  • Epifisiólise femoral proximal, estudo retrospetivo de resultados a longo
           prazo após fixação in situ

    • Abstract: Objetivo: A epifisiólise femoral proximal (EFP) é o distúrbio mais frequente na anca do adolescente. A fixação in situ da epífise femoral é um dos tratamentos cirúrgicos com resultados consistentes, mas as consequências a longo prazo da sua fixação em posição não anatómica têm sido alvo de debate. Este trabalho teve por objetivo a avaliação retrospetiva dos resultados clínicos e funcionais de doentes submetidos a fixação in situ de EFP entre 1989-1999. Materiais e Métodos: Os pacientes submetidos a fixação in situ da epífise femoral por EFP entre 1989 e 1999, na nossa instituição, foram convocados a avaliação clínica presencial e avaliação funcional e de qualidade de vida com questionário HOOS e SF-36v2. Foram criados dois grupos de pacientes, segundo a classificação de Wilson. Os grupos foram comparados utilizando ferramentas estatísticas (t-test, teste exato de Fisher e ANOVA). Resultados: A amostra estudada foi de 27 pacientes (um total de 32 ancas). Na avaliação clínica não se identificaram diferenças entre os grupos. Na avaliação de resultados funcionais, o grupo II apresentou resultados do score funcional HOOS mais baixos que os casos grau I, e resultados do score SF-36 mais baixos do que o grupo I e do que os valores padronizados da população portuguesa. Conclusões: Embora alguns estudos apontem para uma semelhança de resultados a longo prazo para fixação in situ em desvios ligeiros e moderados, neste estudo observa-se uma diferença significativa entre estes grupos, constatando-se que o grau de deslizamento metafisário determina um pior resultado funcional a longo prazo.Objective: Slipped capital femoral epiphysis (SCFE) is the most frequent disorder of the adolescent's hip. In situ fixation of the femoral epiphysis is one of the surgical treatments with consistent results, but the long-term consequences of the fixation of the femoral epiphysis in a non-anatomical position have been debated. This study aimed to retrospectively evaluate the clinical and functional results of patients subjected to in situ fixation of SCFE, between 1989-1999. Materials and Methods: Patients subjected to in situ fixation of femoral epiphysis for SCFE between 1989-1999, in our institution, were invited to clinical examination plus functional and quality-of-life assessment with HOOS and SF-36v2 questionnaires. Two groups of patients were created according to Wilson’s classification. The groups were compared using statistical tools (t-test, Fisher exact test and ANOVA). Results: A sample of 27 patients (a total of 32 hips) was studied. In the clinical evaluation, no differences were identified between groups. In the evaluation of functional results, group II presented lower HOOS scores than grade I cases, and SF-36 scores lower than group I and of the standardized values for the portuguese population. Conclusions: Although some studies point to a similarity of long-term results for in situ fixation in mild and moderate SCFE, a significant difference between these groups is observed in this study, showing that the degree of metaphyseal slip determines a worse long-term functional outcome.
       
  • Validade e Fiabilidade do Teste Stress Manual e Gravitacional nas Fraturas
           Maleolares Equivalentes

    • Abstract: Objetivo: Terá o teste stress manual e gravitacional qualidade para separar populações de indivíduos com fratura maleolar com ou sem lesão do ligamento deltoide' O objetivo primário consiste em determinar a validade e fiabilidade do teste stress manual, comparando-o com a radiografia stress gravitacional. Secundariamente avaliar se o espaço tíbio talar medial ≥4 ou ≥5 mm tem valor preditivo diferente para lesão do ligamento deltoide. Material e Métodos: Efetuamos um estudo coorte prospetivo. A amostra dividiu-se em grupo I (teste stress manual) e grupo II (radiografia gravitacional) com fratura maleolar associada ou não a lesão do ligamento deltoide. Os testes foram exclusivamente realizados pelo autor, embora a sua interpretação tenha sido concretizada por avaliadores independentes, cegos para o resultado entre observadores. Resultados: Na amostra de 65 doentes, obtivemos no grupo I uma sensibilidade de 97%, especificidade e valor preditivo positivo de 100%. Enquanto no grupo II a sensibilidade foi de 100%, o valor preditivo positivo 97% e negativo 100%. Verificámos que o espaço tíbio talar medial ≥5 vs ≥4mm tem uma sensibilidade 100% vs 91%, especificidade 100% vs 100%, valor preditivo positivo 100% vs 100% e negativo 100% vs 60%. Em ambos os testes observou-se uma excelente concordância intraobservador e interobservador. Conclusões. Um espaço tíbio talar medial ≥5 mm tem valor preditivo superior na avaliação da integridade e competência do ligamento deltoide. O teste de stress manual e gravitacional são uma ferramenta eficaz, tornando-se fundamental na decisão terapêutica na fratura maleolar isolada com suspeita de lesão medial.Background: Can the manual and gravity stress test quality to separate populations with a malleolar fracture with or without a deltoid ligament injury' The author intends to assess validity and accuracy of the manual external rotation stress test comparing it with gravity stress radiography. Secondarily evaluate whether the medial clear space ≥4 or ≥5 mm has different predictive value for lesion of the deltoid ligament. Methods: A consecutive series of patients with isolated lateral malleolar fractures were prospectively enrolled. The sample was divided into group I (manual stress) and group II (gravity stress) with malleolar fracture with or without deltoid ligament injury. The medial clear space on the ankle’s mortise radiography was measured by blinded observers. Inter and intraobserver reliability was assessed. Results: In the sample of 65 patients, we obtained in group I a sensitivity of 97%, specificity and positive predictive value of 100%. While in the group II the sensitivity was 100%, the positive predictive value 97% and negative 100%. We found that the medial clear space ≥ 5 vs ≥ 4 mm had a sensitivity of 100% vs 91%, specificity 100% vs 100%, positive predictive value 100% vs 100% and negative 100% vs 60%. In both tests an excellent intraobserver and interobserver reliability was observed. Conclusion: A medial clear space ≥5 mm has a higher predictive value in assessing the integrity of the deltoid ligament. The manual and the gravity stress test are an effective tool in assessing the deltoid competence in isolated lateral malleolus fractures.
       
  • Eixo de Mobilidade Tibio-Talar

    • Abstract: Existe controvérsia acerca da variabilidade da orientação do eixo tibio-talar durante os movimentos de dorsiflexão e flexão plantar do pé em relação à perna. Actualmente pensa-se que existe um eixo variável durante estes dois movimentos. Foi feita uma revisão bibliográfica sobre este tema e testada a aplicação do conceito de eixo variável na colocação dos fixadores externos do tornozelo, com a intenção de diminuir a rigidez resultante da sua utilização. Foi feita uma pesquisa no Pubmed usando os termos “ankle axis” e “tibio-talar axis”, tendo sido selecionados os estudos da biomecânica do tornozelo com foco no eixo tibio-talar. Os estudos de Inman são os que mais suportam a existência de um eixo de rotação único. Actualmente, a literatura aponta mais para a existência de um eixo variável para os movimentos de dorsiflexão e flexão plantar. Estudos como o de Barnett e Napier ou, mais recentemente, de Lundberg, suportam a ideia de um eixo variável. Tendo por base as teorias que defendem um eixo variável e apoiados no conceito (de Hicks) de que não podem existir movimentos simultaneamente nos dois eixos, foi feita a tentativa de aplicação deste conceito na colocação de fixadores externos trans-articulares no tornozelo. Colocando o pino do calcâneo na direcção do eixo que privilegia a dorsiflexão pode-se minimizar a tendência para equino do pé com menor necessidade de pinos adicionais no médio-pé.There is still controversy about the variability of the talo crural axis during plantar and dorsiflexion of the ankle. Currrently, it is thought that there is a variable axis forthese two movements. A literature revision was conducted and we tested the application of the variable axis concept to the external fixation of the ankle, with the main goal of reducing the stiffness of the joint. Pubmed was searched using the key words “ankle axis” and “tibio-talar axis”, and the papers about ankle biomechanics were selected. Inman studies supports the idea of a single axis. Currently, the literature points more to the existence of a variable axis for the plantar and dorsiflexion movements. Studies such as that of Barnett and Napier or, more recently, of Lundberg, support this idea of a variable axis. Based on theories that advocate a variable axis and based on the Hicks concept that there cannot exist movements simultaneously in these two axis, an attempt was made to apply this concept in the placement of trans-articular external fixators in the ankle. By placing the calcaneus pin in the direction of the axis that favors dorsiflexion, the tendency for the foot to fall into equine position can be minimized, without the need for additional pins in the midfoot.
       
  • Distração Fisária no Tratamento de Sarcoma de Ewing do Úmero Proximal
           em Idade Pediátrica

    • Abstract: Nas crianças, cerca de 75% dos tumores ósseos malignos estão localizados na região metafisária dos ossos longos. A resseção epifisária condiciona uma morbilidade significativa, por dismetria e/ou disfunção das articulações adjacentes. Com a evolução das técnicas cirúrgicas, verificou-se que, em crianças com tumores malignos periarticulares, a distração fisária com preservação da epífise apresenta excelentes resultados funcionais. O objetivo deste estudo é descrever um caso clínico de uma criança de 7 anos com sarcoma de Ewing do úmero proximal, tratado por distracção fisária e reconstrução com aloenxerto.In children, about 75% of malignant bone tumors are located in the metaphyseal region of the long bones. Epiphyseal resection results in significant morbidity due to subsequent limb length discrepancy and / or dysfunction of the adjacent joints. With the evolution of surgical techniques, it was verified that, in children with periarticular malignant tumors, physeal distraction with preservation of the epiphysis presents excellent functional results. The objective of this study is to describe a clinical case of a 7-year-old child with Ewing’s sarcoma of the proximal humerus, treated by physeal distraction and reconstruction with allograft.
       
  • Sinovite Vilonodular do Ombro: Um Caso de Sucesso Tratado
           Artroscopicamente

    • Abstract: A sinovite vilonodular pigmentada, englobada no grupo dos tumores tenossinoviais de células gigantes, é uma doença rara, benigna e com incidência de 1.8 casos por milhão de habitantes/ano. Ocorre por proliferação da membrana sinovial das articulações, bursas e das bainhas tendinosas, com pigmentação por depósitos de hemossideria. Afecta sobretudo indivíduos entre os 30 e 40 anos. Geralmente é monoarticular, afectando o joelho em cerca de 80% dos casos e o ombro em apenas 2.4% dos casos. A sua clínica é inespecífica, o ao RX esta entidade geralmente passa despercebida. A Ressonância Magnética, não é específica para o seu diagnóstico, mas é útil para diferenciá-la de outras patologias e estratificar a extensão da doença. O diagnóstico definitivo é histológico. A sinovectomia é o tratamento indicado. É fundamental saber o subtipo da doença para a escolha o método cirúrgico: aberto ou artroscópico. Apresentamos um caso de um tumor tenossinovial de células gigantes do ombro, localizado, tratado com exérese artroscópia. O tratamento foi eficaz, sem recidiva até à data.Pigmented villonodular synovitis, which belongs to the group of giant cell tenosynovial tumors, is a rare benign disease, with an incidence of 1.8 cases per million inhabitants per year. It occurs by proliferation of the synovial membrane of the joints, bursae and tendinous sheaths, with pigmentation by haemosideria deposits. It mainly affects individuals between the ages of 30 and 40. Affects the knee in about 80% of the cases and the shoulder in only 2.4% of the cases. Its clinical manifestations are nonspecific and on XRay this entity usually goes unnoticed. Magnetic Resonance Imaging is not specific for diagnosis but is useful for differentiating it from other pathologies and stratifying the extent of the disease. Definitive diagnosis is histological. Synovectomy is the indicated treatment. Knowing the subtype of the disease is crucial for choosing the surgical method: open or arthroscopic. We present a case of a localized giant cell tenosynovial tumor of the shoulder treated with excision by arthroscopy. The treatment was effective, with no recurrence to date.

    • Abstract: As lesões Jeffery tipo2 correspondem a epifisiólises do rádio proximal com angulação superior a 90º relativamente à metáfise radial. O tratamento deste tipo de epifisiólise deve ser a redução aberta. Os autores descrevem um caso clinico de tacicula radial invertida após redução fechada de uma epifisiólise do rádio proximal e fazem uma breve revisão da literatura.Jeffery type 2 lesions correspond to an epiphysiolysis of the proximal radius with angulations greater than 90° in relation to the radial metaphysis. The treatment of these epiphysiolysis should be open reduction. The authors describe a case of reversed radial head after closed reduction of a proximal radius epiphysiolysis and make the literature review about this issue.
