      Une patiente âgée de 66 ans est atteinte d’un diabète de type 2 mal équilibré et d’une maladie polyartérielle. Elle bénéficie d’une prise en charge globale à laquelle participe le pharmacien. L’ objectif du traitement du diabète de type 2 est le contrôle glycémique et la prévention des risques cardiovasculaires. A 66-year-old patient has uncontrolled type 2 diabetes and polyvascular disease. The pharmacist has a role to play in her global treatment. The goal of treatment for type 2 diabetes is glycemic control and cardiovascular risk prevention.

      La sclérose en plaques est une pathologie auto-immune et inflammatoire qui affecte le système nerveux central. Elle se présente sous différentes formes et entraîne des lésions qui provoquent des troubles moteurs, sensitifs, vésico-sphinctériens et cognitifs, qui peuvent progresser vers un handicap irréversible. Les premiers symptômes apparaissent le plus souvent chez l’adulte jeune. Multiple sclerosis is an autoimmune and inflammatory disease which affects the central nervous system. It manifests itself in different forms and results in lesions which cause motor, sensation-related and cognitive disorders and bladder and bowel difficulties, which can progress towards irreversible disability. The first symptoms usually appear in young adults.

      En France, l’incidence de la sclérose en plaques est en augmentation. Bien que certains facteurs, notamment environnementaux ou génétiques, aient été mis en évidence, ils ne peuvent expliquer à eux seuls le déclenchement de la maladie. L’évolution de l’imagerie médicale et des critères de diagnostic a permis de mieux caractériser cette pathologie afin de débuter le traitement de fond dans les meilleurs délais. In France, the incidence of multiple sclerosis is increasing. While some factors, notably environmental or genetic have been discovered, they alone cannot explain how the disease is triggered. The evolution of medical imaging and diagnostic criteria have improved the way this disease is characterised allowing primary therapy to be started as early as possible.

      Depuis une dizaine d’années, le caractère polysymptomatique de la sclérose en plaques et son impact psychologique sont mieux connus. Cependant, certains symptômes restent sous-évalués et insuffisamment pris en charge. Connaître les spécificités de cette pathologie et prendre en compte les particularités de chaque patient sont les nouvelles voies à emprunter par les pharmaciens d’officine dans le suivi et l’accompagnement des malades. Over the last decade, more has been discovered about the polysymptomatic character of multiple sclerosis and understanding of its psychological impact has improved. However, certain symptoms remain under-evaluated and inadequately managed. Knowing the specificities of this disease and taking into account the particularities of each patient are new approaches to be taken by community pharmacists in the follow-up and support of patients.

In some countries, these activities have been evaluated and are refundable by the national health care system. In France, the involvement of pharmacists in these activities was until recently relatively unknown. A survey of professionals working in health centres provides an initial assessment of the current situation.

      Les prescriptions médicamenteuses inappropriées sont associées à une diminution de la qualité de vie, une augmentation de la morbi-mortalité et des surcoûts pour les systèmes de santé. Les patients âgés, souvent polypathologiques et polymédiqués, sont à haut risque. Des outils ont été développés pour prévenir ces erreurs. Les critères STOPP/START constituent une aide substantielle et validée pour les praticiens dans une démarche de révision et d’optimisation des prescriptions. Potentially inappropriate prescriptions are associated with adverse drug events, increased morbi-mortality and health-care costs, and poorer quality of life. Due to polymorbidity and polypharmacy, it is common among the elderly. Explicit indicators to detect inappropriate prescriptions were developed in geriatrics. The STOPP/START set of criteria is a substantial and validated help for the practitioners in the process of reviewing and optimizing prescription.

Ageing is accompanied by a physiological decline in kidney function, which can lead to an accumulation of medications, responsible for a level of toxicity corresponding to an overdose. A website presenting the correct clinical use of medicines gives dosage adjustments and calculators for estimating kidney function. In the pharmacy, this tool helps to make the dispensing of medicines to elderly people safer.

The medication review is a structured interview with the patient, carried out by the pharmacist in collaboration with the general practitioner with the aim of optimising patient care. Several Anglo-Saxon countries have integrated it into the missions of the community pharmacist. In France, the latest round of collective bargaining with the state health insurance organisation has validated the remuneration of this evaluation judged to be a priority for elderly patients taking multiple medications.

      L’insuffisance cardiaque congestive représente généralement une insuffisance cardiaque droite et gauche combinée, qui provoque une congestion pulmonaire et un œdème périphérique. Le cœur subit d’importants remaniements : la fibrose gêne le fonctionnement électrique et mécanique de l’organe et la fonction ventriculaire s’altère progressivement jusqu’à entraîner le décès. Les différents traitements disponibles permettent de ralentir l’évolution de cette pathologie, prolongeant de ce fait la survie des malades. Congestive heart failure generally represents combined right- and left-sided heart failure, which provokes pulmonary congestion and peripheral oedema. The heart is subject to significant changes: fibrosis affects the electrical and mechanical functioning of the organ and the ventricular function gradually deteriorates eventually leading to death. The different available treatments help to slow down the progress of this disease, thereby extending patients’ survival.

Des trois molécules de la famille des biguanides commercialisées dans les années 1950, la metformine est la seule encore disponible sur le marché en France et reste le traitement médicamenteux de première intention. Son mécanisme d’action est de mieux en mieux connu, notamment son rôle freinateur au niveau du métabolisme cellulaire et sa capacité à améliorer l’insulinorésistance. While the initial treatment of diabetes is based on the respect of dietary and lifestyle guidelines, medication is often necessary to balance blood sugar levels. Of the three molecules of the family of biguanides which arrived on the market in the 1950s, metformin is the only one still available in France and remains the first-line medication. More is now known about its action of mechanism, notably its role in slowing down cellular metabolism and its capacity to reduce insulin resistance.

      Les sulfamides hypoglycémiants sont encore largement utilisés par de nombreux patients diabétiques de type 2, principalement en seconde intention et souvent en association avec d’autres molécules antidiabétiques. L’action des glinides, molécules plus récentes, est très proche de celle des sulfamides. Le seul représentant de la classe actuellement sur le marché est le répaglinide. Sulphonylureas are still widely used by many patients with type 2 diabetes, mainly as a second-line treatment and often in combination with other antidiabetic molecules. The action of glinides, more recent molecules, is very similar to that of sulphonylureas. The only representative of the class currently on the market is repaglinide.

      Les incrétines constituent une option thérapeutique assez récente pour la prise en charge du diabète de type 2. Le manque de recul sur leur bénéfice et leur tolérance à long terme limite encore leur utilisation. Elles sont par conséquent recommandées en deuxième intention et en association. Le pharmacien doit s’assurer du respect des conditions de prescription des incrétinomimétiques et des gliptines, et accompagner le patient dans la prise en charge de son diabète avancé, notamment en pratiquant l’éducation thérapeutique. Incretins constitute a relatively recent therapeutic option for treating type 2 diabetes. The lack of data as to their benefit and their long-term tolerance still limits their use. They are consequently recommended as a second-line treatment and in combination with other therapies. The pharmacist must ensure the prescription instructions for incretin mimetics and gliptins are respected and support the patient in the management of his or her advanced diabetes, notably through therapeutic education.

      L’arsenal thérapeutique permettant la prise en charge du diabète évolue régulièrement. Cependant, d’anciennes classes sont toujours sur le marché, même si elles sont parfois caractérisées par une efficacité modeste comme les inhibiteurs des alpha-glucosidases. En raison d’un très bon profil de tolérance, ces médicaments viennent en complément des autres traitements pour la prise en charge du diabète type 2 mal contrôlé ou en cas de contre-indications aux thérapeutiques recommandées. The available treatments for diabetes are constantly evolving. However, old classes of drugs are still on the market, even if they are sometimes characterised as having a modest efficacy, as is the case of alpha-glucosidase inhibitors. Due to a very good tolerance profile, these medications can be used as a complement to other treatments for poorly managed type 2 diabetes or in cases of a contraindication in relation to recommended treatments.
