  • Nutritional and functional rehabilitation in cirrhotic patients

      Abstract: Introduction: Cirrhotic people often have impaired nutritional and functional status. Objective: To evaluate the effect of multidisciplinary rehabilitation on cirrhotic patients. Method: Controlled clinical trial conducted at the Gastroenterology Ambulatory in southern Brazil, involving 33 patients with liver cirrhosis, through a multidisciplinary rehabilitation program, distributed into the groups: nutritional intervention (n=22) and functional and nutritional rehabilitation (n=11). Nutritional orientation according to European Society for Clinical Nutrition and Metabolism (ESPEN) Guideline. Functional intervention, during a period of 3 months, three weekly sessions of up to fifty minutes of exercise. Clinical and etiological characteristics, Child Pugh, nutritional, anthropometric and functional aspects were collected. Significant associations values were considered when p<0,05.Results: Mostly male patients (60,6%), average age 58 years (± 11,9 years), prevalence of cryptogenic etiology, Child-Pugh A, well-nourished subjective global assessment (SGA). Significant changes in the parameters abdominal circumference (ABC) (p<0,049), muscular arm circumference (MAC) (p <0,001),six-minute walk test (6mWT) (p<0,001) between the groups, with significant improvement in multidisciplinary rehabilitation. Significant improvement in the indirect parameters of sarcopenia, handshake strength (HS) (p<0,008) and 6mWT (p<0,001), especially in the group with functional and nutritional rehabilitation. Body composition revealed a significant change in multidisciplinary rehabilitation, in Weight (p<0,001), BMI (p<0,001), Phase Angle (PA) (p<0,036), ABC (p<0,002), MAC (p<0,001) and thumb adductor muscle (TAM) (p<0,030), which was not observed in the nutritional intervention group.Conclusion: The multidisciplinary intervention shows positive results, as observed, where the group with the functional and nutritional rehabilitation activity was superior to the group with the isolated nutritional intervention.

  • Putative Roles of Aloe Ingredients in Aging and Age-Related Diseases:
           Management of Microbiota Senoinflammatory Process

      Abstract: Aging and age-related diseases of increasing elderly population are the most challenging biomedical problem at present. Age-related chronic inflammatory diseases, metabolic syndrome, diabetes, kidney disease, osteoporosis, and dementia are common occurrences among the elderly. In this review, we described putative roles in intestinal homeostasis by butyrate, evaluation of aloe constituents, on prevention of leaky gut syndrome due to age-related increased intestinal permeability which can lead to many systemic inflammatory and immune-related dysfunctions. Our presentation on the beneficial efficacy of aloe ingredients were discussed based on management of gut microbiota and age-related senescent inflammation, known as senoinflammation.

  • Waterborne Enteric Coronaviruses and the SARS-CoV-2 Disease

      Abstract: Enteric or diarrheal viruses are primarily transmitted through contaminated water, and cause self-limiting gastroenteritis, diarrhea or jaundice. The human coronaviruses (CoV), including the severe acute respiratory syndrome CoV (SARS-CoV-1) and Middle-East respiratory syndrome CoV (MERS-CoV) which are inherently respiratory viruses, also manifest in gastrointestinal tract. Similarly, the novel SARS-CoV-2 also causes diarrhea and liver disorder as well as shed in stool in a proportion of SARS-CoV-2 diseases (COVID-19) patients. This is well supported by detections of SARS-CoV-2 RNA in gut specimen, rectal swabs and stool samples. Recent data on its occurrence in wastewater provides a warning on further spread of COVID-19 in resource-poor countries. Therefore, laboratory test of COVID-19 patients stool along with nasopharyngeal specimen as well as implementation of water surveillances could be very useful in protecting community health.

  • Non-Invasive Predictors of Moderate–Large Esophageal Varices in
           Compensated Cirrhotic Patients

      Abstract: Aim: to identify a non-invasive and low-cost tool that could predict the presence of moderate-to-large EV in compensated cirrhotic patients to lower the number of unnecessary endoscopies.Materials and Methods: A cross-sectional study was conducted in newly diagnosed patients with non-Child-Pugh C liver cirrhosis, without a history of variceal bleeding. The presence and degree of esophageal varices were determined during endoscopy. Univariate and multivariate logistic regression were used to identify independent predictors for the presence of esophageal varices.Results: Of the 180 patients enrolled in this study, EV was found in 99 patients (55%). Moderate-large varices were detected in 23.3% and 51.7% of patients with CP-A and CP-B class, respectively. In univariate analysis, CP class B, low platelet count (≤100 000/μL), increased bipolar diameter (≥135 mm), and ratio of platelet count to bipolar spleen diameter (≤847) were associated with the presence of moderate-to-large EV. In multivariate analysis, CP class and ratio of platelet count to bipolar spleen diameter were associated with the presence of moderate-to-large EV. The AUC, sensitivity, and specificity of combined predictors were 0.76, 88% and 55.4%, respectively.Conclusion: Platelet count/bipolar spleen diameter ratio value of ≤847 could be used as a non-invasive predictor of the presence of moderate-to-large EV in compensated cirrhotic patients. Future validation in a larger cohort is needed before applying this parameter in clinical practice.

  • Prevalence and associated factors of premalignant lesions in patients with
           helicobacter pylori infection

      Abstract: Aim : Helicobacter pylori has revolutionized our thinking on gastric carcinogenesis. However, given the low percentage of infected patients who develop gastric cancer, one may hypothized the presence of factors involving in the carcinogenesis. The aim of our study was to evaluate the prevalence of premalignant lesions: gastric atrophy and intestinal metaplasia in a population infected with Helicobacter pylori ; and to investigate about the factors predicting their development. Methods: This is a prospective study, including 481 patients with H.pylori infection confirmed by histological study of gastric biopsies performed per endoscopy. Results: Among our patients we had 216 (44, 9 %) women and 265 (55,1%) men. Chronic antral gastritis was found in 478 of patients (99,4%) and chronic fundic gastritis in 405  of patients (84,4%). The prevalence of gastric dysplasia was 0.4% . In our patients 12,6 % had gastric atrophy  and  7,7 % had intestinal metaplasia. Multivariate analysis showed that only the activity of gastritis is an independent risk factor for the premalignant lesions :  Gastric Atrophy : Antrum (OR = 2,66 ; p<0,001 ;)  , Fundus : (OR = 2,09 ; p = 0.01 ).Intestinal metaplasia : Antrum : (OR=1,82; p=0,011 ) Fundus (OR=1,8 ; p=0,02   ). Conclusion: The prevalence of gastric atrophy and intestinal metaplasia in patients infected with Helicobacter pylori remains low compared with other countries. The activity of gastritis is significantly associated with these premalignant lesions.Keywords :  Helicobacter pylori . Gastritic Atrophy. Intestinal metaplasia.

  • Actinomycosis of an Appendix with a Cecal Abscess

      Abstract: Appendicitis has an etiology of appendiceal actinomycosis and chronic inflammation in only 0.01-0.06% of the cases, and less than 10% of these cases are diagnosed before surgery. Here we report the case of a 27-year-old Chinese female who was referred to our hospital with right lower abdominal pain and a high fever. Abdominal computed tomography (CT) scan revealed an abscess around the cecum. Appendectomy was performed on the same day, and a tumor-liked edematous appendix was observed. Appendiceal actinomycosis was diagnosed pathologically. A combination of antibiotic therapy and operative treatment resolves actinomycosis in 90% of cases.

  • Portal Venous Gas of Uncertain Etiology with Delayed Portal Vein
           Thrombosis: A Case Report and Review of the Literature

      Abstract: Historically, the presence of portal venous gas (PVG) on plain abdominal films was an indication for exploratory laparotomy and seen as an ominous finding with a mortality of almost 100%. Over the last decades, the introduction of CT scan has facilitated the diagnosis of PVG with a subsequent decrease in the mortality. PVG has been associated with ischemic bowel but also with other more benign conditions. We describe a case of a young patient with PVG of uncertain etiology with delayed portal vein thrombosis, with a discussion on the different conditions associated with PVG and the approach to the patient with PVG.
