  Abstract: Zusammenfassung Die (intensiv-)medizinische Behandlung von Patienten mit Leberdysfunktion stellt im klinischen Alltag eine große Herausforderung dar. Versagen konventionelle Behandlungsmöglichkeiten, können extrakorporale Leberunterstützungsverfahren die Therapieoptionen dieser Patienten erweitern. Insbesondere bei hepatischer Enzephalopathie (HE) und zur Stabilisierung der Kreislaufsituation sind gute klinische Daten und Erfahrung zu Anwendung und Nutzen der Verfahren vorhanden. Ein Überlebensvorteil für Patienten konnte bislang nicht erbracht werden. Neuere Verfahren weisen daher höhere Adsorberkapazitäten und längere Laufzeiten auf. Im akuten Leberversagen scheint die Plasmapherese das transplantatfreie Überleben der Patienten zu verbessern. Jedoch sind auch hier weitere Studien notwendig. Insgesamt sollte die Behandlung von Patienten mit schwerer Leberdysfunktion, insbesondere bei Möglichkeit der Lebertransplantation, an einem hierfür spezialisierten Zentrum erfolgen.

  Abstract: Zusammenfassung Die Therapie bösartiger Neubildungen hat sich in den letzten Jahren und Jahrzehnten deutlich verändert, und die klassischen Chemotherapeutika sind durch neue Substanzen ersetzt oder ergänzt worden. Zu ihnen gehören Substanzen, die gezielt einzelne Moleküle als Zielstruktur haben. Hierzu zählt u. a. auch die Immuntherapie. Neue Medikamente dieser Gruppe basieren u. a. auf Antikörpern und T‑Zellen. Checkpoint-Inhibitoren (CPI), bispezifische Antikörper, CAR(„chimeric antigen receptor“)-T-Zell-Therapien und die allogene Epstein-Barr-spezifische T‑Zell-Immuntherapie sind Beispiele für solche Immuntherapien, die alle mit einem breiten Spektrum von Nebenwirkungen assoziiert sind und auch renale Nebenwirkungen hervorrufen können. CPI zeigen spezifische renale Nebenwirkungen, die immunvermittelt auftreten und sich histologisch häufig als akute tubulointerstitielle Nephritis präsentieren. Ein akutes Nierenversagen unter bispezifischen Antikörpern und T‑Zell-Therapien tritt häufig im Rahmen eines Chemokine-release-Syndroms oder selten im Rahmen eines Tumorlysesyndroms auf.

  Abstract: Zusammenfassung Nierenversagen und onkologische Erkrankungen stehen in einer komplexen Wechselbeziehung. Sie teilen gemeinsame Risikofaktoren und Ursachen, und die spezifische Therapie des jeweils einen Krankheitsbildes kann zur Entstehung des anderen beitragen. Die Inzidenz des chronischen Nierenversagens bei onkologischen Erkrankungen wird in den Risikostratifizierungstools verschiedener Tumorerkrankungen unterschätzt. Eine akute oder chronische Niereninsuffizenz geht jedoch mit erhöhter Morbidität und Mortalität von Tumorpatienten einher. Hier ist ein möglichst frühzeitiges Erkennen der spezifischen Ursache zur Prognosebesserung geboten. Umgekehrt scheint ein chronisches Nierenversagen per se die Tumorinzidenz zu steigern. Eine intensivierte interdisziplinäre Zusammenarbeit zur Früherkennung von Nieren- und Tumorerkrankungen bei Risikopatienten, aber auch zur optimierten pharmakologischen Therapie ist geboten und wird mit dem Begriff Nephroonkologie verdeutlicht. Speziell wir Nephrologen müssen unser Verständnis von Tumorbiologie und aktuellen onkologischen Therapieansätzen erweitern.
