  • Autoimmune thyroiditis associated with primary Sjögren’s syndrome
           complicated with distal renal tubular acidosis and hypokalaemic paralysis:
           A case report

    • Abstract: Primary Sjögren’s syndrome (pSS) is known to present with distal renal tubular acidosis (dRTA), which can manifest as hypokalaemic paralysis. The main involvement of the kidneys in pSS is tubulointerstitial nephritis. Furthermore, pSS is an autoimmune rheumatological condition that rarely co-exists with autoimmune thyroiditis. We report a 42-year-old hypothyroid female, who complained of a one-day history of severe proximal muscle weakness in all four limbs and a two-month history of left ankle joint pain and swelling. Her serum potassium level was 1.7 mEq/L, with biochemical evidence of normal anion gap metabolic acidosis with positive urinary anion gap suggesting dRTA. As she complained of gritty eyes and recurrent stomatitis a diagnosis of Sjögren’s syndrome was sought after. Her serology revealed a high rheumatoid factor titre of 378 (<20) IU/mL with negative anti-cyclic citrullinated peptide levels (<0.5 IU/mL). Positive antinuclear antibodies (1:640), anti-Sjögren’s syndrome-related antigen A (SS-A) antibodies and anti-Ro-52 antibodies in the absence of other extractable nuclear antigen antibodies suggested pSS. This was confirmed with labial biopsy which revealed focal lymphocytic infiltration. Magnetic resonance imaging revealed the “salt and pepper” appearance of bilateral parotid glands with glandular atrophy. Even though she was clinically and biochemically euthyroid, given the background of thyroid dysfunction, we investigated for autoimmune thyroiditis (AIT) which was confirmed with imaging and a high anti-thyroid peroxidase antibody titre 532.71 IU/mL (<5.61). Her functional status markedly improved following the correction of serum potassium values. She was started on long term hydroxycholoroquine and oral potassium citrate.  This is a rare case of AIT associated with pSS complicated with dRTA presenting with hypokalaemic paralysis. This case illustrates the importance of screening for pSS among patients with dRTA and the rare coexistence of pSS and AIT.   Published on 2021-12-30 00:00:00
       
  • Vitiligo as a possible manifestation of infective endocarditis complicated
           with ischemic stroke: A case report

    • Abstract: We report a case of young stroke sequel to sub-acute infective endocarditis with simultaneous occurrence of vitiligo. Since the early days of medical practice, identifying peripheral stigmata of infective endocarditis was highlighted as a way of early diagnosis of infective endocarditis. The literature describes several cutaneous manifestations of infective endocarditis. This is possibly the first that reports an association between vitiligo with infective endocarditis and stroke. The patient was a thirty-four-year-old previously well female who had an intermittent fever for one month with a gradual development of vitiligo involving both hands, feet, and lips. She presented with the development of a left-sided ischaemic stroke one month after the onset of fever. She was diagnosed with having mitral valve infective endocarditis and underwent a six-week course of intravenous antibiotics until regression of the mitral valve vegetation. Vitiligo may occur possibly as an immunological phenomenon of infective endocarditis.  Additionally, it may correlate with the occurrence of ischaemic stroke in patients with infective endocarditis.   Published on 2021-12-30 00:00:00
       
  • Socio-demographic factors, treatment-seeking behaviours and common
           clinical presentations of leprosy patients in Anuradhapura, Sri Lanka

    • Abstract: Leprosy is eliminated from most of the countries while it remains a problem in Sri Lanka. This study was aimed to describe the epidemiology, associated factors, clinical presentation, and treatment-seeking behaviours of leprosy patients. A descriptive cross-sectional study was carried out among leprosy patients diagnosed between 13th February 2019 to 12th February 2020 at the Dermatology Clinic of the Teaching Hospital Anuradhapura. Each patient was interviewed using questionnaires prepared in the Sinhala language. Questionnaires were used to obtain data on the following variables; epidemiology, associated factors, treatment-seeking behaviour, and clinical aspects of the leprosy patients. The study included 66 leprosy patients. Most of the patients (56%) were males, and most (50%) were in the age range of 30-50 years. Only a minority of the cases were childhood leprosy cases (10.6%). Most of the patients were housewives (26%), and 19% were farmers. More than one-third of patients (41%) had income less than 10,000 LKR per month. More than two-thirds of the patients (68%) had prior knowledge about leprosy, and most of them knew its aetiology before being diagnosed as leprosy patients. Approximately 27% of patients had previous contact with a leprosy patient. More than 50% of these patients were not timely referred for treatment due to delay in seeking medical attention, and they were referred one year after acquiring the disease, and 11(16%) patients warranted repeat treatment. Most patients (74%) had paucibacillary leprosy, and most (47%) presented with hypopigmented skin lesions. Most of the Leprosy patients in Anuradhapura share known preventable socio-demographic risk factors. Therefore, authorities should pay special attention to the prevention of leprosy in specified communities in Sri Lanka. Published on 2022-03-23 00:00:00
       
  • Statin-induced autoimmune necrotizing myopathy: A case report

    • Abstract: Statin-induced autoimmune necrotizing myopathy is an extremely rare side effect associated with the treatment of statins. It is characterized by proximal muscle weakness, necrotizing myopathy and the presence of antibodies to HMG-CoA reductase. We report a case of a young female who was treated with atorvastatin and presented with progressive proximal muscle weakness with a very high creatine phosphokinase level, even after discontinuing atorvastatin. Muscle biopsy showed muscle necrosis with myophagocytosis. She showed a good response to treatment with oral prednisolone. Published on 2022-03-23 00:00:00
       
  • Abnormalities in MRI brain in Sri Lankan children with epilepsy: A
           descriptive study

    • Abstract: Epilepsy is an important medical condition among children, with an estimated prevalence of around 6/1000 among children in Asian countries. Neuroimaging has a very important role in the diagnostic workup and treatment planning of epilepsy. Magnetic resonance imaging remains the most useful structural imaging modality for epilepsy. The purpose of this study was to identify the distribution of MRI brain abnormalities among children with epilepsy in Sri Lanka and to correlate with their clinical findings. Data were collected from the patients with epilepsy in the age group of 1 to 15 years, who were referred for MRI brain. Patient’s demographic and clinical data were collected by an interviewer-based data collection sheet. All the MRI studies were performed on a 1.5 tesla MRI machine. Findings of MRI were categorized according to ILAE guidelines for imaging infants and children with recent-onset epilepsy. Out of 175 patients, 88 (50.3%) had normal MRI results and 87 (49.7%) had at least one MRI abnormality. Among the patients with abnormal results, nonspecific findings were seen in 46 patients (52.9%), specific abnormalities were seen in 36 patients (41.4%) and non-related findings were seen in 5 patients (2.6%). There was no significant relationship between the seizure type and the proportion of abnormalities. The proportion of MRI brain abnormalities among the children with epilepsy in our population is higher than reported in other countries. There is a poor correlation of MRI brain abnormalities with the type of seizure.    Published on 2022-03-23 00:00:00
       
  • Successful thrombolysis of a wake-up stroke aided by Computed Tomography
           Perfusion (CTP) imaging: A case report

    • Abstract: An 80-year-old man was admitted with aphasia since wakeup. His Computed Tomography Perfusion (CTP) study of the brain revealed a significant but patchy penumbra with a minimal core. His Magnetic Resonance Imaging (MRI) also confirmed a mismatch between Diffusion-weighted MRI (DWI) and fluid-attenuated inversion recovery (FLAIR) sequences (DWI-FLAIR mismatch) supporting the existence of a significant ischaemic penumbra. He underwent thrombolysis with tenecteplase (TNK) after 5 hours and 40 minutes since waken up and had a good recovery with the improvement of the National Institute of Health Stroke Scale (NIHSS) from 5 to 0. Acute stroke patients could benefit from thrombolysis beyond the window period of 4.5 hours if there is a significant penumbra in the CT perfusion imaging and tenecteplase may have an added advantage over alteplase(r-tPA) in such circumstances.   • Myocarditis and severe neuromuscular paralysis following a suspected
           Common krait (Bungarus caeruleus) envenoming in a child: A case report

    • Abstract: Common krait (Bungarus caeruleus) bites cause significant morbidity and mortality in South Asia. Neurotoxicity is the most important clinical entity associated with common krait envenoming and cardiac effects are only rarely observed. We report a case of a two-year-and ten months old Sri Lankan child who developed myocarditis and severe neuromuscular paralysis following a suspected common krait bite. ECG, echocardiographic changes, and elevation of cardiac troponins as well as creatinine kinase were highly suggestive of myocarditis. With polyvalent anti-venom serum and heart failure standard treatment, the child gradually improved. Complete recovery of cardiac functions was seen within 10 days from the bite. This unique feature of common krait envenoming may be related to the differences in the pharmacokinetics of snake venom in children compared to adults.     • Abstract: Enhanced myometrial vascularity in colour Doppler during ultrasound scan is not an uncommon finding following first-trimester miscarriage. When it is persistent and associated with abnormal uterine bleeding or persistently elevated serum beta hCG, suspicion of a significant pathology such as gestational trophoblastic neoplasm and acquired uterine arterio-venous malformation should be raised. Encysted retained products of conception in myometrium is a rare complication following a miscarriage, which has a similar clinical presentation to enhanced myometrial vascularity, but often the diagnosis is difficult.   • Being late may not be too late! Widening the horizons for stroke
           thrombolysis with Computed Tomographic Perfusion (CTP) and Magnetic
           Resonance (MR) brain imaging

    • Abstract: Stroke is the leading cause of adult disability worldwide. Most strokes are ischemic in nature and amenable to cure if detected and treated early enough with reperfusion therapy. Delay in presentation and unknown onset time are key limiting factors of reperfusion therapy in current practice. Cerebral perfusion computed tomographic imaging (CTP) and magnetic resonance imaging (MRI) with special sequences aid in identifying acute stroke patients who present late or with unknown onset time, yet reperfusion treatment may have a benefit. Establishing such imaging facilities at tertiary care hospitals in Sri Lanka would strengthen acute stroke care by offering hope for a cure. It is cost-effective in long term by reducing the burden of prolonged disabilities in stroke patients. Published on 2022-03-15 00:00:00
