience could moderate the impact of bipolar disorder (BD) on patients’ quality of life (QoL) and psychosocial functioning. This study aimed to (1) explore the relationships between bipolar disorder mood episodes (depression, [hypo]mania, and euthymia) and mental health outcomes of QoL, personal recovery, impaired work, and psychosocial functioning in individuals with BD; and (2) determine whether resilience domains were moderators of these associations. A clinical adult sample of 97 individuals diagnosed with BD (63.90% female, mean age = 45.02 years) completed a battery of BD-specific measures of resilience and mental health-related variables. Multiple regression showed that self-management of BD resilience domain moderated the negative relationship between (hypo)manic episode and personal recovery. Furthermore, self-confidence moderated the positive relationships between (hypo)manic and depressive episodes and impaired functioning. We suggest that BD patients experiencing (hypo)manic or depressive episodes may improve their mental health and wellbeing through interventions that promote disorder-specific resilience factors.RESUMEN La resiliencia podría moderar el impacto del trastorno bipolar (TB) en la calidad de vida (CV) y funcionamiento psicosocial de los pacientes. Los objetivos del presente estudio fueron: (1) explorar la asociación entre los episodios anímicos bipolares (depresión, [hipo]manía y eutimia) y los resultados de la CV en cuanto a salud mental, la recuperación personal y el deterioro del funcionamiento laboral y psicosocial de personas con TB y (2) determinar si los dominios de resiliencia moderaban esta asociación. Una muestra clínica de 97 personas adultas diagnosticadas de TB (63.90% mujeres, edad media = 45.02) cumplimentaron una batería de medidas específicas de TB sobre resiliencia y variables relacionadas con la salud mental. La regresión múltiple mostró que el dominio de la resiliencia de autogestión del TB moderó la relación negativa entre el episodio (hipo)maníaco y la recuperación personal. Además, la confianza en uno mismo moderó la asociación positiva entre los episodios (hipo)maníacos y depresivos y el deterioro del funcionamiento. Se sugiere que la salud mental y bienestar de los pacientes con TB en episodios (hipo)maníacos o depresivos podrían mejorar mediante intervenciones que desarrollen aspectos de resiliencia específicos de este trastorno mental.
       
  • Towards a consensus in the evaluation of the evidence of psychological
           treatments

    • Abstract: ABSTRACT Resilience could moderate the impact of bipolar disorder (BD) on patients’ quality of life (QoL) and psychosocial functioning. This study aimed to (1) explore the relationships between bipolar disorder mood episodes (depression, [hypo]mania, and euthymia) and mental health outcomes of QoL, personal recovery, impaired work, and psychosocial functioning in individuals with BD; and (2) determine whether resilience domains were moderators of these associations. A clinical adult sample of 97 individuals diagnosed with BD (63.90% female, mean age = 45.02 years) completed a battery of BD-specific measures of resilience and mental health-related variables. Multiple regression showed that self-management of BD resilience domain moderated the negative relationship between (hypo)manic episode and personal recovery. Furthermore, self-confidence moderated the positive relationships between (hypo)manic and depressive episodes and impaired functioning. We suggest that BD patients experiencing (hypo)manic or depressive episodes may improve their mental health and wellbeing through interventions that promote disorder-specific resilience factors.RESUMEN La resiliencia podría moderar el impacto del trastorno bipolar (TB) en la calidad de vida (CV) y funcionamiento psicosocial de los pacientes. Los objetivos del presente estudio fueron: (1) explorar la asociación entre los episodios anímicos bipolares (depresión, [hipo]manía y eutimia) y los resultados de la CV en cuanto a salud mental, la recuperación personal y el deterioro del funcionamiento laboral y psicosocial de personas con TB y (2) determinar si los dominios de resiliencia moderaban esta asociación. Una muestra clínica de 97 personas adultas diagnosticadas de TB (63.90% mujeres, edad media = 45.02) cumplimentaron una batería de medidas específicas de TB sobre resiliencia y variables relacionadas con la salud mental. La regresión múltiple mostró que el dominio de la resiliencia de autogestión del TB moderó la relación negativa entre el episodio (hipo)maníaco y la recuperación personal. Además, la confianza en uno mismo moderó la asociación positiva entre los episodios (hipo)maníacos y depresivos y el deterioro del funcionamiento. Se sugiere que la salud mental y bienestar de los pacientes con TB en episodios (hipo)maníacos o depresivos podrían mejorar mediante intervenciones que desarrollen aspectos de resiliencia específicos de este trastorno mental.
