  Abstract: Zusammenfassung Angesichts knapper rheumatologischer Ressourcen wurde eine digital unterstützte Früherkennungssprechstunde (FS) etabliert zur frühen und sicheren Unterscheidung von Patienten mit entzündlich rheumatischen vs. nichtentzündlichen Erkrankungen. Es erhielten 500 Patienten mit Verdacht auf eine rheumatische Erkrankung einen Termin innerhalb von 2 Wochen. Sie wurden mithilfe eines digitalen Fragebogens (RhePort) befragt, kurz symptomorientiert untersucht, rheumatologische Basislaborwerte wurden bestimmt. Die Antworten wurden mit einem in RhePort hinterlegten Algorithmus gescort (von 0 = nichtentzündlich bis 4 = hochwahrscheinlich entzündlich), ebenso die Gesamteinschätzung der Rheumatologen nach Abschluss der FS. RhePort-Score und FS-Score wurden verglichen mit der Referenzdiagnose, die bei einer ausführlichen Zweituntersuchung nach im Mittel 10 Wochen erfolgte. Bei 490 auswertbaren Patienten wurden 133 entzündlich rheumatische (27 %) und 357 nichtentzündliche Erkrankungen (73 %) diagnostiziert. Eine Klassifikation allein aufgrund des RhePort-Fragebogens (Score > 1) erkannte 103 von 129 als entzündlich (Sens. 80 %) und 125 von 355 als nichtentzündlich (Spez. 35 %), was in einer AUC von 0,62 nach ROC-Analyse resultierte. Bei einem Score > 1 klassifizierte die rheumatologische Einschätzung nach FS 130 von 133 Patienten als entzündlich (Sensitivität 98 %), 261 von 357 nichtentzündlich (Spezifität 73 %). Die kombinierte FS kann Sensitivität und Spezifität entscheidend steigern im Vergleich zu einer alleinigen „automatisierten“ Befragung mittels Fragebogen. Neben früher Identifikation und Behandlung von entzündlich rheumatischen Patienten können durch schnelle Identifikation nicht rheumatologisch behandlungsbedürftiger Patienten Kapazitäten für die Versorgung geschaffen werden.

  Abstract: Zusammenfassung Nachdem die Ig(Immunglobulin)G4-assoziierte Erkrankung („IgG4-related disease“ [IgG4-RD]) im Jahr 2001 erstmals vermeintlich voneinander unabhängige klinische Krankheitssymptome zu einer eigenständigen Krankheitsentität mit potenzieller Multiorganbeteiligung zusammenfasste, konnten experimentelle und klinische Studien erstaunliche Fortschritte beim Verständnis der entzündlich fibrosierenden Erkrankung erzielen. Pathophysiologisch scheint ein Zusammenspiel aus B‑Zellen, IgG4+-Plasmazellen, follikulären T‑Helferzellen, zytotoxischen CD4+-T-Zellen und M2-Makrophagen eine Entzündungsreaktion mit profibrotischem Zytokinmilieu zu induzieren, die Fibroblasten zur Sekretion extrazellulärer Matrixkomponenten stimuliert. Diese bindegewebigen Veränderungen spiegeln sich mikroskopisch in den charakteristischen Befunden einer storiformen Fibrose und obliterativen Phlebitis sowie makroskopisch in einer diffusen Organvergrößerung oder in tumorösem Wachstum wider. Makroskopisch und histologisch kann jedoch die Abgrenzung zu granulomatösen Erkrankungen, wie z. B. der ANCA(antineutrophile zytoplasmatische Antikörper)-assoziierten Vaskulitis (AAV) erschwert sein. Die Differenzierung wird weiter dadurch verkompliziert, dass ANCAs bei IgG4-RD gemessen werden können. Darüber hinaus können IgG4-Antikörper bei vielen Differenzialdiagnosen von granulomatösen Erkrankungen wie Infektionen, AAV, Sarkoidose oder Malignomen ebenfalls positiv sein. Diese Arbeit gibt einen Überblick über das mannigfaltige Erkrankungsbild der IgG4-RDs im Hinblick auf Pathophysiologie, Diagnostik und Therapie. Weiterhin wird ein Überblick über Differenzialdiagnosen insbesondere im Hinblick auf granulomatöse Erkrankungen gegeben.

  Abstract: Zusammenfassung Hepatische Granulome können diverse Ursachen haben und bedürfen einer systematischen Diagnostik. Zunächst müssen Risikofaktoren für die verschiedenen granulomatösen Erkrankungen erfragt und extrahepatische Organmanifestationen abgeklärt werden. Es empfiehlt sich ein laborchemisches und serologisches Screening auf die häufigsten zugrunde liegenden Erkrankungen von Lebergranulomen in Deutschland wie die primär biliäre Cholangitis (PBC), die Sarkoidose und infektiöse Ursachen (vorrangig Tuberkulose und Hepatitis-C-Infektionen). Essenziell für die Diagnosesicherung ist die Leberbiopsie, wobei die minilaparoskopisch gesteuerte Materialgewinnung viele Vorteile wie Detektion von Granulomen auf der Leberoberfläche, am Peritoneum oder auf der Milz bietet. Ob sich aus dem Nachweis hepatischer Granulome eine therapeutische Konsequenz ergibt, hängt dabei entscheidend von der zugrunde liegenden Erkrankung ab. Liegen hepatische Granulome ohne begleitenden Leberparenchymschaden oder anderweitige Manifestationen vor, die eine Therapie notwendig machen, ist unter engmaschiger klinischer und laborchemischer Verlaufskontrolle ein abwartendes Verhalten vertretbar. Bei steigenden Leberwerten oder Hinweisen für einen Leberparenchymschaden ist eine Therapie der zugrunde liegenden Erkrankung dringend indiziert.

  Abstract: Introduction This study compared the frequency and severity of depressive disorders in patients with antineutrophil cytoplasmic antibody-associated vasculitis (AAV) before and during the severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) pandemic using the Korean version of the Center for Epidemiologic Studies Depression Scale-Revised (K-CESD-R) and the Korean version of the Profile of Mood States (K-POMS) depression, and further determined predictors of current depressive disorders in the patients during the pandemic. Methods Of the 61 patients with AAV who participated before the pandemic, 8 patients were transferred to other hospitals, 3 patients died, and 2 patients refused to participate in this study. Finally, 48 patients participated in this study. Depression disorders were defined as K‑CESD-R ≥ 16. Results When comparing the patterns of mental health between patients with AAV before and during the pandemic, no change in K‑CESD‑R or K‑POMS subscale scores was observed. Among AAV-related indices, regardless of the pandemic, the short-form 36-item Health Survey (SF-36) mental component score (MCS) and physical component score (PCS) were significantly correlated with K‑CESD‑R and could predict current depressive disorders. When the cut-off of Birmingham vasculitis activity score (BVAS) for depressive disorders was obtained by the receiver operator characteristic curve, it significantly predicted current depressive disorders in patients with AAV during the pandemic, unlike those before the pandemic. Conclusion We verified that SF-36 MCS and PCS could predict current depressive disorders, regardless of the pandemic, and furthermore, we demonstrated for the first time that BVAS was a predictor of current depressive disorders in patients with AAV during the pandemic unlike those before the pandemic.

  Abstract: Objective We retrospectively determined factors predicting biologic treatment discontinuation or tapering in patients with axSpA. Materials and methods We included 63 nonradiographic axSpA (nr-axSpA) and 138 radiographic axSpA (r-axSpA) patients on biologic treatments for at least 1 year. The biologic dosing intervals were increased in patients who had been in remission for at least 6 months. In patients whose biologic dosing intervals could be increased by 100% for at least 6 months, the agents were stopped at the end of that time. In patients for whom the biologic agents were stopped or tapered, relapse was defined as a Bath Ankylosing Spondylitis Disease activity index score > 4 and a CRP level > 10 mg/L. Results The median duration of biologic treatment (all patients) was 2 (1–11) years. Logistic regression analysis did not identify any independent predictor of treatment discontinuation. NSAID use was the only independent predictor of tapering (p = 0.001). The time to relapse after tapering was shorter in patients with r‑axSpA than nr-axSpA (25.97 vs. 39.53 months; p = 0.05). The time to relapse in patients with r‑axSpA was considerably shorter than that in patients with nr-axSpA (5.14 vs. 13 months; p = 0.001). All r‑axSpA patients relapsed over the follow-up period; only 2 nr-axSpA patients did not relapse. Conclusion The most significant independent predictor of relapse was NSAID use during treatment. For axSpA patients in remission, tapering of the biologic dosing intervals is more appropriate than discontinuation.

  Abstract: Zusammenfassung Um die prognostisch bedeutsame Wartezeit bis zur Diagnosestellung und Therapieeinleitung bei entzündlich rheumatischen Erkrankungen zu verkürzen, haben rheumatologische Zentren in vielen Regionen Deutschlands spezifische Frühversorgungskonzepte etabliert und fortwährend weiterentwickelt. Evaluierte Modelle aus Altötting·Burghausen, Berlin Buch, Düsseldorf und Heidelberg und ihre Entwicklungsetappen als Antwort auf interne und externe Herausforderungen werden in dieser Übersicht vorgestellt. Die transparente Offenlegung der Entwicklungsschritte und der Erfahrungsaustausch sollen die Entstehung weiterer Frühversorgungskonzepte in anderen Regionen fördern und den gemeinsamen Dialog zur Verbesserung der Früherkennung und Versorgungsqualität bei entzündlich rheumatischen Erkrankungen in Deutschland fortsetzen.

  Abstract: Zusammenfassung Fieber unklarer Genese („fever of unknown origin“ [FUO]) kann insbesondere bei immunsupprimierten Patienten eine Vielzahl an Ursachen haben. In ca. 25–35 % der Fälle liegt dabei eine infektiöse Ursache zugrunde. Die Diagnosestellung ist häufig erschwert und wird mit zeitlicher Latenz gestellt. Wir schildern den Fall einer 74-jährigen Patientin mit einem seit 5 Monate bestehenden FUO, bei der mittels offener Femurbiopsie und mikrobiologischer Untersuchung die Diagnose einer Neoehrlichiose gestellt werden konnte. Nach Einleitung einer Therapie mit Doxycyclin kam es zu einer raschen und kompletten Besserung der Beschwerden.

  Abstract: Zusammenfassung Patienten mit muskuloskeletalen Erkrankungen sind deutlich weniger körperlich aktiv als Gesunde. Sie können von einer Steigerung der körperlichen Aktivität und einer nachhaltigen Veränderung des Lebensstils in vielerlei Hinsicht profitieren, wozu eine stationäre Rehabilitation in erheblichem Maße beitragen kann. In dieser prospektiven Beobachtungsstudie (Prä-Post-Design) wurden körperliche Aktivität (mit dem Freiburger Fragebogen zur körperlichen Aktivität) und Depressivität (mit dem Beck-Depressions-Inventar [BDI]) bei 202 Rehabilitanden (124 weiblich, 77 männlich) mit muskuloskeletalen Erkrankungen (ICD-Diagnosen M) zu verschiedenen Katamnesezeitpunkten (zu Beginn der Reha, nach 3, 6, 9 und 12 Monaten) erhoben. Die Aktivitätssteigerung wurde in Abhängigkeit vom Ausgangslevel und der Depressivität analysiert. Drei Monate nach Rehabilitation lag das Aktivitätsniveau 47,8 % über dem Ausgangsniveau, was einer Steigerung der Medianaktivität von 5 auf 7,2 h pro Woche entspricht; 78,6 % der Teilnehmer zeigten nach 3 Monaten eine positive Differenz zum Ausgangsniveau. Der BDI-Score nahm im Mittel bei Durchführung der Rehabilitationsmaßnahme ab; eine Korrelation zwischen Abnahme des Scores und Zunahme der körperlichen Aktivität konnte nicht gezeigt werden. Durch eine 1‑malige Intervention (3-wöchige Rehabilitation) gelang eine Steigerung der körperlichen Aktivität über 12 Monate, wobei die Höhe der Aktivitätssteigerung nicht mit dem Ausgangslevel korreliert war, sodass auch bislang inaktive Patienten von der Reha profitierten.

  Abstract: Zusammenfassung Aktuelle Daten belegen erstmalig die Wirkeffekte der multimodalen rheumatologischen Komplexbehandlung (MRKB). Wie 2005 konzipiert, therapiert die MRKB erfolgreich durch rheumatologische und degenerative Erkrankungen verursachte Schmerzen und Funktionseinschränkungen und mindert die Krankheitsaktivität. Neben der Darlegung der aktuellen Evidenz gibt der Beitrag konkrete Handlungsempfehlung zur Durchführung wie auch Hilfe bei Prüfung durch den Medizinischen Dienst.

  Abstract: Zusammenfassung Bei dem systemischen Lupus erythematodes (SLE) und dem Antiphospholipidsyndrom (APS) handelt es sich um verwandte, systemweite Autoimmunerkrankungen unklarer Ätiologie. Zwar sind genetisch prädisponierende Faktoren bekannt; diese können allerdings nicht allein ausschlaggebend sein für den Ausbruch und Schweregrad dieser Erkrankungen. Hier erläutern wir die Rolle des bakteriellen Mikrobioms bei der Entstehung und Progression dieser rheumatischen Erkrankungen. Wir fassen dabei die neuesten Erkenntnisse auf dem Gebiet der Mikrobiomforschung anhand von tierexperimentellen Ansätzen, Patientenkohorten und humanen Proben zusammen. Wir gehen auf verschiedene autoimmunitätsfördernde kommensale Bakterien, sog. Pathobionten ein, die aus dem Darm, der Haut und der Mundhöhle stammen. Wir beschreiben zudem deren unterschiedliche Wirkmechanismen: Enterococcus gallinarum und Limosilactobacillus reuteri, die über Translokation aus dem Dünndarm zur Leber und Milz adaptive Autoimmunität und innate Typ-I-Interferon-Signalwege induzieren; Bacteroides thetaiotaomicron, Actinomyces massiliensis, Pseudopropionibacterium propionicum, Corynebacterium amycolatum, Ruminococcus gnavus und Roseburia intestinalis, die über Kreuzreaktivität mit Autoantigenen (Ro60, dsDNA und β2-Glykoprotein I) dazu führen, dass pathogene T‑Zell- und Autoantikörperantworten entstehen. Abschließend gehen wir auch auf potenzielle, zukünftige Behandlungsansätze ein wie die Immunisierung gegen bestimmte Pathobionten oder die gezielte Beeinflussung des Mikrobioms über Ernährungsansätze, die im Tiermodell erfolgreich Autoimmunpathologien vermindern können.

Placebo and nocebo responses in randomized controlled trials of Janus
kinase inhibitor monotherapy for rheumatoid arthritis

  Abstract: Objective The goal of this meta-analysis was to assess the frequency and magnitude of placebo and nocebo responses in placebo-controlled randomized controlled trials (RCTs) of Janus kinase (JAK) inhibitor monotherapy for rheumatoid arthritis (RA) Methods We performed a meta-analysis on the rates of placebo response, adverse effects (AEs), severe AEs (SAEs) and withdrawal due to AEs in placebo-controlled randomized clinical trials (RCTs) of JAK inhibitor therapy for RA. Results Five RCTs contained a total of 1422 patients (746 trial participants and 676 controls). The pooled incidence of an American College of Rheumatology 20% (ACR20) response rate was 33.0% (95% CI 19.6–44.9%) in placebo-treated patients and 68.3% (95% CI 61.4–74.1%) in active drug-treated patients. A strong negative correlation was observed between drug efficacies (ACR20 response) and AE rates in the placebo arm, indicating that the stronger the placebo response, the weaker the nocebo response (r = −0.906, P = 0.034). The pooled estimate of at least one AE was 54.1% (95% CI 44.6–63.4%) in placebo-treated patients and 54.5% (95% CI 46.2–62.6%) in active drug-treated patients. The pooled SAE rate was 3.9% (95% CI 2.7–5.7%) in placebo-treated patients and 3.8% (95% CI 2.5–5.7%) in active comparator-treated patients. The pooled estimate of withdrawal owing to an AE was 4.1% (95% CI 1.4–11.3%) in placebo-treated patients and 2.1% (95% CI 0.8–5.4%) in active drug-treated patients. However, there were no differences in the pooled risk of AE, SAEs, or withdrawal owing to AEs between the active comparator and placebo groups. A strong positive correlation was observed in AE rates between the placebo and active arms, indicating that the stronger the nocebo response, the higher the AE rate in the active arm (r = 0.957, P = 0.012). Conclusion The frequency of placebo and nocebo responses was 33.0 and 54.1%, respectively, in JAK monotherapy trials for RA. The findings indicated that the strengths of placebo and nocebo responses are inversely proportional and that clinically significant differences were absent between AE, SAE, and dropout owing to AEs.

  Abstract: Zusammenfassung Die Gicht, als häufigste Arthritis, wird durch Harnsäurekristalle ausgelöst. Ursächlich ist die Hyperurikämie, die meist aus einer renalen Harnsäureausscheidungsstörung, einem Zuviel an Purinen, aus Komorbiditäten und serumharnsäureerhöhenden Medikationen resultiert. Der erste Gichtanfall signalisiert abgelagerte Harnsäurekristalle – der Patient erhält eine Schmerztherapie. Danach gerät die Hyperurikämie oft in Vergessenheit, eine Harnsäuresenkung erfolgt nicht. Im interkritischen Intervall schreitet der Ablagerungsprozess fort, um nach Jahren mit Gichtanfällen und Gelenkdestruktionen sowie Nierenfunktionsverlust wieder aufzufallen. Daher fordern die Leitlinien bei sicherer Gicht eine Ursachensuche, diätetische Beratung und zielwertorientierte harnsäuresenkende Therapie. Wegen häufiger kardiometabolischer Begleiterkrankungen ist oft eine komplexe internistische Betreuung erforderlich. Im Folgenden werden die leitliniengerechte Diagnostik und Therapie der Gicht kompakt dargestellt.
