The development of IR condition is very difficult to observe. The initial stage of this condition does not show significant symptoms. Therefore, biomarkers are needed to detect this condition early and accurately. Zonulin is a protein that plays a role in regulating intestinal permeability and might be potential as an IR biomarker. Recent studies have shown the association of zonulin with T2DM and other metabolic diseases. However, there have been no studies showing zonulin profile in subjects at risk of IR and its response towards healthy lifestyle intervention. Therefore, this study aimed to determine the profile of zonulin in the blood plasma of subjects at risk of IR (n = 16 persons) relative to T2DM subjects (n = 3 persons) and healthy subjects (n = 42 persons), as well as changes after the intervention of red rice consumption and physical exercise for 3 months. Yet Medical experts are facing the undesirable augmenting causes of diabetes towards human body as a very important global issue for several years. The study aims to assess the applications of the wireless sensor networks that affect on pancreas within and around the individuals body boundary. Qualitative and quantitative wireless sensor data were obtained from field experiments and secondary data were poised from various sources. The study represents the urinary flow speed fluctuates with association of infection due to misuse of wireless sensor networks towards pancreas at light and dark environment. The research also focuses the more effective augmenting causes of diabetes in dark than light environment. The findings replicate the implication in diabetes through operative prevention and medication that the State provides, which fails to improve due to abusing wireless sensor networks. The study also found the urban hospitals are in risks due to insecure sensor technology. Scientific healthcare awareness is essential for management with modern technological device but such awareness is still below par, which is alarming to individual’s health.            Measures in Diabetic Patients at the Medical-Surgical Clinic of the
           Sylvanus Olympio University Hospital in Lomé (Togo)

The objective of our study was to evaluate the level of knowledge and practice of hygieno-dietary measures (HDMs) in diabetic patients in Togo but also to evaluate their role in achieving glycemic control. This is a descriptive and cross-sectional prospective study that took place over a period of six months (August 1, 2018 - January 31, 2019) at the Medical-Surgical Clinic of the Sylvanus Olympio University Hospital. Included in our study were patients with diabetes all gender included, at least 18 years of age, diagnosed and followed for at least one year, with a glycated hemoglobin (HbA1C) measurement no more than three months old and who agreed to participate in the study. A total of 138 patients were registered. Females accounted for 51.4% with a M / F sex ratio of 0.9. The median glycated hemoglobin level was 7.90% (IQR: 6.90-8.20). Twelve point thirty percent of the patients had a notion about HbA1C. Our study allowed us to estimate that 72.5% of patients had an average level of knowledge about diabetes and its HDMs. Twenty-four point six percent of diabetic patients said they regularly use HDMs. The level of knowledge about diabetes and HDMs was not associated with adherence to HDMs. Snacking (p = 0.035) and patients' opinions about their glycemic control (p = 0.013) were the factors associated with glycemic control.
  • Diagnosis of Diabetes Mellitus in a Resource-Constrained Setting: A
           Comparison of Fasting Plasma and Two Hours Post Prandial Glucose Values

    • Abstract: Publication year: 2020Source: International Journal of Diabetes Research, Volume 9, Number 1Allison Frederick Igila, Ojule Aaron C.Background: Studies has shown that the prevalence and incidence of diabetes mellitus varies with type of diagnostic criteria used: fasting plasma glucose of ≥ 7 mmol/l or a two hours post prandial glucose of ≥ 11.1 mmol/l or a glycated haemoglobin of 6.5%. This mean that the sensitivity and specificity of these parameters may vary. Though the diagnostic criteria of FPG and 2HPPG is used independent of the other, the cost of these tests become important and significant with regards to choice in resource- constrained setting. Aim: This study is to compare the ability of FPG and 2HPPG diagnostic criteria in making a diagnosis of diabetes from oral glucose tolerance test (OGTT) results in this resource constrained locality. Methods: A retrospective study of OGTT results from the laboratory of a tertiary hospital in southern Nigeria. WHO criteria for diagnosing diabetes mellitus using FPG≥ 7mmol/l and/or a 2HPPG ≥ 11.1 mmol/l from each subject was compared for sensitivity, specificity and accuracy. This OGTT results were determined using the glucose oxidase method to evaluate plasma glucose. Results: 150 subjects were diagnosed diabetic when the FPG was used, while 160 subjects were diagnosed diabetic when 2HPPG parameter was used. Those that were simultaneously positive for both FPG and 2HPPG were 120 and were regarded as the true positives (TP) for the purpose of this study. The sensitivity of both FPG and 2HPPG were 58% and 67% respectively while the specificities were both 98%. Conclusion: From this study, it was noted that more subjects were diagnosed diabetic using the 2HPPG which also had a higher sensitivity and positive predictive value. Therefore, the use of 2HPPG should be encouraged for targeted screening of diabetes mellitus to reduce a false sense of security for those who may be misdiagnosed and FPG test used for mass screening of diabetes mellitus to save time.
  • The Influence of Dietary Patterns on Diabetes among Women in Owerri West
           Local Government Area of Imo State, Nigeria

Undoubtedly, many chronic diseases can be traced to the dietary pattern of their victims. The study of dietary pattern in relation to a specific disease emerged recently and has not been fully harnessed especially in Africa where variety of native foods are combined and consumed across cultural and ethnic groups. Research on dietary pattern in relation to a specific disease may be unique according to culture and region. Until recently, no research work on dietary pattern in relation to diabetes was seen to have been carried out in Owerri-West Area of Imo State, Nigeria. The present research is an attempt to initiate a study of dietary pattern in Owerri-West and in particular, examine its influence on diabetes among women. The objective of the present study is to examine the dietary patterns and factors that influence the dietary patterns of women resident in Owerri-West Area of Imo State, Nigeria. A cross-sectional design was employed in the study. The study population consisted of adult females in the communities in Owerri West L.G.A who are within the age range of 18-55 years. A sample size of 382 was taken for the study. A semi structured self-administered questionnaire with diabetic test and anthropometric measurements were used for the study. Descriptive and inferential statistical tools were used to analyse the data obtained. Results obtained show that age, education, occupation, marital status and religion had significant influence on the dietary patterns of the participants and the prevalence of diabetes. The results also show that carbohydrate, protein and lipid rich foods alongside soft drinks and beer were in high consumption among the participants with carbohydrate as the highest.
  • Factors Influencing the Adoption of Health Information Technology for
           Optimal Chronic Disease Management: Diabetes Management in Southwestern
           Nigeria as a Case Study

A.Despite diabetes management techniques already in use in Nigeria, desired care outcomes are not acceptable and due to this, patient satisfaction is not optimal. On this note, and having known the prevalence and the nature of diabetes as a chronic disease that needs optimal management, this study therefore investigates the factors influencing optimal management of diabetes in Nigeria using Health Information Technology. However for this study, the model of Donabedian that proposed a healthcare quality systems-based framework of structure, process and outcome, to measure quality of health care, so as to give an idea of where problems are stirring, and how they could be addressed was used. Both qualitative and quantitative data collection methods were used. The quantitative data were collected through questionnaire, while the qualitative data were collected through key informant interviews. Thirty nine (39) factors were presented, and after using factor analysis and some other statistical tools, the seven most important factors that emerged were inadequate monitoring of patient risky indicators; increased unplanned evil occurrences; unavailability of needed/complete confidential patient data; lack of patient involvement and education; erratic power supply; not reasonable staff to patient ratio; and inadequate staff skills and expertise. The first factor accounted for most variance of 23.03, while the rest of the six factors accounted for 17.81, 17.21, 8.15, 7.46, 5.12 and 4.70, respectively. Thus, all the factors accounted for a cumulative of 83.5%. It was then concluded that the use of Health Information Technology could alleviate some of these factors if used meaningfully.

           Southwestern Nigeria as a Case Study

This study was carried out in Southwestern zone of Nigeria. For the purpose of this study, six hospitals were selected purposively from all the six states of the selected zone, so that each state was represented in the study. Therefore, the six hospitals, which were three federal and three state hospitals, were strategically selected from the states. Some health stakeholders, the Nurses, Doctors (from senior registrar level to consultants), Pharmacists, Medical record officers, ICT unit professionals and Laboratory Scientists (in care of patients’ blood tests, urine tests, x-ray and so on) roles were involved in the study. Altogether, three hundred and thirty six (336) respondents were selected for the survey, chosen across all the six states, distributed as 156 Patients, 24 Nurses, 24 Doctors, 36 Pharmacists, 36 ICT Unit professionals, 24 Medical record officers and 36 Laboratory Scientists. However, there was a response rate of 89.3%, meaning that 336 copies of questionnaire were distributed, while 300 copies were retrieved. Primary data were used to collect the data for the study. The Primary data were in the forms of questionnaire and observation. Of all the twelve Health Information tools considered, it was noted that Digital monitoring devices are the most common tool, as it is owned by hospitals, it is also owned by many of the patients. Therefore, most of the staff and patients selected Digital monitoring devices as an important tool useful for diabetes management.

This longitudinal study was conducted among 138 T2DM patients attending the diabetes clinic at Kitui County Referral Hospital. A structured questionnaire was used to obtain information on respondents’socio economic and socio demographic characteristics and drug adherence practices. Data was analyzed using SPSS version 21. After six months, the proportion of respondents who took their drugs at specific times increased from 58.2% to 74.6% in the intervention group (IG) compared to a decrease of 47.9% to 46.5% in the control group (CG).
