  • Rheumatic manifestations in cancer patients treated with immune checkpoint
           inhibitors

      Abstract: Immune checkpoint inhibitors are a breakthrough therapy for oncological patients. These drugs are considered effective in fighting cancer cells by supporting immune system. Their clinical efficancy is well documented however immune-related adverse events caused by drugs are the subject of interest of many research works. It is estimated that 15% to 90% of patients treated by immune checkpoint inhibitors experienced immune-related adverse events including 0,5-13% that require termination of the oncotherapy and provide immunosuppressive agents. The most common rheumatic manifestations of the immune checkpoint inhibitors therapy was arthralgia, myalgia, inflamatory arthritis, myositis. During the treatment, patients also present symptoms of systemic lupus erythematosus, vasculitis or sicca syndrome.

  • Gout in the course of systemic lupus erythematosus: Literature review and
           case study report

      Abstract: Gout is one of the relatively common inflammatory diseases of the joints. It is caused by the deposition of uric acid crystals in the tissues, which induces an acute or chronic inflammatory process. Elevated serum uric acid levels are usually found long before symptoms appear, and it is worth emphasizing that not every hyperuricemic patient will ever develop gout symptoms. The onset of gout is characterized by periodic joint inflammation, which may be triggered by various stress factors (trauma, infection), certain medications, dietary mistakes, and excessive exercise. Over time, repeated joint inflammation causes permanent joint damage. Most often, deposits of urate crystals are located in places with poorer blood supply and exposed to increased pressure, such as joints and soft tissues (e.g., auricles). The coexistence of gout and autoimmune diseases is relatively rare. While for many years it was believed that gout was not associated with other systemic connective tissue diseases, gout has been described in the course of systemic lupus erythematosus, systemic sclerosis, mixed connective tissue disease, psoriatic arthritis, ankylosing spondylitis, and rheumatoid arthritis. The presented systematic review also describes a case of a patient with longlasting systemic lupus erythematosus who was diagnosed with gout.

  • Autoimmune disorders and thyroid function in patients with rheumatoid
           arthritis after biological treatment

      Abstract: Rheumatoid arthritis (RA) and autoimmune diseases of the thyroid gland (AITD) often occur together. As autoimmune diseases, they share common pathological pathways, reflecting the fact that the treatment of the underlying disease can influence the course of thyroid disorders. The pharmacotherapy of RA is based on the supply of disease-modifying anti-inflammatory drugs with an increasing focus on biologics. This article aims to review the available literature describing the effects of biological treatments on thyroid function. The use of biological drugs may have the potential benefit of regulating the level of anti-thyroid antibodies. On the other hand, tumour necrosis factor-alpha (TNF-a) inhibitors are not indifferent to thyroid function and may increase the incidence of subacute thyroiditis. The coexistence of AITD and RA prompts consideration of the need for routine thyroid screening in RA patients and for RA screening in patients with thyroid disease who report joint problems.

  • Antidiabetic effect of disease-modifying antirheumatic drugs

      Abstract: Disease-modifying antirheumatic drugs (DMARDs) form the mainstay of treatment for chronic arthritis, slow down progression of the disease and can lead to a state of remission. Most of these drugs are immunosuppressive preparations and biologics. There are reports on effects of these preparations in addition to their anti-inflammatory effect, including antidiabetic effect. This paper aims to present the currently available results of studies and clinical observations indicating the potential for multidirectional effects of DMARDs, particularly a risk-reducing effect on the development of diabetes and diabetes-dependent complications.

  • Wrist arthroplasty, good alternative for rheumatoid wrist: Case report
           with literature review

      Abstract: Within the hand, the wrist is the most affected joint by rheumatoid arthritis (RA). Wrist fusion (WF) and wrist arthroplasty (WA) are solutions for severe arthritis of the wrist. WA decreases pain, and, contrary to WF, preserves motion. Reported clinical results of modern prosthesis constructs are good and the patients are satisfied. This research presents a patient with RA treated with WA. The follow-up is 13 years. There were 2 further synovectomies. Some bone erosion was observed. Apart from that, the patient is satisfied with WA. It seems that WA should be considered earlier in the treatment scheme in patients with RA.

      Abstract: Diffuse alveolar hemorrhage can accompany many diseases, including systemic vasculitis and other connective tissue diseases. Rapid diagnostics and immediate implementation of adequate treatment are extremely important. In this paper is presented a 63-year-old patient with fever, dyspnoea and massive haemoptysis, admitted as part of the emergency department.
