  • A comparative study of radiofrequency micro-needling with platelet rich
           plasma and fractional LASER in acne scar management

      Abstract: Background: Acne scars (AS) results in permanent scarring causes facial disfigurement to the patients. The aim of the study was to assess the comparative effectiveness and outcomes of radiofrequency micro needling and fractional carbon dioxide laser methods in treating acne scars.Methods: This comparative observational study was conducted in the Ibn Sina diagnostic and consultancy center, Dhaka, Bangladesh during the period from January 2019 to December 2020. In total 30 patients with acne scar were selected as the study population. Among total participants, 15 patients who underwent radiofrequency micro needling treatment method were denoted as group-1 and the rest 15 patients who underwent carbon dioxide laser treatment method were denoted as group-2 patients. All data were processed, analyzed and disseminated by Microsoft (MS) office and statistical package for the social sciences (SPSS) version 26.Results: In this study, in group A as per Goodman and Baron scale at the baseline 40% had grade-3 and 60% had grade-4  status. In the same group, after treatment 23.33, 30.00%, 20.00% and 13.33% patients improved up to grade 1, grade 2, grade 3 and grade 4 status respectively. On the other hand, in group-2, after treatment 26.67%, 50.00%, 20.00% and 13.33% patients improved up to grade 1, grade 2, grade 3 and grade 4 status respectively. Finally, according to the mean Goodman and Baron scale scores in both the groups, we found extremely significant correlations between both the baseline and after treatment status where <0001.Conclusions: Both radiofrequency micro needling and fractional CO2 laser treatment methods are effective for the treatment of acne scars.

  • The clinical pattern of sexually transmitted diseases in the outpatient
           department (skin and VD), Sher-E-Bangla Medical College Hospital, Barishal

      Abstract: Background: Sexually transmitted diseases (STDs) affect men and women of all backgrounds and economic levels. CDC estimates that 19 million new infections occur each year, almost half of them among young people ages 15 to 24. Common STDs are acquired immune deficiency syndrome (AIDS), chancroid, genital HPV infection, gonorrhea, syphilis, trichomoniasis and viral hepatitis. The aim was to see the pattern of STDs in the skin and VD outpatient department (OPD), Sher-E-Bangla Medical College Hospital, Barishal, Bangladesh.Methods: The prospective observational study was conducted among 100 patients with STDs both male and female patients in the OPD of dermatology department of Sher-E-Bangla Medical College Hospital during the period from July 2015 to December 2015 irrespective of sex and age >15 years to assess the prevalence of sexually transmitted diseases.Results: In this study, prevalence of STDs such as gonorrhea, syphilis, non-gonococcal urethritis, chancroid, genital herpes and genital warts were observed. Gonorrhea was 28%, 13% were syphilis, non-gonococcal urethritis (NGU) was 43% and chancroid, genital herpes, genital warts were 5%, 7% and 4% respectively.Conclusions: STDs are becoming a major public health problem in our county. So, STDs have to be wiped in the bud by early diagnosis, efficient treatment and appropriate preventive and control measures.

  • A comparative study on efficacy of oral terbinafine and itraconazole on
           dermatophytic infections

      Abstract: Background: The increased cases of dermatophyte infections and their poor response to oral drugs with current doses and duration of treatment is common. So, we conducted the study with a goal to assess the efficacies of two antifungal drugs with their increased dosages and duration. Methods: A prospective comparative study was done on 145 patients belonging to age group 18-60 years which were both clinically and microbiologically confirmed cases of tinea cruris and tinea corporis. Patients were randomized into two groups A and B. Group A patients were given capsule itraconazole 100 mg twice daily and group B were given tablet terbinafine 250 mg twice daily, both for 6 weeks. Patients were followed up at 2, 4 and 6 week and at each visit, scores for 3 parameters (erythema, scaling and pruritus) were recorded and were analysed.Results: A total 140 patients completed the study with 72 patients in group A and 73 patients in group B. In group A 72.5% patients achieved complete remission compared to 67.6% in group B. At each follow up patients were evaluated based on total symptom score (erythema, scaling and pruritus) which had shown significant improvement with negative potassium hydroxide results. None of the patients had severe side effects.Conclusions: Study showed that Itraconazole has higher clinical and mycological cure rates as compared to terbinafine. So itraconazole is superior to terbinafine in treatment of tinea corporis and cruris.

  • Diagnostic utility of Chicago sky blue stain in superficial
           dermatophytosis

      Abstract: Background: Dermatophyte infections are one of the commonest presentations to a dermatology OPD. The clinical lesions are highly variable and closely resemble other skin diseases, thus it is important to confirm one’s diagnosis using rapid, cheap methods.Methods: Skin scrapings from the lesions of 352 suspected cases of dermatophytosis were subjected to KOH mount and KOH with Chicago sky blue (CSB) stain for direct microscopic examination and culture using Sabouraud’s dextrose agar (SDA). Taking the culture as the gold standard method for diagnosis, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) of KOH mount and CSB staining method was calculated. Kappa statistics were used to determine the degree of congruence between the various diagnostic methods.Results: Direct microscopy with CSB stain, KOH mount and mycological culture showed positive results in 251 (71.3%), 227 (64.5%) and 292 (82.9%) patients, respectively. Cohen’s Kappa was used to see the agreement between KOH mount and culture (Kappa=0.09) which was statistically significant (p<0.05) at 95% CI. CSB staining and Culture (Kappa=0.09) also showed negligible agreement (Kappa=0.09) which was statistically significant (p<0.05). CSB staining and KOH mount showed substantial agreement (Kappa=0.8) which statistically significant (p<0.05).Conclusions: The addition of CSB stain to KOH provides a colour contrast that highlights the fungal elements well, making interpretation easy which increases the overall diagnostic accuracy. CSB stain is inexpensive, easily available and provides a rapid method to diagnose dermatophytosis.

  • Histopathological and clinical correlation of cutaneous adverse drug
           reaction

      Abstract: Background: The aim was to study the histopathological features of cutaneous adverse drug reactions (ADRs) and its correlation to clinical presentation.Methods: We carried our study on 80 patients of drug reactions presented in OPD over a period of 24 months. We noted offending drug, time gap between drug intake and appearance of lesion clinically and performed biopsy to study histopathological patterns and their utility as an aid to diagnosis. WHO-UMC causality assessment method was used in few cases with due diligence. AGEP (acute generalized exanthematous pustulosis) validation score of the EuroSCAR study group was used and SCORTEN for prognosis of SJS/TEN (Stevens-Johnson syndrome/toxic epidermal necrolysis). Histopathological correlation and clinical correlation were statistically analysed.Results: The most common histopathological finding was vacuolar interface dermatitis and presence of eosinophils. The most common drug responsible was antimicrobials. Histopathological findings were most consistent in cases of FDE and AGEP and differentiating viral exanthems from maculopapular rash was challenging. Erythroderma showed varying patterns of histopathologyConclusions: Identification of histopathological patterns and clinical correlation is important for distinguishing between cutaneous ADR and the other inflammatory dermatoses. Drug reactions pose clinical challenge thus clinicopathological correlation can help in reaching diagnosis.

  • Injectable platelet rich fibrin therapy for androgenetic alopecia: a
           series of 15 cases

      Abstract: Androgen dependent hair loss is the most common cause of progressive hair loss due to androgenetic miniaturization of hair follicles. Grade 1 and 2 of alopecia is managed well with topical and oral modalities, however grade 3 onwards the need adjuvant treatment arises. To evaluate the efficacy, safety profile and feasibility of injectable-platelet rich fibrin (I-PRF) in androgenetic alopecia and assess the patient satisfaction with this new treatment modality, we conducted a pilot study. Here, 15 patients of androgenetic alopecia were treated with injectable PRF and micro-needling at 2 weekly interval for 4 sittings with a background therapy of minoxidil, finasteride and multivitamin supplements. Monitoring was done with clinical photographs and dermoscopic evaluation prior to each session and a standard assessment questionnaire was given at the end of the study. A significant improvement in hair growth was observed clinically with positive score of 7.42 on the patient satisfaction scale and visible changes were noticed on clinical photographs and dermoscopy. Hence, we concluded that I-PRF is safe, easy, time and cost-effective adjuvant modality for managing androgenetic alopecia with some theoretical advantages over PRP.

  • Successful self-healing of vitiligo condition and overcoming psychosocial
           stresses of a female patient using Yoga Prana Vidya protocols with
           persistence and determination: an in-depth case study

      Abstract: Vitiligo is a skin condition with patches of white color occurring on any area of the human body and several treatment approaches are recommended in main stream medicine as well as alternative modes of treatments. Patients suffer psychosocial stress due to social rejection and consequent loss of self-esteem and self-confidence. This paper presented an in-depth case study of a 29-year-old female vitiligo patient, who used and practiced Yoga Prana Vidya (YPV) protocols for self-healing. Case series method is used examining full case papers and narration of major events by the patient on what worked well for her using YPV system intervention. The patient persistently healed herself with 2 to 4 sessions a day and within a time span of 1½ months her all the vitiligo patches had healed almost by 50%. After a year of continuing the YPV intervention, the recovery was sustained and further recovery of 30% was seen as she continued with her practices of YPV techniques. As a result, she regained self-esteem and self-confidence overcoming the accumulated psychosocial stress. During this period, she was not taking any medications, but only doing self-healing using YPV protocols. The overall improvement over a period of one year and three months was nearly 80% and a complete life changing experience with self-acceptance after a prolonged traumatic phase in life. YPV system is proving to be a versatile approach to resolving a variety of health issues, as is evident from the published literature.

  • Granuloma annulare of the external ear, a rare mimic of malignancy: two
           case reports

      Abstract: Granuloma annulare (GA) is a granulomatous inflammatory largely cutaneous and less often subcutaneous condition with varied presentations ranging from localized to disseminated forms manifesting as erythematous papules or plaques affecting all ages and primarily involving the extremities. GA may affect other sites such as the face and cases limited to the orbit and ear are exceedingly rare. No consistent causative factor has been identified although there is an established association with diabetes mellitus, autoimmune diseases, infections, malignancies or localized trauma. The chief histological pattern of palisaded or interstitial granulomatous reaction is diagnostic, providing infections such as mycobacterium have been excluded. Standardized treatment is elusive and different modalities including local/intralesional steroids, cryo/photo/laser therapy or oral retinoids/antibiotics have been used with variable response. We report two cases of GA limited to the ear adding to the 13 previously published cases.

  • Dermoscopic assessment of periungual and genital warts treated by multiple
           modalities: a report

      Abstract: Periungual and genital warts pose a therapeutic challenge to the treating clinician as there is lack of single curative treatment modality. All existing therapeutic options are associated with high rate of recurrence when employed alone. Here authors reported complete cure of periungual and genital warts that were treated by multiple approach consisting of mumps-measles-rubella vaccine, radiofrequency ablation and oral acitretin.  Therapeutic response was assessed by dermoscopy which revealed specific patterns before and after treatment. There was no recurrence of lesions even after 1 year of follow-up period.

      Abstract: The coronavirus disease 2019 (COVID-19) is an acute respiratory disease caused by a novel coronavirus (SARS-CoV-2, previously known as 2019-nCoV). Dermatological disorders are among the numerous observations in COVID-19 patients that are consistently reported from various clinical sites throughout the world, although their relationship with illness severity is unknown. Urticarial rash, maculopapular or morbilliform rash, papulovesicular lesions, chilblain, livedo reticularis, vasculitis-like picture, and skin necrosis have all been reported with different frequencies and associated circumstances. In this review article, we will highlight reports of skin necrosis, one of the rare skin findings that are associated with COVID, and their clinical and pathophysiological aspects, in addition to attempting to determine whether they are one of the possible outcomes of this virus infection or a complication of various treatment programs. and to discuss the overlap between skin necrosis due to COVID-19-associated coagulopathies and disseminated intravascular coagulation. Cutaneous necrosis in COVID-19 patients during the pandemic period, according to this review, is not rare. possibly It is simple to diagnose and recognize this dermatological condition, but determining the causal variables and their function in pathogenesis is a complex and difficult task. It's also tough to categorize because it affects people of all ages, regardless of gender, ethnicity, or health status. All hypotheses discuss the direct viral role, the immune system's hyperinflammatory state with cytokine storm, the hypercoagulable state with vasculitis, anticoagulant therapy complications, and deep secondary necrotizing infections as possible interactions that lead to necrosis, which can result in serious outcomes such as disseminated intravascular coagulation if not properly treated.

  • Basic approach to melanonychia: a review

      Abstract: Melanonychia is a brownish-black staining of the nail plate. It can affect a single or several nails on both the fingers and toes. Melanocytic activation and melanocytic hyperplasia are the two basic mechanisms involved in the development of melanonychia. Melanocytic activation occurs as enhanced melanic pigmentation of the nail matrix epithelium and nail plate without a corresponding rise in the number of melanocytes, resulting in melanonychia. The number of matrix melanocytes increases in melanocytic hyperplasia. Involvement of a pathogen has also been suggested as a mechanism for its development. A thorough clinical history should be taken, including questions regarding various triggers and causal events, as well as the use of a questionnaire. A proper detailed physical examination is of a great need to properly identify the type of melanonychia and to rule out melanoma which is of great importance in clinical practice. Onychoscopy and nail biopsy also acts as an aid in proper diagnosis of melanonychia. Treatment of melanonychia basically depends on the underlying etiology. One major aim is to rule out melanoma if present and its proper management in time.

  • Comparative real-world effectiveness and safety of super-bioavailable
           itraconazole 50 mg bd and 65 mg bd in the management of dermatophytosis

      Abstract: In recent times, India has witnessed a steep surge in prescription of itraconazole in the management of dermatophytosis.1 But due to its pharmacokinetics, it has shown multiple variations in the results. To overcome these challenges, a newer itraconazole formulation i.e. super-bioavailable itraconazole (SBITZ) has been launched recently in India. Currently, there are 4 different strengths of SBITZ which are approved by DCGI.2,3 But there is no any clinical data regarding comparison of different strength of SBITZ. This retrospective analysis aimed to compare two different strengths of SBITZ; 50 mg and 65 mg in the management of dermatophytosis.
