  • Knee Squeaking in Native Joint after Anterior Cruciate Ligament

    • Abstract: Articular squeaking sounds cause discomfort, frustration, and embarrassment for affected patients, especially in social contexts. Postsurgical native knee squeaking is rare, and only limited data on the subject are available in the literature. This study presents a case of knee squeaking after anterior cruciate ligament (ACL) reconstruction. A 22-year-old woman underwent reconstructive ACL surgery for an isolated tear in her right knee using an outside-in bone-patellar tendon-bone technique. Two months after the surgery, squeaking sound at knee flexion in both weight-bearing and non-weight-bearing conditions appeared without any further symptoms. The audible squeaking sound was a source of frustration and embarrassment for the patient. Given the unsuccessful attempts at diagnostic imaging, a diagnostic arthroscopy was performed during which a multistrand, long-chain, ultra-high molecular weight polyethylene suture was found moving freely in the joint. This suture, originally attached to the patellar bone portion of the graft, was removed during the arthroscopy. After the procedure, the patient reported having no more knee squeaking sound episodes. Friction of a nonabsorbable suture between the cartilage of a femoral condyle and a tibial plateau can produce a characteristic articular high-pitch squeaking sound. This should alert the surgeon to the possible presence of a freely moving fixation material in the knee. Diagnosis is clinical, as diagnostic imaging techniques have poor sensitivity.

  • L5-S1 Discoligamentous Distraction Injury following Bilateral Total Hip
           Arthroplasty Using an Anterior Approach: A Case Report

    • Abstract: Although acute spinal injury is rarely reported to complicate total hip arthroplasty (THA), its consequences can be devastating. We present a case of L5-S1 discoligamentous dissociation following single-stage bilateral THA performed sequentially through an anterior approach with a traction hemi-table applied on the operated leg. Internal fixation L5-S1 was required, resulting in prolonged recovery, but without relevant long-term sequelae. Pre-operative assessment of the spine is recommended when considering these procedures to evaluate the risk for this potentially severe complication. Particularly, lateral position may be favoured to avoid extension stresses of the spine. An anterior approach may have to be avoided, as it requires hyperextending the hip. A staged procedure may also be preferred to a single-anaesthesia bilateral procedure, limiting duration of potentially harmful positioning.

  • Bilateral Acromioclavicular Joint Subluxation in an Adolescent with
           Ehlers-Danlos Syndrome: Case Report and Literature Review

    • Abstract: Bilateral acromioclavicular joint (ACJ) dislocation is a rare occurrence, with only one reported case in the literature published in 1984. We present the case of a 15-year-old male with Ehlers-Danlos syndrome (EDS) who presented with complaints of subsequent nontraumatic bilateral ACJ subluxations and pain. ACJ reconstruction via an open mini-Mumford procedure was performed on this patient on two separate occasions with successful outcomes. The patient demonstrated decreased pain and increased stability during the postoperative period. EDS is a rare collagen disorder that is usually characterized by abnormal skin elasticity, bleeding tendencies, and, most importantly for this case, joint hypermobility. It is important to have an elevated index of clinical suspicion for potential joint subluxations and injuries in patients with a known history of collagen disease.

  • Total Knee Arthroplasty with Significant Osteolysis following Gore-Tex
           Anterior Cruciate Ligament Reconstruction: Recommendations on Reducing
           Risk of Arthroplasty Failure

    • Abstract: Synthetic substitutes for ligament reconstruction surgery have been widely used in orthopaedic surgery; however, many have been susceptible to mechanical failure and associated with osteolysis. A 58-year-old male underwent total knee arthroplasty (TKA) for valgus knee osteoarthritis on the background of a Gore-Tex anterior cruciate ligament reconstruction over 30 years prior. Radiographic imaging preoperatively demonstrated extensive osteolysis, particularly on the tibial side. At the time of arthroplasty, the extent of osteolysis was noted to be greater than that on the preoperative imaging. The patient underwent a robotic-assisted TKA with the use of a cemented stemmed tibial prosthesis and bone grafting. The patient was prescribed a period of protected weightbearing following the procedure. Our recommendations for undertaking arthroplasty with a Gore-Tex graft in situ are as follows: (1) thorough preoperative workup including computed tomography imaging to identify tunnel widening and osteolysis; (2) ensure that additional bone allograft is available for the procedure; (3) use of cemented implants where there has been significant osteolysis; (4) removal of the entire synthetic graft with copious irrigation post-removal to ensure the joint does not remain exposed to particulate debris; (5) grafting of the tunnels and bone defects post-removal of synthetic graft; (6) use of a stemmed prosthesis to bypass any cortical defects and reduce the risk of subsidence of the component.

  • Pseudoaneurysm Complicating Proximal Humeral Exostosis: A Case Report of a
           Missed Diagnosis on Computed Tomography Angiographic Examination

    • Abstract: Vascular complications of osteochondromas of the humerus are extremely rare. Only 9 cases with pseudoaneurysms of the brachial artery complicating osteochondromas of the humerus were reported in 7 English articles. The computed tomography (CT) angiographic examination of all of them were positive for the diagnosis of the pseudoaneurysm. Up to our knowledge, this is the first case report in which the CT angiography misdiagnosed such lesion. In this article, we described a case with a pseudoaneurysm complicating a proximal humeral exostosis in a 27-year-old male patient with a known history of hereditary multiple osteochondromas since childhood that was unanticipatedly misdiagnosed by the CT angiographic imaging modality. The participating authors tried to infer reasoned possibilities behind this unreported phenomenon before.

  • Delayed Fluoroquinolone-Induced Tibialis Anterior Rupture: Review and Case
           Report on Achilles and Tibialis Anterior Atraumatic Rupture with Operative

    • Abstract: Tendinopathy is a well-defined adverse effect of fluoroquinolone (FQ) antibiotics with complete tendon rupture being the most serious consequence. The most described FQ-associated tendinopathies are of the Achilles tendon with an onset ranging between 2 h after initial FQ administration to 6 months after completing FQ treatment. We herein report the case of a 50-year-old diabetic woman who suffered from a right Achilles tendon rupture and contralateral nontraumatic tibialis anterior (TA) tendon rupture 11 and 25 months following FQ administration, respectively. Operative management of both injuries identified degenerative changes consistent with FQ treatment. This case is unusual in its involvement of an uncommon tendon in an atypical timeframe. Notably, only one other reported case of FQ-associated TA tendon rupture was identified.

  • Synovial Chondromatosis in the Acromioclavicular Joint: Diagnosis and

    • Abstract: A 31-year-old male presented to clinic with 2 months of aching shoulder pain without inciting injury or recent illness. Radiographs and MRI were obtained and the leading diagnosis was synovial chondromatosis (SC) of the acromioclavicular joint. He was treated with surgical resection. This case represents a rare location for SC, a disease defined by chondral tissue growth within the synovium most commonly affecting the knee and hip. We present this case report and consolidate information published in previous reports to provide guidance and discuss management for a rare and difficult location of this disease.

  • Case Report: Pediatric Posterior Wall Acetabulum with Piriformis

    • Abstract: Pediatric acetabular fractures are rare, accounting for only a small percent of pelvic injuries in children. This is a case of a 12-year-old healthy male who sustained a displaced, right posterior acetabular wall fracture following a ground-level fall. It was ultimately determined to be treated through operative management. Intraoperatively, it was noted that the displaced fragment had become trapped posterior to the piriformis muscle. This prompted the surgical release of the piriformis to mobilize the fragment. Afterward, a quarter tubular plate serving as a buttress and multiple cortical screws were placed to secure the fracture. An additional cortical screw and washer were placed for further compression and rotational stabilization. The piriformis was then repaired. The patient was made nonweight bearing for 6 weeks and seen at routine postoperative visits. At 10 weeks postoperatively, the patient had no complaints of pain or difficulty with ambulation and tolerated full range of motion within his right hip. The patient did not return for follow-up, but the mother was contacted at 1 year postoperatively, and she denied any gait or hip range of motion abnormalities within the patient. Unfortunately, no further follow-up was successfully obtained. While most pediatric pelvic injuries can be treated nonoperatively, it has been shown that nonoperative treatment has increased risk of long-term complications. Therefore, surgical consideration remains important as it presents an opportunity for fewer long-term sequelae.

  • Use of Intraoperative Multidimensional Fluoroscopy to Assess Femoral Neck
           Reduction Quality in a Patient with Displaced Femoral Neck Fracture: A
           Case Report

    • Abstract: We present a case of displaced femoral neck fracture (FNF) in a patient treated with closed reduction and surgical fixation. An anatomic reduction was confirmed using intraoperative multidimensional fluoroscopy following reduction and surgical fixation. At the 1-year follow-up, the patient had returned to all activities and there were no signs of avascular necrosis. The quality of FNF reduction is an important modifiable treatment variable to reduce the risk of post-operative complications. Intraoperative multidimensional fluoroscopy enables the surgeon to utilize closed reduction techniques, limiting the risk of damage to vascular structures, while allowing for critical assessment of FNF reduction.

  • Avascular Necrosis due to Delbet Type I Femoral Neck Fracture in an
           Adolescent: A Case Report and Literature Review

    • Abstract: Among all pediatric fractures, femoral neck fracture is an infrequent injury that occurs due to high-energy trauma. The high risk of complications, such as avascular necrosis (AVN), which is the most common and serious complication, coxa vara, nonunion, premature physeal closure and infections, turns this fracture into an orthopedic emergency and increases the need for early treatment and intervention. Among the classifications of femoral neck fractures, which are known as the Delbet classification, type 1 is the least common but with the highest risk of AVN. Therapeutic action for these patients is close or open reduction with internal fixation under general anesthesia, which according to the evidences, open reduction is a more successful method. Due to the high probability of complications and the urgent need for treatment of femoral neck fracture, early intervention and timely treatment should be performed. Here, we report a 14-year-old boy who was brought to the emergency department with a Delbet type 1B fracture of the left femoral neck with detached epiphyseal portion of the femoral head due to a car accident. After one failed attempt to closed reduction, open reduction and internal fixation with a posterolateral (Kocher) approach was performed under general anesthesia. The reduction was maintained with guide pins and fixed with cannulated screws. After 8 months of follow-up, AVN was noticed.
