      Abstract: Athletic training educators teach in didactic, laboratory and clinical settings, all requiring an array of pedagogical strategies to effectively instruct students. Mosston’s Spectrum of Teaching Styles is a theoretical framework to support pedagogical decision making in physical education. The purpose of the commentary is to examine teaching styles from Mosston’s Spectrum in the context of athletic training education. A general introduction, review of comparative literature and practical application to athletic training education is provided for each of the eleven teaching styles. The examination and application of educational theory from other disciplines may provide athletic training educators additional resources to enhance student learning.

  • Force and EMG Comparison between a weight-bearing clinical assessment of
           hip strength assessment and non-weightbearing tasks

      Abstract: Purpose: Altered hip strength is a risk factor for lower extremity injury but its relationship to biomechanical dysfunction is debated. Hip strength assessment methods are criticized for using unidirectional, non-weight-bearing positions which may not be representative of athletic activity and may affect comparison to biomechanical analysis of athletic tasks. A weight-bearing task may better represent hip muscle function during these movements. The aim of this study was to identify EMG and force differences for a clinical weight-bearing method of hip strength (the squat-hold) to traditional non-weight-bearing maximal voluntary isometric contractions (MVICs) for hip abduction, extension, and external rotation. Methods: Twenty-nine healthy volunteers (23 female, 6 male; 23.3±5.8 years) performed the squat-hold, sidelying abduction, prone extension, and seated hip external rotation MVICs. The squat-hold was performed by exerting a bilateral, maximal force against a rigid strap encircling both knees in a semi-squatted position. Surface electromyography (EMG) recorded peak activation of the gluteus medius (GMed), gluteus maximus (Gmax), and tensor fascia lata (TFL) and a handheld dynamometer simultaneously measured force during all tasks. Peak activation was compared between the squat-hold and each MVIC using paired t-tests. Force was compared across tasks using a one-way ANOVA. Results: Greater force was observed during the squat-hold than the external rotation MVIC, but abduction and extension MVICs yielded greater force than the squat-hold. GMax activation was higher during the squat-hold than the external rotation task. TFL activation was higher during the abduction MVIC than the squat-hold but GMed activation was similar across tasks. Peak GMax activation was similar between the extension MVIC and squat-hold. Conclusions: Squat-hold force may have been reduced due to altered gluteal moment arms, which affected the length-tension relationship. Clinicians should consider the squat-hold as an alternative assessment of external rotation force, but should continue to assess abduction and extension force with MVICs. Researchers should examine positions optimizing length-tension relationships to better relate motor function and movement patterns.

  • Effect of Age at Menarche on Anterior Cruciate Ligament Injury Incidence
           and Anterior Knee Laxity in Collegiate Athletes

      Abstract: Female athletes suffer painful, costly, and career-limiting non-contact anterior cruciate ligament (ACL) injuries more often than males. Previous research suggests that pubertal neuromusculoskeletal development contributes to this sex-bias, but the manner in which variation in pubertal development affects injury risk within females is poorly understood. Age at menarche is a variable, significant pubertal developmental event, signaling the onset of estrogen cycling and affecting musculoskeletal development. Earlier menarche may increase injury risk, possibly by increasing anterior knee laxity through prolonged estrogen exposure. The purpose of this case-control study was to test the primary hypothesis that collegiate athletes with previous ACL injuries have earlier age at menarche than their uninjured peers, and to test the secondary hypothesis that earlier menarche is related to greater anterior knee laxity in injured and uninjured athletes. The study sample consisted of female NCAA Division-I varsity athletes (N=14 injured, N=120 uninjured). Outcome measures included: menstrual history and ACL injury details (injury age, activity at time of injury, contact vs. non-contact), assessed by questionnaire; and anterior knee laxity assessed by KT-1000 arthrometer. Correlation, t-tests, and regression analysis were used to test for associations between age at menarche, injury incidence, and knee laxity. Fourteen athletes reported ≥1 non-contact ACL injury, and had significantly earlier menarche than uninjured athletes (12.6±1.3 y vs. 13.4±1.4 y; P=0.05). Earlier menarche also significantly predicted injury status (Wald c2=7.43; Pb=-1.02±0.37; OR=0.36; 95% CI:0.17-0.75), but was not correlated with anterior knee laxity.   • Adolescent Perceptions of Injury and Pressures of Returning to Sport: A
           Retrospective Qualitative Analysis

      Abstract: The increase in sport participation among adolescents has led to the rise in sport-related injuries, many of which have unique characteristics based on the patient, their perceptions, and the pressures faced when returning to sport. The purpose of this study was to identify the underlying factors that contributed to adolescents’ perceptions of injury and the various pressures they experienced when returning to sport. Two themes emerged from the study: support and fear. Support was provided to participants through development, care, and the environment. Fear was the factor that affected the participant in their return to sport, which came in the form of worry and doubt.

  • Recurrent Patellar Fracture in a Healthy Collegiate Basketball Player: An
           Exploration Clinical Case Report

      Abstract: Recurrent Patellar Fracture in a Healthy Collegiate Basketball Player: An Exploration Clinical Case ReportABSTRACT Purpose: Patellar fractures only account for approximately 1% of all skeletal injuries. Patellar fractures usually result from direct trauma (i.e. falling on the knee, dashboard injury, etc.), or, less frequently, occur as a combination of direct and indirect mechanism (i.e. receiving a direct blow while contracting the quadriceps). While indirect trauma is the least common mechanism, it usually occurs due to an extensor mechanism failure during eccentric loading such as landing. Method: Case report, Level 3: Exploration Clinical Contribution to the Available Sources of Evidence (CASE) Report. Results: A 21-year-old Division I collegiate basketball athlete sustained two separate transverse patellar fractures via non-contact mechanisms in the same knee over a 21-month period. The patient had no prior history of knee injury nor predisposing risk factors for fractures; delayed union or nonunion. Conclusion: The uniqueness of this case is twofold: 1) The rareness of this injury among a healthy athletic population; and 2) The unusual non-contact mechanism of the injury. Patellar injuries are extremely rare in athletics. In NCAA Division I men’s basketball, patellar injury accounts for only 2.4% of all injuries during games and 3.7% during practice. Recommendations: Although rare, it is important for athletic trainers to recognize traumatic high-impact injuries can occur during non-contact activity in a healthy population. In addition, athletic trainers should be mindful to suspect fracture without associated defect when a patient sustains a subsequent injury with an analogous mechanism even following successful rehabilitation.
