  • Factors impeding the practice of liver transplantation in west Africa

      Abstract: Background: The burden of liver disease is increasing globally due to the rising prevalence of its risk factors. This is particularly true in West Africa where hepatitis B virus (HBV) is endemic. Since liver transplantation (LT) was established as a definitive treatment for end-stage liver disease, the centers for liver transplantation have expanded significantly. Apart from South Africa and North Africa, there are no facilities for LT in West Africa. The aim of this study was to determine the factors impeding the practice of liver transplantation amongst hepatologists in West Africa.Methods: This was a questionnaire- based cross-sectional descriptive study conducted among hepatologists in West Africa during a regional hepatitis conference in 2015.Results: Twenty-four hepatologists completed the questionnaire. They were from Nigeria (42%), Ghana (21%), Cameroon (21%) and Ivory Coast (17%) . The indication for LT in their practice was mostly Chronic HBVinfection and its complications (88%). Although there are no facilities for LT in West Africa, late presentation and financial constraints were the major reasons why patients were not referred outside the region for the procedure. More than 70% believe that factors impeding LT in West Africa are multi-factorial and mostly related to funding, infrastructure and lack of organ donation network.Conclusion: There are no facilities for LT in West Africa. Hepatologists are unable to access LT in other Countries for their patients mostly because of late presentation and cost implications.Establishment of a regional liver transplant center for capacity building is desirable.Keywords: Liver transplantation, West Africa

  • Awareness and compliance with the Updated Sydney System recommendations
           among Nigerian gastrointestinal endoscopists

      Abstract: Background: It has been reported that gastric biopsy sets that comply with the Updated Sydney System (USS) biopsy sampling protocol have significantly better histological yield than those that do not for the most common gastric inflammatory conditions: Helicobacter pylori and intestinal metaplasia. We determined the level of awareness and compliance with the USS guidelines among Nigerian gastrointestinal endoscopists.Methods: In a descriptive cross-sectional survey, we interviewed all consenting gastrointestinal endoscopists practicing in Nigeria from March to July 2016. A 13-item self-administered questionnaire consisting of sociodemographic data (6-items), knowledge of the USS guidelines (4-items) and compliance with the biopsy sampling protocol (3-items) was used.Results: A total of 45 respondents in all were included in the final analysis. Of the 45 respondents, 29 (64.4%) were males while 16 (35.6%) were females. The mean age of the participants was 43.70 (±6.91) while the age range was 32-60 years. The mean year of endoscopy practice was 6.82 (±5.55) while the range was 1 to 25 years. Twenty-nine (64.4%) participants were generally aware of the USS, 25 (55.6%) knew the recommended number of biopsies, while 26 (57.8%) knew the 3 recommended gastric regions. Only 13 (28.9%) of the participants regularly took the recommended number of biopsies, 14 (31.1%) regularly sampled all the recommended stomach regions while 25 (55.6%) respondents regularly separated biopsies from different regions of the stomach into different containers, according to the USS protocol. However, the knowledge of the recommended number of biopsies and the recommended regions both had significant associations (p =0.020 and p =0.021 respectively) with correspondent endoscopy practices.Conclusion: While the level of awareness and knowledge of the USS guidelines could be considered fair among Nigerian gastrointestinal endoscopists, the level of compliance with the prescribed biopsy sampling protocol is poor. There is a need for relevant professional bodies like SOGHIN to create the awareness of the practice guidelines among their members with proper emphasis on the positive relationship between compliance and gastric histologic yield.Keywords: Updated Sydney System, Gastrointestinal Endoscopists, Gastric biopsy, Gastritis, Nigeria

  • Factors associated with death and duration of stay of cirrhotic patients
           admitted in the hepato-gastroenterology unit of Lomé Campus Teaching
           Hospital (Togo)

      Abstract: Background: In Togo, the death rate for cirrhosis was estimated at 33.4%. The purpose of this study was to identify the factors associated with the duration of stay and death of cirrhotic patients hospitalized in the hepato-gastroenterology department.Patients and method: A retrospective, descriptive and analytical cross-sectional study was conducted from January 1, 2005 to December 31, 2014 in the Hepato-gastroenterology unit of the Teaching Hospital of Lomé. In-patient records during the study period for cirrhosis and its complications were included. The probability of survival was determined by the Kaplan-Meier method.Results: We enrolled 2152 hospitalized cirrhotic patients during the study period, including 1447 men (67.2%) and 705 women (32.8%). The median age was 48 years. There were 567 deceased patients (26.3%). The median duration of stay was 7 days. Death occurred in 64.4% of cases in the first week of hospitalization (p <0.001). Hepatocellular carcinoma, oedemato-ascitic decompensation, jaundice and gastrointestinal bleeding were the complications significantly associated with death. The Child-Plugh-Turcott C score was significantly associated with death (p < 0.001). In the event of death, the median duration of stay was 1.5 days for patients with hepatic encephalopathy; it was 6 days in case of ascites. The probability of overall survival at the first day of hospitalization was 94.1%; it decreasedto 23% by the 40th day. Survival was significantly associated with the
      Child Plugh Turcott score (p < 0.001). Hepatic encephalopathy had a 5.2-fold higher risk of death than oedemato-ascitic decompensation.Conclusion: The majority of cirrhotic patients hospitalized a Child Pugh score stages B and C; more than half of the deaths occurred in the first week of hospitalization.Keywords: Cirrhosis, Death, Survival, Togo

  • Primary biliary cirrhosis in a 37 year old Nigerian male: a case report

      Abstract: The typical patient with primary biliary cirrhosis (PBC) is a middle aged woman presenting with fatigue, pruritus, and markedly elevated serum alkaline phosphatase (ALP) levels. PBC has a higher incidence and prevalence among Caucasians and Hispanics. We reported this case to reinforce the fact that the “rarities” in our environment may not be so uncommon after all. This article reports a case of PBC in a Nigerian man who presented with pruritus, hyperpigmentation and jaundice. The patient met all 3 diagnostic criteria and responded remarkably to Ursodeoxycholic acid (UDCA) tablets.Primary biliary cirrhosis would be an unusual diagnosis to consider in a young male of African descent. Data on PBC among Africans has remained scanty. A high index of suspicion is key in any patient presenting with jaundice, pruritus and cholestasis.Keywords: Primary biliary cirrhosis, Male

  • Seroprevalence trends of hepatitis B and C among donors in the blood bank
           service of a Nigerian tertiary hospital: a five-year retrospective study

      Abstract: Background and Objectives: Hepatitis B Virus (HBV) and Hepatitis C virus (HCV) infections remain very serious public health risks of blood transfusion globally, and especially in developing regions. In the present study, we sought to retrospectively describe the trends of the seroprevalence of HBV and HCV among blood bank donors in the University of Nigeria Teaching Hospital and explore the implications thereof.Methodology: The blood donors’ screening records over a period of five years, from January 2012 to December 2016, were carefully extracted from the University of Nigeria Teaching Hospital Blood Bank record repository, and subjected to descriptive analysis.Results: Of the 19,667 blood donations recorded within the study period, 775 were hepatitis B positive (3.94%), 251 were hepatitis C positive (1.28%) and 6 were hepatitis B and C co-infected (0.031%). Hepatitis C seropositivity rate was relatively stable across the study period, and varied between 0.45% (2014) and 1.74 % (2012). However, the Hepatitis B prevalence rate appeared to have declined, from 5.51% in 2012 to 2.99% in 2015 and 3.30% in 2016.Conclusions: The overall HCV seroprevalence in the present study concurs with the relatively low prevalence generally reported in our environment, while the HBV seroprevalence in the index study is significantly lower than the previously documented ranges in the general population. However, the declining trend of HBV seroprevalence noted over the study period appears to mirror the reported trend in the general population.Keywords: Hepatitis B virus; Hepatitis C virus; Blood donors; Seroprevalence trend; Nigeria
