  • Introduction: Re-imagining the House of Old / Réinventer la maison de
           vieillesse
    • Abstract: Across Canada, an estimated quarter of a million people currently reside in long-term residential care (LTRC) (Statistics Canada 2011), institutions that we variously refer to as care homes or nursing homes. Nearly half of the women and men residing in the House of Old are 85 years or older, and more than 80% are over 65. These are some of our most disabled and vulnerable citizens. With an aging baby boom generation and increasing numbers of older people living longer with complex health conditions, our LTRC population—and the team of (mostly female) nurses, care aides, rehabilitation therapists, family members and friends, cooks, cleaners, and dietary and laundry workers who support them—is expected to double over ... • There's No Place like Home: Designing for Long-Term Residential Care in
           Canada
    • Abstract: Long-term care residences are easy to spot. From years of observing the building type, we know that most are set back from the road, made of brick, one or two storeys high, and have a prominent entrance, rows of repetitive windows, and pitched roofs.1 Typically, a long-term residential care (LTRC) building spreads horizontally across its site, justifying generous parking lots and expansive lawns. Invisible from the street is the ubiquitous inner courtyard, a chunk of landscape wholly contained by the walls of the facility and small enough to survey at a glance. Wheelchair ramps and orientation devices hint at the building's unique mandate to serve as a home away from home for people who, for one reason or another ... Read More

  • Promising Practices in Long-Term Residential Care: Where Do Physicians Fit
           In (or Don't They)?
    • Abstract: Long-term residential care (LTRC) facilities are places of social, nursing, and medical care for individuals who are unable, by virtue of advanced frailty, dementia, or functional loss, to live independently. Most LTRC facilities in Canada require residents to have a most responsible physician (more recently in some provinces, this may also include most responsible nurse practitioner) as a precondition to be admitted, and many provinces require facilities to have a medical director or co-ordinator (see table 1). Some provinces and jurisdictions have also chosen to invest substantial funds toward enhancing the physician role in residential care facilities (General Practice Services Committee 2015). Despite a ... Read More

  • Family Matters: The Work and Skills of Family/Friend Carers in Long-Term
           Residential Care
    • Abstract: I often tell them "I'm the translator. I translate mom to you. And you to mom." And I do try to make it fun. But I think that it's sort of team building, um, trying to train, on the job training. In the quote above, a daughter explains how she trains and works with long-term residential care (LTRC) staff to ensure that her mother receives appropriate care. This daughter, like many other family members and friends, engages in complex labour as she supports her mother. She sees herself as an interpreter, a teacher, and a team-builder, and also as an important liaison between her mother and LTRC staff. This is an impressive job description, yet family/friend care work—performed primarily by women—is routinely ... Read More

  • Policies and Practices: The Case of RAI-MDS in Canadian Long-Term Care
           Homes
    • Abstract: Residents of long-term care (LTC) or nursing homes are among the most vulnerable members of society. Most are very old and frail, and many are unable to walk, to dress, to feed, and/or to toilet themselves. Well over half are cognitively impaired, often to a substantial degree. Not incidentally, a large majority are women. They need help with some or all of the activities of daily living; such help is primarily provided by other women—women who are themselves vulnerable—whether they are paid to provide help or not.In their vulnerability, LTC residents are subject to certain types of close scrutiny—to measures that are "easy to capture" (Cassel et al. 2014, 2146). These include resident indicators, such as food and ... Read More

  • Changes and Continuities in the Workplace of Long-Term Residential Care in

    • Abstract: Long-term residential care (LTRC) occupies a troubled space in the public imagination (Armstrong and Braedley 2013). Historically and across geographies, LTRC is linked to distasteful memories of the poorhouse, a highly stigmatized place of last resort for the so-called friendless and destitute elderly (Crowther 1981; Katz 1984; Montigny 1997; Struthers 1994). Today, even though more Canadians are living out the final years and months of their lives in LTRC, such facilities are still "something most people don't like to talk about," often unfairly "riddled with notions of failure" (Armstrong and Banerjee 2009, 12). As objects of media attention, they have all too frequently become pinned in the spotlight of scandal ... Read More

  • Intensifying Relational Care: The Challenge of Dying in Long-Term
           Residential Care
    • Abstract: The philosophy and the resources don't match what's actually happening. We're seeing a disconnect. The organization seems to be going one way, we're creating vibrant communities where elders and seniors can thrive but, in fact, what we're seeing most of the time is elders arrive approaching end of life or with very complex diagnoses, and we just don't have the resources to care for them. How can we strengthen the capacity of Canadian long-term residential care (LTRC) facilities to care for elderly residents at the close of life? In this article, we explore the current state of end-of-life care in LTRC from the perspective of those who work in these institutions. Drawing on interview data and on-site observations ... • Imagining Gerontological Nursing: Canadian Nurses and Eldercare,
           1905–70
    • Abstract: Long-term residential care facilities—often known colloquially as nursing homes—are the subject of significant contemporary concern. Media coverage, academic studies, familial complaints, and policy debates all centre on the quality and availability of institutional care for the elderly. Such concerns are not new. As historians of aging have shown, since the mid-twentieth century, welfare states throughout the industrial world struggled to determine the appropriate place for nursing homes in the overall mix of domestic and institutional care. It is ironic, though, that in analyses of nursing homes, nurses themselves are often shadowy figures (Brooks 2011; Pelling and Smith 1991).1 It is true that nursing personnel ... Read More

  • A Humanist in the House of Old: Moyra Jones and Early Dementia Care in
           Canada
    • Abstract: She was a very big woman, right? Big in every way. My memory of her was that she was a lot of fun, she was a visionary, she was one gutsy courageous lady. And she was passionate about educating people, about what seniors needed and about dementia care—and about the conditions of what it is like to live with a dementing disorder.Everyone told me that Moyra Jones was a big woman. She was larger than life; she was articulate, funny, and ferocious; she was a futurist thinker, always at least five years ahead of her professional moment; she wore vibrant coloured clothing; she was a problem solver; she stood up for her colleagues and treated older people with dementia with respect; and she had a deeply kind heart ... • The Canadian Long-Term Residential Care System: A British Perspective
    • Abstract: As a British academic, I welcomed the opportunity to join a research project in which we were invited to re-imagine long-term residential care (LTRC) and to observe first-hand how services are provided in the participating countries. In the United Kingdom (UK),1 LTRC has been through turbulent times over recent decades, and there has been a tendency for research to focus in a limited way on the here-and-now rather than to imagine different possibilities. An important element of the design of this project was to bring the fresh eyes of overseas scholars to the research. The rapid site-switching ethnographic methodology of the project created opportunities for observations of daily life in care facilities in all ... Read More

  • A Norwegian View on Canadian Long-Term Residential Care
    • Abstract: Long-term residential care (LTRC) means different things in different jurisdictions (Harrington et al. 2012). In Norway, LTRC predominantly implies nursing homes, defined as medical institutions subject to health legislation (Hauge 2004). We argue that LTRC in Norway also represents a strong egalitarian ideal of public services (Esping-Andersen 1990), where routines and standardization are supposed to facilitate a fair distribution of service to all. While there is much variation in what LTRC facilities are and how they function, some basic, overarching tensions can be identified across jurisdictions. Whether defined as medical institutions or not, the staff need to strike a balance between cure and care. An LTRC ... Read More

  • Residential Care for Older People: Are There Lessons to Be Learned from
           Sweden?
    • Abstract: It has been argued that Swedes have a passion for equality. This passion, often referred to as an ideal of universalism, was the guiding light for Swedish welfare state architects in the building of eldercare services after the Second World War. A crucial aspect of this equality ambition was (and still is) that the same high-quality services should be offered to and used by all social groups (Blomqvist 2004). A central equality argument behind this aspect of universalism—that all social groups should use the same services—is that less privileged groups benefit if the same services are used also by those with more resources. If welfare services are stratified by income and social status, those with fewer resources ... Read More
