  • Unusual presentation of lumbar chordoma on bone scintigraphy in a young

    • Abstract: Chordoma is a rare bone cancer which arises from undifferentiated notochordal remnants in the axial skeleton. It generally has slow-growing and locally aggressive behavior. This tumor is usually diagnosed by CT and MRI modalities and the role of SPECT/CT is still debated. It shows reduced or normal uptake of radioisotope on bone scanning and increased tracer uptake is infrequently reported. Here we present a 33-year-old man with complaint of low back pain and numbness of his right leg. The whole body bone scan showed relatively uniform radiotracer activity throughout the skeleton. A focal increased uptake in the second lumbar vertebra was noted on SPECT/CT images. SPECT/CT also demonstrated multiple lytic lesions in lumbar vertebrae. The lesions were proven to be chordoma on biopsy. Lumbar chordoma could be one of the differential diagnoses for lytic lesions of the vertebrae which show absent or minimal tracer uptake on bone scintigraphy and SPECT/CT imaging. Our case was unusual as the patient was very young for chordoma diagnosis and bone scan showed increased uptake adjacent to the involved vertebral lesion detected by SPECT/CT.

  • Orbital and brain metastases on 68Ga-PSMA PET/CT in a patient with
           prostate carcinoma ...

    • Abstract: We present a case of metastatic prostate cancer with rare metastases involving the brain and orbit, in addition to liver, skeletal and nodal metastases. The patient had undergone prior hormonal therapy and chemotherapy and had disease progression despite 2 cycles of 177Lu-Prostate specific membrane antigen (177Lu-PSMA) based radioligand therapy. He had a partial response after 2 cycles of 225Ac-PSMA based targeted alpha therapy, as demonstrated on the 68Ga-PSMA PET/CT study. However, the patient had disease progression at the end of 4 cycles of 225Ac-PSMA therapy, evident by rising prostate specific antigen levels and imaging findings. The end of treatment 68Ga-PSMA PET/CT showed additional sites of metastases in the orbit and brain apart from overall disease progression. These are rare sites of distant spread in prostate cancer and require urgent evaluation and local treatment to prevent potential complications. The importance of detection of metastatic sites in closed cavities is because of the requirement for urgent intervention to avoid compression related complications.

  • 18F-THK 5351 and 11C-PiB PET of the Thai normal brain template

    • Abstract: Objective: The aim of the study was to create a local normal database brain template of Thai individuals for 11C-Pittsburgh compound B (11C-PiB) and 18F-THK 5351 depositions using statistical parametric mapping (SPM) software, and to validate and optimize the established specific brain template for use in clinical practice with a highly reliability and reproducibility. Methods: This prospective study was conducted in 24 healthy right-handed volunteers (13 men, 11 women; aged: 42–79 years) who underwent 18F-THK 5351 and 11C-PiB PET/CT scans. SPM was used for the 18F-THK 5351 and 11C-PiB PET/CT image analysis. All PET images were processed individually using Diffusion Tensor Image -Magnetic Resonance Imaging-weighted images (DTI-MRI images), which involved: (1) conversion of Digital Imaging and Communications in Medicine (DICOM) files into an analyzable file extension (.NIFTI) for statistical parametric mapping, (2) setting of the origin (the anterior commissure was used as the anatomical landmark), (3) re-alignment, (4) co-registration of PET with B0 (T1W) and DTI-MRI images, (5) normalization, and (6) normal verification using the Thai MRI standard. We then compared the normal PET template with the abnormal deposition area of different dementia syndromes, including Alzheimer’s disease and progressive supranuclear palsy. Results: This method was able to differentiate cognitively normal from Alzheimer’s disease and progressive supranuclear palsy subjects. Conclusions: This normal brain template was able to be integrated into clinical practice and research using PET analyses at our center.

  • CT-Attenuation correction and its impact on image quality of myocardial
           perfusion imaging in ...

    • Abstract: Myocardial perfusion imaging is a non-invasive procedure that plays an integral role in the diagnosis and management of coronary artery disease. With the routine use of computerised tomography attenuation correction (CTAC) in myocardial perfusion imaging still under debate, the aim of this review was to determine the impact of CTAC on image quality in myocardial perfusion imaging. Medline, Embase and CINAHL were searched from the earliest available time until August 2019. Methodological quality was assessed using the Quality Assessment of Diagnostic Accuracy Studies version 2. Details pertaining to image quality and diagnostic accuracy were analysed and results summarised descriptively. Three studies with ‘unclear’ risk of bias and low applicability concerns (1002 participants) from a yield of 2725 articles were identified. Two studies demonstrated an increase in image quality, and one study found no difference in image quality when using CTAC compared to no attenuation correction. Benefits of CTAC for improving image quality remain unclear. Given the potential exposure risk with the addition of CTAC, patient and clinician factors should inform decision making for using of CTAC in myocardial perfusion imaging for coronary artery disease.

  • Sentinel lymph node biopsy for papillary thyroid cancer: the effect of
           dose, tracer and massage

    • Abstract: Background: In patients with papillary thyroid cancer (PTC), sentinel lymph node (SLN) radio-guided biopsy is not routinely used for detection of involved neck lymph nodes (NLN); 99mTc-sulfur colloid antimony (99mTc-SC) has been used for this purpose. In this study, besides 99mTc-SC another radiotracer, 99mTc-phytate (99mTc-P) with different doses and injection methods were evaluated.Materials and methods: Twenty-two patients, scheduled to undergo thyroidectomy for PTC, were injected for radio-guided SLN biopsy in the morning of operation in 3 groups: intra tumoral injection of about 1 mCi 99mTc-P (group A; n=5); peritumoral injection of less than 3 mCi 99mTc-SC (group B; n=6); and peritumoral injection of 3 to 5 mCi 99mTc-SC with application of massage (group C; n=9). A patient refused to complete the study. No NLN was detected in the pre-operative ultra-sonographic examinations of all patients. Central neck dissection was done for all the participants. The presence of radio guided detected NLN and results of pathology were assessed.Results: In group A and B, no SLN was detected. NLNs were resected in 4 patients in group A and B; 2 of them involved by the tumor. In group C, 6 out of 9 patients (66.7%) had between 1 to 6 SLNs; the procedure failed to detect SLN in a patient in group C with surgically resected reactive NLN (failure rate 1 out of 7). Conclusions: The results underscored the significance of SLN radio guided biopsy in patients with PTC; the radiotracer, dose and method of injection may affect the detection rate.

  • 18F-FDG PET/CT in the evaluation of solitary extramedullary plasmacytoma

    • Abstract: The role of 18F-FDG PET/CT in patients with multiple myeloma (MM) and other plasma cell disorder is well known. Solitary plasmacytoma (SP), an extremely rare form within this entity that accounts for approximately 4% of plasma cell malignancies, can be classified as solitary bone plasmacytoma (SBP) or solitary extramedullary plasmacytoma (SEMP). EMP (extramedullary plasmacytoma) is a rare neoplasm characterized by the monoclonal proliferation of plasma cells outside the bone marrow. Breast and craniocerebral region are the uncommon sites of presentations of EMP and rarely reported in the literature. The most frequent site of presentation is upper airways. EMPs have similar pathogenesis as MM, however, they differ in management as they are radiosensitive in nature, and radiotherapy is the preferred treatment modality. As solitary EMP has a better prognosis than SPB with a lower conversion rate to MM, accurate staging is essential to plan the treatment. 18F-FDG PET/CT has higher sensitivity for treatment response evaluation. In the present case series, it is aimed to depict the role of 18F-FDG PET/CT in newly diagnosed solitary EMP with different sites of origin to exclude further lesions which lead to change in a treatment plan and in treatment response assessment.

  • Incidentally Detected Celiac Disease with Splenomegaly on 18F FDG PET/CT:
           A Potential Lymphoma Mimic

    • Abstract: Celiac disease is an immune-mediated disorder triggered by hypersensitivity to gluten occurring in genetically susceptible individuals. A high-index of suspicion is needed for diagnosis as patients can be asymptomatic or present with atypical symptoms or extra-intestinal manifestations. Typical 18-F Fluorodeoxyglucose (FDG) Positron Emission Tomography (PET)/Computed Tomography (CT) gastrointestinal manifestations of celiac disease include increased multifocal or diffuse jejunal and ileal uptake; focal duodenal uptake is less common. Splenomegaly with increased splenic FDG uptake is also uncommon in celiac disease in the absence of portal hypertension; small-sized spleen and functional hyposplenism are more typical. We report a case of celiac disease diagnosed after PET/CT showed FDG uptake in the duodenum and enlarged spleen. Follow-up after gluten-free diet showed complete metabolic resolution and regression of splenomegaly. The combination of focal bowel and splenic uptake is unusual in celiac disease and may be mistaken for a lymphoproliferative disorder. Awareness of this entity may avoid misdiagnosis and guide appropriate management.

  • Incremental Value of FDG PET/CT in Aggressive High Grade B Cell Lymphoma
           with TdT Expression

    • Abstract: High-grade B-cell lymphoma, an aggressive form of Non-Hodgkin’s Lymphoma, is known as a double or triple hit lymphoma based on the presence of MYC and BCL2 without or with BCL6 genetic rearrangements, respectively. It carries a poorer prognosis, compared to other variants of B-cell lymphoma, and its management also differs which requires more intensive chemotherapy in contrast to the routine regimen. Terminal deoxynucleotidyl transferase (TdT), a marker of immaturity is commonly expressed in B-cell lymphoblastic leukemia or lymphoma (B cell ALL) which is absent in mature forms of B-cell lymphoma. The TdT is expressed in highgrade B-cell lymphoma; therefore, it poses a classification and management dilemma, which should be accurately differentiated from B-cell ALL and mandates molecular analysis. Herein, we report a case of a 52-year-old female with biopsy reported as high-grade B-cell lymphoma with TdT expression. She was referred for Fluor-deoxyglucose (FDG) Positron Emission Tomography-Computed Tomography (PET/CT) scan for staging in the absence of molecular analysis for Bcell ALL. It was diagnosed as lymphoma on FDG PET/CT based on its characteristic findings of extensive extranodal involvement of multiple organs with no significant lymphadenopathy establishing the incremental value of FDG PET/CT scan, which helped the clinician to arrive at a conclusion.

  • A case of cardiac amyloidosis incidentally detected by bone scintigraphy

    • Abstract: A 73-year-old man with lung cancer underwent bone scintigraphy for disease staging. Diffuse myocardial 99m technetium hydroxymethylene diphosphonate (99mTc-HMDP) uptake was incidentally found. A diagnosis of amyloid transthyretin (ATTR) cardiac amyloidosis was suspected, although the patient had no symptoms at this time. Single-photon emission computed tomography (SPECT) showed particularly strong uptake in the ventricular septum. Cardiac magnetic resonance imaging (CMR) showed widespread subendocardial and partly transmural enhancement of the left ventricular myocardium on delayed postcontrast T1-weighted images. These findings were consistent with ATTR cardiac amyloidosis. 18F-FDG uptake in the left ventricle wall was observed on PET/CT. He was finally diagnosed with ATTR by endomyocardial biopsy. There are two major subtypes of cardiac amyloidosis: ATTR amyloidosis and amyloid light-chain (AL) amyloidosis. Endomyocardial biopsy is the gold standard for diagnosis. Recently, however, several reports have shown that bone scintigraphy using a 99mTc-labelled bone-seeking agent can detect ATTR cardiac amyloidosis and differentiate it from AL amyloidosis. Bone scintigraphy may play an important role in the detection and differentiation of ATTR cardiac amyloidosis.

  • Physiological FDG uptake in growth plate on pediatric PET

    • Abstract: Objectives 18F-Fluorodeoxyglucose (FDG) uptake in children is different from that in adults. Physiological accumulation is known to occur in growth plates, but the pattern of distribution has not been fully investigated. Our aim was to evaluate the metabolic activity of growth plates according to age and location. Methods We retrospectively evaluated 89 PET/CT scans in 63 pediatric patients (male : female = 25 : 38, range, 0–18 years). Patients were classified into four age groups (Group A: 0–2 years, Group B: 3–9 years, Group C: 10–14 years and Group D: 15-18 years). The maximum standardized uptake value (SUVmax) of the proximal and distal growth plates of the humerus, the forearm bones and the femur were measured. The SUVmax of each site and each age group were compared and statistically analyzed. We also examined the correlations between age and SUVmax. Results As for the comparison of SUVmax in each location, the SUVmax was significantly higher in the distal femur than those in the other sites (p < 0.01). SUVmax in the distal humerus and the proximal forearm bones were significantly lower than those in the other sites (p < 0.01). In the distal femur, there was large variation in SUVmax, while in the distal humerus and the proximal forearm bones, there was small variation. As for the comparison of SUVmax in each age group, the SUVmax in group D tended to be lower than those in the other groups, but in the distal femur, there was no significant difference among each age group. Conclusion Our data indicate that FDG uptake in growth plates varies depending on the site and age with remarkable uptake especially in the distal femur.

  • Extensive Extranodal Involvement in a case of Non-Hodgkin’s Lymphoma

    • Abstract: Non-Hodgkin lymphoma (NHL) is a group of malignant lymphoproliferative disorders arising predominantly in the lymph nodes with various clinical and histological characteristics. At least 25% of NHL originates from tissues other than lymph nodes and sometimes even from sites that do not contain lymphoid tissue. These are referred to as primary extranodal lymphomas (pENLs). pENL is a universal diagnostic challenge to the clinicians and pathologists due to their varied clinical presentations, morphological mimicry, and molecular alterations. The GIT is the most common site of pENL followed by nasopharynx/oropharynx, testis, uterus/ovary, thyroid, and central nervous system. Long bones (tibia), maxillary sinus, skin, and paraspinal soft tissues are the other rare anatomic sites of pENL. We reported a case of a 60-year-old female presented with pain and mass in the pelvis region. 18F-Fluorodeoxyglucose(FDG) PET/CT was done, which revealed extensive extranodal involvement of the lung, bilateral kidneys, uterus, ovaries, bones, and muscles with no involvement of lymph nodes or lymphomatous organs. Extensive extranodal involvement with sparing of lymphomatous organ has not been reported earlier.

  • Improvement of diagnostic accuracy of Parkinson's disease on

    • Abstract: Objective(s): I-123-ioflupane single photon emission computed tomography (FP-CIT-SPECT) has been used to assess dopamine transporter (DAT) loss in Parkinson's disease. The specific binding ratio (SBR), a quantitative parameter of DAT density in the striatum, may be affected by differences in age, sex, and SPECT system. The purpose of this study was to evaluate the utility of FP-CIT-SPECT using the Japanese normal database (NDB) in the diagnosis of Parkinson's disease. Methods: To standardize the quantitative outcome measures of DAT density obtained with different SPECT systems, striatal phantoms filled with striatal to background materials at ratios between 8:1 and 1:1 were measured using a gamma camera (ECAM) in our institute. Consecutive fifty patients (23 men and 27 women; age range, 40-86 years) with suspected PD undergoing FP-CIT SPECT brain imaging during the period from April to October 2016 were enrolled in this retrospective study. Their final diagnoses were PD in 28 patients and PD in 22 patients. SBRs of the patients were calculated using either new (Japanese database with different age and sex; NEWver) or old (non-Japanese database not specifying age and sex; OLDver) version software (AZE Virtual Place Hayabusa [DaTView], AZE, Ltd. Tokyo, Japan). The McNemar test was used to compare the diagnostic accuracy between old and new versions. Results: Based on the phantom study, the calibrated SBR could be calculated by Y=1.25×Measured SBR+0.78. The sensitivities for OLDver and NEWver were 100% and 93%, respectively (p=0.5), and the specificities were 55% and 100% (p=0.002). The diagnostic accuracy of NEWver (96%) was better than that of OLDver (80%, p<0.001).  Conclusion: FP-CIT-SPECT using the Japanese NDB improved the diagnostic accuracy of PD by improving specificity.

  • Evaluation of D-isomer of 18F-FBPA for oncology PET focusing on the
           differentiation of glioma ...

    • Abstract: Objective(s): L-4-borono-2-18F-fluoro-phenylalanine (L-[18F]FBPA), a substrate of L-type amino acid transporter 1 (LAT1), is a tumor-specific probe used in positron emission tomography (PET). On the other hand, it has not been examined whether another isomer D-[18F]FBPA accumulates specifically in the tumor. Here, we compared the accumulation of D-[18F]FBPA in C6 glioma and inflammation to evaluate the performance of D-[18F]FBPA as a tumor-specific probe. Methods: HEK293-LAT1 and HEK293-LAT2 cells were tested for [14C]-leucine or [14C]-alanine transport, and IC50 values of L- and D-FBPA were evaluated in both cell types. PET was conducted in rat xenograft model of C6 glioma with LAT1 expression and model of turpentine oil-induced subcutaneous inflammation (n=10 for both models). The concentrations of D-[18F]FBPA were compared between glioma and inflammatory lesion using standardized uptake value (SUV). Results: In contrast to L-FBPA, which inhibited substrate uptake in both HEK293-LAT1 and -LAT2 cells, D-FBPA showed no inhibitory effect on both cells, suggesting low transporter selectivity of D-[18F]FBPA against LAT1 and LAT2. Static PET analysis showed low accumulation of D-[18F]FBPA in C6 glioma and inflammatory lesion (SUVmax=0.80±0.16, 0.56±0.09, respectively). Although there was a statistical difference in SUVmax between these tissues, it was difficult to distinguish glioma from inflammation on the PET image due to its low uptake level. Therefore, it was suggested that D-[18F]FBPA is not a suitable tumor-specific probe for oncology PET in contrast to L-[18F]FBPA. Conclusion: This study demonstrated that D-[18F]FBPA is not a LAT1-specific PET probe and shows low uptake in C6 glioma, indicating its unsuitability as a tumor diagnosis PET probe.

  • Preclinical study of a new 177Lu-labeled somatostatin receptor antagonist

    • Abstract: Objective(s): Somatostatin receptor-positive neuroendocrine tumors have been targeted using various peptide analogs radiolabeled with therapeutic radionuclides for years. The better biomedical properties of radioantagonists as higher tumor uptake make these radioligands more attractive than agonists for somatostatin receptor-targeted radionuclide therapy. In this study, we tried to evaluate the efficiency of Luthetium-177 (177Lu) radiolabeled DOTA-Peptide 2 (177Lu-DOTA-Peptide 2) as a new radioantagonist in HT-29 human colorectal cancer in vitro and in vivo. Methods: DOTA conjugated antagonistic peptide with the sequence of p-Cl-Phe-Cyclo(D-Cys-L-BzThi-D-Aph-Lys-Thr-Cys)-D-Tyr-NH2 (DOTA-Peptide 2) was labeled with 177Lu. In vitro assays (saturation binding assay and internalization test) and animal biodistribution were performed in human colon adenocarcinoma cells (HT-29) and HT-29 tumor-bearing nude mice. Results: 177Lu-DOTA-Peptide 2 showed high stability in acetate buffer and human plasma (>97%). Antagonistic property of 177Lu-DOTA-Peptide 2 was confirmed by low internalization in HT-29 cells (<5%). The desired dissociation constant (Kd =11.14 nM) and effective tumor uptake (10.89 percentage of injected dose per gram of tumor) showed high binding affinity of 177Lu-DOTA-Peptide 2 to somatostatin receptors.  Conclusion: 177Lu-DOTA-Peptide 2 demonstrated selective and high binding affinity to somatostatin receptors overexpressed on the surface of HT-29 cancer cells, which could make this radiopeptide suitable for somatostatin receptor-targeted radionuclide therapy.

  • Current state of bone scintigraphy protocols and practice in Japan

    • Abstract: Objective(s): Nuclear medicine technologists in Japan often perform additional single-photon emission computed tomography (SPECT) with or without computed tomography (CT) after whole-body imaging for bone scintigraphy. In this study, we wanted to identify the bone scanning protocols used in Japan, together with the current clinical practices. Methods: The study was conducted between October and December 2017. We created a web survey that was hosted by the Japanese Society of Radiological Technology. The questionnaire included 12 items regarding the demographics of the responders, their scan protocols, and the imaging added to, or omitted from, routine protocols. Results: In total, 228 eligible responses were collected from participants with a mean of 11.6±8.4 years’ experience in nuclear medicine examination. All responders reported using routine scan protocols that included whole-body imaging. However, only 2%, 4%, 20%, and 14% of the responders also acquired single-field SPECT, single-field SPECT/CT, multi-field SPECT, and multi-field SPECT/CT, respectively.  Conclusion: Our survey results indicate that nuclear medicine practice in Japan is beginning to shift from planar whole-body imaging with additional spot planar images to additional SPECT or SPECT/CT. Further study is required to examine the optimal protocols for bone scintigraphy.

  • Who' What' Where' A snapshot of Nuclear Medicine Research

    • Abstract: Objective(s): The aims of this study were to: 1) discover location (by city) of contributors to poster and oral presentations at recent ANZSNM conferences; 2) determine the nuclear medicine themes most commonly explored; 3) establish institutions producing the highest number of oral and poster abstracts and 4) determine publication rates of conference abstracts to full papers from recent ANZSNM conferences. Methods: Retrospective analysis of abstracts published in the Internal Medicine Journal Special Issues 2014–2019 identified 614 abstracts. Invited plenary speaker abstracts were excluded. Descriptive statistics were used in data analysis. Conference abstracts were analysed using the following criteria: poster or oral presentation, author/s, city location, hospital and subject matter. Themes defined by the ANZSNM conference committee for abstract submission were: cardiology, oncology, neurology, therapy, renal/urology, gastrointestinal, paediatrics, musculoskeletal, infection/inflammation, technology, physics, radiation safety, radiopharmacy/radiochemistry, education, or general. Retrospective analysis of 555 conference abstracts (excluding New Zealand and International, 59 abstracts) using Google Scholar, Pubmed and Google databases was undertaken. titles, key words, institutions and/or authors’ names were used to find peer-reviewed papers. Identified papers were authenticated through either open access, publicly available author information or Monash University’s library access. Published paper citations were also recorded (up to 1st July 2019). Results: Analysis of 614 abstracts 2014 – 2019 was performed. Over five years, the average number of poster abstracts was 67.8 and oral 55.0. Sydney submitted the highest number of poster abstracts, while Melbourne the highest number of oral abstracts. Most popular abstract theme was oncology for both poster and oral abstracts. Publications found had in excess of 1250 citations.One hundred and one publications from one hundred and seven conference presentations were identified, distributed across sixty journals. Conference presentation to full publication rate was 18.2%; excluding 2019 conference abstracts the rate was 21.5%. Conclusion: Publishing research findings is a challenging process. A retrospective analysis of research presented at recent ANZSNM conferences by abstract content was undertaken, with conference presentation to full publication rate found to be at the lower end of reported literature findings.

  • Demonstration of focal physiologic in-vivo somatostatin receptor

    • Abstract: We present the case of a 60-year-old man with metastatic neuroendocrine tumor of the ileum following ileal resection, being evaluated for 177Lu-based peptide receptor radionuclide therapy. 68Ga-DOTANOC PET/CT showed focal increased tracer uptake in the scrotal region without any morphologic changes on the corresponding CT images. Similar increased tracer uptake was seen on post-therapy whole-body imaging following 177Lu-DOTATATE therapy. An USG guided FNA revealed no malignant cells on cytopathologic examination. This case illustrates that focal testicular tracer uptake, may not always be pathological and can represent a normal physiologic variant, similar to the diffuse testicular somatostatin receptor expression as previously reported in literature.

    • Abstract: 68Ga Prostate-specific membrane antigen (PSMA) is an increasingly popular radiopharmaceutical tracer in prostate cancer and is becoming increasingly researched in other cancers such as breast cancer, renal cell carcinoma, glioblastoma multiforme, among others. Cholangiocarcinoma is the second most common primary hepatic malignant tumor; it is an aggressive tumor with a 5-year survival rate of less than 5 %. We herein report a case of primary cholangiocarcinoma detected on 68Ga-PSMA PET-CT conducted as part of follow up for prostate cancer and confirmed by biopsy and immunohistochemistry.

  • Inguino-scrotal bladder hernia: an unexpected finding on 68Ga-PSMA PET/CT

    • Abstract: Bladder herniation is an uncommon condition mimicking suspicious metastasis on PET/CT imaging. We report a 67 y/o man with prostate cancer referred for recurrence evaluation with 68Ga-PSMA-11 PET/CT. The scan showed an asymmetric site of intense tracer accumulation in the left inguino-scrotal region with the same SUVmax to the pelvic bladder. Reviewing cross sectional CT images with PET confirmed the inguino-scrotal bladder herniation.

  • The role of FDG PET/CT in the evaluation of treatment response in a case
           of calcified ovarian ...

    • Abstract: Evaluation of calcified metastatic lesions by conventional imaging can be challenging. Ovarian cancer metastases can present with calcification which might increase in size and number following therapy. It is not entirely clear whether these calcifications are associated with tumor response or disease progression. Calcified lesions which do not change in size or configuration are particularly problematic when assessed by RECIST criteria. Positron emission tomography (PET)/computed tomography (CT) is of particular value as it demonstrates the metabolic activity of the calcified lesions, in addition, it might reveal metastases in unexpected sites. We report a case of serous papillary ovarian cancer with extensive abdomino-pelvic calcified metastases referred for evaluation of therapy response. Despite being reported as stable disease on CT evaluation, we observed increased metabolic activity in the calcified lesions both on CT-attenuation corrected and non-attenuation corrected images, which was indicative of inadequate response to therapy. PET/CT is an ideal modality in follow-up of patients with ovarian cancer presenting with calcified metastatic tumoral deposits.

  • Peritoneal Super Scan on 18F-FDG PET/CT in two patients with Lymphoma

    • Abstract: Peritoneal lymphomatosis is an extremely rare presentation of lymphoma. Due to its relative low frequency, it receives much less attention than peritoneal carcinomatosis. The challenge is to differentiate between lymphomatosis and carcinomatosis, as well as peritoneal tuberculosis or other pathologic entities within the peritoneal cavity based essentially on radiologic features. 18F-FDG PET/CT is the main imaging modality for staging and monitoring response to treatment in lymphoma. Peritoneal lymphomatosis is most often associated to aggressive histological subtypes of high-grade lymphoma which are reputed high FDG uptake.   We report two cases of Burkitt’s lymphoma and diffuse large B cell lymphoma (DLBCL), with intense FDG uptake involving the entire peritoneum and decreased FDG activity of the rest of the body including brain and kidneys, giving it an appearance of “peritoneal super scan”, follow up FDG PET/CT showed the disappearance of the peritoneal lymphomatosis and the reappearance of cerebral and urinary activity synonym of complete metabolic response.

  • Sinus Tarsi Syndrome: Diagnosed on 99m-Tc MDP Bone SPECT/CT

    • Abstract: Sinus Tarsi Syndrome is a cause of chronic ankle instability and pain. MRI of the ankle has been the modality of choice for diagnosing the condition. However, SPECT-CT offers an alternate modality for diagnosing and evaluation of the condition. We present the case of a footballer who was suffering from chronic right leg pain despite receiving physiotherapy. He was being managed as a case of a chronic ankle sprain. Meanwhile, he was referred to the department as radiology for MRI of the ankle could not be performed as the patient felt claustrophobic. The patient subsequently underwent a 99mTc-MDP Bone scan. He was diagnosed to be suffering from sinus tarsi syndrome as it showed a characteristic pattern noted on 99mTc-MDP Bone scintigraphy. This case report reveals the potential of SPECT-CT as an alternative in the evaluation of chronic ankle sprain to MRI in segment of cases where MRI is not performed due to various reasons.

  • Lymphoscintigraphy in a case of recurrent chylopericardium

    • Abstract: Chylopericardium is an uncommon and benign condition in which triglyceride-containing chylous fluid collects in the pericardial cavity at high concentrations. Usually, chylopericardium occurs due to congenital malformation of lymphatic vessels or secondary to any trauma, surgeries, neoplasms, etc. However, if exact aetiology cannot be identified, the condition is referred to as Idiopathic chylopericardium which is a very rare presentation in day-to-day clinical practice. General physical examination, routine blood investigations and various anatomical imaging modalities may give a clue in the diagnosis, however, diagnosis can be challenging as they have a variable presentation. Also, optimal treatment poses greater difficulty as it remains controversial in most cases. We report a 47-year-old gentleman who presented with recurrent chylous pericardial effusion with no history of trauma, thoracic surgeries, cardiac disease and neoplasm in the past. Lymphoscintigraphy confirmed the communication between the lymphatic trunk and the pericardial space. The patient was managed conservatively with pericardial drainage and the patient recovered is doing well at present.

  • No needle to fear: An approach to needle phobic patients

    • Abstract:    18F-FDG is the most commonly used radioisotope in PET scanning and is administered intravenously. When patients cannot cannulated, there are limited options available for functional tumour assessment. A fifty year old male presented for investigation of a suspected lung carcinoma identified during investigation of pneumonia. The patient had a severe needle phobia, intellectual disabilities and multiple co-morbidities which made cannulation impossible. An alternative administration method was sought, with successful oral administration occurring in both staging and restaging scans. The scans demonstrated resolution of a suspected lung cancer indicating it was an inflammatory/infective process, preventing the need for more invasive investigative approaches. A non-invasive and positive experience allowed for accurate diagnosis and repeat imaging for this patient, enabling follow up imaging to occur. It is reported that oral administration of 18F-FDG may be useful for assessment of suspected cancers for patients where cannulation isn’t possible, when limitations are taken into consideration.
