  • Visualization of the renal artery in kidney transplant patients using
           time-resolved computed tomography angiography

      BackgroundTransplant renal artery stenosis (TRAS) is a post-operative complication which most often occurs between 3 months and 2 years after transplantation. TRAS is associated with kidney failure and hypertension and, thereby, with an increased risk of cardiovascular events.PurposeThe purpose of this retrospective study was to report our experience of perfusion computed tomography angiography (P-CTA) to identify a 50% lumen reduction (as compared to digital subtraction angiography, DSA), assess its subjective image quality and evaluate if contrast-induced acute kidney injury (CI-AKI) occurred.Material and MethodsAll 13 patients who had undergone P-CTA for suspected TRAS at our institution were retrospectively evaluated. At P-CTA, eight or 12 g of iodine were administered intravenously, and five to seven scan sequences were merged into time-resolved images after motion correction. Eight patients underwent subsequent DSA.ResultsThe average patient weight was 76 kg (range 55–97 kg). Image quality was rated as good or excellent for all patients, and pathological changes were shown in 10 of 13 patients undergoing P-CTA. Two patients had a serum creatinine increase of >26 μmol/L during the first 3 days, but serum creatinine was significantly lower in all patients 1 month after P-CTA (165+/−69 μmol/L versus 232+/−66 μmol/L, P < .01). The diagnosis at P-CTA was verified in all eight patients who underwent DSA. However, in two cases with suspected stenosis, renal function was restored without angioplasty.ConclusionAnatomy and blood flow of the transplant renal artery can be visualized using less than a third of the standard contrast media dose by using P-CTA technique.

  • Diffusion-weighted MR imaging in chronic non-bacterial osteitis:
           Proof-of-concept of the apparent diffusion coefficient as an outcome
           measure

      BackgroundThe apparent diffusion coefficient (ADC), as determined by whole-body diffusion-weighted MRI, may be useful as an outcome measure for monitoring response to treatment in chronic non-bacterial osteitis.PurposeTo test and demonstrate the feasibility of ADC-measurement methods for use as outcome measure in chronic non-bacterial osteitis.Materials and MethodsUsing data from a randomized pilot study, feasibility of change-score ADC between baseline and second MRI (ΔADC12) and third MRI (ΔADC13) as outcome measure was assessed in three settings: “whole-lesion,” “single-slice per lesion,” and “index-lesion per patient”. Bone marrow edema lesions were depicted on short tau inversion recovery sequence at baseline and copied to ADC maps at the three time-points. Correlations between the three settings were measured as were analysis of variances. Discriminant validity was assessed as inter- and intra-observer reproducibility and smallest detectable change.Results12 subjects were enrolled, and MRI was performed at baseline and weeks 12 and 36. Pearson correlation was high (r > 0.86; p ≤ 0.01) for ΔADC between single-slice—whole-lesion and whole-lesion—index-lesion and tended to be significant for single-slice—index-lesion settings (p = 0.06). For ΔADC12 and ΔADC13, Bland–Altman plots showed small differences (0.02, 0.03) and narrow 95% limits-of-agreement (−0.13–0.09, −0.07–0.05 μm2/s) between whole-lesion and single-slice ROI settings. Inter-observer reproducibility measured by intra-class correlation coefficient was poor-to-fair (range: 0.09–0.31), whereas intra-observer reproducibility was good-to-excellent (range: 0.67–0.90). Smallest detectable changes were between 0.21–0.28 μm2/s.ConclusionADC change-score as outcome measure was feasible, and the single-slice per lesion ROI setting performed almost equally to whole-lesion setting resulting in reduced assessment time.

  • Multisystem IgG4-related disease involving the abdomen and coronary
           arteries and causing chronic abdominal pain

      IgG4-related disease was originally discovered in patients with autoimmune pancreatitis accompanied by elevated serum IgG4 levels and has subsequently been described in almost every organ system. IgG4-related disease presents with a variety of symptoms according to the organ affected and may be accompanied by serious complications such as organ dysfunction associated with IgG4-positive cell proliferation. We report a case of IgG4-related abdominal disease in a patient who also had involvement of the coronary artery.

  • A case report: An increased oxygen extraction fraction in remote ischemic
           lesions after revascularization for moyamoya disease with a progression of
           posterior cerebral artery stenosis

      The cerebral metabolism, such as the oxygen extraction fraction (OEF), in remote ischemic lesions following revascularization for moyamoya disease (MMD) has not yet been fully elucidated. We herein report a patient with an increased OEF in a remote ischemic lesion after revascularization in a case of adult-onset MMD. A 21-year-old woman suffered from a left parietal lobe infarction due to MMD. At 2 months after onset, left superficial temporal artery (STA)–middle cerebral artery (MCA) bypass and encephalo-myo-synangiosis (EMS) were performed. The postoperative course was uneventful. 15O-positron emission tomography (PET) performed at 2 months after the first operation revealed an increased OEF in the contralateral (right) frontal lobe that was suspected of being possible remote ischemia. The patient underwent right STA-MCA bypass and EMS. 15O-PET at 14 days after the second operation revealed an increased OEF in the contralateral (left) occipital lobe that was suspected of potentially being remote ischemia caused by a watershed shift. Two years after the second surgery, left occipital artery (OA)–posterior cerebral artery (PCA) anastomosis and EMS were performed due to transient right hemianopsia. Neither rebleeding nor ischemic complications occurred 2 years after the third surgery. We need to be alert for the possible progression of PCA stenosis in MMD after revascularization. It might induce remote ischemia after revascularization. OA–PCA bypass is therefore considered to be an effective treatment option in such cases.

  • Aortic compression of the esophagus: A case report and literature review

      Distal aortic compression of the lower esophagus with consecutive dilatation of the proximal esophagus was first described in the literature in 1932. Here, the authors describe the case of a 66 year-old male complaining of gastroesophageal reflux. Due to a positive family history of carcinoma of the esophagus and compression of the dorsal esophagus during an esophageal barium swallow test, further tests were performed. Endoscopy and CT exam revealed a dilated esophagus due to compression of a crossing aorta. Because of mild symptoms and the absence of dysphagia, no further treatment was necessary except for the use of a proton pump inhibitor and recommended follow-ups every one to two years.

  • Reconstruction of cardiovascular black-blood T2-weighted image by deep
           learning algorithm: A comparison with intensity filter

      BackgroundDeep learning–based methods have been used to denoise magnetic resonance imaging.PurposeThe purpose of this study was to evaluate a deep learning reconstruction (DL Recon) in cardiovascular black-blood T2-weighted images and compare with intensity filtered images.Material and MethodsForty-five DL Recon images were compared with intensity filtered and the original images. For quantitative image analysis, the signal to noise ratio (SNR) of the septum, contrast ratio (CR) of the septum to lumen, and sharpness of the endocardial border were calculated in each image. For qualitative image quality assessment, a 4-point subjective scale was assigned to each image (1 = poor, 2 = fair, 3 = good, 4 = excellent).ResultsThe SNR and CR were significantly higher in the DL Recon images than in the intensity filtered and the original images (p < .05 in each). Sharpness of the endocardial border was significantly higher in the DL Recon and intensity filtered images than in the original images (p < .05 in each). The image quality of the DL Recon images was significantly better than that of intensity filtered and original images (p < .001 in each).ConclusionsDL Recon reduced image noise while improving image contrast and sharpness in the cardiovascular black-blood T2-weight sequence.

  • Intervariability in radiographic parameters and general evaluation of a
           low-dose fluoroscopic technique in patients with idiopathic scoliosis

      BackgroundRadiographic images in adolescent idiopathic scoliosis (AIS) have a potential radiation-induced oncogenic effect; thus lowering radiation dose by using fluoroscopic imaging technique of low-dose fluoroscopic technique (LFT) which might be relevant for clinical evaluation.PurposeTo compare radiographs of LFT with gold standard radiographs for AIS ordinary radiographic technique (ORT).Material and MethodsImage quality was evaluated for LTF and ORT of a child phantom and two 3D-printed models (3DPSs) of AIS. We measured the primary physical characteristics of noise, contrast, spatial resolution, signal-to-noise ratio, and contrast-to-noise ratio. Three independent evaluators assessed the radiographs by observer-based methods of image criteria (ICS) and visual grading analysis(VGAS). Radiation doses were evaluated by the dose-area-product (DAP) of the 25 phantom radiographs. Reliability and agreement of Cobb’s angle (CA) and other radiographic parameters were evaluated on the 3DPSs and reliability on 342 LFT.ResultsThe average noise and contrast were approximately 15-fold higher for LFT. SNR and CNR were similar. Overall, ICS and VGAS were 3-fold higher for ORT than for LFT for L3 and similar for Th6. Reliability and agreement were good for the experimental LFT, and the interclass correlation coefficient for CA was 0.852 for the clinical LFT. The average DAP and effective dose for LFT were 8-fold lower than those for ORT.ConclusionIn conclusion, LFT is reliable for CA measurements and is thus useful for clinical outpatient follow-up evaluation. Even though the image quality is lower for LFT than ORT, the merits are the substantially reduced radiation and a lowered malignancy risk without compromising the measurement of Cobb’s angle, thus following the principles of ALARA.
