  • Implementation of a trauma code

    • Abstract: Resumen Objetivo: Desarrollar una propuesta accesible a la realidad local de un hospital general terciario (Hospital Calderón Guardia) para la implementación de un código de trauma, basada en la mejor evidencia médico científica disponible. Métodos: Se realizó una revisión de la bibliografía; se buscaron los trabajos de investigación publicados a nivel nacional e internacional sobre la conformación y criterios relativos al código de trauma, su implementación, sus desafíos, y sus limitaciones; mediante 3 buscadores: Scielo, Pubmed y Ovid. Se incluyeron estudios con diversa metodología, disponibles en inglés o español. Resultados: Treinta artículos publicados en revistas indexadas fueron seleccionados y la información se agrupó en las siguientes categorías: Conformación del equipo de trauma en la activación de los códigos para cada hospital, criterios de activación, niveles de activación, experiencia local y limitaciones. Dicha información permitió identificar dos elementos principales para conseguir un beneficio: la conformación de un equipo multidisciplinario de primera respuesta para los pacientes más graves y la estandarización de criterios específicos para la activación de dicho equipo; entonces, se procedió a elaborar y proponer un modelo viable y concordante con las características del servicio hospitalario. Conclusión: La implementación hospitalaria de un modelo de código de trauma supone un impacto positivo en los desenlaces de morbi-mortalidad, a través de dos 2 mecanismo principales: la conformación de un equipo multidisciplinario de primera respuesta para los pacientes más graves y la estandarización de criterios específicos para la activación de dicho equipo; por lo que se elaboró un modelo ajustado a las necesidades y recursos del hospital.Abstract Objective: To develop a proposal of a trauma code accessible to the local characteristics of a tertiary general hospital (Hospital Calderón Guardia) based on the best clinical evidence available. Methods: A literary search was made of national and international scientific papers regarding several aspects about trauma code, it´s implementation, it´s challenges, main benefits, and it´s limitations in 3 main web search portals: Scielo, PubMed and Ovid. We included paper studies in English and Spanish. Results: Thirty scientific papers from index journals were selected for review and the following data were extracted: Trauma team conformation, trauma team activation criteria, levels for trauma team activation, local experience, and limitations. That information allowed us to identify 2 main beneficial elements: the conformation of the trauma team and the standardization of the specific criteria necessary for its activation. Also, we elaborated a proposal for a viable model in accordance with our resources. Conclusion: According to scientific review, trauma code implementation in any institution associates a positive impact in clinical patient outcomes through 2 main mechanisms: the conformation of a multidisciplinary trauma team response of severe trauma patients, and the standardization of criteria for activation of the trauma teams. With these findings we elaborated a proposal adjusted to the needs and resources of Hospital Rafael Angel Calderon Guardia.
       
  • Selection method of rescue treatment in patients with HIV-1: response and
           survival

    • Abstract: Resumen Objetivo: Determinar el impacto del uso de la prueba genotípica de resistencia en la respuesta y supervivencia a largo plazo de los pacientes infectados con el VIH-1 que presentaron fracaso a la terapia antirretroviral. Métodos: Se realizó un estudio de cohorte, retrospectivo, se definieron dos grupos basados en la forma de selección de la terapia de rescate utilizada: en base al resultado de la prueba genotípica de resistencia (grupo A) y en base al criterio de expertos (grupo B). Los pacientes fueron evaluados antes del cambio de la terapia de rescate según variables demográficas, clínicas y de laboratorio y evaluados a los 6, 12, 18, y 24 meses del cambio de tratamiento según respuesta virológica, respuesta de células CD4+, incidencia de enfermedades oportunistas y supervivencia. La información fue obtenida de las actas de la Comisión Nacional de Terapia Antirretroviral, la base de datos del IPK y las Historias Clínicas. Se utilizaron números absolutos y porcentajes, media y mediana, con sus respectivas desviaciones estándares (DE), Chi2, se aplicó el Riesgo Relativo (RR), prueba U de Mann-Whitney, y el método de Kaplan-Meier. Resultados: Los pacientes de grupo A tuvieron 1,44 veces mayor probabilidad de alcanzar supresión virológica completa que los pacientes del grupo B a los 6 meses, RR 1,44 (1,046- 2,054) p=0,017. El incremento promedio de Linfocitos T CD4+ fue de 117,40 células/mm3 en pacientes del grupo A y de 30,04 células/mm3 en pacientes del grupo B, p<0,005 a los 12 meses de iniciado el tratamiento. La incidencia de enfermedades oportunistas fue de 25,7% en el grupo B y de 5,6% en grupo A. El mayor porcentaje de sobrevida acumulada se observó en el grupo el grupo A (98,1%), en comparación con el grupo B (79%). Conclusiones: Los pacientes en los cuales el tratamiento de rescate se escogió basado en una prueba genotípica de resistencia tuvieron una mejor respuesta virológica, un mayor incremento de Linfocitos T CD4+ y una mayor supervivencia que aquellos en los que el tratamiento se eligió basado en el criterio de expertos.Abstract Objective: To determine the impact of the use of genotypic resistance testing on the response and long-term survival of HIV-1 infected patients who have failed antiretroviral therapy. Methods: A retrospective cohort study was carried out; two groups were defined based on the method of selection of the rescue therapy used: based on the result of the genotypic resistance test (group A) and based on the criteria of experts (group B). The patients were evaluated before the change of rescue therapy according to demographic, clinical and laboratory variables and evaluated at 6, 12, 18, and 24 months after the change of treatment according to virological response, CD4 + cell response, incidence of opportunistic diseases. and survival. The information was obtained from the minutes of the National Commission for Antiretroviral Therapy, the IPK database and the Medical Records. Absolute numbers and percentages, mean and median, with their respective standard deviations (SD), Chi2, were used, the Relative Risk (RR), the Mann-Whitney U test, and the Kaplan-Meier method were applied. Results: Group A patients were 1.44 times more likely to achieve complete virological suppression than group B patients at 6 months, RR 1.44 (1.046-2.054) p = 0.017. The average increase in CD4 + T lymphocytes was 117.40 cells / mm3 in group A patients and 30.04 cells / mm3 in group B patients, p <0.005 12 months after startin treatment. The incidence of opportunistic diseases was 25.7% in group B and 5.6% in group A. The highest percentage of cumulative survival was observed in group A (98.1%), compared to the group B (79%). Conclusions: Patients in whom salvage treatment was chosen based on a genotypic resistance test had a better virological response, a greater increase in CD4 + T lymphocytes, and a longer survival than those in whom treatment was chosen based on expert judgment.
       
  • Trends in emergency department admissions during the COVID-19 pandemic

    • Abstract: Resumen Objetivo: En Costa Rica se observó una disminución en las atenciones en los Servicios de Emergencias al inicio de la pandemia por lo que el objetivo de este estudio es la descripción de este fenómeno, su duración, así como implicaciones en nuestro país. Métodos: Es un estudio descriptivo observacional retrospectivo de las atenciones en los Servicios de Emergencias de los Hospitales pertenecientes a la Caja Costarricense del Seguro Social durante el 1 de enero al 31 de diciembre del 2020. Resultados: Se incluyeron 25 Servicios de Emergencias. Las atenciones durante el periodo de estudio fueron de 1.549.024 millones de visitas y la cantidad de revaloraciones a 4,038,029. El rango de atenciones en los diferentes SEM tuvo un rango de 19070 mil a 122251 mil durante el periodo del estudio. Se observó que 850 707 (54,9%) eran del sexo femenino. La mayor cantidad de pacientes atendidos en los SEM tenían edades entre los 20 a 44 años con 693 379 (44.7%) atenciones, seguido del grupo entre los 45 a 64 años con 328 979 (21.2%). Lo que respecta a la clasificación de la agudeza de los pacientes por medio del sistema de triaje canadiense, un 40% (620 449) fueron catalogados como categoría 3, un 39.8%(616 855) categoría 4, 14%(218 124), 5.3%(82 360) categoría 2 y por último 0.6%(9206) pacientes como categoría 1. Respeto a pacientes con el diagnóstico asociado a SARS CoV-2 se observó que 88 793 en los Servicios de Emergencias tuvieron dichos diagnósticos durante el periodo del estudio. Conclusiones: La tendencia a la baja en las consultas al Servicio de Emergencias en Costa Rica, reproduce las tendencias reportadas en la bibliografía a nivel mundial. La explicación para este fenómeno es multifactorial. Existen una gran cantidad de cambios “estructurales” tanto en los servicios de emergencias como en hospitalización y el sistema de salud en general, que con mucha probabilidad deberán cambiarse de manera definitiva y prioritaria. No solo para la atención de esta pandemia sino con miras a que dichos sistemas se encuentren mejor preparados para próximos eventos infecciosos de cualquier tipo.Abstract Aim: In Costa Rica, a decrease in the admissions in the emergency departments were observed at the beginning of the pandemic and the objective of this study was to describe this phenomenon, its duration, as well as its implications in our country. Methods: It is a retrospective descriptive observational study of the admissions in the Emergency Departments of the Hospitals belonging to the Costa Rican Social Security from January 1 to December 31, 2020. Results: 25 emergency departments were included. Admissions during the study period was 1,549,024 million visits and the number of re-evaluations to 4,038,029. The range of visits in the different emergency departments were from 19,070,000 to 122,251,000 during the study period. It was observed that 850 707 (54.9%) were female. The lar- gest number of patients treated in the emergency departments were aged between 20 to 44 years with 693,379 (44.7%) visits, followed by the group between 45 to 64 years with 328,979 (21.2%). The acuity of the patients using the Canadian triage system were classified 40% (620,449) as category 3, 39.8% (616,855) as category 4, 14% (218,124), 5.3% (82,360) as category 2 and finally 0.6% (9,206) patients as category 1. In regards of patients with the diagnosis associated with SARS CoV-2, it was observed that 88,793 were seen in the emergency departments with such diagnoses during the study period. Conclusions: The downward trend in consultations to the emergency departments in Costa Rica reproduces the trends reported in the literature worldwide. The explanation for this phenomenon is multifactorial. There are many “structural” changes both in the emergency departments and in hospitalization and the Health System in general, which will most likely have to be changed definitively and as a priority. Not only for the attention of this Pandemic but with a view to future infectious events of any kind.
       
  • Point of care ultrasound diagnostic: a case report of a hepatic abscess

    • Abstract: Resumen El ultrasonido en el punto de atención es una herramienta diagnóstica que ha tomado relevancia en el escenario intra y extrahospitalario, al tratarse de un estudio no invasivo que permite que los hallazgos clínicos se correlacionen directamente con los signos y síntomas que presenta el paciente. El objetivo del presente trabajo es evidenciar la utilidad clínica aplicada al caso de una paciente con una bacteremia persistente, en la cual se identificó el foco de infección por evaluación ultrasonográfica, para dirigir posteriormente la terapia. Presentamos un caso clínico en el que, por medio de esta herramienta, se estableció el diagnóstico de un absceso hepático insospechado clínicamente; se demuestra su utilidad clínica como extensión de la valoración médica especializada al obtener una experiencia favorable de su correcta utilización en un caso complejo.Abstract Point of care Ultrasound is a diagnostic tool that has gained relevance in the hospital and outpatient setting, as it is a non-invasive study that allows direct correlation of clinical findings and the signs and symptoms that the patient presents. The objective of this paper is to show its clinical utility evident as applied to a female patient who had persistant bacteremia, in which the site of infection was identified through ultrasound evaluation. We present a clinical case whose diagnosis of hepatic abscess was possible by the use of ecography and shows its usefullness as an extension of specialized medical evaluation by showing a favorable experience of its correct use in a complex case. We present a clinical case in which the diagnosis of a clinically unsuspected liver abscess was obtained using this tool.
       
  • Bordetella bronchiseptica pneumonia in a patient with a history of
           pulmonary lymphoma

    • Abstract: Resumen Bordetella bronchiseptica es un cocobacilo Gram negativo patógeno de animales que, con poca frecuencia causa infecciones en seres humanos. La mayoría de casos registrados en la literatura están asociados con pacientes que presentan algún tipo de inmunosupresión. Este reporte de caso se refiere a una paciente femenina de 67 años con antecedentes de linfoma pulmonar, que recibió quimioterapia y radioterapia 16 años atrás, fue ingresada al Servicio de Medicina Interna del Hospital Tomás Casas Casajús con un diagnóstico de neumonía bacteriana, tras descartarse infección por Covid19. Unos días después, se aisló una B. brochiseptica de una muestra de esputo y con el reporte de la prueba sensibilidad a los antibióticos, se modificó la terapia de antibióticos que originalmente se había indicado, lo que se conlleva a una mejoría en el estado físico de la paciente. Sin embargo, debido a una aparente infección nosocomial la paciente se contagió de SARS- CO2 y falleció debido a las complicaciones causadas por el Covid19.Abstract Bordetella bronchiseptica is an animal pathogenic Gram negative coccobacillus that infrequently causes human infections. Most of the cases recorded in the literature are associated with patients with some type of immunosuppression. In this case, a 67-year- old female patient with a history of pulmonary lymphoma, who received chemotherapy and radiotherapy 16 years ago, is admitted to the Internal Medicine Service of the Tomás Casas Casajús Hospital, with a diagnosis of bacterial pneumonia, after ruling out Covid19 infection. A few days later, a B. brochiseptica is isolated from a sputum sample and with the report of the antibiotic sensitivity test, the antibiotic therapy that had originally been indicated is modified, which is reflected in an improvement in the physical state of the patient. However, due to an apparent nosocomial infection, the patient becomes infected with SARS-CO2 and dies due to complications caused by Covid19.
       
  • Pancreatic Choristoma within a duodenal diverticulum, an accidental
           finding: apropos of a case

    • Abstract: Resumen El coristoma pancreático, o páncreas heterotópico, es una condición rara en gastroenterología. Esta entidad consiste en el hallazgo de tejido pancreático aberrante, en alguna zona del tracto gastrointestinal, sin continuidad vascular o anatómica con el páncreas normal, es poco reportado y presenta sintomatología variable. Presentamos el caso de un individuo de 46 años con dolor abdominal a quien se le encontró mediante exámenes invasivos un divertículo duodenal, el cual al examen histopatológico e inmunohistoquímico mostró un coristoma pancreático.Abstract Pancreatic choristoma, also called heterotopic pancreas is a rare condition in gastroenterology. This entity consists of the presence of aberrant pancreatic tissue in some other area of the gastrointestinal tract without vascular or anatomical continuity with the normal pancreas; it has been seldomly reported and it could exhibit variable symptomathology. We herein report the case of a 46-year-old male, who presented with abdominal pain and was found, through invasive examinations to have a pancreatic choristoma within a duodenal diverticulum. The diagnosis was made using histopathology and immune-histochemistry testing.
