  • Functional outcome of Austin Moore Hemire Placement Arthroplasty in
           Fracture Neck of Femur in elderly

      Abstract: Objective: To study the outcome of Austin Moore Hemireplacement Arthroplasty in terms of Harris Hip Score and Mobility and Aid Score in fracture neck of femur in elderly.Design: A prospective randomized study carried out between September 2011 to August 2012 AD.Methods: We performed 30 Austin Moore hemiarthroplasties in patients aged 65 and above that sustained a displaced femoral neck fracture and evaluated the outcome in terms of Harris Hip Score and Mobility and Aid Score.Results: Mean HHS at 2 week was 63.93 which increased to 73.83, 80.53 and 86.48 at 6 weeks, 12 weeks and 6 months respectively with overall excellent to good result of 60%. The mean MASM at 6 months was 4.0 whereas the pre-injury MASM was 4.53.Conclusion: Hemiarthroplasty with Austin Moore Prosthesis is safe and effective method of treatment for displaced fracture neck of femur in elderly. There was early post-operative full weight bearing mobilization with reduction in major complications. The operative duration, post-operative complicationsare comparable to other similar studies done elsewhere.   • Stress and Coping Mechanisms among nursing students in Kathmandu

      Abstract: Background: Every person experiences different forms of stress throughout their life. Therefore a student nurse is no exception as they have to adjust to an entirely new environment on joining nursing. Stress in nursing students is an area of growing concern. The stress among the students can have serious effects on their health and studies. It may affect in psychological distress, physical complains, behavior problems and poor academic performance.Objective : The main objective of this study was to assess the level of stress and coping mechanisms among nursing students.Methodology: A descriptive cross-sectional study was done among 283 nursing students of Manmohan Memorial Institiute of Health Sciences (MMIHS), by using purposive sampling technique. The level of stress was assessed using Perceived stress scale-14 (PSS-14) and coping mechanism was assessed using Brief cope inventory. The data analysis was done using SPSS version 20.  Descriptive and inferential statistics was used and Chi-square test was applied for association.Result: The results were found that 61.5% of the respondents have moderate stress and 27.9% of the respondents have severe stress and mild stress only by 10.6%.Likewise 44.5% of the respondents were very often stressed due to test, examination and evaluations where as half of the majority of respondents were some time deal with dying or seriously ill patient.  Similarly 58.3% used Emotion-focused strategies for coping mechanism and 3.9% of the respondents used Problem-focused strategies for coping mechanism.Conclusion: Majority of the nursing students were found to have moderate stress. Perceived stress scale was found to be statistically significant with age, educational status of the respondents, education of father and mother.   • Health Policy in Nepal: An Assessment of Clinical Guidelines in Relation
           to the Burden of Disease

      Abstract: Background: Clinical guidelines provide health workers with valuable information on how to diagnose and treat patients. In many low-income countries, there appears to be a lack of appropriate clinical guidelines for conditions with high rates of morbidity; Nepal is one such nation. It is useful to examine clinical guidelines in relation to disease burden to establish any gaps which might be addressed through changes in policy and practice.Methods: The research adopted a mixed-methods approach, quantifying disease burden both nationally and in five districts served by PHASE Nepal – an NGO working in health and development – before contrasting this with available clinical guidelines in Nepal.  Key informant interviews were subsequently conducted to gain greater insight into disease burden and guideline availability.Results: Of the 10 diseases found to be most burdensome in the PHASE-served districts, 3 lacked any context-specific guidelines. NCDs were found to be prevalent at both district and national level, yet guideline provision for such conditions was minimal. Mental illness was highlighted by interview participants as a growing problem; however, Nepal does not currently have any utilisable clinical guidelines for mental health. Missing data was identified as a significant issue, with lack of mortality data hindering accurate quantification of local and national disease burden.Conclusions: There is evident need for more context-appropriate clinical guidelines in Nepal to effectively diagnose and treat diseases that are most burdensome within communities.   • Depression among Elderly people attending at Senior Citizen Home,
           Bhaktapur

      Abstract: Introduction: Depression is the most common mental disorder in elderly people. The rapidly increasing growth of elderly population in developing countries including Nepal is at risk of increased population with depression.The objective of this study is to assess the prevalence and level of depression among elderly people attending day care center at Sidhi Saligram Senoir Citizen Home.Methodology: A descriptive cross-sectional study design was carried out among the elderly people attending day care center at Siddhi Shaligram Senior Citizens Home, Bhaktapur. Non-probability convenience time frame sampling technique was used to collect the data. The total sample was112 respondents. Face-to-face interview was carried out using Geriatric Depression Scale Long Form. Data analysis was done with SPSS 16 version and simple descriptive statistics and chi-square were applied for data interpretationResults: The study findings show that the prevalence of depression, i.e. 56.2% had depression. Among the depressed respondents, 77.8% had mild depression and 22.2% had severe depression. Depression among elderly people was found to be significantly associated with the sex (p=0.05) and the presence of chronic illness (p=0.000). However association was not seen among the age, educational level, and marital status, type of family, Income and disability.Conclusion: Prevalence of depression was found high among elderly people.   • Post-Traumatic Stress Disorder and associated factors among adolescents
           after 2015 Nepal Earthquake

      Abstract: Background: On April 25, 2015 at 11:56 local time, a great earthquake of massive magnitude-7.8 trembler was experienced in Nepal. Along with the physical and economical loss, such disasters can have devastating consequences in adolescent’s mental health. The purpose of this study is to determine the prevalence of Post-Traumatic Stress Disorder and associated factors among adolescents after 2015 Nepal Earthquake.Methods: A total of 140 adolescents from grade 8, 9 and 10 of two higher secondary schools were recruited in this study. Post-Traumatic Stress Disorder CheckList-5 (PCL-5) of DSM-V criteria was used for the diagnosis of Post-Traumatic Stress Disorder. Chi-square test was used to assess the association.Results: Among 140 adolescents, the age range was 12 to 19 years with a mean age of 14.8 (SD=1.1) where 27.1% of the participants had Post-Traumatic Stress Disorder.  A statistically significant association was found between Post-Traumatic Stress Disorder and grade (χ² = 9.324, p<0.01), earthquake exposure (χ² =32.251, p<0.001) and death of someone close in earthquake (χ² = 9.535, p<0.01). Majority of the respondents reported symptoms of avoiding memories, thoughts, or feelings related to the stressful experience (58.57%).Conclusions: This study hopes to assist the therapist, psychiatrists and planners in the design and implementation of effective and appropriate intervention programs that can help reduce the impact of Post-Traumatic Stress Disorder on adolescents, aid in psychological reconstruction and decrease the long term negative mental health consequences of such disasters especially for those who are at lower grade, were exposed to earthquake and lost their close people in the earthquake.Journal of Manmohan Memorial Institute of Health SciencesVol. 3, No. 1, 2017, page: 45-55

  • Knowledge on occupational health hazard and safety practices among the
           municipal solid waste handler

      Abstract: Background: All over the world nearly 860,000 people are injured every day and 2.3 million deaths occur annually due to occupational accidents and work-related diseases.There are limited studies that specifically determine the knowledge of waste handlers on occupational health problems and associated factors of occupational health problems and injuries in waste handlersin context of Nepal. Therefore, this studyaimed to assess the knowledge of waste handlers on occupational health and safety practices and its association with utilization of PPEs along with the condition of occupational health problems and injuries and factors associated with them among the waste handlers of Kathmandu Metropolitan City.Methods: A descriptive cross-sectional survey was conducted among selected 355 solid waste collectors by using semi-structured questionnaire and observation checklist, which was completed from March 2016 to May 2016. The data was entered into SPSS 20 version for analysis and a chi-square test was performed to see the presence of the association between the dependent and selected independent variables.Results: Among total respondents (355) surveyed 51.5% had got knowledge on occupational health hazard, 16.1 %  had got the knowledge to prevent from work-related health risks, 29% had got knowledge about PPEs and  31.5% had got knowledge about PPEs utilization. About 45.8% of the study participants had practices of the utilization of PPEs. Majority of the respondents i.e. 63.7% reported that they were suffering from occupational health problems. The evidence of the occupational health injuries was found to be low i.e. 25.4%. Similarly, variables which had significant influence on the occupational health problems were knowledge on occupational health hazard (p=0.016), work experience (p=0.021), health checkup (p=0.042) and for the occupational health injuries were monthly income(p=0.036), knowledge to prevent from health risk (p=0.001), gender (p=0.02), and knowledge of occupational health hazard (p=0.011), work experience (p= 0.025), work type (p=0.001), knowledge on PPEs ( p=0.034) and knowledge on work-related health risks (p= 0.027).Conclusion: Based on this study, its recommended to improve occupational health and safety practices.   • History of Pharmaceutical Development in Nepal

      Abstract: The historical event of the development of pharmacy was started during ancient Lichchhavi ruler Amshu Berma date back to 605-620 AD when a Ayurvedic hospital was established. In 1641-1674 AD, King Pratap Malla started ayurvedic medicine production unit in the royal place. Modern allopathic medicines were introduced in Nepal in 1816 AD after Suguali Treaty and establishment of British residency in Nepal. Allopathic medicine manufacturing was started in 1969 in private sector and a government undertaking Royal Drug Limited was established in 1972.  Department of Drug Administration (DDA), a drug regulating Agency of the country was established according to the Drug Act in 1979. The pharmaceutical education was started in Nepal with the commencement of the Proficiency Certificate Level, a two and half year program (Intermediate in Pharmacy that is similar to Diploma of Pharmacy) at the Institute of Medicine, Tribhuban University in 1972.   • Developing an international higher education partnerships between high and
           low-income countries: two case studies
