- ELDERLY SURGERY PATIENTS AND FRAILTY SYNDROME
Authors: Esma ÖZŞAKER
Abstract: Today, in parallel with the increase in the elderly population, the rate of surgicalprocedures in the elderly population is increasing. As many parts of the worldexperience the demographic trend towards aging, fragility is becoming an increasinglyimportant issue. It is expected that frailty rates will increase in parallel withthe increase in the number of elderly people, and the number of fragile patientswho apply for surgery will also increase. Frailty is a state of increased vulnerabilitythat results from decreased physiological reserve and function in multiple organsystems. The fragile individual’s ability to withstand stress factors is impaired. Frailtyis an important issue in surgery because of its association with adverse health eventsand poor postoperative outcomes. While the surgical process causes post-operativecomplications and deterioration in recovery in most elderly patients, this is evenmore important in frail elderly patients. Therefore, a comprehensive geriatric evaluationwith a patient-centered and multidisciplinary approach is recommended inthe preoperative period. Another important issue is that vulnerability is reversiblewhen detected at an early stage. For this reason, it is very important for nurses andother health professionals to identify the frail or pre-frail elderly and take precautionsfor this issue. Many screening tests, evaluations and measurements with provenreliability are used in the detection of frailty. In this review, frailty, risk factors, characteristics, evaluation methods, the effects of frailty in the surgical process andrecommendations for the management of frail individuals are included

- EFFECT OF AROMATHERAPY ON THE CONTROL OF URODYNAMIC PAIN AND ANXIETY
Authors: Elife KETTAŞ DÖLEK; Gülay ALTUN UĞRAŞ
Abstract: Urodynamics is a diagnostic evaluation method consisting of multiple teststo obtain physiological data about the function of the lower urinary tract duringfilling and emptying of the bladder. It can cause problems such as urodynamics, burning in urination, bleeding and pain. Apart from these problems, patients mayexperience embarrassment, discomfort, tension, anxiety and stress due to reasonssuch as placing a catheter in the patient’s bladder, rectum or vagina, the procedurebeing an invasive and traumatizing method, urinating in the presence of a strangerduring the procedure, and observing the urination status during the test. Theincrease in the patient’s stress level during urodynamics may decrease the bladdercapacity by stimulating the sympathetic nervous system and may affect thepatient’s urodynamic results. To reduce the patient’s pain and anxiety during theurodynamic proceduremany pharmacological and non-pharmacological interventionsisimplemented. Aromatherapy is a preferred non-pharmacological practicein patients with anxiety before invasive procedures. When aromatherapy is comparedto pharmaceutical methods; It is a simple, inexpensive, non-invasive and easy-to-apply method. The use of essential oils in nursing science reflects nursing philosophy, theories and ethics. In this context, it has been stated that aromatherapycan be used as a holistic nursing intervention in providing comfort, coping withstress, and reducing pain and anxiety. Clinical aromatherapy strengthens nursingas it contributes to the art of nursing as well as to the science of nursing. However, the important deficiency of aromatherapy in nursing practices is that scientific studiesin this field have not yet turned into studies with a high level of evidence. Thisthe purpose of this review is to convey the use of aromatherapy in reducing thenegative emotions experienced by patients during the urodynamic procedure andthe responsibilities of nurses in the light of current literature.
