  • Open versus Laparoscopic Inguinal Hernioplasty - Outcome Correlation

      Abstract: Background: Repair of inguinal hernias in men is a common surgical procedure, but the most effective surgical technique is still in debate.Methods: We randomly assigned men with inguinal hernias at Mitford Hospital surgery, ward to either open mesh or laparoscopic mesh repair. The primary aim was to detect recurrence of hernias in both groups at 6 month. Secondary aims were to detect complications and patient compliance.Results: of the 70 patients who were randomly assigned to one of the two procedures, 62 underwent operation; 6 month follow-up was completed in 55 (78.6%). Recurrences were only one in the laparoscopic group (3.6%) and 1 in the open group (3.7%). The rate of complications was lower in the laparoscopic-surgery group than in the open-surgery group (17.6% vs. 27%). The laparoscopic- surgery group had less pain initially than the open-surgery group on the day of surgery (difference in mean score on a visual-analogue scale, 10.2 mm; 95 percent confidence interval, 4.8 to 15.6) and at two weeks (6.1 mm; 95 percent confidence interval, 1.7 to 10.5) and returned to normal activities earlier (adjusted hazard ratio for a shorter time to return to normal activities, 1.2; 95% confidence interval, 1.1 to 1.3). Hospital stay was shorter in laparoscopic group (2.6 days vs 3.2 days). Patients’ satisfaction with surgery was 95% in the laparoscopic group and 87% in open group. Nenety six laparoscopic and 87% of open surgery patients perceived that they were healthy after surgery. Total treatment cost was more in laparoscopic group.Conclusions: The laparoscopic technique is superior to the open technique for mesh repair of primary hernias.J Shaheed Suhrawardy Med Coll, June 2016, Vol.8(1); 3-7

  • Clinical Profile of Brain Tumour in Children

      Abstract: Background: This study was designed to evaluate the clinical profile and location of brain tumours in children so that brain tumour cases could be diagnosed and managed earlier.Materials and Methods: This cross sectional study was carried out from April 2011 to April 2012. A total 50 clinically suspected cases of brain tumour and supported by MRI were enrolled in this study.Results: Brain tumour of children was predominant in male where male to female ratio was almost 1.9:1. Supratentorial and Infratentorial brain tumour were found in 36% and 64% cases respectively in children. According to MRI findings in the supratentorial location, craniopharyngioma were commonest (50%) followed by cerebral astrocytoma (22.2%). In the infratentorial region, medulloblastoma was observed in (40.6%), followed by cerebellar astrocytoma (31.3%). Vertigo, weakness of limbs were found in 46.9% and 87.5% cases respectively in infratentorial group which were significantly (p<0.05) higher than supratentorial group. Growth failure and behavioral change were found in 16.7% and 22.2% cases respectively in supratentorial which were significantly (p<0.05) higher than infratentorial group. Papilloedema, reduced muscle power, increased muscle tone, exaggerated deep reflexes, clonus, gait disturbance and cerebellar signs were significantly (p<0.05) higher in infratentorial group.Conclusion: Craniopharyngioma and medulloblastoma were more common findings in MRI evaluation in the supratentorial and infratentorial location respectively. Growth failure and behavioral change were found higher in supratentorial than infratentorial group.   • Association between Helicobacter Pylori and Hyperemesis Gravidarum

      Abstract: Background: Hyperemesis gravidarum (HG) is the severe form of the nausea & vomiting in pregnancy and is associated with weight loss, ketonemia ,ketonuria ,electrolyte imbalances and profound volume depletion. Increased accumulation of fluid caused by elevated steroid hormones in pregnancy, a shift in pH and change of pH in the gastrointestinal tract could result in the manifestation of subclinical Helicobacter pylori (H. Pylori) infection. We therefore examined whether seropositivity for IgG antibodies to H. Pylori may be related to hyperemesis graviderum.Objective: To determine the association of H-pylori infection in hyperemesis graviderum patients.Methodology: A cross sectional study was conducted in the department of obstetrics and gynaecology, Dhaka medical college hospital from January 2007 to December 2008. Total 82 pregnant patients, 40 were hyperemesis gravidarum patients (Cases, group A) and 42 were normal pregnancy without hyperemesis graviderum (Control, group B) for admission and attending for routine ANC in OPD were selected. Purposive sampling method was followed as per inclusion and exclusion criteria. Evaluation of the patients was based on history, physical examination and investigation. Serum IgG antibody response to H.pylori antigen by ELISA were measured from every patients and analysed for its association with HG.Results: Most of the patients of this study were primi gravida, Group-A was 72.5% and Group-B was 66.7%. Maximum number of women in both the groups belonged to lower socio-economic status (Group-A was 85% and Group-B was (76%). Previous H/O hyperemesis gravidarum in the Group-A was 52.5% and Group-B was 07 % (p<.01), that is statistically highly significant. Antibody to Helicobactor pylori in serum was positive in Group-A 47.5%, and Group-B 19%. The difference was statistically significant. (p value-0.006).Conclusion: According to the findings of our study, H.pylori infection would seem to be an important associated factor in the exacerbation of HG, but it does not seem to be the only cause of the disease.J Shaheed Suhrawardy Med Coll, June 2016, Vol.8(1); 13-16

  • Clinico-Epidemiological Study of Self-Poisoning by Different
           Pharmaceutical Agents

      Abstract: Background: In Bangladesh self-poisoning cases are mostly of organophosphorous compound (OPC). It is documented that 14% of all deaths amongst 10 to 50 year old women in Bangladesh were due to poisoning, the majority following suicidal ingestion of pesticides. In tertiary level hospital, a large number of self-poisoning cases are of different pharmaceutical agents, mostly from urban areas and mostly of teenager group. So the trend of self-poisoning is different in urban area than that of rural area. These pharmaceutical agent self-poisoning cases admitted into the hospitals have different modalities of clinical features with variable outcomes and socioeconomic background. But there is no actual data of these self-poisoning by the different pharmaceutical agents. Therefore, this study was aimed to describe the pattern of self-poisoning by different pharmaceutical agents.Methods: A descriptive longitudinal study was conducted in five adult medicine units of a tertiary level hospital from January 2008 to June 2008. All self-poisoned patients with different pharmaceutical agents were included with their prior informed written consent. A pre-tested questionnaire was used for data collection.Results: A total of 281 patients were interviewed. Most of the patients were female (70.8%) with highest age range from 13 to 30 years (91.81%). Most of the patients were students (53%) and 40.9% were secondary school certificate pass. 60.1% patients were unmarried. Most of the patients were from urban area (84%). The commonest background for self-poisoning was family disharmony (74.46%). Most common offending pharmaceutical agents were benzodiazepines (44.10%), collected mostly from local dispensary without prescription (89.33%). Most of the patients consulted within 1 to 2 hours of poisoning (44.5%). Commonest consultation before admission was in a Government hospital (48.40%). Only 3 patients (1.10%) were known to have psychiatric illness. Five patients (1.80%) had previous history of self-poisoning. Most of the patients (89.3%) stayed in the hospital for 1 day with no mortality.Conclusion: Self poisoning by pharmaceutical agents is common especially in urban areas. Common offending pharmaceutical agents were benzodiazepines. The commonest background for self-poisoning was family disharmony.J Shaheed Suhrawardy Med Coll, June 2016, Vol.8(1); 17-22

  • Neonatal outcome of suspected fetal distress cases

      Abstract: Background: Perinatal mortality is closely related to the early and accurate recognition of foetal distress. Objective of the study was to find out neonatal outcome in suspected fetal distress cases.Methods: This was a cross sectional study done in ShSMCH. Purposively 38 samples were taken in three weeks time and clinical state related to fetal distress and neonatal outcome of them were observed.Results: Fetal movement were less in 36.8% cases. Abnormal fetal heart rate were present in 89.4% cases and in 47.4% cases amniotic fluid was meconium stained. First minute Apgar score was low in 36.8% cases and 47.4% were admitted in paediatric ward. The clinical profile for diagnosis of fetal distress was not significantly associated with low apgar scores.Conclusion: There was very little appreciation for assumption of fetal distress clinically.J Shaheed Suhrawardy Med Coll, June 2016, Vol.8(1); 23-25

      Abstract: Background: Portal hypertension is a clinical syndrome defined by a pathologic increase of portal venous pressure. The objectives of this study were to evaluate etiological and clinical presentation of portal hypertension admitted in a tertiary care centre of Bangladesh.Materials and Methods: This cross sectional study was done at the Department of Paediatric Gastroenterology & Nutrition, BSMMU on 100 consecutive cases admitted during the period from July 2013 through June 2015. Confirmation of the presence of portal hypertension was done by demonstration of oesophageal varices during upper GI endoscopic examination. The diagnosis of chronic liver disease was based on a combination of clinical, biochemical (abnormal liver function tests) and ultrasonographic (coarse echotexture of liver) features. Extrahepatic portal hypertension was diagnosed on the basis of clinical, normal liver function test and ultrasonographic evidence of portal or splenic vein thrombosis, with or without cavernous transformation. Doppler ultrasound showed increase portal venous pressure.Results: Patient's age group was 2 to 15 years. 69 cases were extra hepatic portal hypertension. Haematemesis and/or melaena were found in 42(60.9%) and splenomegaly was found in 68(98.6%) cases. Of extra hepatic 69 cases 10% had history of umbilical sepsis. Of the 31cases of CLD with portal hypertension, haematemesis and/or melaena was found in 20(64.5%) cases, splenomegaly in 30(96.8%), ascites 8(25.8%).Most (50%) of CLD cases were cryptogenic followed by Wilson’s disease 29%. Of the 100 cases, endoscopy revealed grade 3 esophageal varices in 45% cases. All the patients were treated with propranolol. EVL was done in 70% cases.Conclusion: In this study, most of the cases were extra hepatic portal hypertension. Gastrointestinal bleeding & splenomegaly were found in most of the cases. No risk factor was found in most of extra hepatic cases. Portal vein thrombosis & cryptogenic were the most common cause in extra-hepatic and intra-hepatic cases respectively.J Shaheed Suhrawardy Med Coll, June 2016, Vol.8(1); 26-29

      Abstract: Gastro Esophageal Reflux Disorders(GERD) are group of conditions where gastric contents are refluxed into the esophagus and produce troublesome symptoms mainly heartburn and regurgitation. It may present with complications as well. Atypical presentation is also not very uncommon. Proper diagnosis and differentiation from other diseases can ensure appropriate treatment and thus better quality of life. Unlike many other diseases, diagnosis of GERD is relatively straight forward through symptomatology and a few investigations, even though few cases require special and technologically newer modalities of investigative tools for confirmation. Option for treatment are many and mostly medical; surgical and other methods are very rarely needed to pose better life.   • Myelolipoma - A Rare Benign Adrenal Tumour: A case report

      Abstract: Myelolipoma is a rare benign hormonally inactive adrenal neoplasm containing mature adipose tissue and a variable amount of haematopoietic elements.Their mostly small size,unilateral site and clinical quiescence are accountable for predominantly incidental disclosure. A symptomatic case of myelolipoma was presented in our hospital with generalized body swelling ,body rash and intermittent dull abdominal pain .He had H/O taking steroid for last 10 yrs to increase body weight and for better felling . All laboratory investigations were found within normal limit and he was referred to our department for radiological investigations. Plain X-ray, USG and CT scan of abdomen was done which revealed an well capsulated circumscribed highly echogenic , fat density soft tissue mass in right lumbar region which was suspected as suprarenal mass possibly myelolipoma and it was confirmed by FNAC as myelolipoma of right adrenal gland.J Shaheed Suhrawardy Med Coll, June 2016, Vol.8(1); 34-36
