Abstract: For women with pre-existing and gestational diabetes, pregnancy involves specialized and intensive medical care to optimize maternal and infant outcomes. Medical management for patients with diabetes in pregnancy typically occurs via frequent face-to-face outpatient appointments. COVID-19-induced barriers to face-to-face care have identified the need for high-quality, patient-centred virtual healthcare modalities, such as mobile health (mHealth) technologies.

Abstract: Though national diabetes guidelines recommend targets for various health parameters, past studies have demonstrated a gap between recommendations and real-life practice. The objectives of our study were to assess 1) measurements in in type 2 diabetes (T2DM) care performed by diabetologists in tertiary care, 2) whether measurements were within recommended targets by Canadian guidelines, and 3) how these measurements compared to previously published Canadian studies.

Abstract: Administration of Δ9-tetrahydrocannabinol (Δ9-THC) to pregnant rats results in glucose intolerance, insulin resistance and reduced islet mass in female, but not male, offspring. The effects of Δ9-THC on other islet hormones is not known. One downstream target of the cannabinoid receptor, stathmin-2 (Stmn2), has recently been shown to suppress glucagon secretion, thereby suggesting Δ9-THC may also affect alpha cell function. The aim of the present study was to determine the effects of in-utero Δ9-THC exposure on the profile of glucagon, insulin and Stmn2 in the rat offspring islet and serum.

Abstract: Reducing unnecessary tests that do not enhance quality can promote healthcare value. Glycated hemoglobin (A1c) is often ordered at a frequency exceeding the recommendation of once every three months. We conducted a Quality Improvement (QI) initiative to reduce unnecessary repeat testing at a tertiary care academic hospital by 75%.

Abstract: Euglycemic diabetic ketoacidosis (DKA) is a potentially life-threatening adverse condition associated with sodium–glucose cotransporter–2 inhibitors (SGLT2i). This risk is further pronounced in the perioperative period. There is no consensus for when SGLT2i should be held preoperatively, and recommendations from various organizations have evolved from 1 day to 3-4 days in the latest American Diabetes Association guidelines. Further study of patients with perioperative euglycemic DKA is required to help clarify the optimal timing of preoperative discontinuation of SGLT2i agents.

Abstract: One in three adults in Canada are overweight and 26.8% experience obesity.1 Bariatric surgery confers effective weight loss and reduces obesity-related complications including Type 2 diabetes,2 but remains an underutilized treatment.3,4,5 Our objective was to determine whether a referral gap exists in bariatric program referrals for patients with Type 2 diabetes seen in endocrinology clinics at an ambulatory tertiary care hospital in Toronto, Canada.

Abstract: In the last one hundred years, since the discovery of insulin, methods of insulin delivery and glucose monitoring have advanced technologically. In particular, the introduction of insulin pumps, providing continuous subcutaneous insulin infusion (CSII), and continuous glucose monitors (CGM) have been revolutionary for people living with type 1 diabetes. In this review, we focus on automated insulin delivery (AID) systems and discuss the implications of both approved and off-label options for the user and healthcare providers.

Abstract: Depression in patients with diabetes mellitus is common and associated with poorer outcomes. This study aims to identify demographic, socioeconomic, and medical factors associated with the initiation of antidepressant medication after a diagnosis of diabetes in adult patients without a prior prescription for antidepressants. We also examined frequency of primary care visits in the year following antidepressant initiation compared to the year before.

This manuscript explores the social determinants of health (SDOH) that influence health behaviors among South Asian adults with T2D from a service provider perspective.

Abstract: Persons with lived experience of homelessness face more challenges in managing their diabetes, including purchasing and storing medications, procuring healthy food and accessing healthcare services. Not only do these individuals have challenges in accessing primary care, they are also seen by diabetes specialists (endocrinologists, diabetes educators, foot and eye specialists) less frequently.

Abstract: The prevalence of gestational diabetes mellitus (GDM) is dramatically higher in Indigenous compared to non-Indigenous populations in Canada. This scoping review synthesizes the existing literature regarding GDM among Indigenous peoples in Canada, including social and structural determinants that contribute to its higher prevalence in this population. Seven themes related to GDM in Indigenous populations emerged from a synthesis of the 44 included articles. The themes were GDM prevalence and trends, risk factors, screening, diagnosis and treatment, maternal outcomes, child outcomes, systemic barriers, and Indigenous perceptions, concerns and health behaviours.

Abstract: Non-severe hypoglycemia in people with diabetes is usually treated with rapid-acting carbohydrate, of which glucose is the most suitable form. A quantity of 15 g is recommended and repeated after 15 minutes if hypoglycemia persists. This recommendation has not changed for several years despite the introduction of continuous glucose monitoring, more flexible insulin regimens and improved insulin delivery. The present review has examined published studies that have explored the effectiveness of defined amounts of carbohydrate in treating insulin-induced hypoglycemia in adults with diabetes.

Abstract: Glucocorticoids are a central part of cancer treatment protocols. Their use in patients receiving chemotherapy increases their risk of hyperglycemia and associated adverse outcomes. Despite this, there have been few published protocols that guide the management of this patient group. In this narrative review, we use the Quadruple Aim as a framework to evaluate the current literature, including interventions, on glucocorticoid-induced hyperglycemia in patients receiving oncologic treatment, with a focus on the outpatient setting.

Abstract: Clinical and community guidelines recommend lifestyle (i.e., diet and physical activity) interventions for cardiometabolic conditions (including type 2 diabetes), yet current evidence suggests limited and variable services in primary care and public health settings. New implementation research studies are needed to ensure maximal effectiveness, equity and efficiency across all population subgroups and within the context of health systems. Such work will benefit from use of similar core measures and outcome indicators across studies.

Abstract: Type 1 diabetes (T1D) increases the risk of chronic kidney disease (CKD) development. The primary objective of this study was to assess the prevalence of abnormalities in estimated glomerular filtration rate (eGFR) in children with T1D. As a secondary objective, we sought to explore the relationship between clinical characteristics and abnormal trends in eGFR.

Abstract: Contemporary guidelines suggest relaxed glycemic targets in populations with type 2 diabetes mellitus (T2DM) at risk of hypoglycemia, including people experiencing multimorbidity, limited life expectancy, or frailty. However, overtreatment remains common. To inform safe deprescribing, a previous systematic review investigated the benefits and harms of deprescribing antihyperglycemics but identified only limited very low-quality evidence. We sought to update that review to identify and describe newly published literature on the effects of deprescribing antihyperglycemics in older adults with T2DM.

Abstract: The impact of type 2 diabetes (T2DM) on biomarkers denoting lipoprotein compositional status was studied in mild and moderate hypertriglyceridemia (HTG). Diabetic dyslipidemia pathophysiology could contribute to differences in lipoprotein compositional status, which could be reflected in the preferred cardiovascular disease risk prediction markers in HTG: non-high-density lipoprotein cholesterol (non-HDLC), and apolipoprotein B (apoB).

Abstract: Emerging adults (ages 18-30) with type 1 diabetes experience sub-optimal glycemic and psychological outcomes compared with other groups. The emotional burden of the unending self-care needs of diabetes management appears to be related to these poor health outcomes. However, there is no validated measure of this emotional burden in the developmental context of emerging adulthood.

Abstract: Existing peer support literature in diabetes has focused predominantly on the health impact it has on the beneficiaries rather than the benefactors. This mixed-methods study examined the effect of delivering peer support (versus receiving) on glycated hemoglobin (HbA1c) and diabetes distress (DD) at 3 and 12 months as part of a larger diabetes self-management support (DSMS) randomized controlled trial. Maintenance or improvement of outcomes were expected. Further, Peer Leader’s experiences with the program were examined.
