  • The Role of Exercise in Treating Low Back Pain

      Abstract: The purpose of this review is to highlight the role of exercise in preventing and managing acute and chronic axial low back pain (LBP). LBP is one of the leading contributors to years lived with disability as well as health care expenditures in the United States. With an expected increase in prevalence due to an aging population, sports medicine providers have a unique opportunity to provide effective treatment strategies incorporating exercise advice and prescription. Although the majority of individuals with acute LBP will have their symptoms resolve spontaneously, almost 40% will have recurrence or develop chronic LBP within 1 year. No single exercise method has been shown to be more effective than another. The evidence for walking programs, aerobic exercise, yoga, Pilates, and tai chi for LBP is discussed. Our review summarizes the beneficial role of a personalized exercise program and related counseling strategies in the prevention and management of LBP.

      Abstract: imageThe Physical Activity Guidelines recommend performing 150 min of moderate- to vigorous-intensity aerobic physical activity (MVPA) per week. These guidelines also recommend muscle-strengthening physical activity (MSPA) on ≥2 d·wk−1 for additional benefits including muscular fitness and bone health. The majority of the scientific evidence supporting the PA recommendations for health comes from studies of MVPA while the possible contributions of MSPA in these findings have been overlooked historically. Emerging evidence suggests that MSPA can independently protect against major cardiometabolic risk factors, chronic diseases, and mortality. Additional data from clinical trials indicate that many of the well-known health benefits of exercise, like improvements in cardiovascular disease risk factors, are more robust with combined MVPA and MSPA. This review will clarify the relative benefits of MSPA versus MVPA on health-related outcomes to determine the best type of PA for health.

      Abstract: imageHypertension is the leading risk factor for cardiovascular disease and an independent predictor of mortality. The prevalence of hypertension has doubled in the last two decades and evidence suggests that almost half the individuals are unaware of their condition. The antihypertensive effects of exercise are now undisputable, and exercise training is recommended by the major professional and scientific societies, including the American College of Sports Medicine (ACSM), as first-line treatment to prevent, treat, and control hypertension. This review aims to overview the evidence supporting the current ACSM Frequency, Intensity, Time, and Type exercise recommendations for hypertension, discuss new and emerging evidence on exercise in the treatment of hypertension from our laboratories; and propose future directions of research integrating this new and emerging evidence.

      Abstract: imageThe Exercise is Medicine On Campus (EIM-OC) program began in 2009 at Chatham University by Dr. Robert Sallis, Dr. Carena Winters, and ACSM leadership. The vision of EIM-OC is “to see all campus and community members across multiple disciplines discover, share, and adopt the principles of EIM that will help change the culture of physical activity and chronic disease prevention and management campus wide.” Although EIM-OC maintains close track of programmatic details, such as the number of registered and recognized institutions, a comprehensive review of EIM-OC publications has not been previously reported. The purpose of this scoping review was to 1) identify and examine all peer-reviewed evidence of EIM-OC, including scholarly articles and published abstracts of presentations; 2) analyze the key themes of EIM-OC implementation and outcomes; and 3) identify gaps in the literature. The scoping review covered all peer-reviewed publications, including scholarly articles and published abstracts, from 2009 to December 2021. In total, 9 scholarly articles and 46 published abstracts were included in this review. The articles and abstracts covered a wide range of topics, including gold level (physical activity assessment and exercise referral), silver level (physical activity education), and bronze level (physical activity awareness and promotion) activities, as well as evaluation of EIM-OC programming. Now that EIM-OC programming is firmly established, we now call on campuses and leaders to strengthen their reporting of EIM-OC outcomes at all levels: gold, silver, and bronze. Publishing research evidence will strengthen EIM-OC programming and initiatives. Specifically, we encourage publishing scholarly articles and using broad means for increasing dissemination.

  • Gender Diversity in Primary Care Sports Medicine Leadership

      Abstract: imageDiversity, equity, and inclusion have been recognized as important drivers of excellence and innovation in the physician workforce. Given the historical underrepresentation of women in medicine, gender diversity is of interest. In this cross-sectional study, we sought to quantify leadership representation of female physicians in primary care sports medicine settings, including primary care sports medicine fellowship programs, select sports medicine societies, and select sports medicine-related scientific journals. Data were collected by querying the corresponding web site for each fellowship program, society, and journal and analyzed in a descriptive manner. Results showed that fewer female physicians hold primary care sports medicine leadership roles than men do. This work establishes a baseline for female representation in primary care sports medicine leadership; efforts should continue to increase the presence of women in leadership positions.
