  • Rizartrosis de dedo pulgar en dentistas: prevención y tratamiento

    • Abstract: RESUMEN La rizartrosis trapeciometacarpal puede afectar el trabajo de los dentistas y es importante su conocimiento para su prevención y tratamiento. Material y métodos: Se ha realizado una búsqueda bibliográfica en PubmedMedline y biblioteca Cochrane. Artículos publicados en español y en inglés sobre prevención y tratamiento de la rizartrosis del dedo pulgar. Resultados y discusión: Se proponen una serie de medidas para frenar la rizartrosis y mejorar la calidad de vida. Hay un tratamiento conservador y un tratamiento quirúrgico. Conclusión: Los movimientos repetitivos y en muchas ocasiones haciendo fuerza que realizan los dentistas pueden conducir a rizartrosis, sobre todo en mujeres.ABSTRACT Trapeziometacarpal rhizarthrosis can affect the work of dentists and it is important its knowledge for prevention and treatment. Material and Methods: The following electronic databases have been searched: Pubmed Medline and the Cochrane Library Plus. Articles published in Spanish and English about prevention and treatment of thumb rhizarthrosis. Results and discussion: A series of measures are proposed to stop rhizarthrosis and improve quality of life. There is a conservative treatment and a surgical treatment. Conclusions: The repetitive movements and in many cases using force that dentists perform can lead to rhizarthrosis, especially in women.
       
  • Rehabilitación completa de paciente geriátrico diabético e hipertenso:
           caso clínico

    • Abstract: RESUMEN El edentulismo es la pérdida total o parcial de dientes de etiología cariogénica, periodontal, traumática. Las enfermedades sistémicas prexistentes deben estar controladas, previo a los tratamientos odontológicos, mejorando el pronóstico. Objetivo: restaurar estética, función, fonética en paciente geriátrico con patologías sistémicas preexistentes controladas aplicando protocolo de rehabilitación oral completa. Presentación del Caso: Mujer de 74 años con hipertensión y diabetes, medicación permanente Glucophage (Metformina) y Losartan (Hidroclorotiazida). Desdentado total superior, desdentado parcial inferior, clase I de Kennedy. Tratamiento en Fases. Fase I planificación en modelos de yeso, modelos virtuales. Fase II quirúrgica instalación de 6 implantes dentales NEODENT. La fase II quirúrgica, se realizó cirugía campo abierto técnica de colgajo en forma de U, se colocaron 6 implantes dentales diámetro 3,75 mm x 11,5 mm. Cono Morse Alvim (13 y 23) ,cuatro implantes de conexión cónica, i.e drive implants (14, 16, 24 y 26). Fase III rehabilitación oral mediante filosofía gnatológica. Endodoncias de dientes inferiores, carillas de disilicato de litio, coronas de metal cerámica con ataches ERA, prótesis parcial removible inferior. Fase IV ejecución de prótesis hibrida superior con abutments multiunit de diferente angulación. Conclusión: el enfoque multidisciplinario en el tratamiento de la cavidad oral en pacientes geriátricos con patologías sistémicas prexistentes, permitieron restaurar función, estética fonética en el paciente.ABSTRACT An edentulous state is the total or partial loss of teeth and is mainly caused by caries and periodontal disease. Pre-existing systemic diseases must be under strict control prior to dental appointments for a favorable prognosis after treatment. Objective: To restore esthetics, function, and phonetic function in geriatric patients with pre-existing systemic diseases by applying a complete oral rehabilitation protocol. Case Presentation: A 74-year-old woman with history of hypertension and diabetes was medicated with Glucophage (metformin) and Losartan (hydrochlorothiazide). She presented a superior total edentulous maxilla and a partial edentulous Kennedy class I in the mandible. Treatment was performed in four phases. Phase I corresponded to the proper planning of the clinical case with plaster and virtual models. Phase II corresponded to the surgical phase, in which the U-shaped flap technique was performed and 6 dental implants with a diameter of 3.75 mm x 11.5 mm were placed. (Two conical connection implants (I.e. Morse Alvim) were placed in the anterior regions (13 and 23), and four other conical connection implants (i.e. Drive implants) were used in the posterior regions (14, 16, 24, and 26). Phase III oral rehabilitation the based on gnathological philosophy. Dental endodontics, lithium disilicate veneers, ceramic metal crowns with ERA attachments and removable partial dentures were used. Phase IV corresponded to the execution of an upper hybrid prosthesis with multiunit abutments of different angles. Conclusion: This minimally invasive multidisciplinary approach is considered predictable regarding esthetics and recommended for the restoration of patients’ appearance and self-esteem..
       
  • Prestación sanitaria dental durante la pandemia de Covid-19 en un Centro
           de Salud urbano de Zaragoza del Servicio Aragonés de Salud

    • Abstract: RESUMEN La labor ejercida por todos los profesionales sanitarios, incluyendo los que ejercen su labor en la Atención Primaria, presenta un papel transcendental durante la situación creada por la pandemia por la Covid-19. En España, tras doblegar la curva de contagios, se ha iniciado el proceso de desescalada, por el cual todas las actividades laborales productivas deben volver a ponerse en marcha, tras el cese forzoso producido por el Estado de Alarma, procurando a la par preservar la seguridad de toda la ciudadanía ante la amenaza latente de la Covid-19. En los Centros de Salud de los Sistemas Sanitarios Públicos Autonómicos, se han modificado los servicios de atención a los pacientes, con el fin de obtener un servicio asistencial de calidad a la par que se procura garantizar la salud de tanto el personal clínico-asistencial, como de los pacientes. Por este motivo, las Unidades de Salud Bucodental de los Centros de Salud de la Comunidad Autónoma de Aragón han tenido que adaptarse a la nueva situación, presentándose en este estudio las características de la atención bucodental en la Unidad de Salud Bucodental del Centro de Salud El Seminario-Romareda de Zaragoza en el primer mes de la desescalada.ABSTRACT The work carried out by all health professionals, including those who work in Primary Care, presents a transcendental role during the situation created by the Covid-19 pandemic. In Spain, after bending the contagion curve, the de-escalation process has begun, by which all productive work activities must be started again, after the forced cessation produced by the State of Alarm, while trying to preserve the security of all citizens in the face of the latent threat of Covid-19. In the Health Centers of the Autonomous Public Health Systems, the patient care services have been modified, in order to obtain a quality care service while trying to guarantee the health of both the clinical-care staff, as of patients. For this reason, the Oral Health Units of the Health Centers of the Autonomous Community of Aragon have had to adapt to the new situation, presenting in this study the characteristics of oral care in the Oral Health Unit of the Seminario Health Center Romareda de Zaragoza in the first month of the de-escalation.
       
  • Variaciones anatómicas en primer y segundo molar permanente mandibular
           con tratamiento endodóntico, evaluadas con tomografía computarizada de
           haz cónico. Revisión narrativa

    • Abstract: RESUMEN Introducción: Una de las dificultades de los tratamientos endodónticos son las variaciones anatómicas de las raíces y sistema de canales radiculares. Existe poca información sobre prevalencia y distribución espacial de estas variaciones en casos reportados con CBCT (del inglés Cone-Beam Computed Tomography). El propósito de la presente revisión fue analizar las variaciones anatómicas en primer y segundo molar permanente mandibular con tratamiento endodóntico evaluadas con CBCT. Materiales y métodos: Se seleccionaron artículos con casos de tratamiento endodóntico en primer o segundo molar permanente mandibular. Se determinaron el número, distribución espacial de variaciones anatómicas radiculares y canales. Además, el número y tipo de alteraciones de forma. Resultados: La mayor prevalencia del número y distribución espacial de raíces en primer molar fue de tres raíces [1M (mesial), 2D (distal)] con un 76,5% y cuatro raíces (2M, 2D) con un 55,6% en segundo molar. La mayor prevalencia del número y distribución de canales radiculares para el primer y segundo molar fue de cuatro canales: 1mesiovestibular, 1mesiolingual, 1distovestibular y 1distolingual, con un 30% y 90,9% de los casos seleccionados respectivamente. La distribución de alteraciones de forma fue de 8,7% para geminación y de 2,1% para taurodontismo. Conclusiones: La variación anatómica de raíces del primer y segundo molar inferior permanente con más frecuencia fue de tres y cuatro respectivamente. La variación anatómica de canales con más frecuencia fue de cuatro: 2M y 2D. El número de alteraciones de forma fueron poco frecuentes, presentando geminaciones y taurodontismo, solo en segundos molares.ABSTRACT Introduction: One of the difficulties of endodontic treatments is the anatomical variations of the roots and the root canal system. There is little information on the prevalence and spatial distribution of these variations in cases reported with CBCT (Cone-Beam Computed Tomography). The purpose of this review was to analyze the anatomical variations in the mandibular first and second permanent molars with endodontic treatment evaluated with CBCT. Materials and methods: Articles with cases of endodontic treatment in the first or second permanent mandibular molar were selected. The number, spatial distribution of anatomical variations of roots and canals were determined. The number and type of shape alterations were also determined. Results: The highest prevalence regarding number and spatial distribution of root in the first molar was three roots (1M (mesial), 2D (distal)) with 76.5% and four roots (2M, 2D) with 55.6% in second molar. The highest prevalence regarding number and distribution of root canals for the first and second molars was four channels: 1 mesiobuccal, 1 mesiolingual, 1 distobuccal and 1 distolingual, with 30% and 90.9% of the cases selected, respectively. The distribution of shape alterations was 8.7% for gemination and 2.1% for taurodontism. Conclusions: The anatomical variation of roots of the first and second permanent lower molars with the most frequency was three and four respectively. The most frequent anatomic variation of channels was four: 2M and 2D. The number of shape alterations were less frequent, presenting geminations and taurodontism, only in second molars.
       
  • Liquen plano oral: recomendaciones para su diagnóstico y tratamiento

    • Abstract: RESUMEN El liquen plano corresponde a una enfermedad crónica de naturaleza inmuno-inflamatoria que puede presentarse en piel y mucosas, siendo la mucosa oral de las zonas más frecuentemente afectadas, incluso como primera única manifestación de la enfermedad. Su etiología no se encuentra totalmente dilucidada, sin embargo, se atribuyen mecanismos autoinmunes como principal causa de la enfermedad. El liquen plano oral (LPO) presenta un amplio espectro de manifestaciones clínicas, pudiendo presentarse como lesiones rojas, blancas o combinaciones de ambas. Sus diversas formas de presentación en la mucosa oral, en conjunto con una histopatología a veces poco específica, determinan que en varios casos su diagnóstico sea complejo para clínicos no familiarizados con la enfermedad. El tratamiento del LPO se enfoca en reducir los síntomas asociados a las lesiones, siendo los corticoides tópicos los fármacos de primera línea. En caso de que el tratamiento con corticoides tópicos no logre controlar la sintomatología, el tratamiento de segunda línea incluye inhibidores de calcineurina o corticoides sistémicos, los que deben ser utilizados con precaución debido a los efectos adversos asociados a estas drogas. La OMS clasifica al LPO como un desorden potencialmente maligno con una tasa de malignización anual estimada en 1.5%-5%, lo que realza la importancia de un correcto diagnóstico y control rutinario, aún en aquellas formas clínicas asintomáticas. Esto último con el fin de poder detectar de manera oportuna cambios clínicos que sugieran su progresión hacia un carcinoma oral de células escamosas.ABSTRACT Lichen planus is a chronic immunological disorder affecting skin and mucous membranes, being the oral cavity one of the more affected areas even as a first or only manifestation of the disease. Its etiology is unclear, but it has been associated with the activation of autoimmune mechanisms. Oral lichen planus (OLP) has a wide spectrum of clinical manifestations including a variety of white lesions, red lesions or a combination of both. In addition, its histopathological features are not always specific. Because of the above, the diagnosis of OLP can be challenging, especially for clinicians who are not entirely familiarized with the disease. OLP treatment is aimed in reducing pain and discomfort, usually with topical steroids. If treatment with topical steroids is unsuccessful, second line treatments include calcineurin inhibitors and systemic steroids. Care has to be taken when using these drugs as they can have considerable adverse side effects. OLP is classified by the WHO as a potentially malignant disorder with a malignant transformation rate estimated between 1.5% and 5%. Thus, its correct diagnosis and proper follow-up are essential even in asymptomatic forms of the disease. This to detect early clinical changes indicative of malignant transformation to an oral squamous cell carcinoma (OSCC).
       
  • Comparación histológica de las superficies radiculares acondicionadas
           con acido citrico y EDTA previo raspaje

    • Abstract: RESUMEN Objetivo: El objetivo de la presente investigación fue determinar la histología de las superficies radiculares acondicionadas con ácido cítrico y EDTA previo raspaje. Material y métodos: El tipo de investigación es experimental ya que se llega a controlar la acción de la variable, es decir la aplicación del ácido cítrico y el EDTA (ácido etilendiamino). Para ello fueron utilizados 40 premolares sanos extraídos por indicaciones ortodonticas. Los dientes fueron distribuidos aleatoriamente en 4 grupos, cada uno de los grupos estuvo conformado por 10 premolares. El grupo 1 estaba constituido por superficies radiculares vestibulares acondicionadas con ácido cítrico previo raspaje, el grupo 2 estuvieron acondicionadas con EDTA previo raspaje, el grupo 3 constituido por premolares que solo recibieron raspaje y el grupo 4 no incluyeron raspaje. Resultados: La valoración macroscópica en relación a las placas de cemento, el raspaje más EDTA EL 10% existe una presencia normal, el 30% una pérdida parcial y 60% una perdida severa lo cual es estadísticamente significativo. Y los túbulos dentinarios En el grupo sin raspaje 100% existe una presencia normal de túbulos dentinarios, lo cual no es estadísticamente significativo. En el grupo con ácido cítrico el 30% tiene una pérdida parcial y el 70% una perdida severa y el grupo de EDTA el 100% presenta una pérdida severa, ambos grupos demostraron tener una significancia estadística. Conclusiones: Las superficies radiculares expuestas al acondicionamiento EDTA previo raspaje, en comparación con las superficies radiculares acondicionadas con ácido cítrico previo raspaje, muestran diferencias significativas, que evidencian que el EDTA induce a la mayor pérdida de estructura de las placas de cemento en la superficie radicular y evidente precipitación del cemento por la desmineralización, condicionando socavados con o sin precipitación de cemento con una mayor exposición de túbulos dentinarios.ABSTRACT Objective: The objective of the present investigation was to determine the histology of the root surfaces conditioned with citric acid and EDTA prior scraping. Material and methods: The type of research is experimental since the action of the variable is controlled, that is, the application of citric acid and EDTA (ethylenediamine acid).
