    • Free pre-print version: Loading...

Abstract: Founded by alumni and members of the National Health Service Corps (NHSC) in 1996, the Association of Clinicians for the Underserved (ACU) is committed to educating, encouraging, and enabling health care providers of all backgrounds to practice in underserved communities nationwide. The organization engages providers early in their clinical careers, helping to support placement in safety-net organizations that provide guidance and experience while simultaneously serving those communities in need. For more than a decade, ACU has been the leading advocate for the NHSC, specifically leading efforts to secure increased funding and resources for the growth and expansion of this federal program. The NHSC has a 50-year ... Read More

Abstract: To the Editor,In addition to Ontario and Manitoba, universal basic income pilot programs have been run in Africa, Asia, Europe, and South America.1 The programs have been a resounding success, giving participants dignity and improving their health.2 For example, there has been a decreased use of alcohol and tobacco and improved sleep and mental health among recipients.1 At the same time, family members reported improvements in their children's school performance, health, nutrition, stability, and social networks.1,3 Unfortunately, despite such successes, the pilot programs have not materialized into permanent national initiatives.4 This is particularly important in the era where many nations face a growing housing ... • Inequities in COVID-19-Related Patient Outcomes by Socio-Demographic
           Characteristics: A Scoping Review

    • Free pre-print version: Loading...

Abstract: The coronavirus (COVID-19) pandemic made the disparate health burden borne by socially marginalized populations more visible. Disparities in disease burden are often due to inequities in exposures to harmful social conditions as well as the exacerbation of morbidity and mortality from chronic diseases.1 When the first cases of COVID-19 were detected, with global spread becoming apparent early in 2020, already strained health resources and systems were further taxed by overwhelming health care needs. There were also significant socioeconomic, social justice, and human rights consequences for many affected populations.2,3 People living in precarious positions due to poverty, poor health, racism, lack of insurance ... • Colorectal Cancer Screening in Free Clinics: A Systematic Review

    • Free pre-print version: Loading...

Abstract: Colorectal cancer (CRC) is the third most diagnosed cancer and the second most common cause of cancer death in the United States.1 The United States Preventive Services Task Force (USPSTF) guidelines were updated in 2021 to recommend CRC screening for all adults aged 45 to 75 years. Screening protocols include fecal occult blood test (gFOBT or FOBT) or fecal immunochemical test (FIT) annually, stool DNA FIT (sDNA-FIT) every one to three years, computed tomography (CT) colonography every five years, flexible sigmoidoscopy every five years, annual FIT with flexible sigmoidoscopy every 10 years, or colonoscopy every 10 years.2 Despite increased CRC screening in recent decades, only 68.8% of all eligible adults are up ... • Rural Hospital Closures: A Scoping Review of Studies Published Between
           1990 and 2020

    • Free pre-print version: Loading...

Abstract: Approximately 46 million people live in rural places across the United States, accounting for nearly 14% of the U.S. population. Rural communities account for two-thirds of primary care health professional shortage areas (HPSAs), a situation that is exacerbated by their disproportionate risk of hospital closure.1–3 Given that rural communities are generally older with higher multi-morbidity and higher preventable mortality,4 the disparate impact of hospital closures is of great concern. In 1993, the Office of the Inspector General (OIG) in the Department of Health and Human Services, published a report on rural hospital closures between 1987 and 1991.5 By breaking from the previous pattern of grouping rural and ... • Free pre-print version: Loading...

Abstract: In the United States, about 582,500 people experienced homelessness on a single night in January 2022.1 Homelessness has been linked with poor health and worse health-related outcomes.2–5 Individuals experiencing homelessness suffer from higher levels of medical and psychiatric illnesses as well as disparities in clinical care. 2,3,6 For example, patients who are experiencing homelessness and being treated for cardiovascular disease and stroke are less likely to receive diagnostic and therapeutic procedures than non-homeless patients.3 In fact, cardiovascular diseases generally start occurring earlier in patients experiencing homelessness than those who are housed, and are tied to more adverse outcomes due to ... • Free pre-print version: Loading...

Abstract: Health insurance protects beneficiaries from unexpected and high medical bills, but rates of uninsurance in the U.S. are high and disparities in coverage remain. Immigrants—who constitute 14% of the population1—experience higher uninsurance rates than U.S.-born individuals because they face additional restrictions for Medicaid and Marketplace eligibility,2,3 are less likely to work in jobs that offer employer-provided insurance,4 and may be less likely to take up public benefits when eligible, particularly with the public charge rule expansion proposed during the Trump Administration,5 which would have expanded the criteria through which immigrant applicants may be denied admission to and residency in the U.S. for ...            Health in Heart Failure: Analysis of a Safety-Net Population

    • Free pre-print version: Loading...

Abstract: Heart failure (HF) is an overwhelming public health problem. Currently, the prevalence of HF in the United States (U.S.) is estimated to be roughly six million,1 with population prevalence projected to reach over 8% by 2030.2 Medical expenditures associated with HF in the U.S. are similarly increasing, predominantly due to direct medical care during admissions and readmissions,3 and are expected to exceed $50 billion in 2030.4Given these trends in HF prevalence and spending, alongside the high HF mortality burden,5 significant emphasis must be placed on defining and effectively addressing all determinants of HF outcomes. Social and behavioral factors are increasingly recognized as important determinants of health; ... Read More

Abstract: The prevalence of adults with current hepatitis C virus (HCV) infection (defined as HCV RNA-positive) in the United States (U.S.) is estimated at one percent, but it is higher among incarcerated populations at approximately 10.7%.1 It is also estimated that nearly one-third of people living with HCV in the U.S. spend at least part of the year in a correctional facility.2 HCV-related mortality among incarcerated populations is high. One study found that viral hepatitis was a causal factor in 34% of 688 chronic liver disease-related deaths among incarcerated men in Texas.3Current guidelines recommend that HCV treatment be offered to all people with current HCV infection, as treatment with antiviral therapy is ... Abstract: Concordance between patients and clinicians on multiple demographic characteristics (e.g., race, language, gender) has been shown to improve outcomes and is associated with patients' perceived quality of care.1 Racial and ethnic concordance is associated with more consistent control of hypertension, increased access to care, and lower health care expenditure.2–4 One study found that after meeting with a racially concordant physician, Black men were significantly more likely to choose to do all preventive care screening recommended. They estimated that Black physicians could significantly reduce the Black-White cardiovascular mortality gap.5 Another study found that newborn-physician racial concordance was ... Read More

Abstract: Diabetes persists as a global health crisis with significant implications worldwide and in the United States particularly. The prevalence of diabetes continues to rise in the Caribbean. A case in point is the prevalence of diabetes in Haiti's capital, Port-au-Prince, which nearly tripled from 6.9% in 2006 to 19.7% in 2018 and is now higher than the prevalence in the United States.1,2 Within the United States, prior limited studies suggest Haitian Americans experience a disproportionately high prevalence of diabetes when compared with both White and Black non-Hispanic Americans.3,4 A small (n=51) study conducted3 in Little Haiti in Miami, Florida, found a diabetes prevalence of 33% among individuals of Haitian ... Abstract: Adolescence, the period between childhood and adulthood, is a critical, formative time where key health indicators predispose to adult health outcomes. For example, overweight/obesity status in adolescence has been shown to increase risk of several comorbid conditions in adulthood.1–3 Lower socioeconomic status (SES), often defined by parent factors such as educational attainment or income, has been shown to be associated with poor adolescent health outcomes related to mental health and weight.4–7 However, less is known about the mechanisms driving relationships among maternal, paternal, and adolescent education measures on adolescent health. Recent studies have shown that educational attainment may be an ... Read More

Abstract: Health equity is at the forefront of recent discourse within the field of public health. To reduce population-level disparities and inequities, researchers, practitioners, policymakers, and other community stakeholders must focus on the implementation of equitable practices and policies aimed at addressing a holistic array of factors affecting health.1 One method to aid stakeholders in assessing the effect of policies and programs on different populations is the use of health impact assessments.Health impact assessments (HIAs) are tools that can be used to consider the effect of programs and policies on social determinants of health and other factors.            Resistance Strategies: Implications for Tobacco Product Use Prevention

    • Free pre-print version: Loading...

Abstract: Since 2011, youths' e-cigarette use (i.e., vaping) has grown into a national epidemic. Citing data from the Centers for Disease Control and Prevention (CDC), King et al.1 described the exponential growth of youth e-cigarette use. Compared with other tobacco products, which have shown relatively unchanged or decreasing trajectories of use since 2011, youth e-cigarette use has rapidly accelerated, particularly since 2017. These rates are particularly concerning, in light of documented cases of e-cigarette/vaping-associated lung injury (EVALI). As of January 2020,* 2,602 cases of EVALI have been reported by the CDC in all 50 U.S. states, with 57 confirmed deaths. Recent data have also indicated that youths' ... Read More

Medical, social, and public health systems struggled to respond, leaving many without necessary basic care and support as they became critically ill and often died. Building on several demonstration grants funded through the Robert Wood Johnson Foundation and the U.S. Department of Health and Human Services (HHS), the Ryan White Comprehensive AIDS Resources Emergency Care (CARE) Act of 1990 [Public Law No: 101-381] funded a comprehensive system of HIV care, treatment, and essential support services to ensure that people with HIV could access care.
