      Abstract: Abstract When you enter in conversation with someone else, waiting for the answer takes a minimal amount of time that if exceeded creates some kind of embarrassment and hindrance. If a third subject is present, she/he will usually intervene either to remove the hindrance, or to solve the embarrassment. But when one subject exceeds systematically the minimal time for answering, she/he is bound to be diagnosed as autistic. Of course, there are other criteria, is not so simple, but if you read novels like Stephen Hero, by Joyce, you can get an idea about what Autism can be conceived from the point of view of literature. In the following work we will read back the main steps that made the autistic guys psychopathologic, with no hope, and the other gaze on Autism, which was always shaped by a kind of admiration in front of a “mysterious” form of existence.

  • How to use psychiatry well: religious communities, medicalization, and the
           person

      Abstract: Abstract How do members of religious communities determine when medicalization—the use of medical models to account for forms of experience and behavior previously understood using religious and spiritual categories—is appropriate? Psychiatry, as a discipline, lacks any consensual, conceptually valid method by which to demarcate the boundaries of its professional and clinical domain: although psychiatry understands itself as the medical discipline dedicated to the prevention, diagnosis, and treatment of “mental disorders”, psychiatry’s most widely accepted definition of “mental disorder” is both practically impotent and conceptually flawed. Psychiatry, therefore, lacks internal conceptual resources adequate to resist inappropriate medicalization. Considered as a social process, however, medicalization is rarely unidirectional: it frequently involves collaboration between clinicians and the individuals and communities affected by a new medical diagnosis or category. This has historically been true of the medicalization of forms of life previously regarded as religious or spiritual matters, as is made clear by the examples of the medicalization of problem drinking, the medicalization of emotional disturbances, and the medicalization of post-combat suffering. Religious communities, therefore, must discern for themselves when psychiatric medicalization is appropriate and when it is not. Four guiding questions can aid religious communities in developing a prudential approach to using psychiatry in the service of the person.

  • Changing my mind: one professor’s story of rethinking psychotropic
           medication

      Abstract: Abstract Over the past few years, I have reconsidered the evidence base for psychotropic medications. Although not widely publicized or recognized, persuasive evidence suggests that psychotropic medications are less effective and more harmful than most believe. This is an emotionally charged topic that threatens the professional identity and livelihood of many individuals, leading to cognitive dissonance and cognitive biases of which we may be unaware. Personally, I experienced distress and cognitive dissonance in response to this new information, and I have begun to change my research goals and other professional activities as a result. I encourage others to: (a) examine divergent bodies of research literature and your reactions to this literature; and (b) be empathic when discussing and writing about these topics. If we can engage in open-minded, critical self-assessment and discourse, then we may ultimately improve how we care for each other during times of distress and suffering.

  • Socioeconomic status and health conditions associated with incidence of
           dental caries in Brazilian children

      Abstract: Abstract The purpose of this study was to assess the prevalence and severity of caries in primary and permanent dentition and to investigate the association between disease distribution and socio-demographic conditions, oral hygiene, and dietary habits of a population of schoolchildren in three districts of Belo Horizonte, Brazil, in 2012. This study involved 753 schoolchildren, aged 3–15 years, of two socio-economic levels: a low level, in a public school and kindergarten; and a high level, in a private school. The mean decayed, missing and filled deciduous infant teeth (dmft) values in children age 6 years and decayed, missing, and filled permanent adult teeth (DMFT) at 12 years were similar between children in the public school (dmft, 1.53; DMFT, 0.56) and the private school (dmft, 1.53; DMFT, 0.28). A low DMFT is associated with the habit of cleaning teeth after eating sweet foods, when the fathers were employed at the time of the child’s birth and if children started to brush their teeth earlier than 3 years of age. The data show that oral health monitoring of schoolchildren is a significant factor that can reduce dental caries, even in children from poor families. It is necessary to educate children to brush teeth before the 3 years of age, and to create prevention programs for children whose fathers are unemployed.

Internal treatment guidelines were developed, available on the computer network of the department, containing a schematic summary 1–2 pages long of each protocol. Furthermore, a home-made database was designed to be used both as a medical record and for prospective data recording. Data entry was subjected to an “independent check” by a second radiation oncologist. Regular meetings were organized to update all staff on the aim and conduct of the various running trials and to promote the enrollment of patients and the involvement of other specialists in clinical research. In a period of about 11 years: (1) 34 protocols for clinical trials were written and approved; (2) 7463 patients had undergone RT; of these patients, 6321 patients (85 %) were enrolled in clinical trials; (3) 345 papers/book chapters/abstracts were published; of them, 127 papers have been published in journals currently featured on PUBMED; (4) 119 scientific meetings were held. This experience shows that an independent clinical research is possible even in a non-academic and peripheral RT department. In particular, our experience suggests that personnel and resource constraints can be at least partially overcome by a strong personal and group motivation and with the adoption of flexible solutions.

      Abstract: Abstract To investigate a scalable version of a global nutrition education model by a novel assessment of the impact of hands-on cooking and gardening classes, Edible Schoolyard (ESY), on the eating patterns of elementary and middle school students. Lunches (n = 1,750) by students (n = 479) including 8,750 food components in an urban charter school in New Orleans, Louisiana, were analyzed over one week using a validated digital photography protocol. Univariate analyses and multivariate logistic regression according to school class were performed. Compared to less ESY exposed grades, eighth graders consumed less saturated fat (2.4 vs. 2.8 g, p < 0.0274), less total fat (11.9 vs. 13.5 g, p = 0.0324), and more fruits (100 vs. 68 %, p = 0.0047). After controlling for caloric intake, this class was nearly twice as likely to consume less than 1,000.0 mg sodium per meal compared to other grades (OR = 1.94, 95 % CI 1.0137–3.7140, p = 0.045), however, eighth graders failed to meet the 650.0 mg sodium target of the Healthy, Hunger-Free Kids Act of 2010. We report lower sodium and fat but higher fruit consumption in students with greater exposure to hands-on nutrition education. This is the first known application of an accurate digital photography protocol to demonstrate dietary improvements of elementary and middle school students with increased exposure to a hands-on cooking, gardening, and nutrition education curriculum. This study suggests that such a curriculum is thus scalable through schools for improving child health.

      Abstract: Abstract Over 90 % of Americans consume an excessive amount of sodium as a key salt ingredient, despite its contribution to morbidity and mortality. No known studies have analyzed the optimal salt reduction level for medical students and staff in characteristic restaurant recipes. Increased studies linking such quality improvement in medical education and care through lifestyle-based modifications with medical professionals in training may provide a promising model for competency-based medical education in the age of healthcare reform. A volunteer sample (n = 105) of medical students and staff was recruited over 3 weeks to test three-course meal components from representative restaurants prepared under a trained chef’s supervision. Subjects were not informed that sodium levels were decreased in three samples by 15, 25, and 35 % with the fourth sample being the control of 0 % reduction. The largest sub-group of subjects identified the 15 % reduced sample as the optimal saltiness sample compared to the 0, 25, and 35 % reduced samples overall among the shrimp, jambalaya, and gumbo dishes (29.23 % vs. 23.59 %, 22.56 %, 24.62 %, p = 0.567). The Kruskal–Wallis test with post hoc analysis with the Wilcoxon-Mann–Whitney test revealed that sodium-reduced samples had a significantly higher rank sum than the zero-reduced sample for favorite dish (p < 0.001). Our results suggest chefs may reduce sodium while still preserving consumer preference. Future studies are needed to investigate the impact of first-hand nutrition study participation on medical students and staff improving their own nutrition education for their future patients.
