  • SGK1 repression by WT1 may confer a survival advantage to leukemic cells
           under stress conditions

      Abstract: Increased WT1 mRNA levels are pervasive in acute myeloid leukemia (AML), and this marker has been used to assess the leukemic compartment size after chemotherapy or hematopoietic cell transplants. Little is known about the effects of WT1 on the leukemic cells and their targets. This work used data obtained from gene expression arrays performed on AML samples with high and low WT1 mRNA levels to pinpoint genes that WT1 can regulate. We singled out SGK1, which showed an inverse correlation between its mRNA levels and WT1 in leukemic cell lines and AML samples. In cellular models, forced expression of WT1 reduced mRNA and protein levels of SGK1. Furthermore, WT1 repressed the SGK1 promoter activity, and accordingly, WT1 knockdown showed an increased expression of SGK1. We also detected an inverse correlation between WT1 and SGK1 during leukemic cell-line differentiation. WT1 genetic knockdown displayed decreased cell viability under nutrient deprivation. By contrast, SGK1 knockdown or pharmacologic inhibition increased resistance to apoptosis in response to serum starvation, acting on cell cycle progression. The effects of SGK1 targeting in hematologic conditions merit further investigation.

  • Clinical significance of C-reactive protein/platelet ratio from diagnosis
           to allogeneic hematopoietic stem cell transplantation in acute myeloid
           leukemia

      Abstract: Previous studies have shown that C-reactive protein/platelet ratio (CPR) before allogeneic hematopoietic stem cell transplantation (HSCT) is a predictor of survival in patients with hematological malignancies. This multicenter retrospective study aimed to evaluate the clinical significance of CPR from diagnosis to HSCT in patients with acute myeloid leukemia (AML) who underwent HSCT. This cohort included patients with AML who underwent their first HSCT between 2016 and 2021. CPR was evaluated at three time points: at diagnosis, after initial therapy, and pre-HSCT. The cut-off value for CPR was set at 0.05 based on previous studies. In total, 196 patients with a median age of 50 years (range: 15–72). High CPR was associated with the myeloblast ratio in the bone marrow at three points and was associated with high transfusion volume and poor performance status at HSCT. Overall survival (OS) at 2 years according to CPR at diagnosis, after initial therapy, and pre-HSCT (low vs. high) was 67.9% vs. 65.6% (P = 0.477), 72.6% vs. 54.8% (P = 0.022), and 73.1% vs. 49.7% (P

  • Second allogeneic transplantation in relapsed acute leukemia – a
           risk worth taking'

      Abstract: Second allogeneic stem cell transplantation (SCT2) presents a potentially curative treatment approach in patients who relapsed after first allogeneic transplant procedure. Currently there is no consensus on second allografting. The aim of our study was to analyze the outcome and prognostic factors of SCT2 in relapsed acute leukemia setting. The study comprised 60 patients at a median age of 44.7 years (37 females, 23 males) who underwent SCT2 due to relapsed acute leukemia between 2000 and 2024 in our center. Median remission duration after SCT1 was 13.6 (IQR 30.1) months and median time from relapse to SCT2 was 4 (IQR 3) months. Disease-risk index at SCT2 was as follows: low − 2 (3.3%), intermediate − 29 (48,4%), high − 27 (45%) and very high − 2 (3.3%). The same donor was used in 52 procedures (86.7%). Six patients died before engraftment due to severe infectious complications. The 2-year probability of overall survival (OS) and leukemia free survival (LFS) were 26.6% and 26.2%, respectively. Median follow-up after SCT2 was 10.7 (IQR 35) months. At the last follow-up, 22 patients (37%) were alive. The 2-year cumulative relapse incidence (RI) was 47.4% and cumulative non-relapse mortality (NRM) was 55.3%. The factors that adversely influenced survival were as follows: early relapse after SCT1 (

      Abstract: Immunotherapy, encompassing immune checkpoint inhibitors, antibody therapy, and chimeric antigen receptor (CAR)-T-cell therapy, has emerged as a groundbreaking approach by harnessing the immune system to selectively target and eliminate malignant cells. However, its widespread application has revealed a spectrum of severe, life-threatening hemostatic abnormalities, including immune thrombocytopenia, Evans syndrome, thrombotic thrombocytopenic purpura, acquired haemophilia A, thrombosis, disseminated intravascular coagulation and CAR-T-associated coagulopathy. These disorders may arise from excessive immune activation, autoantibody production, and intricate crosstalk between inflammatory pathways and coagulation cascades. Despite increasing recognition, their pathophysiological mechanisms remain incompletely understood, and standardized management strategies have yet to be established, posing significant clinical challenges. Given the safety concerns surrounding immunotherapy, timely diagnosis and appropriate intervention of these abnormalities are crucial. This review provides a comprehensive overview of the epidemiology, distinct clinical manifestations, emerging pathophysiological insights, and evolving treatment paradigms of immunotherapy-associated hemostatic abnormalities to enhance clinicians’ understanding and guide therapeutic decision-making.

  • Sickle cell β-thalassemia diagnosed at age 40: a case report

      Abstract: Hereditary hemoglobinopathies, including sickle cell disease and thalassemias, affect thousands of newborns annually, predominantly in low-and middle-income countries. Sickle cell β-thalassemia (HbSβ-thal), a form of compound heterozygosity involving β-thalassemia, presents with a wide range of clinical severity depending on the specific mutations. However, the clinical manifestations remain poorly defined. We report the case of a 40-year-old Greek female patient presenting with symptomatic sickle cell β-thalassemia, symptoms of tissue hypoperfusion caused by markedly low hemoglobin levels and notably, bone marrow necrosis. Remarkably, her condition remained undiagnosed until her admission to the emergency department. This case underscores the importance of maintaining a high index of clinical suspicion for the late-onset diagnosis of HbSβ-thal, particularly considering its increased prevalence in certain countries. The successful treatment strategy employed in this case highlights the critical role of individualized care in managing the severe and multifaceted symptoms associated with this genetic disorder, offering valuable insights for clinicians worldwide.

  • Predictive factors of romiplostim response in patients with refractory
           aplastic anemia: data from two clinical trials

      Abstract: The thrombopoietin receptor agonist (TPO-RA) romiplostim has been shown to be efficacious in the treatment of aplastic anemia (AA) refractory to immunosuppressive therapy. However, there are few reports on predictors of TPO-RA efficacy. We analyzed patient clinical and baseline laboratory data from two clinical trials that enrolled patients with refractory AA, a phase 2 dose-finding study (NCT02094417) and a phase 2/3 study (NCT02773290), to determine predictors of romiplostim efficacy. Data from 66 patients (35 from the phase 2 dose-finding study and 31 from the phase 2/3 study), median age 47.0 (range 20–78) years, were analyzed. Of these, 51.5% (34/66) had non-severe AA, 86.4% (57/66) had received prior therapy with anti-thymocyte globulin plus cyclosporine, and 80.3% (53/66) were transfusion-dependent. At 27 weeks, no patients achieved a complete response, 57.4% (35/61) achieved a partial response, 49.0% (24/49) became red blood cell transfusion-independent, and 56.8% (21/37) became platelet transfusion-independent.   • Genotype-dependent albuminuria in adult sickle cell disease in Kinshasa

      Abstract: Albuminuria, which depends on multiple factors, is common in patients with sickle cell disease and can progress to chronic kidney disease. In this study, we investigated the frequency and determinants of albuminuria according to sickle cell disease genotype. This multicentre cross-sectional analytical study of adults with stable sickle cell disease was conducted in Kinshasa. Genotypes were categorised as follows: homozygous, HbSS; heterozygous, HbAS; albuminuria, urinary albumin/creatinine ratio (mg/g): grade A1, < 30; grade A2:30–300; or grade A3,> 300. In total, 247 patients with sickle cell disease were included: 205 homozygous and 42 heterozygous. Albuminuria was prevalent in 50.5% of homozygous and 56.1% of heterozygous patients. The multivariate analysis revealed that the factors independently associated with albuminuria in the homozygous group were age ≥ 30 years (p = 0.037), leg ulcers (p = 0.010), hypertension (p = 0.038), and C-reactive protein level > 6 mg/L (p = 0.033). In the heterozygous group, only hypertension (p = 0.009), C-reactive protein > 6 mg/L (p = 0.006) and a history of vaso-occlusive crisis (p = 0.014) emerged as factors independent factors. More than half of patients with sickle cell disease had albuminuria, which was independently associated with hypertension and inflammation in both groups. Furthermore, there was also an association between in albuminuria and age ≥ 30 years, manifestations of vasculopathy in the homozygous group and a history of vaso-occlusive crisis in the heterozygous group.

  • Treatment outcomes in elderly patients ≥ 65 years with Large B-Cell
           Lymphoma (LBCL): a real-world single center experience focusing on
           potential CD19-CAR-T eligibility in second line

      Abstract: Elderly LBCL patients have unfavorable clinical and biological features, leading to higher relapse rates. While CD19 CAR-T therapy offers a curative option in second-line, access remains limited by clinical criteria in pts aged ≥ 65 years. In our real-world study, we evaluated 232 LBCL pts ≥ 65 years focusing on first line outcomes and potential CAR-T eligibility. Sixty-four patients progressed or relapsed. Applying AIFA criteria, only 9/37 (24%) R/R LBCL pts aged 65–75 years would have been CAR-T eligible. Among those > 75 years, excluded from CAR-T in Italy, only 3/27 (11%) met eligibility criteria. The main exclusion criteria were ECOG ≥ 2 (34 pts, 53%), CNS involvement (14 pts, 22%) and rapidly progressive disease with life expectancy

  • Avapritinib monotherapy induces rapid and deep remission of heavily
           treated, KIT D816H-mutated t(8;21) acute myeloid leukemia, a case report

      Abstract: Acute myeloid leukemia (AML) with t(8;21) is a subset of core binding factor AML and is considered to be favorable risk disease in patients receiving intensive cytarabine based chemotherapy. However, relapse remains a significant clinical challenge. Mutations in KIT, which frequently co-occur in t(8;21) AML, have been associated with worse relapse free and overall survival. Avapritinib is a novel tyrosine kinase inhibitor targeting KIT mutations that is approved for systemic mastocytosis but doesn’t currently have an established role in the treatment of AML. We present a case of a patient with extensively treated KIT D816H-mutated t(8;21) AML who experienced relapse after an allogeneic hematopoietic stem cell transplant and achieved a deep remission rapidly with avapritinib monotherapy. This case highlights the potential role of avapritinib as a targeted therapy for relapsed t(8;21) AML with KIT mutations, warranting further clinical investigation.

      Abstract: Abstract Allogeneic stem cell transplantation (SCT) remains the only curative therapy for patients with high-risk myelodysplastic syndromes (MDS). Due to age, comorbidities, or lack of urgency, many receive only palliative therapies to improve quality of life. Some patients remain untreated due to a lack of symptoms or low progression risk. Data on the impact of palliative therapies on overall survival (OS) and leukemia-free survival (LFS) are limited. Using the Düsseldorf MDS Registry, we compared outcomes of patients receiving red blood cell transfusions (RBCT) alone to the outcome of patients receiving RBCT combined with iron chelation therapy (ICT), erythropoietin (EPO), antithymoglobulin (ATG), or lenalidomide (LENA). Daratumumab, an anti-CD38 human IgG1κ monoclonal antibody has been used as single agent in CD38 positive r/r T-LBL. We administered a salvage treatment with daratumumab in combination with bortezomib and dexamethasone regimen followed by allogeneic stem cell transplantation from HLA-haploidentical related donor in a 50 years old patient affected by cortical CD38 positive T-LBL, refractory to 2 prior therapies including first-line cyclophosphamide, vincristine, doxorubicin hydrochloride and dexamethasone (hyper-CVAD) regimen plus autologous stem cell transplantation. After 29 months he is alive and in sustained complete remission, emphasizing the role of daratumumab in combination with bortezomib and dexamethasone as a salvage treatment option in CD38 positive r/r T-LBL patients.

  • Inborn errors of immunity presenting with lymphoproliferation: lessons
           from a case series

      Abstract: Lymphoproliferation can represent the first or leading disease sign in different inborn errors of immunity (IEI), which include autoimmune lymphoproliferative syndrome (ALPS), common variable immunodeficiency (CVID), combined immunodeficiencies, activated phosphoinositide 3-kinase δ syndrome (APDS), Epstein-Barr (EBV)-related disorders, and others. The genetic and molecular background and the clinical implications of IEI presenting with lymphoproliferation are partly unexplored. Therefore, the diagnosis and clinical management of this category of patients is particularly challenging. As treatments targeting the specific genetic defect are available for some of the IEIs associated with lymphoproliferation, identifying the molecular diagnosis is of great clinical relevance and could significantly improve the patient’s long-term outcome. To this purpose, a first-level immunological assessment is strongly recommended in patients with persistent or recurrent unexplained lymphoproliferation, and specific second-level analysis can orient towards the correct clinical suspicion. The mechanisms responsible for polyclonal, benign lymphoproliferation in IEI also cause an increased susceptibility to clonal, malignant lymphoproliferation. Therefore, a careful follow-up is recommended in all the patients with IEI and polyclonal lymphoproliferation, to promptly identify the development of clonality. Moreover, as malignant lymphoproliferation can be the first disease sign of some IEI, there is increasing interest in the possibility of recognizing IEIs in patients presenting with lymphoid malignancies. This work, by describing some relevant case studies, reviews the role of lymphoproliferative features in three of the most paradigmatic conditions, particularly ALPS, CVID, and APDS, and provides an updated discussion on the diagnosis, treatment, and follow-up of patients with IEI and lymphoproliferative features.

  • Prognostic factors of survival in second allogeneic hematopoietic
           transplantation

      Abstract: Allogeneic stem cell transplantation (alloSCT) remains a potentially curative treatment for patients with hematologic malignancies. Nevertheless, relapse and graft failure continue to be major barriers to long-term success. In these high-risk situations, a second alloSCT may represent the only curative option, although outcomes are frequently compromised by high non-relapse mortality and disease progression. Despite improvements in conditioning regimens, donor availability, and supportive care strategies, clinical results remain suboptimal and underscore the importance of careful patient selection.In this study, we report the 15-year experience of our institution—a national reference center for alloSCT in Spain—in managing patients undergoing a second alloSCT. Our objective is to evaluate relevant clinical and transplant-related factors associated with survival outcomes. Ultimately, we aim to enhance the selection process and contribute to a more personalized approach to 2nd-alloSCT, helping clinicians make better-informed decisions about which patients are most likely to benefit from this high-risk but potentially life-saving procedure.

      Abstract: Circular RNA (circRNA) is crucial in various biological processes, particularly in the onset and progression of different diseases. Nevertheless, the function of circMUC16 in acute lymphoblastic leukemia (ALL) remains unclear. The expression of circMUC16, miR-1182 and TPPP3 was evaluated by qPCR. The role of circMUC16 in the progression of ALL was investigated bymcherry-EGFP-LC-3, cell cycle, flow cytometry apoptosis, caspase-3 activity, western blotting, and an in vivo xenograft model. The interactions among circMUC16, miR-1182, and TPPP3 were explored by using qPCR, western blot, dual-luciferase reporter, and RNA immunoprecipitation assays.In this study, it was found that circMUC16 was increased in ALL cells. Down-regulation of circMUC16 reduced autophagy, cell proliferation and increased apoptosis in vitro while inhibiting cancer in vivo. circMUC16 acted as a molecular sponge of miR-1182. The carcinogenic effect of circMUC16 was partially mediated by miR-409-3p. TPPP3 was shown to be a direct target of miR-1182 and to be competed with circMUC16. Overexpression of TPPP3 partially counteracted the inhibitory effects of sh-circMUC16 inhibition on malignant behaviors of ALL cells. This study revealed that the circMUC16/miR-1186/TPPP3 axis plays a crucial role in the progression of ALL, suggesting a novel therapeutic target for ALL therapy.

  • Impact of ETV6 mutations on clinical outcomes in acute myeloid leukemia: a
           real-world retrospective cohort study

      Abstract: As a member of the ETS family, ETV6 has been demonstrated to be implicated with the molecular etiology of various hematopoietic diseases. However, the clinical impact of ETV6 mutations (ETV6mut) in acute myeloid leukemia (AML) remains unclear. Hereon, we included 879 consecutive newly diagnosed AML patients in our institution to elucidate the prognostic impact of ETV6 mutation status. In the overall cohort, ETV6mut were found in 24 cases (2.7%) and were associated with lower hemoglobin levels. The predominant common mutation types were missense mutations (15/31, 48.4%) and frameshift mutations (14/31, 45.2%). ETV6 mutations often occurred in conjunction with U2AF1 and ASXL1 mutations. Moreover, ETV6mut was associated with a lower complete remission (CR) rate (45.8% vs. 69.1%, P = 0.015) and shorter OS (P = 0.048) compared to the ETV6 wild-type (ETV6wt) group. Notably, the achievement of CR did not contribute to survival benefit in AML with ETV6mut. In multivariate analysis, ETV6 mutation was shown to be an independent adverse factor for OS in AML patients (HR: 1.72, 95% CI: 1.03–2.89; P = 0.040). Taken together, our study shows a mutational profile of ETV6 in AML and suggests that ETV6 mutations are associated with poor prognosis in AML patients.
