Antibody responses of COVID-19 patients according to symptoms and the
presence of pneumonia

  Abstract: Introduction: The aim of the study was to examine the 30-day total SARS-CoV-2 antibody positivity in patients across a clinical spectrum ranging from asymptomatic to pneumonia.Methods: This prospective cohort study consisted of 51 consecutive patients who were RT-PCR positive and diagnosed COVID-19 pneumonia (Group 1) and 58 consecutive patients who were also RT-PCR positive but were asymptomatic or had mild symptoms (Group 2). On the 30th day from the date of symptom onset, the patients were called for examination and blood samples were taken for the detection of SARS-CoV-2 antibodies.Results: Patients with pneumonia, fever, muscle pain, and loss of taste and smell had significantly higher rates of antibody positivity (p= 0.001, 0.003, 0.030, and 0.018, respectively). Antibody positivity was found to be significantly higher in patients with at least one symptom on admission compared to asymptomatic patients (p = 0.001). While the antibody positivity rate was 96.1% in Group 1 (patients with pneumonia), it was 50% in Group 2 (patients without pneumonia), and 77.7% in patients with at least one symptom on admission compared to 33.3% in asymptomatic patients (p=0.001).Conclusions: Patients with COVID-19 pneumonia have significantly higher disease-specific total antibody positivity rates than patients without pneumonia. Considering the 50% antibody positivity in patients who had COVID-19 infection who were asymptomatic or had symptoms other than pneumonia, the issue of COVID-19 re-infection and immunity is much more important than it appears.

The determination of cut-off points of the visceral adiposity index in
predicting metabolic syndrome, insulin resistance, diabetes mellitus, and
hypertension

  Abstract: Introduction: The visceral adiposity index is a reliable indicator of visceral adipose tissue dysfunction. The aim of this research was to determine the cut-off points of the visceral adiposity index in predicting metabolic syndrome, insulin resistance, type 2 diabetes mellitus, and hypertension at specific age ranges and in both sexes separately.Methods: This research is both descriptive and analytical. The research was conducted with 951 participants aged 18 and over between July 2019 and July 2020. 51 participants that did not meet study criteria were excluded from the research. The research was completed with a total of 900 participants, 577 females and 373 males. A physical examination and anthropometric measurements (height, weight, waist circumference, and blood pressure) of all participants were conducted. After 12 hours of fasting, the HDL, TG, glucose, and insulin levels of participants were measured. The blood pressure of participants was measured after 15 minutes of rest. Adult treatment panel 3 criteria were used for the diagnosis of metabolic syndrome. A homeostatic model assessment was calculated. The visceral adiposity index is calculated in the entire population for the prediction of metabolic syndrome, insulin resistance, diabetes, and hypertension: women, men, age groups 18–30, 31–40, 41–50, 51–65, 66 and over.Results: Cut-off points of the visceral adiposity index in predicting metabolic syndrome were 4.53 in the total population and males, 4.28 in females, 4.76 in the 18-30 age range, 4.96 in the 31-40 age range, 4.87 in the 41-50 age range, 5.04 in the 51-65 age range, and 4.59 in the age range 66 and above. Cut-off points of the visceral adiposity index in predicting insulin resistance were 4.24 in the total population and females, 4.68 in males, 3.45 in the 18-30 age range, 4.15 in the 31-40 age range, 4.66 in the 41-50 age range, and 4.87 in the 51-65 age range. Cut-off points of the visceral adiposity index in predicting type 2 diabetes mellitus were 4.89 in the total population, 6.43 in females, 7.02 in the 31-40 age range, 7.29 in the 41-50 age range, and 5.98 in the 51-65 age range. Cut-off points of the visceral adiposity index in predicting hypertension were 4.26 in the total population, 4.07 in females, 4.64 in the 18–30 age range, and 4.43 in the 51–65 age range.Conclusions: The visceral adiposity index can be used to predict metabolic syndrome, insulin resistance, type 2 diabetes mellitus and hypertension.

An uncommon cause of anterior knee pain in a child: Osteoid Osteoma of the
patella and a review of the literature

  Abstract: Introduction: Osteoid osteoma (OO) is a small, benign bone neoplasm that has a well demarcated nidus surrounded by a reactive zone of sclerosis. It is most common in the long bones and only rarely in the patella.Case Presentation: An eight-year-old girl was admitted to the outpatient clinic with the complaint of anterior knee pain. She had been suffering from pain for approximately one year. The patient had a patellar osteoid osteoma. Surgical curettage of the lesion was performed, and the patient was free of pain the day after the surgery.Conclusions: Although Patellar OO is rare, it should be included in the differential diagnosis of persistent knee pain in children and young adults.Keywords: Osteoid Osteoma, patella, knee pain, curettage, delayed diagnosis, bone neoplasms

Turkey and Europe data comparison in terms of the course of the COVID-19
pandemic and health expenses

  Abstract: Introduction: The COVID-19 pandemic has affected the entire world and has been a challenge to the health prospects of the nations infected. The elderly population of the nations and parameters like the countries' health system and the allocation of budget to health are critical factors for explaining the epidemic's development with different mortality rates in various countries. The purpose of the study was to assess the characteristics of the Turkish population, COVID cases, the progression of diseases, and health expenditures compared to those in Europe.Methods: For this study, we used the following databases: the Worldometer digital database, the database of the Organization for Economic Cooperation and Development, and the Turkish National Databases on Economic and Finance references, such as the Turkish National Strategy and Budget Presidency and the database of the Turkish Statistical Institute. The data was for the date 15.12.2021 and the general annual for 2020.Results: By the investigation date as of 15/12/2021, the total number of COVID-19 cases was 9209740 in Turkey and 81556853 in the European Union countries. The incidence of the disease was 107.500 in Turkey and 135.525 in Europe. While the total change/ increase in last-week-new cases was -5% and 8% in Turkey and Europe, respectively, the real change/ increase in last-week-deaths was -1% to -8% in Turkey and Europe. The number of COVID screening tests per one million population was higher in Europe (1.343.256 to 8.790.27 in Turkey and Europe, respectively). The GDP and health expenditure per capita were distinctly higher in Europe.Conclusion: The fact that there is a higher percentage of elderly residents in European countries than in Turkey, that the number of new cases detected every week is higher, and that the death rate and the gradual decline in cases are linked to the large share of health expenditures distributed by nations for the fight against the disease.

The effect of depression and related psychosocial factors on disease
activity in rheumatoid arthritis

  Abstract: Introduction: Rheumatoid arthritis is one of the most common inflammatory chronic diseases worldwide. Depression is common in patients with rheumatoid arthritis. The relationship between depression and rheumatoid arthritis is a matter of curiosity, and it is thought that there is a bidirectional interaction between them. Depression is an important health problem that requires treatment. Therefore, it is essential to determine the conditions that call for more attention in terms of depression when following rheumatoid arthritis patients in primary care.Methods: Patients who applied to the rheumatology outpatient clinic, followed up for at least 1 year, and were diagnosed with rheumatoid arthritis according to the American College of Rheumatology (ACR) criteria were consecutively invited to the study. A total of 110 people, 18 men and 92 women, accepted the invitation. Beck Depression Inventory (BDI), Health Assessment Questionnaire (HAQ), and visual analogue scale (VAS) were filled together with the questionnaire developed by the researcher. In the joint examination of the patients, the number of tender joints and swollen joints were noted. Disease Activity Score (DAS28) was calculated and noted by measuring C Reactive Protein and erythrocyte sedimentation rates (ESR) in earlier tests performed on the patients. Rheumatoid factor status was recorded at the time of diagnosis or by checking the follow-up records.Results: In rheumatoid arthritis (RA) patients, the BDI score was higher in people with other chronic diseases and those who felt the need to apply to a psychiatry clinic. The BDI score was higher in patients with other chronic diseases and those who felt the need to apply to a psychiatry clinic for rheumatoid arthritis. RA patients with high BDI scores also had high DAS28 and HAQ scores. In addition, disease activity was significantly correlated with RF positivity.Conclusion: Rheumatoid arthritis patients should be evaluated for depression. Depression evaluation should be done more carefully in cases with high disease activity, other chronic diseases, and those who feel the need to apply to a psychiatry clinic.

Evaluation of whole blood, mean platelet volume (MPV) and
neutrophil-lymphocyte ratios (NLR) in people with B12 vitamin deficiency

  Abstract: Introduction: It was aimed to investigate the effects of vitamin B12 deficiency on blood tests, mean platelet volume (MPV), and neutrophil-lymphocyte ratio (NLR), which is one of the inflammation markers.Methods: Our study is a case-control study of 375 patients. 170 patients with low B12 (<130 pg/dL) were accepted as the experimental group, and 205 individuals with normal B12 levels were accepted as the control group. Between these two groups, white blood cells (WBC), platelet count (Plt), hemoglobin (Hgb), mean erythrocyte volume (Mean Corpuscular Volume-MCV), mean platelet volume (Mean Platelet Volume-MPV), neutrophil, lymphocyte counts, and neutrophil -lymphocyte ratios (NLR) were examined.Results: The mean age was 48.45 ±17.497 years in the B12 deficient group and 51.93 ±16.175 years in the control group. The mean of vitamin B12 in the deficient group was 101.41 ± 20.50 pg/mL (min-max 37-130), while it was 257.24 ± 88.13 pg/mL (131–498) in the control group. It was observed that there was no statistically significant difference between WBC, Hgb, Plt, neutrophil, lymphocyte, MVC, MPV and NLR values.Conclusion: In our study, we could not find a significant relationship between B12 deficiency and blood tests. In previous studies, there has been information that MPV values are affected by B12 deficiency. In our study, we found that there was no significant relationship between B12 deficiency and MPV or NLR (p > 0.05). There is a need for new studies on how B12 deficiency will change as the degree and duration of B12 deficiency increases, especially its effect on atherogenic events.
